Allopurinol lowers urate production through the inhibition of xanthine oxidase. It is oxidatively hydroxylated to oxypurinol and is the most prescribed medication for gout treatment. Although it has a beneficial effect in the treatment of this common disease, like many medications, it is also known for having numerous adverse effects. Stevens-Johnson syndrome (SJS) and toxic epidermal necrolysis (TEN), diseases that exist on a spectrum, are two of the most dangerous adverse effects associated with allopurinol use. These immune-mediated disease processes involve almost every organ system. They are essential to recognize as early as possible, as they could potentially be deadly, …

A Confirmatory Factor Analysis Of The Psi-4-Sf In Parents Of Children With Autism Spectrum Disorder, Paige Weir
LSU Doctoral Dissertations
The factor structure of the Parenting Stress Index - Short Form (PSI-4-SF) was evaluated among parents of children with autism spectrum disorder (ASD). Two hypotheses were tested: (1) the original PSI-4-SF model would yield an unsatisfactory fit, and (2) a three-factor alternative model would improve fit compared to the original and other models. A sample of 205 parent-child dyads was analyzed using confirmatory factor analysis (CFA) with the lavaan package in R Studio. Preliminary analyses included descriptive statistics, normality checks, and assessments of skewness and kurtosis. Results supported the first hypothesis, showing poor fit indices for the original PSI-4-SF model. …

Effective synthesis and application of single-atom catalysts on supports lacking enough defects remain a significant challenge in environmental catalysis. Herein, we present a universal defect-enrichment strategy to increase the surface defects of CeO2-based supports through H2 reduction pretreatment. The Pt catalysts supported by defective CeO2-based supports, including CeO2, CeZrOx, and CeO2/Al2O3 (CA), exhibit much higher Pt dispersion and CO oxidation activity upon reduction activation compared to their counterpart catalysts without defect enrichment. Specifically, Pt is present as embedded single atoms on the CA support with enriched surface defects (CA-HD) based on which the highly active catalyst showing embedded …

Vitamin D is a pleiotropic hormone that regulates calcium and phosphate homeostasis. The roles of vitamin D can be divided into skeletal and nonskeletal. Its skeletal roles include promoting calcium absorption in the intestines, maintaining adequate levels of calcium and phosphate in the bloodstream, and supporting bone mineralization, thus preventing conditions such as rickets and osteoporosis. Nonskeletal roles encompass a range of functions, including immune system modulation, cardiovascular health, and cell growth regulation. Sufficient concentrations of vitamin D in the bloodstream are 20–100 ng/ml in men, women, and children, but individuals with more pigmented skin, live in environments with less …

BACKGROUND: The aetiology of lung cancer among individuals who never smoked remains elusive, despite 15% of lung cancer cases in men and 53% in women worldwide being unrelated to smoking. Epigenetic alterations, particularly DNA methylation (DNAm) changes, have emerged as potential drivers. Yet, few prospective epigenome-wide association studies (EWAS), primarily focusing on peripheral blood DNAm with limited representation of never smokers, have been conducted.
METHODS: We conducted a nested case-control study of 80 never-smoking incident lung cancer cases and 83 never-smoking controls within the Shanghai Women's Health Study and Shanghai Men's Health Study. DNAm was measured in prediagnostic oral rinse …

This review comprehensively explores the complex interplay between extracellular vesicles (ECVs)/exosomes and circadian rhythms, with a focus on the role of this interaction in hepatocellular carcinoma (HCC). Exosomes are nanovesicles derived from cells that facilitate intercellular communication by transporting bioactive molecules such as proteins, lipids, and RNA/DNA species. ECVs are implicated in a range of diseases, where they play crucial roles in signaling between cells and their surrounding environment. In the setting of cancer, ECVs are known to influence cancer initiation and progression. The scope of this review extends to all cancer types, synthesizing existing knowledge on the various roles …

Low Remnant Cholesterol And In-Hospital Bleeding Risk After Ischemic Stroke Or Transient Ischemic Attack, Zhiyuan Wu, Haiping Zhang, Yi Xu, Xia Li, Xingang Li, Lois Balmer, Xiuhua Guo, Qi Zhang, Xiang Han, Lixin Tao
Research outputs 2022 to 2026
BACKGROUND: Bleeding risk brought by intensive lipid-lowering therapy and low low-density lipoprotein cholesterol is concern-ing, while evidence regarding the relationship between remnant cholesterol and bleeding is frightening. This study aimed to investigate the association between remnant cholesterol at admission and an in-hospital bleeding event after acute ischemic stroke or transient ischemic attack (TIA). METHODS AND RESULTS: A total of 3222 eligible patients admitted to Shanghai Huashan Hospital between 2015 and 2021 with complete lipid data were analyzed. Patients were classified into low (<20.0 mg/dL), moderate (20.0–29.9 mg/dL), and high (≥30 mg/dL) groups by remnant cholesterol. The mean age of patients was 63.0± 13.1 years, including 2301 (71.4%) men and 651 (20.2%) with TIA. The median (interquartile range) of remnant cholesterol was 18.6 (13.5–25.9) mg/dL. After adjustment for confounding variables, patients with low remnant cholesterol had a higher risk of bleeding events (odds ratio, 2.56 [95% CI, 1.12–6.67]) than those with moderate remnant cholesterol. The high remnant cholesterol group was not significantly associated with bleeding risk. Combined assessment of low-density lipoprotein cholesterol and remnant cholesterol further identi-fied patients with the highest risk of bleeding events. CONCLUSIONS: Low remnant cholesterol levels were associated with bleeding events during the acute stage of ischemic stroke and TIA. The assessment of remnant cholesterol could inform the bleeding risk during hospitalization both for patients and physicians in clinical practice.

Background: Even with efforts to encourage mothers to breastfeed exclusively for the first 6 months, the majority of women are not able to undertake breastfeeding as the sole means of infant nutrition provision for various reasons. The role of occupational therapists working with mothers who breastfeed is complex and not strongly supported in the literature. Therefore, this qualitative study was designed to examine the role that occupational therapists may engage in supporting mothers and infants in the co-occupation of breastfeeding.
Method: A qualitative phenomenological study design was applied and included 11 occupational therapists working with breastfeeding dyads. The …

The Open Journal of Occupational Therapy
Background: Physical agent modalities (PAMs) are used by occupational therapists to prepare for or facilitate participation in purposeful activities. However, little has been published in the last quarter century regarding how U.S. occupational therapists use and are prepared to use of PAMs in contemporary clinical practice. In this study, U.S.-based occupational therapists were surveyed regarding their use of PAMs, preparation for use, and perceptions of that preparation.
Methods: Survey data was collected from a purposive sample of 141 U.S.-based occupational therapists and occupational therapy assistants as part of this exploratory-descriptive, mixed-methods study. Thematic and descriptive statistical analyses were …

The Open Journal of Occupational Therapy
Background. The World Health Organization recognizes that participation is important to individuals’ well being; yet, autistic children participate in fewer activities compared to neurotypical children. This article aims to describe the role of autism assistance dogs (AADs) in promoting participation and engagement in occupations and activities of daily living (ADLs) for autistic children.
Method. A qualitative interpretative phenomenological design was used. Two semi-structured interviews from a narrative perspective were conducted with each participant. Four volunteer parents and primary handlers of their child’s AAD participated in the study. Inductive analysis was used to interpret open-ended questions. Following transcription, Creswell’s …

Current Landscape Of Interventions That Promote Self-Determination In Adolescents With Intellectual And Developmental Disabilities In Secondary Transition: A Scoping Review, Chia-Yang Chiang, Tsu-Hsin Howe
The Open Journal of Occupational Therapy
Purpose: Although promoting self-determination in secondary transition is an important goal when working with adolescents with intellectual and developmental disabilities, the conceptualization and definitions of self-determination and its associated skills are not consistent. The aim of this scoping review was to examine how self-determination and its associated skills have been conceptualized and defined in interventions that support secondary transition in adolescents with intellectual and developmental disabilities and how self-determination is operationalized accordingly in those interventions.
Method: Studies published between 2006 and 2021 were searched using databases, including MEDLINE, PubMed, ERIC, PsycINFO, Education Source, and OTseeker. Hand-searching of bibliographies was …
