Acute confusion in older adults is a common emergency department presentation with a broad differential diagnosis. Tick-borne illnesses can cause significant systemic disease and may present with nonspecific neurologic or constitutional symptoms, making early recognition challenging. Babesiosis is a tick-borne parasitic infection transmitted by Ixodes ticks and may occur concurrently with other infections such as Lyme disease, further complicating diagnosis. We present a case of a 67-year-old male who presented with acute confusion and was found to have concurrent babesiosis and Lyme disease. This case highlights the importance of maintaining a high index of suspicion for tick-borne co-infections in patients …

Over 1.5 billion of the poorest people are infected with soil-transmitted helminths (STH) such as whipworm, hookworm, and Ascaris which have devastating effects on human growth, nutrition, and cognition, perpetuating the cycle of poverty. The World Health Organization only approves four anthelmintic drugs to treat STH for mass drug administration (MDA), with Albendazole being the drug of choice. Due to these drugs only having two mechanisms of action, resistance to these drugs has been reported over the years. We aim to better understand STH susceptibility to drugs at multiple points in their life stage. Previously, the effects of Albendazole and …

Over 1.5 billion individuals a year are infected with soil-transmitted helminths (STH) such as whipworm, hookworm, and Ascaris. These infections have detrimental effects on human growth, nutrition, and cognition leading to a cycle of poverty. Currently, The World Health Organization (WHO) recognizes four drugs to treat STH for mass drug administration which consist of two mechanisms of action. These fall into two classes, nicotinic acetylcholine receptor agonists and benzimidazoles. Albendazole, the drug of choice, is a beta-tubulin inhibitor that induces the immobilization and death of STH (Malik and Dua, 2023). The widespread use of these drugs and the limited mechanism …

Research Colloquium
Malaria is a serious disease caused by various species of the Plasmodium genus. Among these, Plasmodium falciparum is widely considered the most dangerous, due to its ability to infect both young and mature red blood cells (RBCs). Malarial disease, caused primarily by P. falciparum, is a major cause of morbidity and mortality in Sub-Saharan Africa, and notably in Chad, accounting for over 10,000 deaths each year and with an incidence rate of over 3 million. Anyone can acquire falciparum malaria after being bitten by infected anopheles mosquitoes, but in pregnant women and young children, this disease is often deadly. A …

We report a case of a 40-year-old male who presented with sensation of throat closing and shortness of breath while at work at a meat processing facility. The differential diagnosis of shortness of breath is throat closing includes, but is not limited to, allergic reactions to antibiotics in meat, allergy to food additives, Symptom triggers are useful in identifying causes and a good supporting history and exposures can help with diagnosis. In this case, the patient had a known diagnosis of Alpha-gal syndrome (AGS) which is a tick-bite associated, possibly life-threatening immunoglobin E-mediated (IgE) hypersensitivity to galactose-alpha-1,3-galactose (alpha-gal). The patient …

Babesiosis is a tick-borne illness caused by the parasitic protozoa Babesia transmitted by the Ixodes tick. Babesia infect the red blood cell of its host and requires the cell for its replication. It is a disease process that can range from asymptomatic infection to organ failure and death. This case presentation reviews a 68-year-old male who presented to the emergency department with flu like symptoms and was found to have babesiosis.

Targeting Cyp51 In The Treatment Of Acanthamoeba Keratitis, Ashkan Hassani, Vineet Sadarangani, Brad Haubrich
Annual Research Symposium
Acanthamoeba castellanii causes a rare but serious ocular infection called Acanthamoeba keratitis. Risk factors for infection include contact with contaminated water and poor hygiene of contact lenses. The first-line treatment is comprised of a months-long schedule of antibiotics. However, in cases where pharmacological interventions fail to address the infection, surgery may be necessary, with the continued risk of potential vision loss. Consequently, new treatments are needed, whether new pharmacological agents or new combination therapies with current agents. As an aerobic parasite, Acanthamoeba synthesizes ergosterol as a cell membrane insert, and ergosterol biosynthesis inhibitors (EBIs), common in antifungal pharmaceuticals, could potentially …

Microsporidiosis: Reviewing Clinical Presentation And Treatment Strategies, Julienne Zhou, Belinda Liu, Megan Dearmond, David Banach, Brad Haubrich
Annual Research Symposium
Microsporidiosis is an opportunistic infection caused by obligate intracellular pathogens within the taxon Microsporidia. Infection is zoonotic and has been associated with patients experiencing immunodeficiency, though incidence is increasing among other populations. Most commonly, the disease is gastrointestinal, typically by Enterocytozoon bieneusi and less commonly by Encephalitozoon intestinalis. Symptoms present with diarrhea and vomiting, and severe cases can lead to wasting and mortality. While the incorporation of anti-retroviral therapy for HIV patients has significantly decreased mortality in a subset of the vulnerable populations, treatment for microsporidiosis is still not standardized or well explored. Resistance to first-line treatment albendazole is increasing, …

Schistosomiasis is a leading parasitic infection that debilitates many individuals in sub Saharan Africa. Because this parasitic infection involves a snail as an intermediate host, we use a native prawn species as a natural predator to decrease the population of this snail community. This study focuses on Tomefa, a primarily fishing peri-urban community located oi the outskirts of Accra, the capital city of Ghana. A survey was also given to the local community measuring economic characteristics. In the laboratory, Macrobrachium spp prawn exhibit preference of Bulinus snails over Biomphalaria snails while non-parasitic snails were consumed in higher proportion to parasitized …

Development Of An Interleukin-4-Inducing Principle From Schistosoma Mansoni Eggs (Ipse)-Specific Pcr Assay As A Quantitative Predictor Of Schistosomiasis-Associated Morbidity, Dannah Farah, Evaristis Mbanefo, Michael H. Hsieh
GW Research Days 2016 - 2020
Schistosomiasis is a neglected tropical disease affecting between 200-500 million people worldwide. The two species causing most human cases of schistosomiasis are Schistosoma mansoni and Schistosoma haematobium. The gold standard for diagnosis is parasitological detection of parasite eggs in stool using the Kato-Katz method. Counting eggs shed in stool is labor-intensive and inaccurate. Interleukin-4- inducing principle from Schistosoma mansoni eggs (IPSE) is the most abundant secreted protein from schistosome eggs. We hypothesized that the mRNA transcripts of the IPSE protein may be found in the liver tissue and stool of experimentally infected animals, and that these transcripts can be specifically …
