Background: The initiation of antiviral treatment for coronavirus disease 2019 (COVID-19) is based on the patient's risk of developing severe symptoms. Molnupiravir and remdesivir are new antiviral agents with limited data in real-world practice, especially in Thailand.
Objectives: This study aimed to evaluate the treatment outcomes, adverse drug reactions (ADRs), and factors associated with treatment failure in mild-moderate symptoms of adult COVID-19 who received molnupiravir or three days- intravenous remdesivir at the initiation of treatment.

Background: The gold standard for diagnosing cerebral amyloid angiopathy (CAA), full brain post-mortem examination, is rarely performed. Current diagnostic criteria, primarily based on clinico-radiological features, were developed from Western populations and may have limited applicability to Asian populations.
Objective: We aimed to evaluate the accuracy of current diagnostic criteria and examine the clinico-radiological characteristics of Thai CAA patients.

Background: Recent studies have highlighted the relationship between gut microbiome and Alzheimer’s disease (AD), revealing the ways of how gut microbial balance impacts conditions ranging from inflammatory bowel diseases to neurological disorders. The emerging field of the gut-brain-axis suggests production of microbial metabolites influences AD progression. However, there is limited understanding of molecular mechanisms involved in neurological disease when gut microbiome dysbiosis occurs. Persistent endodontic infections can endure despite technically adequate root canal treatment, reflecting a biological transition from microbial invasion to a host-driven chronic inflammatory state. This study applied a computational network biology approach to model host–microbe molecular interactions in chronic apical periodontitis and to identify key network hubs sustaining inflammation and periapical bone resorption.

Background: This systematic review of reviews and meta-analyses provides education leaders with ten years of evidence-based strategies for effective adolescent substance use prevention. Methods: Following the PRISMA 2020 protocol for systematic reviews, we searched the PubMed and PsychINFO databases on October 19, 2023, and identified 96 systematic literature reviews and meta-analyses involving adolescent drug use prevention from 2103-2023. Twenty-four papers met the criteria for final inclusion. - Occupational therapy is a cornerstone rehabilitation profession that provides client-centered care for individuals experiencing a loss of independence at any age.
- Over 90% of occupational therapy educational programs use simulation.6
- Research suggests that simulation can be used to prepare occupational therapy students for their full-time clinical placements.1,5

In this quality improvement project, I assessed a telehealth-enhanced care coordination model in a rural primary care clinic to improve chronic disease management for adults with limited access to in-person services. The issues addressed included delayed follow-up, inconsistent self-management support, and limited continuity of care, which significantly affect rural populations and health equity. The purpose of this doctoral project was to evaluate whether a structured telehealth follow-up protocol improved key care coordination outcomes, appointment adherence, follow-up timeliness, and documentation completeness for rural adults receiving chronic disease management services. Introduction: Total knee arthroplasty (TKA) is a common orthopedic procedure that is increasing in incidence and prevalence and is rising in younger populations. The Rio Grande Valley (RGV) represents a medically underserved region in the southern United States characterized by high rates of chronic conditions such as obesity, diabetes, and arthritis. Autism spectrum disorder (ASD) affects approximately 18 million individuals in India, yet the factors influencing parental care-seeking behavior were not well understood. Guided by the health belief model, the purpose of this basic qualitative study was to explore how parents’ perceived severity of a child’s symptoms, parental beliefs about the causes of autism, and perceived mental and physical health outcomes of caregiving influence parents’ intention to seek services for children aged 1–10 years diagnosed with ASD in India. Same-day outpatient procedural cancellations represent a persistent challenge for healthcare organizations, negatively affecting patient safety, access, operational efficiency, and financial performance. This Doctor of Nursing Practice (DNP) executive leadership project proposes the implementation of a nurse-led, artificial intelligence (AI)–enhanced preprocedural readiness program within an outpatient procedural department. The initiative integrates standardized nurse-led readiness checklists, AI-supported clinical decision support embedded in the electronic health record, structured nurse-led outreach, and standardized patient and staff education to proactively identify and address preventable barriers to procedural readiness. Purpose:
Misophonia and hyperacusis are distinct sound sensitivity conditions that can co-occur and are linked to heightened emotional distress and impaired functioning. This study investigated the convergence between audiometric-based and self-reported indicators of hyperacusis and examined how each related to misophonia severity and psychological outcomes in adults with misophonia.
Method:
Participants (N = 60) completed self-report measures and underwent loudness discomfort level (LDL) testing.
Results:
Analyses revealed discrepancies between self-reported and LDL-defined hyperacusis, with no significant associations between LDL-based classifications and psychological or misophonia-related outcomes. In contrast, hyperacusis identified via a standardized self-report measure was significantly …

Throughput widely affects all healthcare organizations. The patient care quality gap centers on throughput because poor flow and boarding causes overcrowding, leading to adverse outcomes and mortality. The practice-focused question was: How can the hospital throughput be improved to help prevent adverse effects when boarding in the emergency room (ER) for a long period of time? This process improvement project was implemented by multidisciplinary teams who addressed barriers in patient flow in real-time. Malaria is a highly prevalent infectious disease caused by Plasmodium parasites. Plasmodium intraerythrocytic replication leads to hemolysis-driven intermittent febrile crises in patients. In addition, the lysis of unparasitized red blood cells contributes to anemia and endotoxemia. Because thrombocytopenia is an important feature of vivax and severe falciparum malaria, we hypothesized that increased hemolysis in malaria contributes to severe thrombocytopenia by releasing endogenous and parasite toxins (ie, heme and hemozoin) capable of inducing programmed cell death in platelets. The Primary Immune Deficiency Treatment Consortium performed a retrospective analysis of 133 patients with severe combined immunodeficiency (SCID) receiving matched sibling donor (MSD) hematopoietic cell transplantation (HCT) between 1980 and 2023 at 30 North American institutions. In this largest cohort of MSD outcomes in patients with SCID to date, we examined the impact of conditioning regimen and graft-versus-host disease (GVHD) prophylaxis on survival and immune recovery. Outcomes after MSD HCT for SCID were excellent. Patients without an active infection or failure to thrive (FTT) at the time of HCT had 5-year overall survival superior to those with infection or FTT. …

The incidence of venous thromboembolism (VTE) in patients with acute lymphoblastic leukemia (ALL) receiving asparaginase-based induction is high despite primary thromboprophylaxis. Our aim was to derive and externally validate a VTE risk prediction model in patients with ALL receiving asparaginase-based induction. We conducted a multicenter retrospective cohort study of patients (aged ≥18 years) with newly diagnosed ALL receiving asparaginase-based induction. The derivation and external validation cohorts included 306 and 94 patients, respectively. Primary outcome was VTE at any site. This study presents a content analysis of 470 doctoral dissertations published between 2022 and 2024, examining emerging trends, theoretical frameworks, and research approaches related to international students. Using a qualitative content analysis approach and descriptive statistics, the study systematically analyzed the dissertations to identify recurring themes and issues. The key themes identified include teaching and learning; academic practices; acculturation and intercultural competence; career development and leadership; experiences and challenges; mobility and policy contexts; social diversity and inclusion; institutional support; and well-being. No-show and late appointment cancellations remain significant challenges in outpatient psychiatry clinics, negatively impacting continuity of care, clinic efficiency, and patient outcomes. Motivational interviewing (MI) and culturally responsive communication are evidence-based approaches that support patient engagement and understanding, yet they are not consistently incorporated into staff education related to appointment adherence. This project evaluates whether staff education on MI and culturally responsive communication increases staff knowledge and skills in educating patients about no-show and appointment cancellation policies and whether this education contributes to improved appointment attendance. Vigorous debate has erupted over the trustworthiness of scientific research findings in a number of domains. The question "what makes research findings trustworthy?" elicits different answers depending on whether the emphasis is on research integrity and ethics, research methods, transparency, inclusion, assessment and peer review, or scholarly communication. Each provides partial insight. We offer a systems approach that focuses on whether the research is accountable, evaluable, well-formulated, has been evaluated, controls for bias, reduces error, and whether the claims are warranted by the evidence. We tie each of these components to measurable indicators of trustworthiness for evaluating the research itself, …

Study objectives: GGIR is an R package for processing raw acceleration data to estimate sleep health parameters. We aimed to (1) assess the performance of three sleep algorithms within GGIR against PSG for detecting sleep/wake in clinically referred, typically-developing children (criterion validity); and (2) describe GGIR-derived sleep estimates from typically developing children enrolled in multiple cohort studies (face validity).
Methods: For criterion evaluation, children (8-16 years, N = 30) wore an actigraphy device for one night during in-lab polysomnography with performance assessed using epoch-by-epoch analyses. Nav1.7 is a voltage-gated sodium channel (VGSC) subtype predominantly expressed in sensory neurons, amplifying threshold currents. Here, we identify that Uvarigranol D (UGD) suppresses human (h) Nav1.7 with a much greater maximal inhibition than other VGSC subtypes, despite having similar apparent affinities. We demonstrate that Thr1398 determines the greater inhibitory efficacy of UGD, the leftward shift in voltage-dependence and faster inactivation kinetics of hNav1.7. UGD binds to the inactivated state, with Gln360, Ile394, Lys1395, Phe1737, and Tyr1744 being critically involved. Two closely homologous channelrhodopsins from Hyphochytrium catenoides, known as HcKCR1 (kalium channelrhodopsin 1) and HcCCR (cation channelrhodopsin), exhibit >100 times different K+/Na+ relative permeabilities and are emerging optogenetic tools for controlling neurons and cardiomyocytes. Key residue motifs and trimeric organization relate them structurally to bacteriorhodopsin-like cation channelrhodopsins (BCCRs). Here, we demonstrate that they utilize a specific gating mechanism previously suggested for the earlier-discovered BCCR from cryptophytes. Using a comparative analysis of transient absorption changes and photocurrents under single-turnover conditions, we identified an early, far-UV-absorbing photocycle intermediate that precedes channel opening in wild-type HcKCR1 and HcCCR. Objectives: This study aimed to determine associations between hypothyroidism during pregnancy and selected maternal and neonatal outcomes. Methods: A retrospective, cohort study was conducted between January 2018 and December 2020 at the two main tertiary hospitals in Muscat, Oman. The study included 408 Omani pregnant women aged 18–45 years. Participants were divided into two groups: those diagnosed with overt or subclinical hypothyroidism (n = 201; exposed group) and those with normal thyroid function (n = 207; unexposed group), matched for relevant variables.
