Background: Total femur replacement (TFR) exists as a salvage procedure option after multiple failed hip and knee arthroplasties. It can restore a patient’s quality of life and function but is associated with a high complication rate. Historically, the procedure has been performed utilizing en bloc resection of the femur. More recently, intramedullary TFR has been utilized as a method of preserving critical soft tissue attachments. More studies are needed describing intramedullary TFR as to enhance the technique and outcomes for future procedures. Granulomatosis with polyangiitis (GPA) is a necrotizing vasculitides subset of ANCA-associated vasculitis (AAVs) that involves small-sized arteries affecting multisystemic organs. Rheumatoid arthritis (RA) is a chronic autoimmune disease characterized by inflammatory polyarthritis involving the small joints. GPA and RA can have overlapping clinical presentations, including vasculitis, ocular inflammation, interstitial lung disease, and arthritis, but existing evidence indicates they are distinct conditions. Vasculitis-induced biologic TNF inhibitors have been reported (1-3), particularly cutaneous vasculitis and lupus-like syndromes. Background: Plasmablastic lymphoma (PbL) has been recognized by the World Health Organization (WHO) classification as a distinct subtype of diffuse large B-cell lymphoma (DLBCL), which are characterized with a strong expression of plasma cell markers instead of conventional B-cell markers. Although strongly associated with HIV-positive patients, it has also been commonly described in patients with solid organ transplants as well as immunocompetent patients. Plasmablastic lymphoma has shown to have an increase affinity for the oral cavity, followed by digestive tract. The Coronavirus S protein induces neutralizing antibodies, specific receptor binding, and membrane fusion between the virus and host cells. These changes are crucial to understanding the mechanism, genetic correlates, and prophylactic treatment of viral infection. In this study, the spike (S) protein structure and phylogenetic association of CCoV and FcoV were analyzed using bioinformatics. The results showed that the S proteins of CCoV and FCoV contained transmembrane regions and signal peptides. The S proteins of these coronaviruses were soluble. Cranial cruciate ligament rupture (CrCLR) and medial patellar luxation (MPL) are common problems that cause hindlimb dysfunction, lameness and pain in dogs. Although surgical intervention is recommended for these conditions, it may not be suitable for an individual with some underlying diseases. The objective of the present study was to compare the outcomes between surgical and non-surgical treatments for concomitant CrCLR and MPL in small-breed dogs. Consistent and effective blood glucose (BG) control is crucial in the treatment of diabetes mellitus (DM) and insulin resistance (IR). In this study, the surrogate indices for monitoring insulin sensitivity and the preliminary clinical approach for detecting IR were established. Fasting serum C-peptide was measured by a commercial enzyme-linked immunosorbent assay (ELISA) kit and utilized as an endogenous insulin representation. The homeostasis model assessment of insulin resistance (HOMA-IR) was then calculated. To detect the IR, the relationship between HOMA-IR and fasting serum C-peptide in each individual subject was analyzed, and designed cut-off values. The results revealed that the HOMA-IR of …

Blastic Plasmacytoid Dendritic Cell Neoplasm (BPDCN) is a rare hematologic cancer, accounting for less than 1% of acute leukemias in the U.S. Diagnosis involves detecting markers like CD123, CD4, CD56, TCL1, and TCF4. Treatment typically involved acute leukemia therapies, but Tagraxofusp, a targeted therapy, was recently approved. Despite advancements, prognosis remains grim, with a median survival of around 1 year. Atraumatic splenic rupture (ASR) is a rare complication of this condition, with only five cases reported from 1994 to 2018. Here we present a case of BPDCN complicated by ASR. Multiple myeloma (MM) is a relatively uncommon neoplastic proliferation of monoclonal plasma cells. Common manifestations are related to infiltration of plasma cells into bones and other organs, causing an increased total serum protein concentration, hypercalcemia, kidney injury, and anemia. The most common type of MM is IgG Kappa, and the second most common type is the IgA subtype. Hyperviscosity (HVS) is a rare presentation of MM and management includes prompt plasmapheresis, resulting in significant reduction of serum viscosity and symptomatic relief. We present a case of IgA Lambda MM presenting with HVS.

Metronidazole-induced encephalopathy (MIE) is a rare toxic encephalopathy. We describe a reversible MIE case in a patient with hereditary hemorrhagic telangiectasia (HHT), treated with metronidazole for brain abscess, who developed dizziness, weakness, dysarthria, and severe dysmetria. His Magnetic Resonance Imaging (MRI) brain revealed bilateral, symmetric lesions in bilateral symmetrical regions of increased intensity in the medullary olives, cerebellar dentate nuclei, and the dorsal pons, all characteristic of MIE. Upon metronidazole discontinuation, the patient experienced significant symptom improvement, with subsequent MRI showing resolution of the lesions

This meta-analysis assesses the recent Food and Drug Administration (FDA)-approved antifungal, rezafungin, for treating candidemia and invasive candidiasis— both are significant health concerns with limited treatment options. Two randomized controlled trials comparing rezafungin to caspofungin were meta-analyzed, revealing no significant differences in global cure rates and 30-day all-cause mortality. While rezafungin's unique attributes, like a novel mechanism and once-weekly dosing, may enhance patient adherence, concerns arise about its clinical relevance given the substantial investment. The study emphasizes the need for ongoing research, post-marketing surveillance, and real-world data to determine rezafungin's true value in managing these life-threatening fungal infections. บทคัดย่อ
การวิจัยนี้มีวัตถุประสงค์เพื่อการศึกษาสถานการณ์โรคไม่ติดต่อและปัจจัยที่สัมพันธ์กับการใช้บริการรักษาพยาบาลแบบผู้ป่วยนอกและผู้ป่วยใน ในประชากรไทยอายุตั้งแต่ 45 ปีขึ้นไป โดยการใช้ข้อมูลจากโครงการสำรวจด้านสุขภาพ การสูงอายุ และการเกษียณในประเทศไทย (Health, Aging, and Retirement in Thailand: HART) รอบที่ 3 ปี พ.ศ. 2564 เป็นการวิจัยเชิงวิเคราะห์แบบภาคตัดขวาง (cross-sectional analytical study) ผลการวิจัยพบว่า ปัจจัยด้านอายุ เพศ สถานภาพสมรส การศึกษา การสูบบุหรี่ สถานะสุขภาพกาย สถานะสุขภาพจิต ภูมิภาค และสิทธิหลักประกันสุขภาพ สัมพันธ์กับการเจ็บป่วยด้วยโรคไม่ติดต่อ ยกเว้นการออกกำลังกาย สำหรับการใช้บริการรักษาพยาบาล พบว่า ผู้ที่ป่วยด้วยโรคไม่ติดต่อตั้งแต่ 2 โรคขึ้นไป มีโอกาสในการใช้บริการรักษาพยาบาล จำนวนครั้ง (กรณีผู้ป่วยนอกและผู้ป่วยใน) และจำนวนวันนอนรักษาในโรงพยาบาล (กรณีผู้ป่วยใน) สูงกว่าผู้ที่ป่วยด้วยโรคไม่ติดต่อเพียง 1 โรค ปัจจัยที่สัมพันธ์กับการใช้บริการรักษาพยาบาลผู้ป่วยนอก ได้แก่ ปัจจัยด้านการออกกำลังกาย สถานะสุขภาพจิต และภูมิภาค ในขณะที่บริการรักษาพยาบาลผู้ใน พบว่า การเจ็บป่วยด้วยโรคไม่ติดต่อตั้งแต่ 2 โรคขึ้นไป อายุ และสถานะสุขภาพกาย สัมพันธ์กับการใช้บริการรักษาพยาบาลผู้ป่วยใน นอกจากนี้แล้ว การวิจัยนี้ไม่พบว่า สิทธิหลักประกันสุขภาพที่แตกต่างกันสัมพันธ์กับการใช้บริการรักษาพยาบาลทั้งผู้ป่วยนอกและผู้ป่วยในที่แตกต่างกัน ข้อค้นพบจากการวิจัยนี้สามารถนำไปใช้ประโยชน์ในการกำหนดนโยบายสาธารณสุขด้านโรคไม่ติดต่อและเพื่อพัฒนาระบบสาธารณสุขของประเทศไทยต่อไปได้

The aminoacyl-tRNA synthetases (aaRS) are a large group of enzymes that implement the genetic code in all known biological systems. They attach amino acids to their cognate tRNAs, moonlight in various translational and non-translational activities beyond aminoacylation, and are linked to many genetic disorders. The aaRS have a subtle ontology characterized by structural and functional idiosyncrasies that vary from organism to organism, and protein to protein. The social distance scale developed by Bogardus decades ago, which allows us to measure levels of acceptance among members of diverse social, ethnic, or racial groups, has been widely used in different contexts. The objective of this study is to evaluate the social distance accepted by pro-vaccine and anti-vaccine groups. Following the principle of the Bogardus scale, a questionnaire was created regarding different degrees of closeness. Respondents also completed a scientific skills self-assessment. BACKGROUND: Reductions in opioid prescribing by health care providers can lead to a decreased risk of opioid dependence in patients. Peer comparison has been demonstrated to impact providers' prescribing habits, though its effect on opioid prescribing has predominantly been studied in the emergency department setting.
OBJECTIVE: The purpose of this study is to describe the development of an enterprise-wide opioid scorecard, the architecture of its implementation, and plans for future research on its effects.

Sharing neuroimaging datasets enables reproducibility, education, tool development, and new discoveries. Neuroimaging from many studies are publicly available, providing a glimpse into progressive disorders and human development. In contrast, few stroke studies are shared, and these datasets lack longitudinal sampling of functional imaging, diffusion imaging, as well as the behavioral and demographic data that encourage novel applications. This is surprising, as stroke is a leading cause of disability, and acquiring brain imaging is considered standard of care. The first release of the Aphasia Recovery Cohort includes imaging data, demographics and behavioral measures from 230 chronic stroke survivors who experienced aphasia. …

BACKGROUND: Data dashboards are published tools that present visualizations; they are increasingly used to display data about behavioral health, social determinants of health, and chronic and infectious disease risks to inform or support public health endeavors. Dashboards can be an evidence-based approach used by communities to influence decision-making in health care for specific populations. Despite widespread use, evidence on how to best design and use dashboards in the public health realm is limited. HIV infection of CD4+ T Helper lymphocytes results in a gradual loss of immune competence, which ultimately leads to development of the acquired immune deficiency syndrome (AIDS). HIV-1 expresses two regulatory proteins, Tat and Rev, which act to upregulate viral transcription and facilitate mRNA transport enhancing the rate of translation. Inhibition of Tat and Rev have been shown to block virus replication. Therefore, Tat and Rev represent critical key points in the virus replication cycle. Rationale: Diabetic cardiomyopathy is one of the major diabetic cardiovascular complications in which fibrosis plays a critical pathogenetic role. However, the precise mechanisms by which diabetes triggers cardiac fibrosis in the heart remain elusive. Small extracellular vesicles (sEVs) play an important role in the cellular communication. Nevertheless, whether and how diabetes may adversely alter sEVs-mediated cardiomyocyte-fibroblast communication, promoting diabetic cardiac fibrosis and contributing to diabetic cardiomyopathy, has not been previously investigated. Myalgic Encephalomyelitis/Chronic Fatigue Syndrome (ME/CFS) is a complex, multifaceted disease characterized by persistent, severe fatigue lasting six months or longer that is not relieved by rest. An estimated 20 million people worldwide suffer from ME/CFS. ME/CFS has long been suspected to be a post-viral illness, as viral infections can stimulate inflammatory cytokine production. Millions of coronavirus disease 2019 (COVID-19) patients are also predicted to develop persistent symptoms that mimic ME/CFS symptomology after the resolution of the acute infection, referred to as Post-Acute Sequelae of COVID-19 (PASC). Background
Historically, tobacco companies have used flavored tobacco products to enhance the appeal of tobacco consumption, encourage initiation and experimentation of tobacco use, and contribute to sustained tobacco use. While flavored tobacco products are regulated in several countries, there is no existing regulation on flavored tobacco products in the Philippines, specifically for cigarettes and cigars. This study aims to update evidence on the flavored tobacco product landscape in the Philippines by assessing both the flavor descriptors and flavor imagery featured on cigarette and cigar packaging.

BACKGROUND: A wealth of clinically relevant information is only obtainable within unstructured clinical narratives, leading to great interest in clinical natural language processing (NLP). While a multitude of approaches to NLP exist, current algorithm development approaches have limitations that can slow the development process. These limitations are exacerbated when the task is emergent, as is the case currently for NLP extraction of signs and symptoms of COVID-19 and postacute sequelae of SARS-CoV-2 infection (PASC).

Critical care medicine has undergone significant evaluation in the 21st century, primarily driven by advancements in technology, changes in healthcare delivery, and a deeper understanding of disease processes. Advancements in technology have revolutionized patient monitoring, diagnosis, and treatment in the critical care setting. From minimally invasive procedures to advances imaging techniques, clinicians now have access to a wide array of tools to assess and manage critically ill patients more effectively. Interferon-stimulated gene-15 (ISG15) is an interferon-induced protein with two ubiquitin-like (Ubl) domains linked by a short peptide chain and is a conjugated protein of the ISGylation system. Similar to ubiquitin and other Ubls, ISG15 is ligated to its target proteins through a series of E1, E2, and E3 enzymes known as Uba7, Ube2L6/UbcH8, and HERC5, respectively. Ube2L6/UbcH8 plays a central role in ISGylation, underscoring it as an important drug target for boosting innate antiviral immunity. Depending on the type of conjugated protein and the ultimate target protein, E2 enzymes have been shown to function as monomers, dimers, or both. UbcH8 …
