Distinguishing adenoid cystic carcinoma from polymorphous low-grade adenocarcinoma of the salivary glands is important for their management. We studied the expression of several myoepithelial and basal/stem cell markers (smooth muscle actin, calponin, smooth muscle myosin heavy chain, metallothionein, maspin, and p63) by immunohistochemistry in 23 adenoid cystic carcinoma and 24 polymorphous low-grade adenocarcinoma, to identify the most useful marker or combination of markers that may help their diagnoses. The results were analyzed using hierarchical cluster analysis and χ² test for trend. Since 1975, the Hmong refugee population in the U.S. has increased over 200%. However, little is known about their dental needs or self-rated oral health (SROH). The study aims were to: (1) describe the SROH, self-rated general health (SRGH), and use of dental/physician services; and (2) identify the factors associated with SROH among Hmong adults. A cross-sectional study design with locating sampling methodology was used. Oral health questionnaire was administered to assess SROH and SRGH, past dental and physician visits, and language preference. New molecules were designed for efficient intra-oral delivery of antimicrobials to prevent and treat oral infection. The salivary statherin fragment, which has high affinity for the tooth enamel, was used as a carrier peptide. This was linked through the side chain of the N-terminal residue to the C-terminus of a defensin-like 12-residue peptide to generate two bifunctional hybrid molecules, one with an ester linkage and the other with an anhydride bond between the carrier and the antimicrobial components. The aim of the present investigation is to purify, characterize, and determine the chemical and biological properties of Porphyromonas gingivalis free dihydroceramides (fDHC). Effects were tested by examination of human gingival fibroblast morphologic changes in addition to their synthesis of prostaglandin E2 (PGE2) and prostaglandin F2α (PGF2&#;) produced in vitro. Free dihydroceramides as well as phosphoglycerol dihydroceramide (PGDHC), and de-esterified phosphoglycerol dihydroceramide (PGDHC-C15:0) were obtained by total lipid extraction of P. gingivalis followed by high performance liquid chromatography (HPLC). A cantilever and three point bend model were used to assess degree of bracket play, deactivation force, and superelastic range. Oral mucositis refers to erythematous and ulcerative lesions of the oral mucosa observed in patients with cancer being treated with chemotherapy, and/or with radiation therapy to fields involving the oral cavity. Lesions of oral mucositis are often very painful and compromise nutrition and oral hygiene as well as increase risk for local and systemic infection. Mucositis can also involve other areas of the alimentary tract; for example, gastrointestinal (GI) mucositis can manifest as diarrhea. Thus, mucositis is a highly significant and sometimes dose-limiting complication of cancer therapy.

Lack of curricular time, faculty time, and funding are potential limitations for communication skills training in dentistry. Interdisciplinary collaboration amongst health care faculties could address these limitations. This article describes the development, implementation, and student perceptions of a communication skills program in dentistry. The program has four components: Knowledge, Observation, Simulation, and Experience (KOSE) and spans over the second and third years of dental school. KOSE allows students to obtain knowledge of and observe effective communication skills and practice these skills in the simulated and nonsimulated environment. วัตถุประสงค์ เพื่อทําความเข้าใจ ความเชื่อและพฤติกรรมการบริโภคอาหารของชาวบ้านที่เกี่ยวข้องกับโรคฟันผุ ในเด็กปฐมวัย วัสดุและวิธีการ การวิจัยเชิงคุณภาพ โดยใช้การสังเกตแบบมีส่วนร่วม การสัมภาษณ์อย่างไม่เป็นทางการ และ การสัมภาษณ์เจาะลึก จากผู้ให้ข้อมูลสําคัญ 36 ราย ผลการศึกษา ผู้เลี้ยงดูเด็กส่วนใหญ่จะเลี้ยงเด็กด้วยนมแม่ ไม่นิยมใช้ขวดนม มีการเคี้ยวข้าวเหนียวป้อนเด็กและ ใช้อาหารเสริมซีรีแลค ชาวบ้านเชื่อว่าการบริโภคขนมเป็นเรื่องธรรมดาของเด็ก และเฉพาะลูกอมเท่านั้นที่ทําให้ ฟันผุ พฤติกรรมเหล่านี้มีที่มาจากบรรพบุรุษ สื่อโทรทัศน์ การบอกต่อกันของชาวบ้าน และจากคําแนะนําของ เจ้าหน้าที่สาธารณสุข พฤติกรรมการเลี้ยงเด็กด้วยนมแม่ ไม่นิยมใช้ขวดนม ควรสนับสนุนให้คงอยู่ ความเชื่อว่า วิธีการให้นมไม่เป็นสาเหตุของฟันผุ การบริโภคขนมเป็นเรื่องธรรมดาของเด็ก ลูกอมเท่านั้นที่ทําให้ฟันผุ และ พฤติกรรมการเคี้ยวข้าวเหนียวป้อนเด็ก ควรได้รับการปรับเปลี่ยน ส่วนพฤติกรรมการใช้อาหารเสริมซีรีแลค ซึ่งไม่ ทราบผลที่มีต่อสุขภาพช่องปากอย่างชัดเจน ควรทําการศึกษาต่อ เพื่อหาข้อสรุปและใช้เป็นแนวทางการให้คําแนะนําในวงกว้าง สรุป การศึกษานี้ทําให้เกิดความเข้าใจถึงระบบคิดของชาวบ้าน สามารถใช้เป็นแนวทางในการกําหนดมาตรการที่ 4 ซึ่งสอดคล้องกับวิธีคิดและวิถีชีวิตของชาวบ้าน อันจะนําไปสู่การมีสุขภาพช่องปากที่ดีของเด็กต่อไปเหมาะสม (ว ทันต จุฬาฯ 2551;31:69-86)

Objective Sporicidal activity is a good indicator for testing the efficacy of chemical disinfectants in infection control. We tested the sporicidal effect of common germicides used in Thailand. Materials and methods The spore-forming bacteria Bacillus atropheas and Geobacillus stearothermophilus were chosen to test four commercial chemical agents: sodium hypochlorite, 2% glutaraldehyde, 35% hydrogen peroxide, and iodophore. Staphylococcus aureus, Pseudomonas aeruginosa, and Salmonella typhi were also used to ensure compliance with EPA (Environmental Protection Agency) and AOAC (Association of Analytical Communities) laboratory standards for hospital disinfectants. วัตถุประสงค์ เพื่อเปรียบเทียบระดับความเข้มข้นของไอระเหยของสารประกอบซัลเฟอร์ในผู้ป่วยจัดฟันไทย ก่อนและหลังการติดเครื่องมือชนิดติดแน่น วัสดุและวิธีการ ผู้ป่วยไทยที่มารับบริการจากคลินิกภาควิชาทันตกรรมจัดฟัน คณะทันตแพทยศาสตร์ จุฬาลงกรณ์ มหาวิทยาลัย จํานวน 46 ราย (เพศชาย 16 ราย เพศหญิง 30 ราย อายุเฉลี่ย 18.5 + 5.3 ปี) ได้รับการวัดระดับ ความเข้มข้นของไอระเหยของสารประกอบซัลเฟอร์ในช่องปาก ซึ่งได้แก่ ไฮโดรเจนซัลไฟด์ เมทิลเมอแคปแทน และไดเมทิลซัลไฟด์ ก่อนและหลังการติดเครื่องมือชนิดติดแน่นไปแล้ว 4.5 + 0.7 เดือน ด้วยเครื่องตรวจวัดกลิ่นปาก ยี่ห้อออริลโครมา เปรียบเทียบระดับความเข้มข้นของไอระเหยของสารประกอบซัลเฟอร์ก่อนและหลังการติดเครื่องมือ โดยใช้การทดสอบเชิงเครื่องหมายและลําดับที่แบบวิลคอกชัน ที่ระดับนัยสําคัญ .05 ผลการศึกษา ภายหลังการติดเครื่องมือชนิดติดแน่น ระดับความเข้มข้นของไดเมทิลซัลไฟด์และไอระเหยของ สารประกอบซัลเฟอร์รวม มีค่าสูงขึ้นอย่างมีนัยสําคัญทางสถิติ (ค่าพี่เท่ากับ .019 และ 024 ตามลําดับ) แม้ว่า ไฮโดรเจนซัลไฟด์และเมทิลเมอแคปแทนมีค่าสูงขึ้น แต่ไม่แตกต่างอย่างมีนัยสําคัญทางสถิติ (ค่าพี่เท่ากับ 102และ .342 ตามลําดับ) สรุป ระดับความเข้มข้นของไดเมทิลซัลไฟด์และไอระเหยของสารประกอบซัลเฟอร์รวม เพิ่มขึ้นหลังการติดเครื่องมือจัดฟันชนิดติดแน่น (ว ทันต จุฬาฯ 2551;31:33-42)

การรักษาฟันหลุดจากเบ้า: รายงานผู้ป่วย 2 ราย, ชเนตตี พันธกิจไพบูลย์

รายงานการรักษาฟันหลุดจากเบ้า ในผู้ป่วย 2 ราย หลังจากติดตามผลการรักษาเป็นเวลา 3 ปี ให้ผลการ รักษาที่แตกต่างกัน ในเด็กหญิงอายุ 14 ปีให้ผลการรักษาเป็นที่น่าพอใจมากกว่า เนื่องจากมีระยะเวลาฟันแห้ง นอกปากประมาณ 25 นาที และได้รับการแช่ในน้ํานมอีก 30 นาที ก่อนปลูกฟันคืน สําหรับการรักษาฟันหลุดใน เด็กหญิงอายุ 9 ขวบ ซึ่งปลายรากฟันเปิดเล็กน้อย ได้เกิดการละลายของรากฟันและมีการแทนที่ด้วยกระดูก และมี ฟันงอกต่ํากว่าระดับการสบฟัน 1.5 มิลลิเมตร ฟันซี่นี้มีระยะเวลาฟันแห้งนอกปากนาน 40 นาที ก่อนได้รับการแช่ ในน้ําประปาอีก 20 นาที หลังจากนั้นจึงได้รับการแช่ในน้ํานมอีก 15 นาทีก่อนปลูกฟันคืน ผู้ป่วยทั้งสองรายได้รับ การรักษาคลองรากฟันโดยการใส่แคลเซียมไฮดรอกไซด์ไประยะหนึ่ง จนการละลายของรากฟันหยุด จึงได้รับการ อุดคลองรากด้วยกัตตาเปอร์ชาและซิงค์ออกไซด์ยูจินอลรูทคะแนลซีลเลอร์ โดยวิธีแลทเทอราลคอนเดนเซชัน (ว ทันต จุฬาฯ 2551:31:103-14)

วัตถุประสงค์ เพื่อออกแบบเครื่องสํารวจทางทันตกรรม ที่มีส่วนประกอบน้อยชิ้น และมีค่าใช้จ่ายการผลิตต่ําเพื่อเหมาะสมกับพื้นฐานเศรษฐกิจของประเทศไทย วัสดุและวิธีการ ศึกษาผลิตภัณฑ์นําเข้าเพื่อใช้เป็นตัวอย่างเปรียบเทียบ จากนั้นสังเคราะห์แบบและสร้างเครื่องกล โดยใช้วัสดุที่มีการขึ้นรูปมาก่อนประกอบกับปรัชญาการออกแบบที่ทําให้การผลิต และทํางานง่ายขึ้น การลด จํานวนชิ้นส่วนและลดชิ้นส่วนที่ต้องสร้างเป็นการจําเพาะทําให้ค่าใช้จ่ายการผลิตลดลงมาก ผลการศึกษา แบบที่นําเสนอสําหรับใช้เพื่อการเรียนการสอนและการใช้ข้างเก้าอี้ทําฟันนี้ เป็นการประยุกต์ใช้โครงสร้างที่มีพอลิเมอร์เป็นส่วนประกอบหลัก ราคาขายต่ํากว่าผลิตภัณฑ์นําเข้าที่ใช้งานอย่างเดียวกันประมาณสิบห้าเท่า สรุป นวัตกรรมการออกแบบเป็นการผลิตเครื่องมือกลทางทันตกรรมที่ชิ้นส่วนต่าง ๆ ส่วนใหญ่หาได้ในประเทศไทย การประดิษฐ์นี้เป็นหนึ่งในความพยายามอย่างยากลําบาก ที่ต่อกรกับการนําเข้าผลิตภัณฑ์ และอาจช่วยลด รายจ่ายของชาติที่ใช้ซื้อเครื่องมือที่มีเทคโนโลยีปานกลาง การพัฒนาผลิตภัณฑ์ และการส่งเสริมประชาสัมพันธ์ น่าจะเป็นการกระตุ้นนักประดิษฐ์และผู้ใช้ ให้ประจักษ์ต่อค่าของงานใหม่ที่แตกต่าง อันทรงคุณต่อการพึ่งพาตนเองเพื่อมาตุภูมิ (ว ทันต จุฬาฯ 2551;31:1-10)

รายงานผู้ป่วยชายไทย อายุ 42 ปี มาโรงพยาบาลด้วยสาเหตุพบรูเปิดของหนองบริเวณใต้คางด้านซ้าย จากการวินิจฉัยพบว่าฟันตัดข้างซ้ายล่าง มีโพรงหนองรอบปลายรากชนิดเรื้อรัง จึงได้รับการรักษาคลองรากฟัน แบบเสร็จในหลายครั้ง จากการติดตามผลหลังการรักษาเป็นเวลา 6 เดือน ได้ผลดีเป็นที่น่าพอใจ (ว ทันต จุฬาฯ 2551:31:115-24)

วัตถุประสงค์ เพื่อศึกษาผลการตอบสนองของสโตรมัลเซลล์จากไขกระดูกมนุษย์ต่อออสทิโอพอนหินและคอลลาเจนในแง่ของการเหนี่ยวนําการเปลี่ยนแปลงจํานวนเซลล์ และการยึดเกาะของเซลล์ที่เพาะเลี้ยงในห้องปฏิบัติการ วัสดุและวิธีการ ทําการศึกษาโดยการเพาะเลี้ยงสโตรมัลเซลล์จากไขกระดูกมนุษย์ร่วมกับคอลลาเจนที่สกัดจากผิวหนังวัวและรีคอมบิแนนท์ออสทิโอพอนทินของหนู จากนั้นศึกษาการเพิ่มจํานวนเซลล์และความเป็นพิษต่อเซลล์ ด้วยวิธีสอบวิเคราะห์เอ็มทีทีในเซลล์ที่เพาะเลี้ยงให้สัมผัสกับพื้นผิว 4 สภาวะ ได้แก่ พื้นผิวที่ไม่ได้รับการฉาบ สารละลายใด ๆ พื้นผิวที่ฉาบด้วยสารละลายคอลลาเจน ด้วยสารละลายออสทิโอพอนทิน และด้วยสารละลาย คอลลาเจนผสมออสทิโอพอนหิน และศึกษาลักษณะการยึดเกาะของเซลล์ด้วยกล้องจุลทรรศน์อิเล็กตรอนชนิดส่องกราด ทง 4 สภาวะ ผลการศึกษา สารละลายคอลลาเจนและสารละลายคอลลาเจนผสมออสทิโอพอนทินกระตุ้นการเพิ่มจํานวน สโตรมัลเซลล์ โดยเพิ่มขึ้นเป็นร้อยละ 106.52 + 4.08 และ 114.78 + 6.82 ตามลําดับ แต่สารละลายออสทิโอพอนทินมีผลลดจํานวนเซลล์อย่างมีนัยสําคัญทางสถิติ โดยลดลงเหลือร้อยละ 53.48 1 12.20 เมื่อเปรียบเทียบกับกลุ่มควบคุม การศึกษาด้วยกล้องจุลทรรศน์อิเล็กตรอนชนิดส่องกราด พบว่าในกลุ่มควบคุมและกลุ่มที่ฉาบด้วยสารละลาย คอลลาเจนผสมออสทิโอพอนทิน เซลล์มีการแผ่ตัวและยึดเกาะดี ในขณะที่กลุ่มที่ฉาบด้วยด้วยสารละลายคอลลาเจน สารละลายออสทิโอพอนทิน พบว่าเซลล์มีการยึดเกาะดี แต่แผ่ตัวน้อยกว่าใน 2 กลุ่มแรก สรุป สารละลายคอลลาเจนมีการกระตุ้นให้เกิดการเพิ่มจํานวนและการยึดเกาะของสโตรมัลเซลล์จากไขกระดูก มนุษย์ได้ดีโดยสารละลายคอลลาเจนผสมออสที่โอพอนหินให้ผลดีที่สุด ผลจากการศึกษานี้แสดงให้เห็นว่าคอลลาเจนและออสทิโอพอนทินให้ผลดีต่อสโตรมัลเซลล์ และอาจมีประโยชน์ในการนํามาใช้เพื่อทําให้เกิดการงอกใหม่ของแผลกระดูก (ว ทันต จุฬาฯ 2551;31:19-32)

To assess those knowledge, skill and attitude among Chulalongkorn dental graduates (years 2003-2005) and compare levels of knowledge, skill and attitude between graduates with different characteristics. Objective This study aims to determine the prevalence of dens evaginatus and apical periodontitis in dens evaginatus in a group of Thai schoolchildren. Materials and methods In carrying out the study, a total number of 9,279 schoolchildren, aged between 9-18, from 18 schools in Muang District, Kanchanaburi Province were examined, preliminary at their schools. การวางแผนเลือกพื้นที่ปฏิบัติงานในกรอบเวลาระยะสั้นและระยะยาวของนิสิตทันตแพทย์ จุฬาลงกรณ์มหาวิทยาลัย, ชาญชัย ให้สงวน

วัตถุประสงค์ การวิจัยนี้มีวัตถุประสงค์เพื่อเปรียบเทียบแบบแผนการเลือกพื้นที่ปฏิบัติงานในกรอบเวลาระยะสั้น และระยะยาวของนิสิตทันตแพทย์ จุฬาลงกรณ์มหาวิทยาลัย และระบุปัจจัยที่มีความสัมพันธ์กับแผนการเปลี่ยน พื้นที่ปฏิบัติงาน วัสดุและวิธีการ กลุ่มตัวอย่างเป็นนิสิตทันตแพทย์ จุฬาลงกรณ์มหาวิทยาลัย ชั้นปีที่ 1 3 และ 6 จํานวน 258 คน ใช้แบบสอบถามชนิดตอบด้วยตนเองเก็บข้อมูลเกี่ยวกับแผนการเลือกพื้นที่ปฏิบัติงานในกรอบเวลาระยะสั้นและระยะยาว รวมทั้งข้อมูลที่เกี่ยวข้อง ใช้สถิติถดถอยโลจิสติกแบบขั้นตอนเพื่อระบุปัจจัยทํานายที่มีความสัมพันธ์กับ การเปลี่ยนพื้นที่ปฏิบัติงาน ผลการศึกษา นิสิตทันตแพทย์ จุฬาลงกรณ์มหาวิทยาลัย ส่วนใหญ่ร้อยละ 94.8 คิดจะปฏิบัติงานใช้ทุน แต่มี เพียงร้อยละ 34.8 ที่คิดจะอยู่ต่อในระบบราชการหลังใช้ทุนครบ 3 ปี นิสิตร้อยละ 91.6 วางแผนปฏิบัติงานใน กรอบเวลาระยะสั้นอยู่นอกพื้นที่กรุงเทพมหานครและปริมณฑล แต่ในกรอบเวลาระยะยาว นิสิตส่วนใหญ่ร้อยละ 65.7 เลือกที่จะปฏิบัติงานในเขตกรุงเทพมหานครและปริมณฑล เมื่อเปรียบเทียบระหว่าง 2 กรอบเวลา มีนิสิตร้อยละ 57.0 ที่แสดงความคงตัวของพื้นที่ปฏิบัติงาน และร้อยละ 43.0 แสดงแผนการเปลี่ยนแปลงพื้นที่ปฏิบัติงาน ผลการวิเคราะห์ถดถอยโลจิสติก พบว่ามี 2 ปัจจัยทํานายที่มีความสัมพันธ์กับแผนการเปลี่ยนแปลงพื้นที่ปฏิบัติงาน คือ การมีภูมิลําเนาในกรุงเทพมหานครและปริมณฑล และการวางแผนปฏิบัติงานระยะยาวอยู่ในภาคเอกชน สรุป การเลือกพื้นที่ปฏิบัติงานในกรอบเวลาระยะยาวมีแบบกระส่วนต่างจากกรอบเวลาระยะสั้นโดยนิสิตทันตแพทย์ส่วนใหญ่แสดงถึงความตั้งใจที่จะโยกย้ายเข้ามาปฏิบัติงานในกรุงเทพมหานครและปริมณฑล อันอาจสะท้อนให้ เห็นถึงการปรับตัวในระระยาวเพื่อคลี่คลายสภาวะการพลัดถิ่นฐานทางสังคม (2 ทันต จุฬาฯ 2551;31:87-102)
