Transformative Experiences, Anticipatory Regret, And Informed Consent, Elizabeth Lanphier, Shannon Fyfe
Scholarly Articles
Regret risk is not consistently part of information sharing within informed consent. Yet two kinds of decisions that often invoke concerns about future regret, abortion and sterilization, raise considerations for the role of regret in clinical decision-making and informed consent, particularly regarding decisions about potentially transformative experiences. We distinguish between first-personal and second-personal anticipatory regret and argue that first-personal anticipatory regret can play a productive role, but second-personal anticipatory regret can function in ways that are pernicious. Introducing second-personal anticipatory regret into medical informed consent processes is, we argue, not only not required for informed medical decision-making, but impermissible within …

Antimicrobial resistance (AMR) poses a global health threat, severely impeding the effective treatment of bacterial infections and jeopardizing the safety of routine medical procedures. Methicillin-resistant Staphylococcus aureus (MRSA) is particularly problematic because of its resistance to beta-lactams and the ability to form resilient biofilms. Conventional antibiotics, including last-resort options, have serious side effects and may contribute to further resistance. Chitosan, a natural biopolymer, offers a promising alternative due to its biocompatibility and antimicrobial properties, though its effectiveness against biofilms is limited. This study addresses the scientific tendency to misattribute the higher levels of belief in extra-sensory perception (ESP) of adult survivors of childhood trauma to cognitive errors when they may be a result of actual experience with ESP. Dissociative tendencies may contribute to precognition, which would be an adaptive skill for children living in unpredictable and traumatic environments. This study investigated the correlation between self-reported childhood abuse and neglect and precognitive abilities in 227 adults. The participants completed the Childhood Trauma Questionnaire, comprising subscales measuring emotional, physical, and sexual abuse, and physical and emotional neglect, and completed four online tasks measuring …

Introduction: Acute ischemic stroke (AIS) results in death of brain tissue necessitating prompt intervention to restore blood flow and minimize neurological damage. Tissue plasminogen activator (tPA), specifically alteplase, has long been the standard treatment for AIS. However, recent research explores the potential advantages of tenecteplase, an alternative thrombolytic agent.
Methods: PubMed was searched, utilizing free text, randomized control trials, systematic reviews, and meta-analyses from 2019-2024. The literature search yielded four relevant results to include in this review comparing tenecteplase and alteplase, focusing on efficacy, safety, and overall clinical outcomes.

Introduction: Pre-eclampsia is a leading cause of maternal mortality worldwide. The purpose of this clinical review is to analyze the efficacy of aspirin vs placebo in the prevention of pre- eclampsia in high-risk pregnant women.
Methods: PubMed, COCHRANE, and JAMA Network were searched with terms “pre- eclampsia” AND “prevention” AND “aspirin." Operators and filters were added to find RCTs, reviews, and meta-analyses within the last five years. One article was found using article reference searching. Four articles were chosen for review.

Introduction: The anterior cruciate ligament (ACL) is one of the most common injuries in orthopedic medicine, resulting in 400,000 reconstructions every year in the United States. Bone-Patellar Tendon-Bone (BPTB) and Hamstring Tendon (HT) grafts are the most common graft types used. The objective of this review is to compare HT grafts and BPTB grafts when it comes to return to sport, returning to pre-injury level, and overall long-term functional outcomes.

Introduction: Secondary Mitral Regurgitation due to heart failure is a major cause of morbidity and mortality. There is debate about surgical versus medical treatment options. This paper aims to evaluate the efficacy of adding transcatheter mitral valve repair (TMVR) with standard treatments for heart failure.
Methods: Using Pubmed, the search terms “transcatheter mitral valve repair” AND “heart failure” resulted in 835 articles that were then filtered to <5 >years, English language, and type of article. This integrative essay reflects on member-checking and engaged research approaches in the study of exceptional human experiences (EHEs)—subjective accounts that often challenge conventional views of consciousness and physical reality. Member-checking, or inviting research participants to review data or interpretations, can improve credibility by aligning research with their lived experience. While benefits include transparency and insight, risks include confirmatory bias, emotional distress, power asymmetries, and logistical complexity. The evolving nature of EHEs further complicates participant validation. We therefore advocate a six-level "fit-for-purpose" system to balance rigor with ethical care and responsiveness. Kutob is a Filipino concept that usually refers to a sense of what may happen. It is often experienced as an omen of negative events. Despite its apparent ubiquity in everyday conversations, there is a lack of sincere academic reflection surrounding the topic of kutob. Relying on foreign (that is, western) approaches and frameworks to understand this concept may only limit it to certain categories of psychological thought. In this intrinsic case study, the author explores the value of creative expression as a process-oriented approach to transformative learning through her experience of designing and facilitating a graduate art-therapy studio workshop. The author grounds her teaching in a humanistic view of creativity and transpersonal, transformative learning theories pertaining to expressive ways of knowing. Arguably transpersonal education is still in its infancy in relation to defining the area and from this, implementing into practice. While broad themes exist, such themes tend to apply to graduate or postgraduate education. The purpose of this article is to revisit definitions of transpersonal education in relation to compulsory schooling (elementary school through to the end of secondary school). This article addresses the “why,” “what,” and “how” of teaching for transpersonal learning. Why transpersonal education is relevant for addressing modern social problems and the global ecological crisis, what it is like to teach for transpersonal learning, and how transpersonal learning becomes transformative are discussed. Three promising options in technology and consciousness studies for transpersonal learning—transpersonal media, multistate studies, and entheogenic education—are described and the challenges of introducing transpersonal education into academic settings and assessing educational (student) learning outcomes are reviewed. Patients who have had near-death experiences are often profoundly changed by the event, and they and their families can find these phenomena bewildering or even distubing. Despite this, awareness of near-death experiences appears to be minimal among health care providers in India. This cross-sectional study was conducted with 100 individuals who attend patients at the Amrita Institute of Medical Sciences in Kochi, Kerala, India, and with 100 physicians at the same institution. The COVID-19 pandemic, distinguished by both its duration and widespread impact, stands apart from other crises typically faced by schools, such as natural disasters, school violence, or isolated traumatic incidents. Background: Preoperative anesthesia evaluations of elective procedures are important to anesthesia care and imperative to prevent postoperative complications and unnecessary surgical cancellations¹. Triaging cases are complex, requiring clinical skills to facilitate the process. Triaging is cumbersome and, in most organizations, done manually. In 2021, our healthcare institution transitioned to EPIC² and formed a team of anesthesiologists, surgeons, informaticists, and EPIC analysts to automate the triage process which was nonexistent.

Breastfeeding is widely acknowledged for its significant benefits for mother and child. Despite the many advantages for breastfeeding, disparities in breastfeeding rates persist, particularly among non-Hispanic Black women in Montgomery County, OH. Rates for babies born in 2020 were low for those who were ‘ever’ breastfed (NH-Black 68.8% vs white 76.6% for ‘ever’ breastfed at hospital discharge) and even lower for those who were exclusively breastfed (NH-Black 38.2% vs. NH-white 57.9% exclusively breastfed at hospital discharge).1 Ohio's breastfeeding initiation rate remains among the lowest in the nation ranking 41st out of 50 states and falling short of the Healthy People …
