Currently there are barriers to establish a new patient with a primary care. The workflow for patients who self-refer currently requires 10 steps and is performed by 7 care team members.

  • 4 PSR at the front desk complete paper intake forms that are then manually entered into a spreadsheet by 3 other care team members.

Developing and maintaining good communication is central in nursing because the goal is to achieve the best outcomes for the patient. In the past, written handover reports have been the standard way nurses communicate with each other, especially in the hospital and in home-based practice. Despite this rationale, this approach has considerable weaknesses: its high risk of mistakes and misunderstanding could negatively influence patient outcomes. The goal of this article is to describe an integrated parallel process for the co-development of written and computable clinical practice guidelines (CPGs) to accelerate adoption and increase the impact of guideline recommendations in clinical practice. From February 2018 through December 2021, interdisciplinary work groups were formed after an initial Kaizen event and using expert consensus and available literature, produced a 12-phase integrated process (IP). The IP includes activities, resources, and iterative feedback loops for developing, implementing, disseminating, communicating, and evaluating CPGs. Patients leaving against medical advice (AMA) presents a challenge to hospitals as they try to manage costs and improve patient outcomes in an ever-increasing competitive market. Investigating AMA discharges that occurred during the early COVID-19 pandemic presents a unique opportunity to better understand this phenomenon and be better prepared for the future.

Methods

Currently, at my internship at Salinas Valley Health Orthopedics, we face a large number of uninsured patients.
