Impact Of Ehr On Security Check And Patient Outcomes, Arun Kumar Marumudi

This study aims to explore patient perspectives on how electronic health record (EHR) security protocols influence care quality and outcomes. In the context of methodology, an online survey was conducted in March 2024 with 10 participants from the USA who provided informed consent. Questions assessed experience with EHRs, security concerns, perceptions of authentication impacts, satisfaction with balancing priorities, and open-ended feedback. Descriptive analysis was used to identify key themes.

Most respondents had direct EHR exposure, which validated their viewpoints. Universal concerns co-existed with preferring digital records due to facilitated sharing when regulated appropriately. One-third reported occasional access issues correlating with …

OBJECTIVE: Elevated rates of gluconeogenesis are an early pathogenic feature of youth-onset type 2 diabetes (Y-T2D), but targeted first-line therapies are suboptimal, especially in African American (AA) youth. We evaluated glucose-lowering mechanisms of metformin and liraglutide by measuring rates of gluconeogenesis and β-cell function after therapy in AA Y-T2D.

METHODS: In this parallel randomized clinical trial, 22 youth with Y-T2D-age 15.3 ± 2.1 years (mean ± SD), 68% female, body mass index (BMI) 40.1 ± 7.9 kg/m2, duration of diagnosis 1.8 ± 1.3 years-were randomized to metformin alone (Met) or metformin + liraglutide (Lira) (Met + Lira) and evaluated before …

Electrochromic (EC) glazing has garnered significant attention recently as a crucial solution for enhancing energy efficiency in future construction and automotive sectors. EC glazing could significantly reduce the energy usage of buildings compared to traditional blinds and glazing. Impact Of Empathic Concern On Prosocial Behavior In Gain And Loss Contexts: Evidence From Event-Related Potential, Xi Luo, Taowen Yu, Min Tan, Yiping Zhong

This study employs event-related potential (ERP) to examine the impact of empathic concern on prosocial decision-making with costs in both gain and loss contexts. In this experiment, participants can choose between two types of lottery tickets and pay lottery money to help the target person gain more money or lose less money. The behavioral results showed that regardless of the context of the decision (financial loss or gain), participants tended to help individuals who had induced high empathic concern. ERP results show that compared to the low-empathic-concern condition, the high-empathic-concern condition induced greater P3 amplitude in the gain context. However, …

Studies on muscle strength and physical function after fracture are focused on short follow-ups and adjacent anatomical region. We compared loss of muscle strength and physical function in men after fracture with normal ageing-related decline. In 823 men aged 60-87, measurements of grip strength and clinical tests (chair stands, balance) were performed every 4 years for 12 years. In 155 men with incident fracture, we compared the status after vs. before the fracture. In men without fracture (controls), we compared the status on the first follow-up (4 years) vs. baseline. Frailty is associated with declines in physiological capacity across sensory, neurological, and musculoskeletal systems. An underlying assumption is that the frailer an individual, the more likely they are to experience falls and fractures. We examined whether grades of frailty can assess the long-term risk of hospitalized falls, fractures, and all-cause mortality in 1261 community-dwelling older women (mean age [SD] of 75.1 [2.7] yr) over 14.5 yr. Frailty was operationalized using a frailty index (FI) of cumulative deficits from 33 variables across multiple health domains (physical, mental, comorbidities) at baseline. The global incidence of human papillomavirus (HPV) associated head and neck carcinoma is on the rise, in response to this a tetravalent therapeutic vaccine named Qβ-HPVag was developed. This vaccine, utilizing virus-like particles (VLPs) loaded with toll-like receptor ligands and chemically coupled to four HPV16-derived peptides, demonstrated strong anti-tumor effects in a murine head and neck cancer model. Qβ-HPVag impeded tumor progression, increased infiltration of HPV-specific T cells, and significantly improved survival. The vaccine`s efficacy was associated with immune repolarization in the tumor microenvironment, characterized by expanded activated dendritic cell subsets (cDC1, cDC2, DC3). Objective: Development of clinical phenotypes from electronic health records (EHRs) can be resource intensive. Several phenotype libraries have been created to facilitate reuse of definitions. However, these platforms vary in target audience and utility. We describe the development of the Centralized Interactive Phenomics Resource (CIPHER) knowledgebase, a comprehensive public-facing phenotype library, which aims to facilitate clinical and health services research.

The retinoblastoma (RB) transcriptional corepressor 1 (RB1) is a critical tumor suppressor gene, governing diverse cellular processes implicated in cancer biology. Dysregulation or deletion in RB1 contributes to the development and progression of various cancers, making it a prime target for therapeutic intervention. RB1's canonical function in cell cycle control and DNA repair mechanisms underscores its significance in restraining aberrant cell growth and maintaining genomic stability. Chinese medicinal herbs have been found to contain bioactive phytochemicals with anti-cancer properties. Herbal extracts induce apoptosis in cancer cells by modulating levels of pro- and anti-apoptotic proteins. A previous study showed that a commonly used herb, Scutellaria barbata, significantly induced apoptosis in the breast cancer cell line MDA-MB-157 and the therapy-resistant breast cancer cell line 93B. However, the effects of two different Chinese herbs, Oldenlandia diffusa (OD) and Bryophyllum pinnatum (BP) on these breast cancer cells are not known. Bone marrow transplants are vital for those with blood cancers and disorders, but not every ethnic group has an equal chance of finding a match. Some of the human leukocyte antigen (HLA) cell markers used to match donors and recipients are unique to certain ethnic groups, making it harder for members of those groups to find a match. Chinese medicinal herbs, such as Oldenlandia diffusa, contain phytochemicals that exhibit antitumor, chemopreventive, and antiangiogenic effects in cancer cells. In this study, both Oldenlandia diffusa and Bryophyllum pinnatum (a lesser-studied herb) were tested on breast cancer cell lines MDA-MB-157 and 93A by administering an aqueous extract. The effectiveness between cell lines was also compared. The apoptotic effect both herbs showed was quantified by the levels of pro and anti-apoptotic proteins expressed in each cell line. Continuous monitoring of biomarkers at locations adjacent to targeted internal organs can provide actionable information about postoperative status beyond conventional diagnostic methods. As an example, changes in pH in the intra-abdominal space after gastric surgeries can serve as direct indicators of potentially life-threatening leakage events, in contrast to symptomatic reactions that may delay treatment. Here, we report a bioresorbable, wireless, passive sensor that addresses this clinical need, designed to locally monitor pH for early detection of gastric leakage. The Omicron variant of SARS-CoV-2, characterized by multiple subvariants including BA.1, XBB.1.5, EG.5, and JN.1, became the predominant strain in early 2022. Studies indicate that Omicron replicates less efficiently in lung tissue compared to the ancestral strain. However, the infectivity of Omicron in the gastrointestinal tract is not fully defined, despite the fact that 70% of COVID-19 patients experience digestive disease symptoms. Here, using primary human colonoids, we found that, regardless of individual variability, Omicron infects colon cells similarly or less effectively than the ancestral strain or the Delta variant. Diffuse midline glioma, H3 K27-altered (DMG-H3 K27) is an aggressive group of diffuse gliomas that predominantly occurs in pediatric patients, involves midline structures, and displays loss of H3 p.K28me3 (K27me3) expression by immunohistochemistry and characteristic genetic/epigenetic profile. Rare examples of a diffuse glioma with an H3 p.K28M (K27M) mutation and without involvement of the midline structures, so-called "diffuse hemispheric glioma with H3 p.K28M (K27M) mutation" (DHG-H3 K27), have been reported. INTRODUCTION: Surgical patients over 70 experience postoperative delirium (POD) complications in up to 50% of procedures. Sleep/circadian disruption has emerged as a potential risk factor for POD in epidemiological studies. This protocol presents a single-site, prospective observational study designed to examine the relationship between sleep/circadian regulation and POD and how this association could be moderated or mediated by Alzheimer's disease (AD) pathology and genetic risk for AD.

Swine health is critical to maintaining a profitable and productive swine operation. Biosecurity and sanitation are important practices that help maintain herd health. This study aimed to discover how biosecurity protocols and management practices differ between swine operations of different sizes. With diseases such as Porcine Reproductive and Respiratory Syndrome Virus (PRRSV), Brucellosis, and Porcine Circovirus type 2 (PCV2) being common in the swine industry, a strong biosecurity protocol protects swine farmers from financial losses associated with disease outbreaks. There has been remarkable growth in consumption of botanical dietary supplements (BDS), making it important to understand the safety profile of BDS with respect to the pharmacokinetic properties for any potential of botanical-drug interactions. One such botanical interactions which has gained significant attention involves inhibition of cytochrome P450 (CYP450) enzymes by co-administered drugs. Our study involved examining two widely used botanicals, ashwagandha and açaí for any potential inhibition of CYP450. Four different ashwagandha extracts were tested, resulting in inhibition of CYP2B6 with IC50 < 100 µg/ml of extract and showed potential of time-dependent inhibition for CYP2C9 and CYP2D6 with an IC50 ~ 200 µg/ml of extract. In the case of açaí, seven different extracts were tested and only CYP2C9 was inhibited. The acidic methanol extract of açaí formulation showed an IC50 of 0.3 µg/ml of extract indicating potent inhibition of CYP2C9, while the methanol (IC50 ~ 91.75 µg/ml) & ethanol extracts of Mountain Rose açaí powder showed weak inhibitory effect with an IC50 < 100 µg/ml of extract for CYP2C9. The results reflect that both botanical extracts showed potential of CYP450 inhibition, suggesting that compounds in BDS can prolong the half-lives of medications leading to extended action or toxicity. Next step involves, testing subfractions of these extracts to identify the compounds responsible for the observed inhibition.
