บทนำ รากฟันแตกในแนวดิ่งเป็นหนึ่งในสาเหตุสำคัญของการสูญเสียฟันในฟันที่ผ่านการรักษาคลองรากฟันมาแล้ว ฟันที่มีรากฟันแตกในแนวดิ่งนั้นมักวินิจฉัยได้ยาก จึงได้นำภาพรังสีซีบีซีทีซึ่งสามารถทำให้มองเห็นคลองรากฟันได้ทั้งสามมิติมาตรวจวินิจฉัย แต่ข้อจำกัดของภาพรังสีซีบีซีทีคือเมื่อมีวัสดุทึบรังสีอยู่ในคลองรากฟันจะก่อให้เกิดสิ่งแปลกปนที่มีลักษณะคล้ายกับรอยแตก จุดประสงค์ของการศึกษานี้จึงต้องการศึกษาถึงความเที่ยงตรงของภาพรังสีซีบีซีทีในการตรวจหารอยแตกในแนวดิ่งในคลองรากฟันที่ถูกปิดกั้นด้วยวัสดุทึบรังสีชนิดต่างๆ วัสดุและวิธีการ ใช้ฟันกรามน้อยที่มีรากตรงรากเดียวจำนวนหกสิบซี่ โดยฟันสี่สิบซี่จำลองให้เกิดการแตกของรากฟันในแนวดิ่ง นำฟันทุกซี่ถ่ายภาพรังสีไมโครซีทีเพื่อยืนยันว่าฟันที่จำลองรอยแตกมีรอยแตกเกิดขึ้นจริง ฟันทุกซี่จะถูกถ่ายภาพรังสีซีบีซีที โดยไม่มีวัสดุทึบรังสีในคลองรากฟันใช้เป็นกลุ่มควบคุม และมีการใส่เดือยฟันโลหะเหวี่ยง เดือยฟันเส้นใยเรซิน และกัตตาเพอร์ชาเป็นวัสดุทึบรังสีในคลองรากฟัน ผู้แปลภาพรังสีสี่คนทำการแปลภาพรังสี และคำนวณหาค่าความไว ความจำเพาะ และความเที่ยงตรงของภาพรังสีซีบีซีทีในการตรวจหารากฟันแตกในแนวดิ่ง ผลการทดลอง ในคลองรากฟันที่ไม่มีวัสดุทึบรังสีพบว่าพบว่าซีบีซีทีมีค่าความไว ความจำเพาะ และความเที่ยงตรงอยู่ที่ ร้อยละ 77.5 ร้อยละ 88.75 และร้อยละ 81.86 ตามลำดับ หากมีวัสดุทึบรังสีปรากฏในคลองรากฟันจะทำให้ค่าความจำเพาะและความเที่ยงตรงของซีบีซีทีในการตรวจหารากฟันแตกในแนวดิ่งลดลง สรุป ภาพถ่ายรังสีซีบีซีทีมีความเที่ยงตรงสูงในการตรวจหารากฟันแตกในแนวดิ่งในคลองรากฟันที่ไม่มีวัสดุทึบรัง ซึ่งหากมีวัสดุทึบรังสีปรากฏในคลองรากฟันแล้วจะทำให้ความสามารถของซีบีซีทีในการวินิจฉัยรากฟันแตกในแนวดิ่งลดลง

การศึกษานี้มีวัตถุประสงค์เพื่อศึกษาผลของการปรับเปลี่ยนชีวภาพเนื้อฟันด้วยสารสกัดจากเมล็ดองุ่นในการลดปริมาณแอคทีฟเจลาติเนส เมื่อใช้ร่วมกับสารยึดติดยูนิเวอร์แซล เอทช์แอนด์รินส์โหมด โดยใช้ผงเนื้อฟันที่เตรียมจากฟันกรามใหญ่ซี่ที่สามของมนุษย์ในการวัดปริมาณแอคทีฟเอ็มเอ็มพี ชนิดที่ 2 และใช้ชิ้นตัวอย่างการยึดติดเรซินและเนื้อฟันเพื่อศึกษาตำแหน่งที่มีแอคทีฟเจลาติเนสในชั้นไฮบริด แบ่งกลุ่มการทดลองเป็น 6 กลุ่ม คือ เนื้อฟันที่ไม่ปรับสภาพผิวใด ๆ กลุ่มเนื้อฟันที่กัดด้วยกรดฟอสฟอริก กลุ่มเนื้อฟันที่ใช้สารยึดติดยูนิเวอร์แซล เอทช์แอนด์รินส์โหมด ซิงเกิลบอนด์ ยูนิเวอร์แซล หรือ จี พรีมิโอ บอนด์ และกลุ่มที่มีการปรับสภาพเนื้อฟันด้วยสารสกัดจากเมล็ดองุ่นนาน 1 นาที หลังกัดด้วยกรดฟอสฟอริก ร่วมกับการใช้สารยึดติดดังกล่าว วิเคราะห์ข้อมูลทางสถิติด้วยการวิเคราะห์ความแปรปรวนทางเดียวและการทดสอบบองเฟอโรนี ที่ระดับนัยสำคัญทางสถิติ 0.05 พบว่า กระบวนการยึดติดเนื้อฟันด้วยสารยึดติดยูนิเวอร์แซล เอทช์แอนด์รินส์โหมด มีผลลดปริมาณแอคทีฟเอ็มเอ็มพี ชนิดที่ 2 และการปรับเปลี่ยนชีวภาพเนื้อฟันด้วยสารสกัดจากเมล็ดองุ่นยิ่งทำให้ลดปริมาณแอคทีฟเอ็มเอ็มพี ชนิดที่ 2 โดยเฉพาะอย่างยิ่ง เมื่อใช้สารยึดติด จี พรีมิโอ บอนด์ สอดคล้องกับผลจากการศึกษาตำแหน่งที่พบแอคทีฟเจลาติเนสในชั้นไฮบริดลดลงเมื่อใช้สารสกัดจากเมล็ดองุ่นก่อนการทาสารยึดติด จี พรีมิโอ บอนด์ อาจสรุปได้ว่าสารสกัดจากเมล็ดองุ่นช่วยส่งผลดีในการลดปริมาณแอคทีฟเจลาติเนส โดยเฉพาะอย่างยิ่งเมื่อใช้ร่วมกับสารยึดติดที่มีความไม่ชอบน้ำสูง แต่อย่างไรก็ตาม ยังต้องศึกษาเพิ่มเติมเกี่ยวกับกลไกการยับยั้งการทำงานของเอนไซม์ย่อยโปรตีน ผลต่อค่ากำลังการยึดติดและประสิทธิภาพของสารสกัดจากเมล็ดองุ่นในการยืดอายุของการยึดติดเนื้อฟัน

Objectives: To investigate craniofacial and dental characteristics and identify mutations of the patients affected with osteogenesis imperfecta (OI) and dentinogenesis imperfecta (DGI). Methods: Patients included in this study were diagnosed with OI. Physical, oro-dental, and radiographic examinations were performed. Mutation was identified by whole exome sequencing. Tooth characteristics including mineral density, tooth color, surface roughness, hardness, elastic modulus, mineral compositions, and ultrastructure of DGI teeth were studied compared to the controls. Objective: To investigate oro-dental characteristics and amelogenesis imperfecta (AI)-causing genomic variants associated with AI. Methods: Three Thai families affected with autosomal dominant hypocalcified amelogenesis imperfecta (ADHCAI) were recruited. Whole exome sequencing was used to identify mutations, confirmed by sanger sequencing. Clinical and radiographic features were recorded. The extracted teeth obtained from 3 patients were subjected for investigations of their properties and ultrastructure by micro-computerized tomography, surface profilometer, nano-base indentation system, EDX analysis, and SEM. Angiomyolipoma (AML) is a benign tumor that usually affects the kidney. Extrarenal AML is uncommon. Intraoral AML is extremely rare and only 13 cases have been reported in the English-language literature. Although oral AML shares the terminology with renal AML, it presents as solitary mass not associated with tuberous sclerosis complex and is negative for human melanoma marker HMB-45. This article reports a rare case of intraoral AML presenting on the hard palate of a 52-year-old male patient. Background Prostaglandin E2 (PGE2) is the main mediator for receptor activator of nuclear factor-κB ligand (RANKL)-mediated osteoclastogenesis and bone resorption in periodontitis. Bone marrow stromal cells (BMSCs) secrete PGE2 in response to pro-inflammatory cytokines, such as tumor necrosis factor-alpha (TNF-α). Curcumin, a polyphenolic compound from the turmeric plant, possesses an anti-inflammatory effect by downregulating key mediators of inflammation. Objectives The aim of this study is to evaluate the technical image quality and radiation dose of the DentiiScan 2.0 cone-beam computed tomography (CBCT) scanner before widespread clinical use. Materials and methods Image quality was measured for 4 scanning protocols of the DentiiScan 2.0, using 2 dedicated test objects: the SEDENTEXCT IQ phantom and the QRM-ConeBeam phantom. Objective To investigate the effect of adding ferrous sulphate (FeSO4) as a chemical activator to hydrogen peroxide tooth bleaching gels on color change and treatment time. Methods One hundred and twenty extracted human upper premolars, hue A or B, were selected. One enamel-dentin specimen was prepared from each tooth. The specimens were divided into twelve groups (n=10), based on bleaching product, FeSO4 and treatment time. การคืนกลับแร่ธาตุบนรอยผุจำลองระยะแรกที่ผิวเคลือบฟันที่ได้รับการเคลือบด้วยวัสดุเคลือบหลุมร่องฟันต่างชนิด: การศึกษาในช่องปาก, วิชชุดา ผดุงลาภพิสิฐ

การวิจัยนี้เป็นการศึกษาในช่องปากร่วมกับห้องปฏิบัติการ มีวัตถุประสงค์เพื่อประเมินผลการคืนกลับแร่ธาตุของวัสดุเคลือบหลุมร่องฟันต่างชนิด บนรอยผุจำลองระยะแรกที่ผิวด้านเรียบของฟัน โดยตัดชิ้นฟันจากด้านประชิดของฟันกรามน้อยจำนวน 24 ซี่ จำนวน 3 ชิ้นต่อซี่ ทำให้เกิดรอยผุจำลองระยะแรกบริเวณผิวเคลือบฟัน แบ่งชิ้นฟันเป็น 3 หน้าต่าง ได้แก่ หน้าต่างทดลอง หน้าต่างรอยผุจำลองระยะแรกก่อนการทดลอง และหน้าต่างควบคุมที่ไม่ได้เคลือบวัสดุ บริเวณหน้าต่างทดลองจะถูกเคลือบด้วยวัสดุเคลือบหลุมร่องฟันเรซินที่มีฟลูออไรด์ (เดลตัน-เอฟเอสพลัส) หรือวัสดุเคลือบหลุมร่องฟันเรซินที่ไม่มีฟลูออไรด์ (เดลตัน) หรือกลาสไอโอโนเมอร์ (ฟูจิเซเว่น) ติดชิ้นฟันบนแบร็กเกตที่ยึดกับแถบรัดจัดฟัน และสุ่มชิ้นฟันตัวอย่างติดบนฟันกรามแท้บนซี่ที่หนึ่งทั้งสองข้างของอาสาสมัครจำนวน 24 คน เป็นระยะเวลา 28 วัน มีช่วงพักการทดลอง 7 วัน หลังจากนั้นใช้ชิ้นฟันตัวอย่างที่เหลือติดในการทดลองช่วงที่ 2 ก่อนการทดลอง 7 วันจนสิ้นสุดการทดลองอาสาสมัครใช้ยาสีฟันที่มีส่วนผสมของฟลูออไรด์ วันละ 2 ครั้ง วัดค่าความหนาแน่นแร่ธาตุทั้ง 3 หน้าต่าง ด้วยเครื่องไมโครคอมพิวเตตโทโมกราฟี ภายหลังการทดลอง พบว่าวัสดุเคลือบหลุมร่องฟันทั้ง 3 ชนิด เดลตัน เดลตัน-เอฟเอสพลัส และฟูจิเซเว่น มีค่าความหนาแน่นแร่ธาตุเฉลี่ยเพิ่มขึ้นอย่างมีนัยสำคัญทางสถิติ (p=0.018 และ p=0.001 และ p=0.000 ตามลำดับ) และเดลตัน-เอฟเอสพลัสกับฟูจิเซเว่นมีค่าร้อยละการเปลี่ยนแปลงความหนาแน่นแร่ธาตุเฉลี่ยมากกว่ากลุ่มควบคุมในช่องปากอย่างมีนัยสำคัญทางสถิติ (p=0.003 และ p=0.001 ตามลำดับ) นอกจากนี้ยังพบว่ากลุ่มที่เคลือบด้วยฟูจิเซเว่น มีค่าร้อยละการเปลี่ยนแปลงความหนาแน่นแร่ธาตุเฉลี่ยสูงสุด และมีค่ามากกว่าเดลตันอย่างมีนัยสำคัญทางสถิติ (p=0.01) อย่างไรก็ตามไม่พบความแตกต่างระหว่างกลุ่มฟูจิเซเว่นกับเดลตัน-เอฟเอสพลัส อย่างมีนัยสำคัญทางสถิติ (p=0.625) จึงสรุปได้ว่า การเคลือบหลุมร่องฟันสามารถเพิ่มการคืนกลับแร่ธาตุให้กับรอยผุได้ โดยวัสดุเคลือบหลุมร่องฟันกลาสไอโอโนเมอร์ สามารถเพิ่มการคืนกลับแร่ธาตุได้ดีกว่าวัสดุเคลือบหลุมร่องฟันชนิดเรซินที่มีและไม่มีฟลูออไรด์ ตามลำดับ

การศึกษานี้มีวัตถุประสงค์หลักเพื่อศึกษาความสัมพันธ์ระหว่างระดับความแข็งเหนียวของอาหารไทยจากความรู้สึกโดยใช้แบบสอบถามเทียบกับการทำงานของกล้ามเนื้อบดเคี้ยว เมื่อพิจารณาค่าพารามิเตอร์ต่างๆ ของคลื่นไฟฟ้ากล้ามเนื้อ ในกลุ่มตัวอย่างเพศชายและหญิง อายุ 18-25 ปีของประเทศไทยที่สุขภาพดี ไม่มีปัญหาในการบดเคี้ยวอาหาร จำนวน 32 คน อาหารไทยที่มีความแข็งเหนียวแตกต่างกัน 5 ชนิด จะถูกทดสอบค่าแรงกดอาหารด้วยเครื่องทดสอบแรงกด แล้วหลังจากนั้นจึงให้กลุ่มตัวอย่างประเมินระดับความแข็งเหนียวของอาหารเหล่านี้ด้วยแบบสอบถามมาตรวัดด้วยสายตา (วีเอเอส) เทียบกับการบันทึกคลื่นไฟฟ้ากล้ามเนื้อบดเคี้ยว ผลการศึกษาพบว่าทั้งคะแนนความแข็งและความเหนียวของอาหารจากแบบสอบถาม มีความสัมพันธ์อย่างมีนัยสำคัญทางสถิติกับค่าคลื่นไฟฟ้ากล้ามเนื้อบดเคี้ยวทุกๆ พารามิเตอร์ที่ระดับนัยสำคัญ 0.05 (ค่าสัมประสิทธิ์สหสัมพันธ์เพียร์สัน อยู่ระหว่าง 0.224-0.384) นอกจากนี้ผู้วิจัยยังพบความสัมพันธ์ในระดับสูงระหว่างค่าแรงกดและคะแนนความแข็งของอาหารจากแบบสอบถาม (ค่าสัมประสิทธิ์สหสัมพันธ์เพียร์สันมีค่าเท่ากับ 0.750) โดยสรุปแล้วผู้วิจัยพบความสัมพันธ์ระหว่างการทดสอบอาหารด้วยแบบสอบถามกับการทำงานของกล้ามเนื้อบดเคี้ยวในระดับต่ำถึงปานกลาง อาจเนื่องมาจากกล้ามเนื้อบดเคี้ยวมีการปรับตัวที่ดีต่อความแข็งเหนียวของอาหาร การใช้แบบสอบถามมาตรวัดด้วยสายตาในการประเมินระดับความแข็งเหนียวของอาหาร จึงบ่งบอกถึงการทำงานของกล้ามเนื้อบดเคี้ยวได้ในระดับหนึ่ง

This study investigated the effect of curcumin on the expression of wound healing-related genes including transforming growth factor beta 1 (TGF-β1), transforming growth factor beta receptor type I (TGFβR I), transforming growth factor beta receptor type II (TGFβR II) and vascular endothelial growth factor (VEGF) in human gingival fibroblasts. The cytotoxicity of curcumin was determined by MTT assay. Then, cells were treated with non-cytotoxic concentrations of curcumin for 24 hours and the level of gene expression was determined by quantitative polymerase chain reaction (qPCR).
