Bedside Point Of Care Ultrasound Diagnosing Bilateral Valve Involvement In Infective Endocarditis, Corey Cole, Alfredo Tirado, Jonathan Schwadron

West Florida Division GME Research Day 2020

Endocarditis is a well known complication of intravenous drug abuse which classically affects the tricuspid valve. It is usually diagnosed by formal echocardiogram and positive blood cultures. Vegetations involving both the mitral and tricuspid valves are rare, occurring in less than 5% of all cases of infective endocarditis. 1 Here we report a case of a patient with vegetations on both valves which was discovered by point of care ultrasound.

A Comparison Of Models For Evaluation And Transfer Of Patients With Suspected Large Vessel Occlusions, Edward Jauch, Parita Bhuva, Alex Schneider, Jason Thurman, Josh Lewis, Kathryn Mccarthy, Debra Philpot, Robin Jones, Albert Yoo, Don Frei Feb 2020

A Comparison Of Models For Evaluation And Transfer Of Patients With Suspected Large Vessel Occlusions, Edward Jauch, Parita Bhuva, Alex Schneider, Jason Thurman, Josh Lewis, Kathryn Mccarthy, Debra Philpot, Robin Jones, Albert Yoo, Don Frei

Neurology

Introduction: Mechanisms for the hospital identification of patients with suspected large vessel occlusions (LVO) vary widely based on referring and receiving stroke center resources. We compared processes for identifying patients with suspected LVO who may be candidates for endovascular therapy (EVT) at non-EVT hospitals and the criteria for interfacility transport to Comprehensive Stroke Centers (CSC) for potential EVT. Methods: Data were obtained from 4 CSC which serve distinct geographic regions, ranging from rural to urban environments, over a continuous 12 month period of time beginning year 2018. Patients transferred from all referring hospitals to these facilities were considered regardless of …

Is The Thomas Ring An Effective Approach To Prevent Recurrence Of Persistent Atrial Fibrillation With Advanced Atrial Fibrosis?, Ruifang Yang Md, Jose Luis Henao Md, David Goldgrab Do, Sokol Kalaveshi Md, Stephen Mester Md, Christian Perzanowski Md

Cardiology

Introduction | Objectives: Pulmonary vein isolation (PVI) with the Thomas ring (TR, single ring posterior wall isolation) has been shown to reduce recurrence of primarily paroxysmal atrial fibrillation (AF). Atrial fibrosis, as an important pathophysiological contributor, is directly linked to AF recurrence and resistance to therapy. The effect of TR ablation on outcomes of persistent AF (peAF) with advanced atrial fibrosis has not been adequately studied.

Methods: Observational series of three peAF patients (Tab.) who underwent TR. The degree of atrial fibrosis was determined by electroanatomic bipolar voltage mapping (CARTO, Diamond Bar, CA, USA). The reduced voltage areas (Fig.) reflected …

Endovascular Management Of Free Floating Common Carotid Thrombi, Kevin Carr Md, Zachary Thwing Md, Ravishankar Shivashankar Md, Yafell Serulle Md Jul 2019

Endovascular Management Of Free Floating Common Carotid Thrombi, Kevin Carr Md, Zachary Thwing Md, Ravishankar Shivashankar Md, Yafell Serulle Md

Radiology

  • The incidence of symptomatic free-floating thrombi of the common carotid artery (CCA) is as low as 0.24-5% incidence in stroke victims. Patients are predisposed to a prothrombotic state
  • Historical management options were limited to surgical and medical options both with individual attendant risk profiles
  • Large thrombi can present with various degrees of clinical severity
  • The endovascular approach is a viable tool to manage these patients

Community Stroke Practice Experience With Small Vessel Thrombectomy, Kevin Carr Md, Zachary Thwing Md, David Pasquale Md, Ravishankar Shivashankar Md, Yafell Serulle Md

Radiology

  • Emergent endovascular revascularization is currently mainstay for patients presenting with emergent Large Vessel Occlusion (LVO) of the intracranial circulation
  • Stroke symptoms affecting smaller vessels can carry significant morbidity
  • Newer technologies and greater experience with the endovascular approach make treatment for small vessel occlusions increasingly more manageable
  • This is more relevant in patients presenting outside the tPA window with poor collateral circulation
  • We present our technical institutional experience with the endovascular management of acute occlusions of the M2-M3 MCA, A1-A2 ACA and P1-P2 PCA territories.

Prognosis Of Patients With Aki After Percutaneous Mechanical Thrombectomy: A Retrospective Cohort Analysis, Hongchuan H. Coville Md, Tien-Min Lee Md, Hong Liang, Paul Alfino Md, Christopher Bray Md, Matthew Calestino Md Apr 2019

Prognosis Of Patients With Aki After Percutaneous Mechanical Thrombectomy: A Retrospective Cohort Analysis, Hongchuan H. Coville Md, Tien-Min Lee Md, Hong Liang, Paul Alfino Md, Christopher Bray Md, Matthew Calestino Md

Internal Medicine

Percutaneous Mechanical Thrombectomy (PMT) is an endovascular procedure utilized in both arterial and venous thrombosis. Acute kidney injury (AKI) has been recognized as one undesired sequela of PMT. There is no study to investigate the outcomes in this specific patient population.
