There are inaccuracies and inconsistencies of radiographic interpretation among clinical instructors. The purpose of this investigation was to determine if a training program could improve the accuracy and consistency of instructors’ ratings of bone loss. A total of thirty-five clinical instructors consisting of periodontal faculty (periodontists and general dentists), dental hygiene faculty, and periodontal graduate students viewed projected digitized radiographic images and quantified bone loss for twenty-five teeth into four descriptive categories. Accurate and consistent radiographic interpretation among clinical instructors is needed for assessment of teaching, student performance, and patient care. The purpose of this investigation was to determine if the method of radiographic viewing affects accuracy and consistency of instructors’ determinations of bone loss. Forty-one clinicians who provide instruction in a dental school clinical teaching program (including periodontists, general dentists, periodontal graduate students, and dental hygienists) quantified bone loss for up to twenty-five teeth into four descriptive categories using a view box for plain film viewing or a projection system for digitized image viewing. ความสัมพันธ์ระหว่างความกว้างของฟันหน้าบนกับความกว้างของปีกจมูกและระยะห่างระหว่างหัวตา, สิทธิเดช นิลเจริญ

วัตถุประสงค์ เพื่อศึกษาว่า ความกว้างของฟันหน้าบนทั้ง 6 ซี่ มีความสัมพันธ์กับความกว้างของปีกจมูกและ ระยะห่างระหว่างหัวตาหรือไม่ เพื่อจะนําผลที่ได้จากการศึกษาไปเป็นแนวทางในการเลือกขนาดฟันหน้าที่เหมาะสมใน การทําฟันเทียม เนื่องจากผลการวิจัยส่วนใหญ่เป็นการศึกษาในคนเชื้อชาติอาหรับและคนผิวขาว ซึ่งโครงสร้างใบหน้ามีความแตกต่างไปจากคนเอเชียค่อนข้างมาก จึงต้องการตรวจสอบว่าจะสามารถนําผลการวิจัยครั้งนี้มาใช้กับประชากรไทยได้หรือไม่ โดยใช้ระเบียบการวิจัยเชิงพรรณนา วัสดุและวิธีการ ดําเนินการเก็บบันทึกข้อมูลจากผู้ป่วยอายุมากกว่า 17 ปีขึ้นไปที่เข้ามารับบริการที่กลุ่มงาน ทันตกรรม โรงพยาบาลกลาง จํานวน 303 คน แยกเป็นชาย 141 คน หญิง 162 คน อายุระหว่าง 18-71 ปี (อายุเฉลี่ย 35.2 + 13 ปี) โดยที่กลุ่มตัวอย่างจะต้องมีฟันหน้าบนอยู่ในสภาพที่สมบูรณ์ครบทั้ง 6 ซี่ ไม่เคยได้รับการรักษาด้วย การจัดฟันมาก่อน ในกรณีที่มีความผิดปกติบนใบหน้าซึ่งเป็นมาแต่กําเนิดหรือได้รับอุบัติเหตุในภายหลัง มีโรค ทางตาหรือได้รับการผ่าตัดบนใบหน้ามาก่อน มีช่องฟันห่าง มีการสึกเหตุบดเคี้ยวจนถึงบริเวณสัมผัส มีรอยผุและ วัสดุอุดฟันด้านประชิดหรือมีการบูรณะด้วยครอบฟันจะถูกคัดออก ทําการเก็บข้อมูลโดยใช้วิธีการวัดระยะห่าง ระหว่างส่วนที่กว้างที่สุดของปีกจมูกแต่ละข้าง ระยะห่างระหว่างจุดบรรจบของหนังตาบนและล่างของหัวตาแต่ละข้างและระยะรวมของส่วนที่กว้างมากที่สุดของฟันหน้าบนแต่ละซี่ตั้งแต่ฟันเขี้ยวบนขวาถึงฟันเขี้ยวบนซ้ายรวม 6 ทําการวัด 3 ครั้งในแต่ละตัวแปร นํามาหาค่าเฉลี่ย และส่วนเบี่ยงเบนมาตรฐาน นําค่าเฉลี่ยที่ได้มาทําการวิเคราะห์ และหาความสัมพันธ์ระหว่างความกว้างของฟันหน้าบนกับความกว้างของปีกจมูกและระยะห่างระหว่างหัวตา โดยใช้ค่าสัมประสิทธิ์สหสัมพันธ์เพียร์สัน (Pearson's correlation coefficients) ( A = 0.05) ด้วยโปรแกรม เอสพีเอสเอส สําหรับระบบปฏิบัติการวินโดว์ เวอร์ชั่น 11.5 (SPSS 11.5 for Windows) ผลการศึกษา ค่าเฉลี่ยความกว้างของฟันหน้าบนทั้ง 6 ที่มีค่า 46.86 + 2.49 มม. ค่าเฉลี่ยความกว้างของปีกจมูก มีค่า 41.32 + 3.86 มม. และค่าเฉลี่ยระยะห่างระหว่างหัวตามีค่า 34.81 + 2.78 มม. จากการวิเคราะห์ค่าสัมประสิทธิ์ สหสัมพันธ์เพียร์สันระหว่างค่าเฉลี่ยความกว้างของฟันหน้าบนกับค่าเฉลี่ยความกว้างของปีกจมูกและค่าเฉลี่ยระยะห่างระหว่างหัวตา พบว่า ค่าเฉลี่ยความกว้างของฟันหน้าบนมีความสัมพันธ์กับค่าเฉลี่ยความกว้างของปีกจมูกอย่าง มีนัยสําคัญทางสถิติ …

วัตถุประสงค์ เพื่อประเมินการเปลี่ยนแปลงปริมาณและสัดส่วนของแบคทีเรียในพ็อกเก็ต และลักษณะทางคลินิก หลังการขูดหินน้ําลายใต้เหงือก ด้วยหัวขูดอัลทราโซนิกส์คล้ายคิวเรตต์แบบมินิทิป ในผู้ป่วยโรคปริทันต์อักเสบเรื้อรัง วัสดุและวิธีการ การวิจัยนี้ใช้กลุ่มตัวอย่าง 12 คน ตําแหน่งทดลองแบ่งออกเป็น 2 กลุ่ม ได้แก่ กลุ่มที่มีพ็อกเก็ต 3.1-5.0 ม.ม. จํานวน 97 ตําแหน่ง และกลุ่มที่มีพ็อกเก็ต 5.1-7.0 ม.ม. จํานวน 43 ตําแหน่ง การศึกษาผลทาง จุลชีววิทยาด้วยกล้องจุลทรรศน์ชนิดเฟส-คอนทราสต์ และการวัดผลทางคลินิกด้วยค่าความลึกของพ็อกเก็ต และ ระดับการยืดติดของอวัยวะปริทันต์ ในช่วงเวลา 8 สัปดาห์ จํานวนแบคทีเรียใต้เหงือกที่นับได้ให้คํานวณค่าด้วย ลอการิทึมฐาน 10 ก่อนนําไปวิเคราะห์ผลด้วยสถิตินั้นพาราเมตริก ชนิดการทดสอบมันน์-วิตนีย์ทดสอบความแตกต่าง ระหว่างกลุ่มทดลอง และการทดสอบเชิงเครื่องหมายและลําดับที่แบบวิลคอกชันทดสอบความแตกต่างภายในกลุ่ม ทดลองของแต่ละช่วงเวลา การเปรียบเทียบความแตกต่างของค่าเฉลี่ยความลึกของพ็อกเก็ต และระดับการยืดตัว ของอวัยวะปริทันต์ ระหว่างกลุ่มทดลอง โดยใช้สถิติการทดสอบสําหรับกลุ่มตัวอย่างสองกลุ่มที่เป็นอิสระกัน และ การทดสอบที่สําหรับกลุ่มตัวอย่างสองกลุ่มที่สัมพันธ์กันทดสอบความแตกต่างภายในกลุ่มทดลอง ที่ระดับความเชื่อมั่นร้อยละ 95 ผลการศึกษา ผลการวิเคราะห์ระหว่างกลุ่ม พบว่า สัดส่วนของแบคทีเรียทุกประเภทแตกต่างอย่างมีนัยสําคัญทาง สถิติ (p < .05) ในสัปดาห์ที่ 8 โดยสัดส่วนของแบคทีเรียรูปกลม และรูปแท่งเคลื่อนที่ไม่ได้จะเพิ่มขึ้น แต่สัดส่วนของ แบคทีเรียรูปแท่งเคลื่อนที่ได้ และสไปโรคีตส์จะลดลงในทุกช่วงเวลาเมื่อเทียบกับสัปดาห์ที่ 0 อย่างมีนัยสําคัญทางสถิติ (p < .05) นอกจากนี้ ค่าร้อยละของสไปโรคีตส์ในตําแหน่งที่มีพ็อกเก็ตลึกจะมากกว่าตําแหน่งที่มีพ็อกเก็ตขึ้น การเปลี่ยนแปลงลักษณะทางคลินิกได้แก่ ความลึกของพ็อกเก็ตลดลง และระดับการยึดติดของอวัยวะปริทันต์เพิ่มขึ้น ในแต่ละช่วงเวลาอย่างมีนัยสําคัญทางสถิติ (p < .05) ตลอดช่วงเวลาการศึกษา เมื่อเทียบกับสัปดาห์ที่ 0 นอกจากนี้ การเปลี่ยนแปลงลักษณะทางคลินิกระหว่างกลุ่มทดลอง จะแตกต่างกันอย่างมีนัยสําคัญทางสถิติ (p < .05) เช่นเดียวกัน สรุป การใช้หัวอัลทราโซนิกส์ขูดหินน้ําลายคล้ายคิวเรตต์แบบมินิทิป จะเปลี่ยนแปลงปริมาณและสัดส่วนของ แบคทีเรียใต้เหงือก และลักษณะทางคลินิกของอวัยวะปริทันต์เมื่อเทียบกับก่อนการรักษา โดยความลึกของพ็อกเก็ตมีแนวโน้มต่อการเปลี่ยนแปลงสัดส่วนของแบคทีเรียใต้เหงือกภายหลังการรักษา (ว ทันต จุฬาฯ 2549;29:22-32)

วัตถุประสงค์ เพื่อศึกษาเปรียบเทียบความแข็งแรงดัดขวางชนิดไบแอกเชียลของวัสดุเรซินคอมโพสิตในปัจจุบัน จํานวน 5 ผลิตภัณฑ์ที่มีฟิลเลอร์และเรซินเมทริกซ์ต่างกัน วิธีการทดลอง วัสดุเรซินคอมโพสิตที่ใช้ในการทดลองนี้จํานวน 5 ผลิตภัณฑ์คือ Admira (Voco), CeramX (Dentsply), Filtek Supreme translucent (3M ESPE), Filtek Supreme standard (3M ESPE) และ Z250(3M ESPE) แต่ละผลิตภัณฑ์จะถูกเตรียมเป็นชิ้นทดสอบรูปแผ่นกลมเส้นผ่านศูนย์กลาง 13 มม.หนา 2 มม. จํานวน 15 ชิ้น และเก็บไว้ในน้ํากลั่นอุณหภูมิ 37 องศาเซลเซียสก่อนการทดสอบ ค่าความแข็งแรงตัดขวางชนิด ไบแอกเชียลของวัสดุเรซินคอมโพสิตได้จากการทดสอบด้วยเครื่องทดสอบสากล ที่ความเร็วการกด 0.5 มม. ต่อ นาที จากนั้นนําข้อมูลมาวิเคราะห์ด้วยสถิติความแปรปรวนแบบทางเดียว และการทดสอบทุกย์ ผลการศึกษา วัสดุ Filtek Supreme translucent มีค่าความแข็งแรงดัดขวางชนิดไบแอกเชียลเฉลี่ย 167.13 เมกะปาสคาลซึ่งสูงกว่าวัสดุทุกชนิดที่นํามาทดสอบ ค่าความแข็งแรงดัดขวางชนิดไบแอกเชียลเฉลี่ยของ Filtek Supreme standard และ Z250 เท่ากับ 154.78 และ 147.01 เมกะปาสคาลตามลําาดับโดยแตกต่างกันอย่างไม่มี นัยสําคัญ วัสดุ Admira มีค่าความแข็งแรงดัดขวางชนิดไบแอกเชียลเท่ากับ 122.67 เมกะปาสคาลและวัสดุ CeramX มีค่าความแข็งแรงดัดขวางชนิดไบแอกเชียลเท่ากับ 109.89 เมกะปาสคาล โดยวัสดุ Admira มีค่า ความแข็งแรงต่ํากว่าวัสดุ Z250 แต่สูงกว่าวัสดุ CeramX อย่างมีนัยสําคัญ สรุป จากวัสดุเรซินคอมโพสิตที่ใช้ในการศึกษานี้ วัสดุเรซินคอมโพสิตที่มีฟิลเลอร์ขนาดเล็กมีค่าความแข็งแรงดัดขวางชนิดไบแอกเซียลสูงกว่าวัสดุมีขนาดฟิลเลอร์ใหญ่กว่าและวัสดุเรซินคอมโพสิตที่มีโมเลกุลของออโมเซอร์ เป็นส่วนประกอบไม่ได้มีความแข็งแรงดัดขวางชนิดไบแอกเชียลมากกว่าวัสดุเรซินคอมโพสิตชนิดอื่น (ว ทันต จุฬาฯ 2549;29:1-12)

การใช้หน้ากากดึงขากรรไกรบน (facemask) เป็นวิธีหนึ่งในการแก้ไขการสบฟันแบบคลาสทรี ที่เกิด จากโครงสร้างขากรรไกรบนมีตําแหน่งถอยหลังในผู้ป่วยที่ยังมีการเจริญเติบโตอยู่ ทําให้เกิดการเปลี่ยนแปลงของ โครงสร้างใบหน้า โดยเกิดการเคลื่อนของขากรรไกรบนและฟันบนมาข้างหน้า นิยมทําการขยายขากรรไกรบนร่วมด้วย เพื่อชดเชยกับการหดตัวในบริเวณส่วนหน้าของขากรรไกรบนขณะทําการดึงมาข้างหน้า แม้ก่อนการรักษาไม่ปรากฏว่าขากรรไกรบนแคบก็ตาม ตําแหน่งที่ใช้ดึงยางควรดึงจากบริเวณส่วนหน้าของขากรรไกรบนในทิศทางลงล่าง เพื่อลด การหมุนส่วนหน้าขึ้นของขากรรไกรบน ช่วงระยะเวลาที่เหมาะสมในการดึงขากรรไกรบน ควรทําก่อนที่จะมีการ เจริญเติบโตสูงสุด เพื่อให้เกิดการเปลี่ยนแปลงทางโครงสร้างมากกว่าการเคลื่อนของฟัน ปริมาณแรงที่ใช้เพื่อให้เกิดการเคลื่อนของโครงสร้างนั้นมีปริมาณมากกว่าการเคลื่อนของฟัน ระดับแรงที่พบว่าทําให้เกิดการเคลื่อนที่ของขา กรรไกรบนมาข้างหน้ามีค่าประมาณ 300-500 กรัมต่อข้าง ผู้ป่วยควรใส่เครื่องมืออย่างน้อย 10-12 ชั่วโมงต่อวัน หากปริมาณแรงน้อย ระยะเวลาการใส่แต่ละวันและช่วงระยะเวลาการรักษาก็จะยาวนานขึ้น ทั้งนี้ขึ้นกับระดับความ รุนแรงและอายุของผู้ป่วยด้วย หลังการขยายและดึงขากรรไกรบนมาข้างหน้าจนสําเร็จแล้ว ควรติดตามผลหลังการ รักษา และ/หรือให้ผู้ป่วยใส่เครื่องมือคงสภาพเฉพาะตอนกลางคืนจนกว่าผู้ป่วยหมดการเจริญเติบโต (ว ทันต จุฬาฯ 2549;29:44-51)

วัตถุประสงค์ เพื่อประเมินผลการใช้สารลดการเสียวฟันชนิดสารเรซินที่มีตัวเติมต่อการป้องกันการสึกกร่อนของเนื้อฟันจากการแปรงฟันในห้องทดลอง วัสดุและวิธีการ การวิจัยใช้ฟันกรามน้อย 40 ซี่ นํามาเตรียมชิ้นตัวอย่าง โดยตัดแต่งและกําจัดชั้นเคลือบรากฟัน ออกจะได้ชิ้นเนื้อฟันบริเวณรากฟันทั้งหมด 40 ชิ้น นําไปอาบกัมมันตรังสี และแบ่งเป็น 4 กลุ่มตามชนิดของสารที่ใช้ เคลือบบนผิวเนื้อฟัน ได้แก่ กลุ่มที่หนึ่งใช้สารเรซินที่มีตัวเติม Seal&ProtectTM กลุ่มที่สองใช้สารเรซินชนิดเดียวกับ กลุ่มที่หนึ่งแต่ไม่มีตัวเติม Seal&ProtectTM กลุ่มที่สามใช้สารเรซินที่ไม่มีตัวเติมคือใช้ All-bond 2 และกลุ่มที่สี่ เป็นกลุ่มควบคุมที่ไม่มีการเคลือบด้วยสารใด ๆ บนผิวเนื้อฟัน นําชิ้นตัวอย่างไปทดสอบการสึกกร่อนของเนื้อฟันด้วยวิธี เรดิโอเทรเซอร์ โดยดัดแปลงจากวิธีของเอดีเอ วัดปริมาณสารกัมมันตรังสีในสารละลายยาสีฟันที่ได้จากการแปรงฟัน ด้วยเครื่องแปรงฟันวี8 โดยตรวจนับปริมาณและคํานวณหน่วยเป็นจํานวนนับต่อนาที ด้วยเครื่องตรวจนับ กัมมันตรังสี และวิเคราะห์ทางสถิติด้วยการวิเคราะห์ความแปรปรวนทางเดียว ที่ระดับนัยสําคัญ 05 ผลการศึกษา ปริมาณการสึกกร่อนของเนื้อฟันมีความแตกต่างระหว่างกลุ่มอย่างมีนัยสําคัญทางสถิติ (p < .05) โดยกลุ่มที่เคลือบสารเรซินที่มีตัวเติม Seal&ProtectTM มีค่าเฉลี่ยจํานวนการสึกกร่อนของเนื้อฟันน้อยที่สุด และ แตกต่างจากกลุ่มอื่นอย่างมีนัยสําคัญทางสถิติ สรุป ผลจากการวิจัยนี้แสดงว่าสารลดการเสียวฟันชนิดสารเรซินที่มีตัวเติม Seal&ProtectTM สามารถลดการ สึกกร่อนของเนื้อฟันจากการแปรงฟันได้ และอาจเป็นแนวทางในการนําไปใช้รักษาผู้ป่วยที่เสียวรากฟัน อย่างไรก็ดีควรมีการประเมินผลทางคลินิกด้วย (ว ทันต จุฬาฯ 2549;29:13-21)

Purpose. Dental hygiene ethics is an essential component of the dental hygiene curriculum. The accreditation standards for dental hygiene education state that graduates must be competent in applying ethical concepts to the provision and/or support of oral health care services.1 Although the standards for entry into the profession of dental hygiene emphasize the importance of ethical reasoning, there is little published research specific to ethics instruction in dental hygiene programs. The purpose of this study was to compare the longevity of crowns versus large multisurface restorations in posterior teeth. The investigation used the treatment database at Virginia Commonwealth University School of Dentistry. The inclusion criteria for the final data set used for analysis were: only one restored tooth per patient, premolars with three or more restored surfaces, molars with four or more restored surfaces, molars and premolars restored with complete veneer metal crowns, or crowns veneered with metal and porcelain. The purpose of these case reports is to introduce an innovative orthodontic appliance to effect forced eruption. This device is useful when attempting to erupt teeth in the esthetic zone with minimal effect on adjacent teeth. It is especially indicated for adjacent teeth with esthetic restorations. The appliance is easy to fabricate, cost-effective and very useful in forced eruption efforts for the “nonrestorable” tooth.

The calpain family of cysteine proteases has undergone intense study since its discovery in 1976. Although numerous functions and proteolytic substrates have been linked to this 14-member family, little is known about the five atypical calpain isoforms. Over the past 25 years, investigators have identified ‘calpain-like’ activities in mitochondrial fractions isolated from various tissues. The studies undertaken in this dissertation were designed to positively identify the mitochondrial calpain present in renal cortical mitochondria (RCM) and to elucidate its function and proteolytic targets under both basal and CA2- -challenged conditions. Purpose. The purpose of this study was to evaluate the relationship between volatile sulfur compounds (VSC) and gingival health status and to determine if volatile sulfur compounds can detect early dental plaque-induced gingival disease. Methods. A split-mouth design with randomly selected quadrants of the mandibular arch enabled 39 participants to serve as their own controls. At baseline and at three subsequent appointments (days 7, 14, and 21) gingival inflammation (GI), bleeding on probing (BOP), and sulfide levels (SUL) were measured using the Gingival Index and the Diamond Probe/Perio 2000 System. Prevalence information on excessive gingival display in post-orthodontic patients is limited. By studying one aspect, namely the size relationship of the clinical crowns of teeth, in an orthodontic population, we can begin to quantify their need for periodontal plastic surgery. In this two part study, 200 plaster models were used as subjects, followed by a clinical exam of 31 of those subjects. These models represented patients before and directly after orthodontic therapy, and the Part 2 clinical exams were performed at least five years later. The lengths and widths of the six anterior teeth were measured and these values were …
