The purpose of this study was to test the explanatory power of The Health Belief Model with the added environmental condition of perceived humanistic attitude of a dental hygienist in its ability to predict individual's performance of oral disease prevention behaviors. A cross section survey of an intact group of 1,943 full time employees of Old Dominion University was conducted. The response rate was 52% (n = 1001). An exclusionary rule eliminated respondents who did not have a dental hygienist provide preventive services during dental visits. Seven hundred and fifty four were included in the study analyses. Effect Of Ca(Oh)2 On Survival Ability Of A Bacterial Mixture: An In Vitro Study, Nishan Antranik Odabashian

The purpose of this in vitro investigation was to evaluate the antibacterial effect of calcium hydroxide on a mixture of nine bacteria associated with endodontic infections at different time intervals and to examine the survival of the same bacteria when in mixture. A total of 102 teeth were used in the first part of the experiment. Eighty teeth were used in eight experimental groups of 10 each, 16 teeth were used for positive and negative controls, and six teeth were used for scanning electron microscopy (SEM). The purpose of this project was to examine state dental practice laws and the extent to which they encourage alternative models of delivering preventive oral health care. This project encompassed two distinct study components: 1) an analysis of existing state dental statutes and regulations; and 2) case studies to examine the enactment of public health-oriented provisions encouraging alternative models of delivering preventive oral health care to low-income children; and the development and implementation of such alternative models.

ความรู้และพฤติกรรมในการสร้างเสริมป้องกัน ทันตสุขภาพของประชาชนในชุมชนเขตบางรักกรุงเทพมหานคร, มยุรี ตติยกวี

วัตถุประสงค์ เพื่อศึกษาความรู้และพฤติกรรมในการสร้างเสริมและป้องกันทันตสุขภาพของประชาชนในชุมชน เขตบางรัก กรุงเทพมหานคร วัสดุและวิธีการใช้แบบสอบถามเก็บข้อมูลโดยสุ่มตัวอย่างอย่างง่าย รวบรวมข้อมูลและวิเคราะห์ด้วยโปรแกรม SPSS ใช้ค่าสถิติ 2. test กําหนดค่านัยสําคัญ p < 0.05 ผลการศึกษา พบว่าจากประชาชนกลุ่มตัวอย่าง 235 ราย มีความรู้ในเรื่องการแปรงฟันที่ถูกวิธี สาเหตุโรคฟันผุ และโรคเหงือก การป้องกันโรคฟันผุ การป้องกันโรคเหงือก คิดเป็นร้อยละ 74, 35.1, 86, และ 70.2 ตามลําดับ มี ความรู้ในเรื่องการเคลือบฟันด้วยฟลูออไรด์ในเด็ก การเคลือบหลุมร่องฟัน การรับประทานฟลูออไรด์เสริม และ การตรวจฟันด้วยตนเอง คิดเป็นร้อยละ 39.6, 57.9, 62.6, และ 38.7 ตามลําดับ ในด้านพฤติกรรมการดูแลสุขภาพ ฟันนั้น พบว่าร้อยละ 91.1แปรงฟันวันละ 2 ครั้งขึ้นไป ร้อยละ 64.3 พบทันตแพทย์เมื่อมีปัญหาทันตสุขภาพ และ ร้อยละ 16.2 ไปพบทันตแพทย์เพื่อตรวจฟันประจําปี จากการวิเคราะห์ทางสถิติในกลุ่มตัวอย่างนี้พบว่า ผู้หญิง ผู้มี การศึกษาสูง และผู้ที่มีรายได้เพียงพอ มีความรู้และพฤติกรรมทางทันตสุขภาพดีกว่า ผู้ชาย ผู้มีการศึกษาที่ต่ํากว่า และผู้ที่มีรายได้ไม่เพียงพอ การเรียนรู้ทันตสุขภาพจากโทรทัศน์และจากโฆษณา มีความสัมพันธ์กับการเรียนรู้วิธี แปรงฟันที่ถูกต้องและการป้องกันโรคเหงือก และความต้องการให้สอนทันตสุขศึกษาทางสื่อต่าง ๆ มีความสัมพันธ์กับ การเรียนรู้จากโทรทัศน์ อย่างมีนัยสําคัญ (p < 0.05)สรุป ประชาชนในชุมชนเขตบางรัก กรุงเทพมหานคร ยังขาดความรู้ด้านการสร้างเสริมและป้องกันทันตสุขภาพอีก หลายประการ ความรู้และพฤติกรรมในการสร้างเสริมป้องกันทันตสุขภาพมีความสัมพันธ์ กับ เพศ อายุ ระดับ การศึกษา และรายได้ของบุคคล สื่อโทรทัศน์มีอิทธิพลต่อการเรียนรู้ของประชาชน

การสูญเสียฟันในผู้ป่วยทันตกรรมสูงอายุและความสัมพันธ์กับโรคทางระบบ, อารีย์ เจนกิตติวงศ์

วัตถุประสงค์เพื่อให้ได้ข้อมูลของอุบัติการและรูปแบบของการสูญเสียฟันในผู้ป่วยทันตกรรมสูงอายุ และเพื่อหาความสัมพันธ์ของการสูญเสียฟันกับโรคทางระบบประชากร กษาและวิธีการ การศึกษาทําในผู้ป่วยที่มาขอรับการรักษาทางทันตกรรมที่คณะทันตแพทยศาสตร์ จุฬาลงกรณ์มหาวิทยาลัย อายุ 60 ปีหรือมากกว่า จํานวน 510 คน โดยการสัมภาษณ์เกี่ยวกับโรคทางระบบ และ ตรวจช่องปากบันทึกซี่ฟันที่หายไป ผลการศึกษา อุบัติการของการสูญเสียฟัน ภาวะสูญเสียฟันทั้งปาก และจํานวนพื้นที่หายไปเพิ่มตามอายุที่สูงขึ้น - อย่างมีนัยสําคัญ (p < 0.001) ผู้สูงอายุส่วนใหญ่ (83.1%) มีฟันหายไปอย่างน้อย 1 ปี ขณะที่ 12.5% ของผู้ สูงอายุไม่มีฟันทั้งปาก ค่าเฉลี่ยจํานวนฟันที่หายไปในผู้สูงอายุจํานวน 510 คน คือ 16.7 ต่อคน ไม่พบความแตกต่าง ของค่าเฉลี่ยจํานวนฟันที่หายไประหว่างเพศ พบว่าฟันที่หายไปบ่อยที่สุดคือ ฟันกรามใหญ่ ฟันที่หายไปน้อยที่สุดคือ ฟันเขี้ยว ค่าเฉลี่ยจํานวนรากฟันเหลือค้างในช่องปากของผู้สูงอายุ 136 คนคือ 2.3 รากต่อคน พบรากฟันเหลือค้าง ในขากรรไกรบนมากกว่าขากรรไกรล่าง ไม่พบความแตกต่างของอุบัติการและค่าเฉลี่ยจํานวนรากฟันเหลือค้างระหว่างเพศและอายุ ผู้สูงอายุจํานวน 421 คนมีโรคทางระบบอย่างน้อยหนึ่งชนิด ค่าเฉลี่ยจํานวนฟันที่หายไปใน ผู้สูงอายุที่มีโรคทางระบบไม่แตกต่างจากผู้สูงอายุที่ไม่มีโรคใด ๆ และไม่พบความสัมพันธ์ระหว่างภาวะสูญเสียฟันทั้งปากกับการที่มีโรคทางระบบสรุป การศึกษานี้สนับสนุนการศึกษาอื่น ๆ ที่รายงานว่าอุบัติการของการสูญเสียฟันและจํานวนพื้นที่หายไปสูงใน ผู้สูงอายุ และแปรตามอายุที่สูงขึ้น แต่การศึกษานี้ไม่พบความสัมพันธ์ของการสูญเสียฟันกับโรคทางระบบ

เปรียบเทียบปริมาณฟลูออไรด์ที่ขับออกภายหลัง การกินยาเม็ดฟลูออไรด์พร้อมน้ํา นมธรรมดา และนมแคลเซียมสูง, ยุทธนา ปัญญางาม, พรศรี ปฏิมานุเกษม, กฤษนันท์ ประคองทรัพย์, สานเดิม มุทธสกุล

วัตถุประสงค์เพื่อเปรียบเทียบปริมาณฟลูออไรด์ที่ขับออกมาในปัสสาวะภายในเวลา 3 ชั่วโมงหลังกินยาเม็ดไซเดียม ฟลูออไรด์พร้อมน้ํา นมธรรมดา และนมแคลเซียมสูง วัสดุและวิธีการ อาสาสมัครสุขภาพสมบูรณ์ อายุระหว่าง 19-21 ปี จํานวน 10 คนกินยาเม็ดโซเดียมฟลูออไรด์ 0.25 มิลลิกรัม 1 เม็ด พร้อมน้ํา 250 มิลลิลิตร เก็บปัสสาวะทุก 30 นาที หลังกินยาจนครบ 3 ชั่วโมง วัดปริมาตร ของปัสสาวะและความเข้มข้นของฟลูออไรด์อิออนในปัสสาวะด้วยฟลูออไรด์อีเล็คโทรด จํานวนปริมาณฟลูออไรด์ เฉลี่ยที่ถูกขับออกมาในปัสสาวะในเวลา 3 ชั่วโมง และร้อยละเฉลี่ยเมื่อเทียบกับปริมาณฟลูออไรด์ที่กิน ทําการ ทดลองซ้ําโดยให้กินยาเม็ดไซเดียมฟลูออไรด์พร้อมนมธรรมดา 2 ชนิด และนมแคลเซียมสูง 2 ชนิด โดยเว้นระยะ อย่างน้อย 24 ชั่วโมงสําหรับนมแต่ละชนิด เปรียบเทียบความแตกต่างทางสถิติของปริมาณฟลูออไรด์เฉลี่ยที่ถูกขับออกมาในปัสสาวะภายหลังการกินพร้อมน้ําและนมแต่ละชนิดด้วยการวิเคราะห์ความแปรปรวนแบบจําแนกทางเดียว (One-way ANOVA) และเปรียบเทียบคู่ที่แตกต่างกันด้วยวิธีนัยสําคัญน้อยที่สุด (Least significance difference) ที่ระดับนัยสําคัญ 0.05 ผลการศึกษา ปริมาณฟลูออไรด์เฉลี่ยที่ถูกขับออกมาในปัสสาวะในเวลา 3 ชั่วโมงหลังกินยาเม็ดโซเดียมฟลูออไรด์ พร้อมน้ํา นมธรรมดาชนิดที่ 1 นมธรรมดาชนิดที่ 2 นมแคลเซียมสูงชนิดที่ 1 และนมแคลเซียมสูงชนิดที่ 2 เท่ากับ 0.139 + 0.014, 0.114 + 0.026, 0.113 + 0.034, 0.079 + 0.022 และ 0.080 4 0.024 มิลลิกรัม คิดเป็นร้อยละ ของปริมาณฟลูออไรด์ทั้งหมดที่กินเท่ากับ 55.6, 45.6, 45.2, 31.6 และ 32.0ตามลําดับ ปริมาณเฉลี่ยของฟลูออไรด์ ที่ถูกขับออกมาในปัสสาวะหลังกินพร้อมน้ํามากกว่าปริมาณเฉลี่ยของฟลูออไรด์ที่ถูกขับออกมาหลังกินพร้อม นมทั้ง 4 ชนิดอย่างมีนัยสําคัญทางสถิติ (p < 0.05) ปริมาณเฉลี่ยของฟลูออไรด์ที่ถูกขับออกมาหลังกินพร้อม นมธรรมดาทั้ง 2 ชนิดมากกว่าปริมาณฟลูออไรด์ที่ถูกขับออกมากหลังกินพร้อมนมแคลเซียมสูงทั้ง 2 ชนิดอย่าง มีนัยสําคัญทางสถิติ (p < 0.05) สรุป ปริมาณแคลเซียมในนมที่เพิ่มขึ้นเป็นผลให้ปริมาณฟลูออไรด์ที่ถูกขับออกมาในปัสสาวะลดลง

ผลของเอพิแกลโลคาเทชิน แกลเลต ในการยับยั้งการกระตุ้นการทํางานของเอนไซม์ Mmp-2 ที่เหนี่ยวนําโดยสารละลายคอลลาเจนในเซลล์ไลน์ ของมะเร็งชนิดสแควร์มัสเซลล์ที่ได้จากช่องปาก, นิรชา สารชวนะกิจ, อมรรัตน์ สุวรรณชัย, อรฉัตร บุณยเกียรติ, อาทิพันธ์ พิมพ์ขาวขำ, ประสิทธิ์ ภวสันต์

วัตถุประสงค์ เพื่อศึกษาผลของเอพีแกลโลคาเทชิน แกลเลต (Epigallocatechin gallate; EGCG) ซึ่งเป็นสารสกัด จากใบชาในกลุ่มโพลีฟีนอล ในการยับยั้งการกระตุ้นการทํางานของเอนไซม์ MMP-2 ในเซลล์ไลน์จากมะเร็ง ชนิดสแควร์มัสเซลล์ที่พบในช่องปาก วัสดุและวิธีการ ทําการศึกษาในห้องปฏิบัติการ โดยใช้ HSC-3 และ HSC-7 ซึ่งเป็นเซลล์ไลน์ของมะเร็ง ชนิดสแควร์มัสเซลล์ และ HFF ซึ่งเป็นเซลล์ไลน์ของเนื้อเยื่อเกี่ยวพัน เซลล์ไลน์ทั้งสามถูกเลี้ยงในสภาวะที่เดิม สารละลายคอลลาเจน 200 ไมโครกรัมต่อมิลลิลิตร เพื่อเหนี่ยวนําการกระตุ้นการทํางานของเอนไซม์ MMP-2 จากนั้นเติม EGCG ที่ความเข้มข้น 0, 1, 10 ไมโครโมลาร์ เป็นเวลา 48 ชั่วโมง ระดับของ แอคทีฟ MMP-2 ถูก ตรวจวัดจากอาหารเลี้ยงเซลล์ด้วยเทคนิคเจลาตินไซโมกราฟฟิผลการศีกษา พบว่า EGCG ที่ความเข้มข้น 10 ไมโครโมลาร์ ลดระดับการกระตุ้นการทํางานของเอนไซม์ MMP-2 ใน HSC-3 และ HFF และยับยั้งการกระตุ้นการทํางานของเอนไซม์ MMP-2 ใน HSC-7 สรุป ผลการศึกษาแสดงให้เห็นความสามารถของ EGCO ในการลดการกระตุ้นการทํางานของเอนไซม์ MMP-2 ซึ่งมีบทบาทสําคัญในกระบวนการแพร่กระจายของมะเร็งโดยทั่วไปรวมทั้งมะเร็งชนิดสแควร์มัสเซลล์ ซึ่งเป็น คุณสมบัติอีกด้านหนึ่งของ EGCG นอกเหนือจากการเป็นสารแอนติออกซิแดนท์ด้านการเกิดมะเร็งดังที่เคยมี รายงานในมะเร็งหลายๆชนิด

Enamel Shear Bond Strength Of Different Sealants: In Vitro Study, Supaporn Chongvisal, Piyanuch Saisuwan, Thipawan Tharapiwattananon

Objective The aim of this study was to investigate the enamel shear bond strength of four sealants in vitro. Materials and methods Sixty caries-free human premolars were randomly divided into four groups of 15. The materials used were a local-made sealant (LM), Super-Bond C&B (SB), Delton® (D) and Concise™(C). Each material was prepared according to the instruction of the manufacturer. The sealant was placed in a mold, cured, stored in distilled water for 24 hr and shear bond strength determined by the Instron Universal Testing Machine at a crosshead speed of 0.5 mm/min. วัตถุประสงค์ เพื่อหาความสัมพันธ์ระหว่างความสามารถในการควบคุมระดับน้ําตาลในเลือดกับการเกิดโรคปริทันต์อักเสบในผู้ป่วยเบาหวานชนิดที่ 2 ในคนไทย วัสดุและวิธีการศึกษาในผู้ป่วยเบาหวานชนิดที่ 2 จํานวน 205 ราย ในคลินิกโรคเบาหวาน ของโรงพยาบาลเมือง ฉะเชิงเทรา โดยผู้ป่วยที่เป็นกลุ่มตัวอย่างนี้ ต้องเป็นผู้ป่วยที่มารับการรักษาจากคลินิกดังกล่าว ติดต่อกันอย่างน้อย สองครั้ง ต้องเป็นผู้ป่วยที่มีฟันดัชนีที่จะสามารถประเมินสภาวะต่าง ๆ ทางปริทันต์ได้ครบ 6 ปี ในวันที่ทําการศึกษา หลังจากเจาะเลือดเพื่อตรวจหาค่า บ่งชี้ความสามารถในการควบคุมระดับน้ําตาลในเลือดโดยวัดระดับฮีโมโกลบินที่ มีน้ําตาลเกาะ (Hemoglobin A : HbA) จึงทําการตรวจและจดบันทึกสภาวะต่าง ๆ ทางปริทันต์ ผลจากการ ตรวจวัดค่าเฮโมโกลบินเอวัน พบว่าในผู้ป่วย 205 รายนี้มีผู้ป่วยที่สามารถควบคุมระดับน้ําตาลในเลือดได้ 46 ราย จึงนํารายละเอียดจากแบบบันทึกสภาวะปริทันต์ของกลุ่มตัวอย่างอีก 45 ราย ที่มีอายุและเวลาป่วยเป็นเบาหวาน ใกล้เคียงกับกลุ่มแรกมากที่สุดแต่ควบคุมระดับน้ําตาลในเลือดไม่ได้มาศึกษาเปรียบเทียบโดยใช้โปรแกรมสําเร็จรูป เอสทีเอส.บล/ที ผลการศึกษา พบว่าเมื่อศึกษารวมทั้งกลุ่ม โดยสหสัมพันธ์ของเพียร์สัน ค่าเฉลี่ยฮีโมโกลบินเอวัน สัมพันธ์กับ ค่าเฉลี่ยการสูญเสียการยึดเกาะของอวัยวะปริทันต์อย่างมีนัยสําคัญทางสถิติ p = 0.01 (p < 0.05) โดยมีค่า สัมประสิทธิ์สหสัมพันธ์ (1) = 0.3 เมื่อใช้การทดสอบไค-สแควร์หาความสัมพันธ์ของความสามารถในการควบคุม ระดับน้ําตาลในเลือดกับการเกิดโรคปริทันต์อักเสบ โดยเปรียบเทียบจํานวนผู้ป่วยที่มีการสูญเสียการยึดเกาะของ อวัยวะปริทันต์ 25 มิลลิเมตรมากกว่าร้อยละ 10 ของด้านฟันขึ้นไป พบว่ามีความสัมพันธ์กันอย่างมีนัยสําคัญทาง สถิติ ที่ความเชื่อมั่นร้อยละ 95 p = 0.012 (p < 0.05) ค่า odds ratio - 3.96 (95% CI = 11.43, 1.37) สรุป ในผู้ป่วยเบาหวานชนิดที่ 2 ความสามารถในการควบคุมระดับน้ําตาลในเลือดมีความสัมพันธ์กับการเกิดโรค ปริทันต์อักเสบ โดยผู้ป่วยเบาหวานที่ควบคุมระดับน้ําตาลในเลือดไม่ได้มีโอกาสเป็นโรคปริทันต์อักเสบคิดเป็น 4 เท่าของผู้ป่วยเบาหวานที่สามารถควบคุมระดับน้ําตาลในเลือดได้

รอยโรคไลเคนอยด์ในช่องปากที่เกิดจาก ยานาพรอกเซน: รายงานผู้ป่วย 1 ราย, สโรชรัตน์ ปิตุวงศ์

รายงานผู้ป่วยรอยโรคไลเคนอยด์ในช่องปาก ที่มีความสัมพันธ์กับการได้รับยานาพรอกเซนที่ใช้รักษาโรค ข้ออักเสบ ผู้ป่วยได้รับประทานยานี้เป็นเวลานานประมาณ 2-3 ปีติดต่อกัน อาการและลักษณะทางคลินิกของ รอยโรคภายในช่องปากดีขึ้นภายหลังที่ให้ผู้ป่วยหยุดยานี้ จากการติดตามผลการรักษาเป็นระยะ ๆ เป็นเวลา 1 ปีไม่พบว่ามีรอยโรคนี้เกิดขึ้นอีก

โรควอนวิลลีแบรนด์ (von Willebrand's Disease) เป็นโรคเลือดออกผิดปกติที่เป็นมาแต่กําเนิด ถ่ายทอด ทางพันธุกรรมแบบออโตโซมอล โดมิแนนท์ (autosomal dominant) เกิดในผู้ป่วยที่มีความผิดปกติของวอลวิลลี - แบรนด์แฟคเตอร์ และไปทําให้มีความผิดปกติของแฟคเตอร์ 8 แอนติฮีมโมฟิลิค กลอบบูลีน (antihemophilic globulin ) และมีความผิดปกติในหน้าที่ของเกร็ดเลือด ในโรคนี้มีจํานวนเกร็ดเลือดและการแข็งตัวของลิ่มเลือดปกติ แต่ระยะเวลาเลือดออกจะยาวนานกว่าปกติ ดังนั้นการทําฟันจะต้องทําด้วยความระมัดระวัง โดยเฉพาะการถอนฟัน หรือขูดหินปูน จําเป็นจะต้องทําการรักษาร่วมกับแพทย์ทางโลหิตวิทยา แก้ไขปัญหาทางการแพทย์ ก่อนการให้ การรักษาทางทันตกรรมเพื่อป้องกันไม่ให้เกิดภาวะแทรกซ้อนเลือดออกผิดปกติ ในบทความนี้ได้รายงานผู้ป่วย 2 ราย ที่เป็นโรควอนวิลลีแบรนด์ร่วมกับการรักษาทางทันตกรรม
