Because of the enormity of the HIV-AIDS epidemic and the urgency for preventing transmission, HIV prevention programs are a high priority for careful and timely evaluations. Information on program effectiveness and efficiency is needed for decision-making about future HIV prevention priorities. General characteristics of successful HIV prevention programs, programs empirically evaluated and found to change (or not change) high-risk behaviors or in need of further empirical study, and economic evaluations of certain programs are described and summarized with attention limited to programs that have a behavioral basis. HIV prevention programs have an impact on averting or reducing risk behaviors, particularly …

This cross-sectional analysis of the 1991 CDC Youth Risk Behavior Survey explored factors associated with an early age at first sexual intercourse. Almost 18% of White males, 49% of Black males, 5% of White females and 12% of Black females were sexually active before age 13. Carrying a weapon to school, fighting, and early (< age 13) experimentation with cigarettes and alcohol were associated with early initiation of sexual activity for all four race and gender groupings. Those initiating sexual activity early had greater numbers of partners but were 50% less likely to use condoms regularly and were two-seven times more likely to have been pregnant or caused a pregnancy. Females who initiated sexual activity early were more likely to have had a sexually transmitted disease (STD). Interventions to postpone sexual activity need to be tailored to the ethnic and gender differences observed in these analyses. Interventions must begin before age 13 and should be comprehensive school-based efforts.

An immunohistochemical assay for the measurement of estrogen receptor (ER) has been evaluated on 290 consecutive human breast biopsy and mastectomy specimens in the year 1992 at The Aga Khan University Hospital laboratories. Immunohistochemical localization of estrogen receptor on frozen/paraffin section was scored in a semi-quantitative fashion incorporating both the intensity and the distribution of specific staining. Histologic grading of the tumour was performed according to Bloom’s method. In this study, 21% of the tumours were estrogen receptor negative, 15% were weak positive, 25% intermediate positive and 39% strong positive. Background: Past studies suggested an association between human papillomavirus (HPV) and cervical intraepithelial neoplasia (CIN). In 1987, University of North Carolina (UNC) Hospitals Family Practice Center clinicians were approached for a control population to study this association. Methods: One hundred fives patients attending the UNC Hospitals Neoplasia Clinic with biopsy-proven CIN 2 or 3 and 268 control patients attending the UNC Family Practice Center for a routine Papanicolaou smear were enrolled in this case-control study. Problem. Few attempts have been conducted to assess knowledge of HIV/AIDS in the adolescent population attending Seventh-day Adventist secondary schools. This present study was to investigate the HIV/AIDS-related beliefs, HIV/AIDS-related knowledge, and HIV/AIDS-related behaviors of adolescents according to gender, grade level, ethnicity, geographical location, and religious affiliation.

Method. The subjects for this study were 1,292 9th- through 12th-grade students attending selected non-boarding Seventh-day Adventist academies representing three geographical regions of the United States and the Virgin Islands. The human immunodeficiency virus (HIV) and its latter stage, the acquired immune deficiency syndrome (AIDS), are still a major health problem in the world and the full extent of the epidemic is unknown. The HIV virus in Australia has been spread mainly through homosexual contact, but this mode of transmission is decreasing while transmission from heterosexual contact and from IV drug use is increasing. The proposed mathematical model concentrates on transmission of the HIV virus through heterosexual contact and IV drug use among heterosexuals in Western Australia. Finding Hiv-Infected Women - The Clinician's Role, Mary Guinan

An estimated 100,000 women are currently infected with the human immunodeficiency virus (HIV) in the United States, and a great majority of them are unaware of their condition. Approximately 20,000 HIV-infected women were identified through publicly funded HIV screening programs in 1989 and 1990, and an unknown number through private screening. Because most HIV-infected women are believed to be in the lower socioeconomic strata, it is unlikely that a significant number were identified in the private sector. Therefore, up to 80% of HIV-positive women may not know they are infected.

The impact of various child care arrangements on the health of infants and preschool children is not known in any systematic way, yet by 1990 more than 10 million of these children may be receiving their care in day care facilities (1). Concerns over the health of these children and health practices within day care facilities have already led some States to place regulation of day care facilities under the jurisdiction of the department of health (2,3), and others are presently considering such legislation. We studied the prevalence of antibody to Herpes simplex virus types I and 2 (HSV-I and HSV-2) in 766 randomly selected Costa Rican women 25-59 years of age in a national household survey in 1984-1985. Overall, 97.1% were seropositive for HSV-l and 39.4% for HSV -2. Only 1.1% of HSV -2 seropositive women gave a history of symptomatic genital herpes. HSV-2 virus antibody increased with age and with the number of lifetime sexual partners. HSV -2 seroprevalence among women who reported only 1 lifetime sexual partner was almost twice as high as the prevalence among women who denied sexual experience …

Mathematical modeling of the AIDS epidemic can be useful for policymakers even though precise projections are not possible at this time. Models are useful in establishing ranges for current and future prevalence of HIV infection and incidence of AIDS, as well as in predicting the effect of a given intervention strategy. Most decision makers are using models implicitly when they use epidemiological information as a basis for policy; formulating a model explicitly permits examination of the underlying assumptions. By creating and testing a variety of models, an investigator can determine whether the models reflect more the underlying assumptions or the …

Recently, we analyzed the reported cases of acquired immunodeficiency syndrome (AIDS) in women in the United States.1 We found two areas of great concern: first, women are more likely than men to acquire AIDS through heterosexual intercourse, and second, women are the source of disease for about 80% of children with AIDS. In this column we will discuss the source and distribution of AIDS in women, and in a future piece we will discuss women as the source of infection for pediatric AIDS cases.

Bone loss in women accelerates immediately after menopause, often leading to osteoporosis and an increased risk of bone fractures. In 1985,247,000 hip fractures occurred in people over 45 years of age. Among those who live to be 90, one-third of women and one-sixth of men will experience hip fractures. Six months after fracture, 25% of victims will not be able to carry out their usual activities, and 50% will need assistance to do so. Hip fractures are not only a serious cause of morbidity but they also increase the risk of mortality in older people. Because the proportion of the …

More than 500 cases of acquired immune deficiency syndrome (AlDS) in children (ages I3 and under) had been reported to the Centers for Disease Control as of August , 1987. Of these children, 78% were infected perinatally by their mothers. The ratio of AIDS cases among women to AIDS cases among ; i children has remained stable at approximately 5:1 for several years. So, for the foreseeable future, we can expect that for every five reports of women with AIDS, one child with AIDS will be reported. Recent reports of 12 confirmed cases of acquired immunodeficiency syndrome (AIDS) in Costa Rica have generated interest in the extent of spread of human T-lymphotropic virus type III/LAV (HTLV-III) in Latin America (1). To date, all cases of AIDS in Costa Rica have occurred in members of previously identified, high-risk groups, chiefly hemophiliacs. To examine the possibility of heterosexual transmission in Costa Rica, we utilized sera collected in a population-based case-control study of cervical cancer, conducted by the Costa Rican Demographic Association and other institution.

There is evidence that specific nutrients may modify hormonal balance and contribute to breast cancer etiology. To investigate the relationship between dietary nutrients and plasma reproductive hormones and to determine hormone-nutrient interrelationships, we measured plasma estrogens, androgens. progesterone and prolactin levels in 10 premenopausal Seventh-day Adventist (SDA) vegetarian and 10 premenopausal SDA nonvegetarian women. Over three days in each of three consecutive months. diet records and fasting midluteal blood samples were collected. The nonvegetarians consumed significantly more protein, total and saturated fats, oleic and linoleic acids, and cholesterol than did the vegetarians. Hormonal status of the two groups did not …

Nonadherence to therapeutic regimens which necessitate major lifestyle change is recognized as one of the most serious problems in medical practice today. Attempts to prevent and control chronic disease, including the development and progression of cardiovascular disease, are only effective when recommendations to the patient are carried out. Few studies have systematically studied the relationship of psychosocial and behavioral factors to adherence outside the theoretical framework of a specific behavioral model; no study has done so with a carefully measured diet as the outcome variable in a controlled prospective clinical trial with patients suffering from cardiovascular disease.
