ฟันคุดในฟันกรามแท้ล่างซี่ที่สองพบได้ไม่บ่อยแต่เมื่อตรวจพบมักมีปัญหายุ่งยากในการรักษาซึ่งสิ่งสําคัญคือการวินิจฉัยและการรักษาให้เร็วที่สุดเท่าที่จะทําได้ เพราะการรักษาในระยะที่ช้าเกินไปมักจะมีความยุ่งยากจากการ สบฟันที่ผิดปกติทําให้ต้องใช้เวลานานในการรักษา ทั้งยังอาจทําให้ฟันที่เหลืออยู่มีประสิทธิภาพในการบดเคี้ยวลดลงรายงานนี้กล่าวถึงผู้ป่วยหนึ่งรายที่มีฟันคุดในฟันกรามแท้ล่างซ้ายซี่ที่สองซึ่งได้รับการรักษาโดยการผ่าตัดนําฟันคุดออกเพื่อให้ฟันกรามแท้ล่างที่สามขึ้นมาแทนที่และได้ผลเป็นที่น่าพอใจ (2 ทันต จุฬาฯ 2550;30:51-60)

ฟันผุแอบแฝงบนด้านบดเคี้ยวของฟันกรามแท้ใช้อธิบายถึงฟันผุที่พบในชั้นเนื้อฟันซึ่งด้านบดเคี้ยวถูก วินิจฉัยทางคลินิกว่าไม่เป็นฟันผุหรือมีรอยผุปริมาณน้อย ๆ เนื่องจากด้านบดเคี้ยวของฟันมีลักษณะผุเพียงเล็กน้อยไม่เป็นโพรงฟัน หรือมีเพียงการสูญเสียแร่ธาตุเพียงเล็กน้อยบนผิวเคลือบฟัน แต่สามารถตรวจพบเงาดําภายใน ชั้นเนื้อฟันจากภาพรังสีไบท์วิ่ง รายงานฉบับนี้ได้ตรวจพบฟันผุแอบแฝงในผู้ป่วย 2 ราย ที่การตรวจช่องปากทาง แต่มีรอยผุขนาดใหญ่ซึ่งตรวจพบได้จากภาพรังสีไบท์วิ่ง และการรักษารอยโรคเหล่านั้นอย่างเหมาะสม โดยรอยโรคไม่ลุกลามจนเป็นอันตรายต่อโพรงในฟัน ซึ่งทําให้คลินิกพบรอยยุบนด้านบดเคี้ยวของฟันเพียงเล็กน้อยสามารถป้องกันการสูญเสียฟันและไม่ต้องมีการรักษาที่มีค่าใช้จ่ายสูงขึ้น (ว ทันต จุฬาฯ 2550;30:61-72)

Antibacterial Activity Of Mangosteen Pericarp Extract Against Cariogenic Streptococcus Mutans, Kitti Torrungruang, Piraporn Vichienroj, Suchada Chutimaworapan

Objective The purpose of this study was to examine the antibacterial activity of extract from mangosteen pericarp against Streptococcus mutans, bacteria associated with dental plaque formation and caries development. Materials and methods Bacterial strains used in this study were S. mutans ATCC 25175 and KPSK2. Antibacterial activity was expressed as minimum inhibitory concentration (MIC) and minimum bactericidal concentration (MBC). The rates at which bacteria were killed were also determined by exposure to the extract at 2x and 4x MBC for varying time periods. Results The extract was effective against both strains of S. mutans. โครงการสร้างเสริมสุขภาพช่องปากในโรงเรียนตามหลักการสุขภาพแบบองค์รวมและชุมชนมีส่วนร่วม, สุดาดวง เกรันพงษ์, วนิดา นิ้วสุวรรณ

วัตถุประสงค์ เพื่อดําเนินโครงการสร้างเสริมสุขภาพช่องปากตามหลักการสุขภาพองค์รวมและการมีส่วนร่วมของ ชุมชน และเพื่อแสดงให้เห็นกระบวนการและการประเมินผลโครงการในขั้นตอนต่าง ๆ วิธีการ งานวิจัยเชิงปฏิบัติการระยะเวลาหนึ่งปีในโรงเรียนอนุบาลหมู่บ้านเด็กสานรัก ดําเนินโครงการ 4 ขั้นตอน ได้แก่ เตรียมความพร้อม สํารวจปัญหา ดําเนินการ (ประกอบด้วย ขั้นปฏิบัติการสร้างเสริมสุขภาพ ประเมิน ผลลัพธ์ของงานสร้างเสริมสุขภาพ และผลลัพธ์ทางสุขภาพระดับกลาง) และสรุป การเก็บข้อมูลและการประเมินผล ใช้วิธีการเชิงคุณภาพร่วมกับเชิงปริมาณ โดยใช้แบบบันทึก การสัมภาษณ์ สังเกตการณ์ และวิเคราะห์โภชนาการ ของเด็ก (สารอาหาร กลุ่มอาหาร และน้ําตาล) เปรียบเทียบก่อนและหลังดําเนินโครงการ ผลการศึกษา เด็กส่วนใหญ่มีฟันผุ ด้อยพัฒนาการเจริญเติบโต แปรงฟันด้วยวิธีที่ไม่เหมาะสม โภชนาการของ โรงเรียนยังขาดความสมดุลและครบถ้วนตามเกณฑ์ ทีมผู้วิจัยและทางโรงเรียนได้ร่วมพัฒนาโครงการย่อย 2 โครงการ คือ “โภชนาการแบบองค์รวม” และ “แปรงฟันด้วยกันก่อนอาบน้ํา” ที่ประกอบด้วยมาตรการทาง สังคมและสิ่งแวดล้อมต่าง ๆ ซึ่งสามารถปรับปรุงโภชนาการของเด็กได้อย่างชัดเจนโดยเฉพาะการลดปริมาณ น้ําตาลลงได้ถึงร้อยละ 80 และปรับปรุงการแปรงฟันของเด็กทุกคนตอนเช้าและเย็นให้สม่ําเสมอและบ้วนน้ําเพียง หนึ่งครั้งได้ ร้อยละของเด็กที่แปรงฟันได้นาน 2 นาทีเพิ่มขึ้นจาก 6.4 เป็น 63.3 โดยใช้วิธีแปรง 12 จังหวะที่พี่เลี้ยง เป็นต้นความคิด สรุป การศึกษานี้แสดงให้เห็นถึงการนําหลักการเรื่องสุขภาพแบบองค์รวมและการมีส่วนร่วมของชุมชนมาใช้ในการดําเนินงานสร้างเสริมสุขภาพช่องปาก โครงการสามารถปรับปรุงโภชนาการและนิสัยการแปรงฟันของเด็กได้ซึ่งน่าจะเป็นแนวทางสําหรับการผนวกการสร้างเสริมสุขภาพช่องปากให้เป็นส่วนหนึ่งในการดําเนินงานโรงเรียนส่งเสริมสุขภาพได้ (ว ทันต จุฬาฯ 2550:30:11-28)

Objectives: No instrument for assessing dentistsû job stress has been developed in Thailand. The objectives of this study were to describe the development of a Job Stress Inventory (JSI), a new self-report measure, and to assess its reliability, validity and factorial structure. Methods: A systematic random sample of 733 practicing dentists in Thailand was sent a mailed questionnaire that included the JSI and other questions. The JSI consisted of 30 items in a 5-point Likert format; a higher score reflected a higher level of job stress. Reliability was assessed by Cronbachûs alpha internal consistency coefficients. Studies from which we derive our recommendations need to have been conducted with scientific rigor and need to be confirmed with other welldesigned studies. Given the numerous, long-term, peer-reviewed published studies on antimicrobial mouthrinses with consistent statistically and clinically significant outcomes, it is time to change our professional thinking and practices.

A Comparison Of Flexural Fracture Of Three Different Nickel-Titanium Rotary File Systems, Matthew Warren Lloyd

The purpose of this study was to compare the number of rotations to failure of three different rotary file systems. ProFile, Sequence, and Liberator files in sizes 25 and 40 with 0.04 taper were divided into groups of five and rotated against a grooved metal block mounted to a Universal testing machine at 31 and 34 degrees. Each file was rotated at 300 rpm until fracture occurred. The number of rotations to fracture were calculated. Use of a three-way ANOVA and Tukey's HSD multiple comparison tests revealed significant differences for the angle of deflection, size, and type of file. An …

The purpose of this prospective clinical study was to investigate differences in outcomes from two common treatment modalities used to reduce deep overbite: maxillary incisor intrusion using an intrusion arch and posterior tooth eruption using an anterior bite plate. Pre-treatment, post-overbite correction and post-treatment records were gathered from 32 patients who presented with deep overbite malocclusions to the Virginia Commonwealth University orthodontic clinic. Both groups of patients experienced reductions in overbite and maxillary incisor display as well as maxillary and mandibular incisor proclination and mandibular incisor occlusal movement during treatment. Osteosarcoma is the most predominant primary bone cancer among pediatric patients. The pathogenesis and mechanism of osteosarcoma, however, as yet remains unclear. This study investigated the hypothesis that Ca2+/calmodulin-dependent protein kinase II (CaMK II), activated by Ca2+ and its intracellular receptor, calmodulin, plays a critical role in the growth of human osteosarcoma cells. The inhibition of CaMK II with its pharmacologic antagonist KN-93 was used to investigate the role of CaMK II in the growth of human osteosarcoma cells because our preliminary data showed that =-CaMK II is expressed in several human osteosarcoma cell lines. The periodontal ligament (PDL) is thought to include progenitor cells capable of forming fibroblasts, osteoblasts and cementoblasts. However, characterization of PDL stem cell populations (SC) remains undetermined due in part, to the diversity of cell types that conform to the periodontal apparatus. Objective: To isolate and characterize PDLSC and assess their pluripotent capabilities to differentiate into bone, cartilage and adipose cell types. Methods: PDL was scraped from human teeth, enzymatically digested, and cells stained for STRO- 1 (SC marker) using fluorescent immunohistochemistry. Adhesively cemented prefabricated posts are frequently employed for the restora-tion of endodontically treated teeth. Bonding agents may improve the post and core per-formance. Correct dentin bonding procedures are a critical factor in the bonded post re-tention, but adequate bonding in the canal space has been reported difficult to achieve. The operator is unable to visualize the depth of conditioner gel penetration into the canal, and its removal cannot be easily confirmed.
