Odontogenic myxoma (OM) is a rare, benign but locally invasive intraosseous neoplasm. It is known to arise from the mesenchymal portion of the tooth germ. Although it is a slow-growing tumor, maxillary odontogenic myxomas can lead to increased morbidity caused by invasion into the maxillary sinus and middle cranial fossa. We herein present a case of a giant cell fibroma in an older adult male that clinically has a pebbly or bosselated surface architecture, mimicking a squamous papilloma. The giant cell fibroma is a fibrous tumor distinct from the common irritation fibroma and may easily be mistaken for a papilloma based on clinical features. This case is reported to provide diagnostic value to the general practitioner.

For many decades, titanium implants have achieved a remarkably high success rate. However, there are cases in which osseointegration does not occur as predicted or desired. In these cases, an analysis of the factors that led to the implant failure must be conducted. There are multiple, widely accepted potential causes of such implant failures. There have been 17 reported cases of isolated amyloid deposition (amyloidoma) of the hard palate (Table 1). We present an 18th case, with a review of the literature on amyloidosis. Systemic conditions, such as multiple myeloma, must be ruled out when amyloid deposition is present. However, localized amyloidoma of the palate is rare, and there are few cases reported in the literature.

Oral Myeloid Sarcoma: Rare Presentation Of Refractory Acute Myeloid Leukemia With Adverse Molecular Profile, Lauren A. Musial, Naomi Ramer Dds

Myeloid sarcoma is a rare extramedullary presentation of acute myeloid leukemia and is especially uncommon in the oral cavity. The presence of myeloid sarcoma may be the initial manifestation of an underlying hematolymphoid disorder or may indicate relapsing disease. We report a case of a 48-year-old male with refractory acute myeloid leukemia who presented with a painful, yellow, plaque-like ulceration on the buccal mucosa. Incisional biopsy demonstrated ulcerated squamous mucosa with a dense infiltrate of large, atypical immunoblastic cells admixed with a prominent population of morphologically atypical eosinophils. This literature review examines the potential use of peanut proteins embedded within toothpaste formulations as a novel avenue for inducing tolerance in individuals with peanut allergies. Gorlin-Goltz syndrome is a hereditary autosomal dominant disorder. It has multiple clinical manifestations caused by mutation in the patched gene (PTCH), which is responsible for the growth and development of healthy tissue and for regulating the cell cycle. HAART and PrEP have demonstrated reduction of HIV-related mortality and HIV infection, respectively. A range of orofacial side effects has been associated with both HAART and PrEP. As PrEP usage has increased, there has also been an increase in sexually transmitted infections that have orofacial symptoms. As oral health-care providers, it is important that we recognize these orofacial side effects and symptoms. Further, we offer clinical recommendations for dental professionals treating patients who are on either HAART or PrEP and their associated risk factors.

The purpose of the study was to conduct a retrospective analysis of histopathological diagnosis of all pericoronal tissues associated with impacted teeth and to correlate it with a hypothetical classification of pericoronal follicular space to improve our clinical management of treating impacted teeth. The pyogenic granuloma is a benign vascular proliferation which may present either on the skin or intraorally. While the majority of oral pyogenic granulomas remain less than 1 cm in size, they can grow to large sizes if care is neglected. Herein, we report case of a large pyogenic granuloma on the mandibular alveolar ridge gingiva which grew to 10x10cm in size due to failure to seek treatment.

Langerhans cell histiocytosis (LCH) is a destructive process that may involve both hard and soft tissues. While rare, it has a predilection for the oral and maxillofacial region. LCH occurs across the age range, yet is often diagnosed in the pediatric population. The dentigerous cyst (DC) is the most common developmental odontogenic cyst. It typically presents in young adult patients as a solitary lesion associated with impacted third molars. Here we present a case of LCH occurring in the wall of a DC in a 12-year-old male.

Peripheral ameloblastoma is a benign, painless, and slow-growing odontogenic tumor, which commonly affects gingival soft tissues or edentulous alveolar areas. Peripheral ameloblastoma is typically found during the 5th to 7th decades of life, with a mean patient age of 52 years, and exhibits a male predilection. Histopathology features are consistent with those of conventional ameloblastoma. The treatment of choice is conservative surgical excision with minimal disease-free margins and appropriate follow-up.

Sulfamethoxazole- trimethoprim (ST) is a sulfonamide derivative antibiotic that interferes with bacterial folic acid synthesis and growth by blocking dihydrofolic acid formation from para-aminobenzoic acid. One of the potential side effects of sulfamethoxazole-trimethoprim is thrombocytopenia. Thrombocytopenia is a blood disorder characterized by a decreased number of circulating platelets. We present two cases of ST induced thrombocytopenia where the oral manifestations were the first indication of the low platelet count leading to life saving interventions. Our aim is to educate dentists about the oral manifestations of thrombocytopenia and its association with ST as their examination may be instrumental in directing treatment.

Squamous odontogenic tumors (SOT) are slow-growing, benign, rare intraosseous neoplasms. SOT are typically found in the premolar-canine region of the maxilla and the molar region of the mandible. These lesions have no gender predilection and show peak occurrence in the third decade of life.

Oral Manifestations Of Metastatic Diseases: A Clinical And Histologic Analysis Of 7 Cases, Zeming Zheng Dds, Elizabeth Philipone Dmd

Metastatic diseases comprise 1-3% of malignant oral neoplasms with nearly one-third of patients being unaware of having a primary cancer1-4,9. This retrospective analysis identified all oral lesions diagnosed as metastatic disease in the CDM Department of Oral Pathology’s oral biopsy database from 2015-2020 and reported on patient demographics, clinical presentation and diagnosis, origin of the primary, and whether the patient had known history of the disease. From 17,500 total cases, 7 cases were identified as metastatic disease and described in detail. Clinicians’ recognition and awareness of these lesions’ often evasive, ambiguous oral presentations may help facilitate diagnosis, treatment, and prognosis.

Adenoid ameloblastoma with dentinoid (AAD) is a rare benign neoplasm of odontogenic epithelium, with under 50 cases having been reported in the literature. AAD shares histopathological characteristics with both ameloblastomas and adenomatoid odontogenic tumors. They are unique because of dentinoid deposition without enamel. We present a case of AAD in the maxilla of a 13 year old. The patient was treated with left infrastructure maxillectomy and left partial infratemporal fossa dissection. This case reports on an extremely rare disease process in an uncommon location in a pediatric patient.

The aim of the present study is to determine the prevalence and demographics of temporomandibular joint (TMJ) condylar changes in the asymptomatic general population of a hospital based Oral and Maxillofacial Surgery (OMFS) practice. We retrospectively reviewed cone beam computed tomography (CBCT) scans of 272 TMJs from 136 consecutive patients to detect osteophyte, subchondral cyst, sclerosis, cortical erosions, and flattening. While gender did not appear to play a significant role in TMJ pathology, age was positively correlated with prevalence of condylar changes. Osteomas are frequently reported in the maxillofacial region with much lower incidences in the maxillary and sphenoid sinuses. An unerupted third molar within the maxillary sinus coexisting with a maxillary sinus osteoma is an extremely rare pathologic finding. Such cases can be managed endoscopically or intraorally through a Caldwell-Luc approach. The following case report deals with a patient who presents with an uncommon pathological finding: an osteoma embedding an ectopic tooth within the maxillary sinus.

Individuals with symptomatic third molar pathologies frequently seek dental care at emergency departments or urgent dental care centers. This is becoming a dental public health problem, when repeated dental emergency appointments are sought for treatment of symptomatic contralateral wisdom teeth. Body: We designed a retrospective review study of adults 18 years and older with an initial dental emergency visit related to third molar pathology at a single dental urgent care center, at the University of Rochester. Conclusion: A sub-optimal oral health is a predictor for future emergency dental visits, due to pathology of remaining third molars.

Diagnosis And Management Of Nasopalatine Duct Cyst: A Report Of Two Cases And Literature Review, Feiyi Sun Dds, Harry Dym Dds

The nasopalatine duct cyst (NPDC) is one of the most common non-odontogenic, non-neoplastic developmental cysts. It arises from the epithelial remnants of the nasopalatine duct within the incisive canal and crosses the midline of the anterior maxilla. Most cases are asymptomatic and either are discovered by chance on radiographs or present as a soft tissue swelling, pain, or drainage in the palate. The present study describes the diagnosis and surgical management of two NPDC cases at the Department of Oral and Maxillofacial Surgery at the Brooklyn Hospital Center.

BACKGROUND: The investigators aim to determine the frequency of systemic medications in patients with oral lichen planus or oral lichenoid lesions. METHODS: A retrospective study of lichenoid diagnoses in the New York University College of Dentistry Oral Medicine Clinic. Data obtained included patient demographics, prescription medications and pathologic diagnosis. Comparative analysis was performed. RESULTS: A total of 162 oral lichenoid lesion/oral lichen planus patients were identified: The most frequently found medications were anti-hypertensives, anti-hyperlipidemics followed by metformin, proton pump inhibitors and levothyroxine. CONCLUSION: The findings emphasize the importance of clinicians’ awareness of potential oral adverse reactions from certain medications.

Amelogenesis Imperfecta: Case Series And Review Of The Literature, Andrew T. Moshman Dmd, Gwen Cohen-Brown Dds, Faaomp

Amelogenesis imperfecta (AI) describes a group of genetic conditions affecting the thickness, microstructure, and degree of mineralization of the enamel. Correct diagnosis may be important for managing other serious medical conditions. AI patients are highly susceptible to caries, pain, tooth wear, and loss of the vertical dimension of occlusion, often requiring significant dental treatment throughout their life. Early diagnosis and timely preventative and restorative treatments are crucial to circumvent AI’s psychological, social, and functional sequalae. This case series highlights AI in a 19-year-old patient, with suggestions for further testing and monitoring of the condition.
