Fibromyalgia (FM) is a pain disorder that involves a variety of symptoms including painful joints, fatigue, muscle stiffness, and sleep disturbances. Cognitive symptoms are also a hallmark of FM, which result in difficulties with thought articulation, concentration, and mental fatigue. There is a gap in research substantiating increased levels of sensory defensiveness symptoms in individuals with FM and the effects in daily life. Fibromyalgia is a complex and challenging disorder of unknown etiology; however the management of patients with fibromyalgia is becoming facilitated more often by Advance Practice Registered Nurses (APRN). Unfortunately, the lack of clarity in diagnosis, variances in practice guidelines, time constraints and the resources required for APRNs to adequately manage fibromyalgia can be extensive. The evidence supporting nonpharmacological strategies is strong; yet implementation of these strategies into routine care has not been sufficient.

Although the causes of fibromyalgia remain unknown, recent advances help elucidate the underlying pathophysiologies of this condition, and show that it is likely not a musculoskeletal condition, but rather a pathoneurological condition.

Objective: The objective of this systematic review is to determine whether or not Sodium Oxybate is a safe and effective treatment of pain and sleep-related symptoms of fibromyalgia.

Study design: Review of three English language primary randomized controlled trials published between 2003-2010.

Data Sources: Randomized, controlled clinical trials comparing Sodium Oxybate to a placebo group were found using PubMed and Cochrane database.

Introduction: Fibromyalgia (FMS) is a chronic pain syndrome characterized by non-restorative sleep, and fatigue. Over 75% of individuals with FMS complain of poor sleep quality and fatigue. These have been ranked by patients with FMS as having great impact on quality of life. A literature review suggested that poor sleep quality may be a predictor of increased stress and FMS symptom onset in those affected. Fibromyalgia is a female dominant chronic syndrome of diffuse muscle pain on palpation of at least 11 of 18 syndrome-associated tender points present for 3 months or longer. There is no cure. Self-care management involving multimodal integrative medicine approaches may increase treatment involvement resulting in a sense of control and pain relief. Objective The purpose of the present investigation was to determine if variation in the catechol-O-methyltransferase (COMT) and mu-opioid receptor (OPRM1) genes is associated with pain-related positive affective regulation in fibromyalgia (FM). Design Forty-six female FM patients completed an electronic diary that included daily assessments of positive affect and pain. Between- and within-person analyses were conducted with multilevel modeling. Main Outcome Measure Daily positive affect was the primary outcome measure. Abstract: Prevalence of tender points (TP), and widespread pain and fibromyalgia, as well as the relationship between TP and widespread pain and mobility, was examined in 585 community-dwelling older adults (mean age 78.2 years, 63.4% female). Pain was based on location (none, single site, multisite, widespread). Mobility was measured by the Short Physical Performance Battery (SPPB), gait speed, and self-reported (S-R) mobility difficulty. Tender-point count and health characteristics (ie, BMI, chronic conditions, analgesic use, number of medications, depression, and blocks walked per week) were assessed. Researchers studying fibromyalgia strive to identify objective, measurable biomarkers that may identify susceptible individuals, may facilitate diagnosis, or that parallel activity of the disease. Candidate objective measures range from sophisticated functional neuroimaging to office-ready measures of the pressure pain threshold. A systematic literature review was completed to assess highly investigated, objective measures used in fibromyalgia studies. To date, only experimental pain testing has been shown to coincide with improvements in clinical status in a longitudinal study. Concerted efforts to systematically evaluate additional objective measures in research trials will be vital for ongoing progress in outcome research and translation into clinical …

Urinary tract sensory symptoms provide an additional symptomatic burden for women with fibromyalgia. The urogenital symptoms of both fibromyalgia and interstitial cystitis have been described as non-infective sensory disorders, suggesting the existence of substantial clinical overlap between the two conditions. Research suggests that although interstitial cystitis has been treated as a specific bladder condition, it may be part of the disorder of central processing of sensory information as seen in fibromyalgia.

This field study examined the influence of moderate intensity training on gait patterns and work capacity of individauls with fibromyalgia syndrome (FS). FS is a chronic condition of unknown etiology, characterized by muscle tenderness, pain and stiffness and often accompanied by depression and fatigue which seems to occur primarily in middle aged females. There is no known cure for FS but treatment often includes a prescription of mild exercise. Fibromyalgia is a nonarticular rheumatic disease of unknown origin. It is diagnosed by the presence of widespread pain of more than three months duration and tenderness at 11 or more of 18 tender point sites. Typical symptoms of this syndrome are generalized pain, stiffness, and chronic fatigue due to sleep disturbances. Other uncommon symptoms include headaches, anxiety, depression, irritable bowel syndrome, and numbness. These symptoms become worse with the presence of modulating factors such as weather, stress, fatigue, overexertion, or inactivity.
