Extracranial carotid artery aneurysm is a rare sequela of atherosclerosis, trauma, and conditions of chronic inflammation. Standard treatments include antiplatelet medications, open resection, or endovascular stenting. Transcarotid artery revascularization (TCAR) has become a well-recognized alternative treatment in the past few decades. This case study documents the first reported case of Horner’s syndrome post-TCAR for an internal carotid artery (ICA) aneurysm. A 68 year-old male with intermittent double vision was referred to vascular surgery clinic with brain CT angiography showing a 9mm saccular aneurysm with dissection flap of the right cervical ICA. Polypharmacy is a ubiquitous concern among patients, especially geriatric patients, who are at a higher risk of clinical burden. It is associated with an increase in healthcare costs, decreased functional status, and adverse drug events, contributing to geriatric syndromes, such as falls, cognitive decline, and urinary retention. Geriatric care is multi-tiered, requiring assistance and care from a thorough physician who is attune to the potential risks of medications that are commonly prescribed. Introduction: A majority of case reports and review studies exploring the incidence of colovaginal fistulas typically involve patients of older age with a history of both a hysterectomy and at least one episode of symptomatic diverticular disease. Background: Urinary tract infections (UTIs) are a commonly encountered diagnosis at pediatric urgent care (UC) centers. The urinalysis (UA) is usually the initial study in UC settings used to guide decisions regarding initiating empiric antibiotics and/or pursuing urine culture. However, studies in pediatric UC settings examining the ideal threshold for a positive result are lacking.

Methods: UA result data were extracted from the records of 6,327 pediatric patients, which were collected as part of a previous QI project. Logistic regression was used to determine the predictors of positive urine cultures. Decision trees for a positive UA result for both clean …

Background: Precise description of the skin is a pillar of dermatology. For dermatologists, skin pigment is akin to a ‘vital sign’ in that it represents a basic, yet illuminating, piece of information for each patient. The current method used to describe skin color is the Fitzpatrick scale. The Fitzpatrick scale was designed to measure skin burning and the scale is inadequate for precise measurement of skin color in a diverse population. Taste And Learning In The Gustatory Cortex, Martin Raymond

This research investigates the organization of taste representations in the gustatory cortex (GC) through two experiments utilizing miniscope imaging in mice. The first experiment focuses on conditioned taste aversion (CTA) and reveals that taste representations in GC are heavily dominated by palatability, with shifts in representational space occurring as a function of learning and extinction. The second experiment explores the interrelation of aversive stimuli, the impact of novelty on taste representations, and the role of somatosensory feedback. Findings suggest that familiarity and palatability emerge as positive signals with experience, shaping taste representations in GC. The study contributes to a nuanced …

Proteomics, the study of complex protein mixtures in which liquid chromatography-mass spectrometry (LC-MS) is used to analyze peptide sequences, is commonly utilized to study the double membrane, cell-derived vesicles known as exosomes. Exosomes are double membrane, cell-derived vesicles that contain cell components such as DNA, lipids, and proteins. They function to transport proteins and cellular materials between cells and are involved in the progression and metastasis of cancer. The impact of these extracellular vesicles on cancer spread can be analyzed using proteomics, through the purification of the proteins isolated from the vesicles. Background: In the United States (U.S.), falls are the leading cause of injury among adults 65 and over, resulting in 36 million falls yearly (Moreland et al., 2020). According to the Centers for Disease Control and Prevention (CDC, 2023), one in four older adults experiences a fall each year. Falls are the world's second most prominent cause of accidental deaths (World Health Organization [WHO], 2021). Falls are the leading cause of both fatal and non-fatal injuries among older adults (Moreland et al., 2020).

Purpose: This narrative review aims to summarize the evidence regarding taste manipulation as a dysphagia management strategy within the evidence-based practice framework, with a focus on clinical application for speech-language pathologists.

Method: First, we present background information on the physiology of taste and swallowing. A summary of research evidence on the effect of taste on swallowing is presented, followed by suggested methods for incorporating taste into swallowing evaluation and intervention. An overview of individual factors that can impact taste perception is provided. Finally, we discuss methods of clinical application and future directions for clinicians.

Bowel ultrasonography (BUS) is emerging as a promising noninvasive tool for assessing disease activity in inflammatory bowel disease (IBD) patients. We evaluated the diagnostic accuracy of BUS in IBD patients against the gold standard diagnostic method, standard colonoscopy.

Methods

Major databases were searched from inception to May 2023 for studies on BUS diagnostic accuracy in IBD. Outcomes of interest were pooled sensitivity, specificity, positive (PPV), and negative (NPV) predictive values. Endoscopic confirmation served as ground truth. Background: Copper (Cu), an essential trace mineral regulating multiple actions of inflammation and oxidative stress, has been implicated in risk for preterm birth (PTB).

Objectives: This study aimed to determine the association of maternal Cu concentration during pregnancy with PTB risk and gestational duration in a large multicohort study including diverse populations.

Methods: Maternal plasma or serum samples of 10,449 singleton live births were obtained from 18 geographically diverse study cohorts. Glucagon-like peptide receptor agonists (GLP-1 RA) are FDA-approved medications for the treatment of type 2 diabetes mellitus, and obesity, and are also beneficial in cardiovascular risk reduction. However, due to known GLP-1 RA adverse effects, notably delayed gastric emptying, the ASA has implemented GLP-1RA guidelines aiming to minimize potential complications related to GLP-1 RA use in patients undergoing procedures requiring general anesthesia.
