Instability of the occlusion is a common problem following orthodontic treatment. The purpose of the present research was to evaluate posttreatment relapse at long-term recall (> 10 years) in cases (A) treated with temporary fixed mandibular retention plus Hawley retainers compared to (B) a similar group retained with just removable retention (standard Hawley type retainers alone). Fixed retainers were removed after having been in place for about 2.5 years. Following this period, use of removable retention was left up to the discretion of the patient in both groups. Defensins are cationic (positive-charged) peptides with broad-spectrum antibiotic activity. In humans, there are two types of defensins, alpha (α) and beta (β). Human neutrophils contain four α-defensins known as Human Neutrophil Peptide (HNP) 1-4. Epithelial cells produce four β-defensins known as Human Beta Defensin (HBD) 1-4. Gram-negative anaerobic bacteria that are associated with periodontal disease are resistant to human α-defensins, but are killed by β-defensins.
HBD-3 is the most active β-defensin. HBD-3 is a longer peptide than HNP 1-4. HBD-3 has additional amino acid residues with hydrophobic side chains near the N-terminus and residues with cationic side chains at the …

Craniofacial Changes Following Nonextraction Orthodontic Treatment: A Long-Term Cephalometric Analysis, James Austin Rahaim

Long-term posttreatment cephalometric changes from late adolescence into early adulthood were analyzed in this study. Lateral cephalometric radiographs from a sample of 30 Class II division 1 Caucasian females treated without extractions were evaluated at posttreatment (mean age = 15.9 years) and recall (mean age = 28.3 years). All of the subjects were treated in the private practice of a single, experienced practitioner. The cephalograms were examined to investigate changes in the cranial base, midface, maxilla, mandible, maxillomandibular relationships, dental relationships, and the soft tissue profile that occurred at an average of 12.4 years posttreatment. Background: Dental caries is the most common chronic disease of children ages 6 to 11. According to the U.S. Department of Health and Human Services, 41% of the children in this age group experience decay in their primary teeth. Since 1980 the number of overweight children ages 6 to 11 has doubled. Childhood obesity is currently the most prevalent nutritional condition of children in the U.S.
Purpose: The purpose of this observational study was to determine if there is an association between childhood dental caries and childhood obesity. The study also explored the effects of diet and other covariates.

The vast majority of periodontal pathogens are classified as gram negative bacteria. These bacteria have an outermost membrane consisting of lipopolysaccharide (LPS), a molecule that can act as an endotoxin and elicit an immune response. The lipopolysaccharide structure consists of three major regions: oligosaccharide, core, and lipid A. Oligosaccharide is exposed on the cell surface, and lipid A anchors LPS in the outer membrane. LPS has been known to hinder certain defensive mechanisms of the immune system by secreting products that interfere with signals that promote chemotaxis of leukocytes enabling the pathogens to go unrestricted. Achieving long-term stability after orthodontic treatment is a major challenge for orthodontists. The previous literature shows that relapse occurs at a considerable magnitude. Dental caries is present worldwide and has afflicted over 90% of all adults in the United States. Biological determinants of the disease are well understood but social determinants and how they interact with biological factors are not. Programs designed to reduce incidence and prevalence of caries often target certain individuals and involve behavioral change. Some programs are more successful when environmental and societal issues are also addressed (Watt, 2002). The use of cone-beam computed tomography (CBCT) technology has been shown to be more accurate in measuring individual incisor tooth widths than the use of wax exemplars. There were fewer differences by investigators using CBCT than others using an F-test in a mixed model of the measurement differences of investigators, wax type, and which tooth was measured. In addition, the frequency of outliers was less in the CBCT method (a total of 5) as compared to the two-dimensional measurements in ether Aluwax (a total of 8) or Coprwax (a total of 12). Background/Aim: Experimental studies have identified differences in the effect of physicochemical properties of beverages on the etiology of erosive tooth wear (ETW). Little is known from epidemiological studies about the relationship between ETW and consumption of juices, drinks and milk. This study examined the relationship between the consumption of juices, drinks, milk and ETW in children in the United States. Methods: The National Health and Nutrition Examinations Survey data for 2003–2004 was analyzed. Trained and calibrated examiners used the modified Smith and Knight Tooth Wear Index from a 1998 United Kingdom Adult Health Survey to measure ETW. Beverage consumption collected …

Osteomas are benign tumors composed of mature compact or cancellous bone. They represent an uncommon lesion that occurs mainly in craniofacial complex bones. In jaws, they can appear on the bone surface as a polypoid or sessile mass, characterizing a peripheral osteoma (PO), or can be a lesion in the medullar space, then it is called central osteoma. In view of the scarcely reported cases about POs, this article presents a case of PO of the maxillofacial area that was surgically resected using hemicoronal approach.

Purpose: The specific aim of this study is to evaluate the trends in dental health care for individuals with ectodermal dysplasia. Methods: This was a cross sectional analysis of subjects recruited through the National Foundation of Ectodermal Dysplasia (NFED). From 1997 to 2000, individuals with ectodermal dysplasia or their caregiver (if the individuals were too young to selfreport) voluntarily completed questionnaires. The questionnaire consisted of 37 items consisting of demographics, ectodermal dysplasia diagnosis, access to dental care, level of dental utilization, and type of dental services received. Purpose: The purpose was to examine the maternal influences on the development of infant oral biofim and dominant bacterial strains of at risk populations. Methods: The study used a cross-sectional design to examine factors influencing biofilm colonization and the identification of bacterial strains transmitted from mother to child. Participants were enrolled in Children’s Health Involving Parents of Greater Richmond (CHIP). Plaque and saliva samples were collected from mothers and their children ages 6-36 months. Purpose: To assess the effect of dental Medicaid reforms in the state of Virginia on dental plan performance. Methods: This project is a retrospective cohort study of 825,000 dental claims obtained from the Virginia Department of Medical Assistance Services. This cohort includes dental claims for children enrolled in Virginia’s Medicaid program from July 1, 2002 through June 30, 2008. The independent variable was enrollment pre or post policy reform with July 1, 2005 as the period dividing date. Teeth are brittle and highly susceptible to cracking. We propose that observations of such cracking can be used as a diagnostic tool for predicting bite force and inferring tooth function in living and fossil mammals. Laboratory tests on model tooth structures and extracted human teeth in simulated biting identify the principal fracture modes in enamel. Examination of museum specimens reveals the presence of similar fractures in a wide range of vertebrates, suggesting that cracks extended during ingestion or mastication. The use of ‘fracture mechanics’ from materials engineering provides elegant relations for quantifying critical bite forces in terms of characteristic tooth …

Purpose: The purpose of this pilot study was to determine if there is a difference in post-operative pain experience for children following dental restorations and/or extractions under general anesthesia (GA), with and without local anesthetic (LA). The alternative hypothesis is that children will experience less post-operative discomfort and soft tissue trauma when using intra-ligamental local anesthetic during the intra-operative time period. Those with physical and mental disabilities are at an increased risk of oral complications such as gingivitis and periodontal disease. In comparison with this risk, this population struggles to access appropriate dental care that accommodates those with disabilities. This thesis reviews the demographics of those with special needs, dental education barriers to working with this population and financial barriers that hinder access to care. Our objective was to compare frictional resistance evident in aesthetic archwires to traditional (non-aesthetic) archwires. Methods: Archwires ligated with elasatics to fixed brackets were pulled through these brackets while frictional resistance (in lbf) was measured. Results: There were no confirmed significant differences between the frictional resistance of the aesthetic arch wires compared to the traditional non-coated wires for all wire sizes tested Conclusions: Our data suggests that a sacrifice of clinical performance with these aesthetic archwires as compared to traditional archwires is not likely

On January 7th, 2011 the U.S. DHHS recommended setting the level of fluoride in drinking water to the lowest end of the current optimal range (0.7 to 1.2 ppm). EPA is initiating a review of the maximum amount of fluoride allowed in drinking water based on scientific assessments of health effects from too much fluoride. MCL (Maximum Contaminant Level) is the legal threshold limit on the amount of a substance allowed in public water systems. EPA sets the MCL to address adverse health effects. Current MCL for fluoride is 4 mg/L. The EPA is unsure if they will revise the …
A Comparative Study Of Rotary Instrumentation Of The Maxillary First Premolar Buccal Root Utilizing Cone Beam Computed Tomography, Stephan Zigo

The study objective was to determine cementum-dentin wall thickness along the furcation groove in maxillary bifurcated first premolars after preparation with three successively larger, 0.04 tapered, nickel titanium rotary files. Pre-instrumentation and post-instrumentation imaging was accomplished utilizing Cone Beam Computed Tomography. All data was analyzed using an ANOVA. Instrumentation resulted in a significant reduction in dentin-cementum wall thickness (p < .001). At mid-groove, predicted dentin-cementum wall thickness (95% CI) was equal to or less than the proposed standard (0.50 mm) for apical file sizes 30, 35, and 40 respectively. Instrumentation of the mid-groove in maxillary first premolars reduces dentin-cementum wall thickness to levels that may be insufficient to ensure tooth integrity.

The purpose of this study was to determine the number of rotations to fracture (cyclic fatigue) of the Profile® Vortex™ files (Dentsply Tulsa Dental Specialties, Tulsa, OK) compared to the EndoSequence™ files (Brasseler USA, Savannah, GA) using an in-vitro apparatus simulating a curved canal. Two hundred Profile® Vortex™ files of 25mm length were divided equally into ten groups, one for each of the Profile® Vortex™ files 20/0.04, 20/0.06, 25/0.04, 25/0.06, 30/0.04, 30/0.06, 35/0.04, 35/0.06, 40/0.04, and 40/0.06. The author reflects on the need of legal counsel for the dental students before signing any employment contract in a private dental practice firm. He states that the counsel can save both the students and owners from the problems which he called "mines". He further elaborate these "mines" which include lack of revenue or patients at the work place, compensation scheme causing too much profit for the owner and little or no discussion of the practice's value with the new associate.

Purpose: The purpose of this study was to determine policies and practices regarding magnification loupes among accredited dental hygiene programs as measured by a 31- item, self-designed questionnaire titled Magnifying Loupes in U.S. Entry Level Dental Hygiene Programs. In addition, the study compared policies among dental hygiene programs in two year versus four year programs in terms of requirements for the use of magnification loupes.
Methods: After institutional review board approval, the questionnaire was emailed via Survey Monkey to 303 entry-level dental hygiene programs. An overall response rate of 75% was obtained. Data were analyzed using descriptive statistics and …

Introduction: The polymerization and acid<–>base reactions in resin<–>modified glass<–>ionomers (RMGI) are thought to compete with and inhibit one another. The objective of this study was to examine the effect of visible light<–>cure (VLC) delay on the polymerization efficiency and orthodontic bond strength of a dual<–>cured RMGI.
Methods: An RMGI light<–>cured immediately, 2.5, 5, or 10 minutes after mixing comprised the experimental groups. Isothermal and dynamic temperature scan differential scanning calorimetry (DSC) analysis of the RMGI was performed to determine extents of VLC polymerization and acid<–>base reaction exotherms.
