Jefferson Nursing Oral Histories

This oral history was completed with Genevieve (Jenny) Williams about her older sister Stella Jedrziewski Wawrynovic, a 1940 graduate of Jefferson's Nursing Training School.

Stella Jedrziewski Wawrynovic was born in Osceola Mills, Pennsylvania to Polish immigrants. Her parents championed the importance of education for all of their children, and so when the oldest daughter Stella graduated from high school in 1936 she moved to Philadelphia to pursue a nursing degree at Jefferson's Nursing School. She began her career at Jefferson before joining the Army during WWII to work as a nurse. After the war she returned to Jefferson, where she …

The Institute of Medicine’s (IOM) report (2010) on the “Future of Nursing” emphasized the need for nurses to lead health care change. One of the key messages in this report is a call to action for nursing schools to re-envision nursing education that focuses on a population-based perspective and emerging roles for nurses across the care continuum. Purpose:

Nursing students are unfamiliar with roles and responsibilities of their healthcare colleagues and how partnering with them can impact patient health outcomes. Many are unaware of how an interprofessional healthcare team contributes to preventing and resolving healthcare issues affecting care transition from tertiary settings back to the community. Case “Ivan” is a real lived experience that was shared to prepare nursing and allied health students for interprofessional, patient-centered care practice.

Recently vaccination has become a controversial topic. There is a growing number of people who believe that vaccines carry great health risks to patients and therefore refuse to be vaccinated or to vaccinate their children. This ill-informed view of immunizations is beginning to cause serious problems in the United States as growing numbers of disease cases are being seen. A closer look into the science of vaccines and the benefits they have brought, clearly show that not only do vaccines carry very little risk to patients, but they are responsible for the eradication and reduction of multiple debilitating diseases.

Thermal Burns And Smoke Inhalation Injuries, Anna Cox

In this pathophysiology paper, the reader is presented with a profile of an elderly patient who recently suffered thermal burns and smoke inhalation injuries as a result of a nursing home fire. This patient’s severe burns were classified as deep partial-thickness and full-thickness and her total body surface area (TBSA) of burns was over 15%. This paper details the different types of burns, the varying clinical manifestations of thermal burns, smoke inhalation injuries, laboratory values associated with burns, and the multitude of treatment necessary for each stage of burn management. Background: To address the shortage of PhD-prepared nursing faculty, universities in the United States of America offer direct entry BSN-to-PhD nursing programs. Little research has been done to explore students’ perceptions of these programs and formally evaluate the successes and opportunities for growth of this academic track. Methods: Focusing on the perceptions and experiential reflection of BSN-to-PhD education, a survey with open-ended questions was distributed among voluntary participants who are current BSN-to-PhD students or recent graduates (years) from various universities in the country. Improving Patient Satisfaction Through Reducing Nurse Overtime And Redesigning Nurse Staffing And Scheduling, Abby Romme

Increasing cardiac device patient demands within a large Midwest healthcare institution resulted in significant increases in staff shift requirements and corresponding patient complaints. The quality improvement project aimed to decrease patient complaints by executing a standard baseline daily full time equivalent (FTE) staffing strategy for future schedules, increasing nursing FTE, and maintaining institutional staffing standards. Standard calculated daily nursing requirements targeted 17.0 FTE. Implementation occurred over a three month period including schedule reprocessing, a pending incremental nursing FTE request, and a restructuring of unit based scheduling and paid time off guidelines. The completed schedule accurately captured staffing requirements for 100% …

Manager. “Clinical Outcomes Manager uses data to change practice and improve outcomes”

(AACN, 2013). With the assistance of my preceptor, lean team members, department managers,

and administration, this project will aim to reduce delayed patient transfers times between the

emergency department and inpatient units by 50%. This project goal will be achieved by writing

and setting in place a new guideline for 45 minute black-out transfer times. The anticipated goal

date for this project is set for mid-August, 2015.

Ventilator-associated pneumonia (VAP) is a common but preventable health-care associated infection that affects up to 20% of mechanically ventilated adult patients, resulting in estimated mortality rates ranging from 13% to 55% (Chahoud, Semaan, Almoosa, 2015; Melsen et al., 2013). In an effort to reduce morbidity, mortality and related costs, the Centers for Disease Control and Prevention (CDC) and the National Healthcare Safety Network (NHSN), proposed ventilator-associated pneumonia prevention as a national patient safety goal. To explore factors that influence postpartum weight self-management behaviours. Transitions Theory and the Integrated Theory of Health Behaviour Change guided selection of variables. Transition conditions, level of patient activation and social facilitation were examined for association with postpartum weight self-management behaviours.

Background

Retention of pregnancy weight increases risk of overweight and obesity later in life. Little is known about what women do to self-manage return to pre-pregnant weight and how providers can influence their behaviours.

Design

Prospective, longitudinal, correlational.

Aim: The aim of this study was to report a study protocol that examines feasibility, preliminary efficacy and satisfaction of a text message intervention on the outcome of medication adherence in adult patients prescribed oral anti-cancer agents.

Background: Administration of oral anti-cancer agents occurs in the home setting, requiring patients to self-manage the regimen as prescribed. However, many barriers to medication adherence exist: regimens are often complex, with cycling of two or more medications; side effects of treatment; most cancer patients are older with comorbid conditions and competing demands; and cognitive decline and forgetfulness may occur. In an attempt to look into ways to reduce attrition rates in nursing programs, we did a study to look into what non-academic factors can be looked at in addition to the current standard admission data. The research question of interest was “What non-academic characteristics of students can lead to best chance of success in nursing school?” Our sample included 115 students at Georgia Southern University School of Nursing, who have already completed at least one semester of nursing school. Delirium Education For Nurses On A Medical-Surgical Unit, Carole Santos

Failure to recognize delirium compromises patient safety and may lead to harmful outcomes. By June 2015, nurses on medical-surgical units will screen 100-percent of patients for delirium according to an established protocol. The project mesosystem is a 411-bed, level I trauma center and teaching hospital; the microsystem is a 28-bed medical-surgical telemetry unit.

An audit of microsystem data collected over four months in 2014 revealed that delirium was consistently underidentified (10% in September, 26% in October, 34% in November, and 22% in December), suggesting that medical-surgical nurses were not equipped to effectively perform delirium screening.

Medicare-certified hospice home care agencies must provide a 24/7 on call system for after business hours patient concerns. An agency identified a problem with a high volume of after-hours hospice calls for symptom management. How these calls are handled impact patient and family outcomes and satisfaction. A literature review provided evidence that hospice nurse education and appropriate telephone support improves symptom management, enhances family support, provides a sense of security, reduces anxiety, and promotes comfort. Complications from CABG surgery are one of the top causes of 30-day hospital readmissions. Readmissions after CABG surgery are often preventable and add to health care costs. Recent government legislation mandates penalties for hospitals with excess readmission rates. It is estimated that three million people in the United States are on oral anticoagulation therapy (warfarin). Populations studied have been found in therapeutic range 64% of the time (Pernod, Labarere, & Bosson, 2008). The low percentage of therapeutic range is attributable to (a) adherence, (b) monitoring infrequency, (c) Vitamin K antagonists, and (d) the effects of aging and comorbidities.
