BACKGROUND: Many older adults with cancer have at least one comorbidity. Type II diabetes mellitus (T2DM) is a common comorbidity among patients with prostate cancer (PC) and represents an important consideration in this population. Comorbid T2DM can negatively impact cancer-specific outcomes for patients, suggesting the need for better glycemic management, especially for patients on active cancer treatment. In this article, we sought to understand the factors associated with emergency department (ED) utilization by patients after metabolic and bariatric surgery (MBS). The hopes were that modifiable risk factors could be identified to reduce ED visits after MBS.

We analyzed data from the Metabolic and Bariatric Surgery Accreditation and Quality Improvement Program database (MBSAQIP) which is comprised of data from all nationally accredited bariatric centers in the United States. Metformin-associated lactic acidosis (MALA) is a rare but life-threatening complication with mortality rates of 30-50%. Although metformin is the most widely prescribed antidiabetic medication, severe MALA requiring emergent hemodialysis remains uncommon, with an estimated incidence of 2-9 cases per 100,000 patient-years. Risk factors include acute kidney injury, dehydration, and tissue hypoxia. Early hemodialysis initiation is independently associated with improved survival. This case highlights how rapid interprofessional collaboration from the emergency department to the intensive care unit enabled in a patient presenting with profound acidemia and survival multiorgan dysfunction

Background: SGLT2 inhibitors improve cardiovascular and renal outcomes in type 2 diabetes mellitus (T2DM), but their safety in cirrhosis remains uncertain given risks of volume depletion, infection, and decompensation. We evaluated the association between SGLT2 inhibitor use and hepatic decompensation and healthcare utilization in cirrhotic patients with T2DM.

Methods: A retrospective cohort study was performed using the TriNetX multicenter electronic health record network. Background: Hypertriglyceridemia is a recognized cause of acute pancreatitis, but the prognostic impact of markedly elevated triglycerides (TG) on clinical outcomes remains unclear. We evaluated whether TG ≥500 mg/dL at presentation predicts worse pancreatitis outcomes.

Methods: Using the TriNetX multicenter electronic health record network, adults hospitalized with acute pancreatitis and a TG measurement within 48 hours of admission between 2018–2024 were identified. Patients with TG ≥500 mg/dL were compared with those with TG < 500 mg/dL after 1:1 propensity score matching on demographics and comorbidities (n=7,014 per group). Primary outcomes included ICU stay, pancreatic complications, need for procedure, and all-cause mortality. Secondary outcomes included acute renal failure, cardiovascular events, repeat pancreatitis, infection, and liver failure. Odds ratios (OR) with 95% confidence intervals were estimated.

Results: Compared with TG < 500 mg/dL, patients with TG ≥500 mg/dL had higher risks of ICU admission (25.3% vs. 17.2%; OR 1.63), pancreatic complications (5.3% vs. 4.5%; OR 1.19), and mortality (4.6% vs. 3.3%; OR 1.39). Need for procedural intervention was lower in the high-TG group (1.2% vs. 3.6%; OR 0.34). Secondary outcomes including acute renal failure (15.5% vs. 10.7%; OR 1.54), repeat pancreatitis (58.6% vs. 51.9%; OR 1.31), and infection (13.7% vs. 12.0%; OR 1.17) were all significantly elevated. Cardiovascular events and liver failure did not differ significantly.

Conclusions: TG ≥500 mg/dL in acute pancreatitis was associated with greater ICU utilization, complications, and mortality, but lower procedural intervention rates. Admission triglyceride …

SGLT2 inhibitors have emerged as a drug class with cardiorenal benefits beyond glycemic control in patients with type 2 diabetes mellitus (T2DM) and chronic kidney disease (CKD). This review compares two widely used SGLT2 inhibitors dapagliflozin and empagliflozin across efficacy, safety, and cost-effectiveness outcomes in patients with comorbid CKD and T2DM. A systematic literature search of PubMed, Scopus, and Embase yielded 10 final articles after screening. Both agents demonstrated comparable cardiorenal benefits, with dapagliflozin showing advantages in weight, blood pressure, and glycemic markers, while empagliflozin proved more cost-effective for CKD-specific disease management. Myxedema coma is an endocrinologic emergency that requires prompt recognition and treatment. It is described as severe hypothyroidism characterized by life-threatening slowing of metabolic processes, often occurring in the setting of precipitating factors such as trauma, infection, medications, or cold exposure. This case report presents a clinical case in which early suspicion of myxedema coma in an elderly female patient led to prompt treatment and life-saving measures.

Background: Chronic kidney disease (CKD) is known to influence clinical outcomes across a range of procedures, but its impact on endoscopic retrograde cholangiopancreatography (ERCP) outcomes remains poorly characterized. We evaluated the association between CKD and post-ERCP survival and procedural complications.

Methods: A retrospective cohort study was conducted using the TriNetX database. Adults with CKD stages 1–5 undergoing ERCP between 2010–2024 were identified and compared to patients without CKD undergoing the same procedure. Propensity score matching (1:1) was performed to balance known demographic and clinical confounders. Glucagon-like peptide 1 receptor agonists (GLP-1 RAs) are widely used for the management of type 2 diabetes mellitus (T2DM) and obesity, with growing evidence of their anti-inflammatory effects. We report the case of a 54-year-old woman with obesity, T2DM, and biopsy-confirmed plaque psoriasis who developed joint pain and stiffness consistent with psoriatic arthritis (PsA). Her symptoms persisted despite appropriate trials of topical corticosteroids, calcipotriene, and oral apremilast. She also declined methotrexate due to concerns about side effects. Imaging of her hands revealed distal interphalangeal joint space narrowing, marginal erosions, and periosteal reaction, supporting the diagnosis of PsA. The patient was …

Advances in Clinical Medical Research and Healthcare Delivery

Thyroid disease, particularly hypothyroidism, can significantly affect the cardiovascular system, leading to structural, functional, and conduction abnormalities. Hypothyroidism decreases inotropy and chronotropy and may cause bradycardia or conduction defects. Although these changes are typically reversible with thyroid hormone replacement, severe cases may necessitate pacemaker placement. This case describes an 82-year-old female with hypothyroidism, atrial fibrillation, and left bundle branch block (LBBB) who presented with recurrent syncope due to hypothyroidism-induced bradycardia with sinus pauses. Her symptoms resolved with thyroid hormone optimization, highlighting the importance of prompt recognition and treatment of hypothyroidism-related cardiac manifestations to prevent unnecessary pacemaker implantation.

Background: Abnormal levels of VWF (von Willebrand Factor) are a risk factor for venous thromboembolism (VTE) and bleeding. Genome-wide association studies for VWF have identified novel candidate genes that may regulate VWF levels in humans, including RAB5C (RAS-associated protein RAB5C). We hypothesized that RAB5C regulates VWF release from endothelial cells.

The reviewed literature focuses on the clinical application of subcutaneous (SQ) insulin, particularly rapid-acting insulin analogs (RAIAs), in treating selected patients with mild to moderate diabetic ketoacidosis (DKA). The use of SQ insulin offers a viable alternative to traditional intravenous (IV) insulin infusions, enabling DKA management outside intensive care unit (ICU) settings and reducing healthcare resource utilization.
