Globally, prostate cancer is the second most common cancer for men. Prostate cancer patients presenting metastasis exhibit a poor prognosis, consequently leading to diminished survival rates. Thus, 68Ga-PSMA-11 PET/CT examination plays a critical role in prostate cancer diagnosis. Parametric imaging techniques offer the potential to improve the accuracy of quantification analysis in PET/CT diagnosis, but their effectiveness is constrained by lengthy acquisition protocols. ภูมิหลัง: สถาบันโรคผิวหนังได้จัดตั้งระบบให้คำปรึกษาทางไกลด้านโรคผิวหนังแก่แพทย์ทั่วไปผ่านแอปพลิเคชันไลน์ตั้งแต่ปี 2560 โดยมีการขอคำปรึกษามากกว่า 3,000 ครั้ง แต่แนวโน้มการส่งเคส กลับลดลงทุกปี วัตถุประสงค์: เพื่อศึกษาปัญหาอุปสรรคที่เกิดขึ้นจากการใช้งานระบบให้คำปรึกษาทางไกลด้านโรคผิวหนังภายใต้มุมมองของผู้มีส่วนได้ส่วนเสีย นำมาสู่การพัฒนาข้อเสนอแนะเชิงนโยบายเพื่อขับเคลื่อนการดำเนินงานให้มีประสิทธิภาพ วิธีการ: การวิจัยเพื่อการนำนโยบายไปสู่การปฏิบัติ โดยศึกษาสถิติการให้คำปรึกษาย้อนหลังการสำรวจความคิดเห็นของแพทย์ทั่วไปที่ขอรับคำปรึกษา จำนวน 215 คน การสัมภาษณ์ผู้มีส่วนได้ส่วนเสียจำนวน 45 คน และการระดมสมองในกลุ่มผู้มีส่วนได้ส่วนเสีย จำนวน 7 คน เพื่อจัดทำข้อเสนอแนะเชิงนโยบายที่เอื้อต่อการดำเนินงานของระบบให้คำปรึกษาทางไกลด้านโรคผิวหนัง จากนั้นส่งข้อเสนอแนะเชิงนโยบายให้ผู้บริหารพิจารณา และวัดผลการยอมรับต่อข้อเสนอแนะ ผล: แพทย์ในโรงพยาบาลชุมชนมีการใช้ระบบมากที่สุด ร้อยละ 57.34 โดยแพทย์ผิวหนังสามารถให้คำแนะนำได้ถึง ร้อยละ 98.85 อย่างไรก็ตามยังพบว่า แพทย์มีความกังวลด้านความปลอดภัยของข้อมูล และความไม่สมบูรณ์ของข้อมูลที่ส่งปรึกษา ซึ่งส่งผลต่อประสิทธิภาพการให้คำปรึกษา ข้อเสนอแนะที่ได้จากการระดมสมองพบว่า ควรเพิ่มการประชาสัมพันธ์เพื่อเพิ่มจำนวนผู้ขอรับคำปรึกษา ควรใช้ Platform ที่ปลอดภัยสำหรับการส่งข้อมูล ควรสร้าง Checklist สำหรับการส่งข้อมูล ซึ่งข้อเสนอทั้งหมดนี้ได้รับการสนับสนุนจากผู้บริหาร รวมถึงการพิจารณาการเบิกจ่ายค่าตอบแทนให้กับแพทย์ เพื่อรองรับการขยายการดำเนินงานในอนาคต สรุป: การมีระบบที่เป็นมาตรฐาน ปลอดภัย และตอบสนองต่อความต้องการของผู้ใช้งานได้อย่างมีประสิทธิภาพ จะช่วยให้แพทย์ทั่วไปในเขตสุขภาพสามารถเข้าถึงบริการด้านโรคผิวหนัง เพิ่มความมั่นใจในการรักษา และลดการส่งต่อผู้ป่วยได้

Objectives: Homonegativity adversely affects the health and well-being of homosexuals in society, making it vital to identify factors associated with it. This study investigates whether active membership in voluntary organizations correlates with homonegativity, examining how this varies by gender and age.Methods: Using the World Values Survey data (2017-2022) from 87,777 participants in 63 countries, we performed binary logistic regression to assess relationships between homonegativity and factors including socioeconomic status, demographics, and voluntary activity participation.Results: Our findings suggest that active membership in certain voluntary organizations correlates with homonegativity levels among both men and women across various age groups. งานวิจัยนี้เป็นการวิจัยพรรณนาเชิงภาคตัดขวาง มีวัตถุประสงค์เพื่อศึกษาความชุกความเครียดจากงาน และศึกษาปัจจัยที่มีความสัมพันธ์กับความเครียดจากงานในพนักงานของโรงเรียนแพทย์แห่งหนึ่งในประเทศไทย โดยเลือกกลุ่มตัวอย่างแบบจำเพาะเจาะจง คือ พนักงานฝ่ายปฏิบัติการของโรงเรียนแพทย์ ที่มีสำนักงานในเขตกรุงเทพมหานคร โดยเก็บตัวอย่างพนักงานโรงเรียนแพทย์ทั้งหมดโดยไม่สุ่ม จำนวน 401 คน ทำการศึกษาระหว่าง เดือน พฤษภาคม ถึง กรกฎาคม พ.ศ. 2567 แบบสอบถามประกอบด้วย ข้อมูลปัจจัยทั่วไปส่วนบุคคล ข้อมูลปัจจัยด้านการทำงาน ข้อมูลความเครียดจากงาน ประเมิน โดยแบบสอบถามที่เป็นฉบับภาษาไทย Thai Job Content Questionnaire วิเคราะห์ข้อมูลโดยใช้สถิติเชิงพรรณนา และสถิติเชิงอนุมาน ประกอบด้วยการวิเคราะห์เปรียบเทียบระหว่างตัวแปรสองตัว และวิเคราะห์การถดถอยลอจิสติกเชิงพหุ โดยกลุ่มตัวอย่างมีอัตราตอบกลับร้อยละ 55.61 ความชุกความเครียดจากงาน เท่ากับ ร้อยละ 18.45 เมื่อควบคุมตัวกวนแล้วพบว่าปัจจัยที่มีความสัมพันธ์กับความเครียดจากงานอย่างมีนัยสำคัญทางสถิติ ได้แก่ ได้แก่ ดัชนีมวลกายต่ำกว่าเกณฑ์ (aOR=2.61, 95% CI: 1.19 - 5.76) การมีหนี้สิน (aOR=2.00,95% CI: 1.08 - 3.73) ตำแหน่งอาชีพพนักงานสำนักงาน (aOR = 4.38, 95% CI: 1.10 - 17.44), การทำงานแบ่งกะ (aOR=2.37,95% CI: 1.01 - 5.54) การเรียกร้องจากงานทางกาย (aOR=2.71,95% CI: 1.18 - 6.20) อันตรายภายในงาน (aOR=3.89,95% CI: 2.05 - 7.41) ดังนั้น จึงควรจัดการแก้ไข และส่งเสริมในการป้องกันความเครียดจากงานไปสู่การป้องกันในเชิงนโยบายต่อไป

การเข้าสู่วัยหมดระดูเป็นสิ่งที่ไม่สามารถหลีกเลี่ยงได้ในชีวิตของเพศหญิง การศึกษานี้มีวัตถุประสงค์ในการศึกษาความสัมพันธ์ของอาการวัยหมดระดูและการสูญเสียผลิตภาพในการทำงาน ความชุกของการประสบอาการวัยหมดระดูในพยาบาลในกรุงเทพมหานครและปัจจัยที่เกี่ยวข้อง การศึกษานี้เป็นการศึกษาภาคตัดขวางเชิงวิเคราะห์ ศึกษาในพยาบาลอายุ 45 ถึง 60 ปีทั้งหมด ณ โรงพยาบาลแห่งหนึ่งในกรุงเทพมหานคร ข้อมูลทั่วไป ข้อมูลสูตินรีเวช ข้อมูลด้านการทำงาน ข้อมูลอาการวัยหมดระดู และข้อมูลการสูญเสียผลิตภาพในการทำงานถูกเก็บในรูปแบบของแบบสอบถาม การวิเคราะห์การถดถอยโลจิสติกและการวิเคราะห์การถดถอยโลจิสติกเชิงพหุถูกใช้เพื่อวิเคราะห์ปัจจัยที่เกี่ยวข้องกับอาการวัยหมดระดู การวิเคราะห์การถดถอยเชิงเส้นและการวิเคราะห์การถดถอยพหุคูณถูกใช้เพื่อวิเคราะห์ความสัมพันธ์ของอาการวัยหมดระดูกับการสูญเสียผลิตภาพในการทำงาน การศึกษานี้มีผู้เข้าร่วมทั้งหมด 182 คน ความชุกของอาการวัยหมดระดูอยู่ที่ 74.2% จำนวนและร้อยละของอาการวัยหมดระดูโดยแบ่งกลุ่มตามความรุนแรง ได้แก่ กลุ่มไม่มีอาการ 47 คน (25.8%) กลุ่มรุนแรงเล็กน้อย 38 คน (20.9%) กลุ่มรุนแรงปานกลาง 65 คน (35.7%) และกลุ่มรุนแรงมาก 32 คน (17.6%) จากการศึกษาพบว่าการมีอาการวัยหมดระดูในระดับรุนแรงมากมีความสัมพันธ์กับร้อยละการมาทำงานแต่ไม่ได้งานและการสูญเสียผลิตภาพในงานโดยรวมอย่างมีนัยสำคัญทางสถิติ โดยมีผลต่างค่าเฉลี่ยและช่วงค่าความเชื่อมั่น 95% อยู่ที่ 20.84 (11.10-30.59) และ 20.49 (10.39-30.58) ตามลำดับ พยาบาลที่มีอาการวัยหมดระดูในระดับรุนแรงควรได้รับการวินิจฉัย รักษา หรือช่วยเหลือในด้านต่างๆ จากทั้งแพทย์เฉพาะทาง หัวหน้าแผนก และเพื่อนร่วมงาน เพื่อป้องกันการสูญเสียผลิตภาพในการทำงาน เพิ่มประสิทธิภาพในการทำงาน ลดการเกษียณอายุงานก่อนเวลา และรักษามาตรฐานการให้บริการของโรงพยาบาลต่อไปในอนาคต

ปัจจุบันแพลตฟอร์มพาณิชย์อิเล็กทรอนิกส์มีบทบาทสำคัญในระบบสุขภาพในด้านเป็นแหล่งข้อมูลและช่องทางการเข้าถึงผลิตภัณฑ์เสริมอาหารในกลุ่มผู้บริโภคไทย แต่ขณะเดียวกันกลับพบการกระทำผิดกฎหมายเป็นจำนวนมาก วัตถุประสงค์ เพื่อพัฒนาเครื่องมือปัญญาประดิษฐ์ช่วยตรวจสอบความถูกต้องตามกฎหมายของข้อมูลผลิตภัณฑ์เสริมอาหารบนแพลตฟอร์มพาณิชย์อิเล็กทรอนิกส์ในประเทศไทย ใช้รูปแบบการวิจัยแบบผสมผสาน มีกระบวนการ 3 ระยะ ระยะที่ 1 เป็นการพัฒนาต้นแบบปัญญาประดิษฐ์ช่วยตรวจสอบความถูกต้องตามกฎหมายของข้อมูลผลิตภัณฑ์เสริมอาหารบนแพลตฟอร์มพาณิชย์อิเล็กทรอนิกส์ ระยะที่ 2 เป็นการทดสอบความไวและความจำเพาะของต้นแบบ และระยะที่ 3 เป็นการศึกษาความเห็นของผู้ใช้งานเป้าหมายต่อการนำปัญญาประดิษฐ์ไปใช้งาน ผลการวิจัย ระยะที่ 1 พบความชุกของการกระทำผิดเกี่ยวกับการจำหน่ายผลิตภัณฑ์เสริมอาหารบนแพลตฟอร์มพาณิชย์อิเล็กทรอนิกส์ที่นิยมในประเทศไทยอยู่ที่ร้อยละ 84.6 ผู้มีส่วนได้ส่วนเสียหลักมีความเห็นในการพัฒนาต้นแบบว่าควรมีการจัดการที่มาของนวัตกรรมที่น่าเชื่อถือ การเชื่อมโยงการทำงานกับผู้ให้บริการ การออกแบบให้สามารถจำแนกความผิดและรวบรวมข้อมูลส่งกลับให้ผู้เกี่ยวข้องได้ เป็นการสร้างรายได้ การสร้างแรงกดดันทางสังคม และการนำเสนอคุณค่า ในระยะที่ 2 การทดสอบต้นแบบปัญญาประดิษฐ์มีค่าความไว ร้อยละ 56.4 และความจำเพาะ ร้อยละ 89.0 ส่วนระยะที่ 3 พบว่าตัวแทนองค์ผู้ใช้เป้าหมายให้ความสนใจปัญญาประดิษฐ์ อย่างไรก็ตามควรปรับปรุงให้การจำแนกความผิดมีความซับซ้อนลดลงแต่สอดคล้องกับการนำไปใช้งานมากขึ้น กลยุทธ์ที่ควรทำคือ การสื่อสารคุณค่าของปัญญาประดิษฐ์ในส่งเสริมการค้าเชิงสร้างสรรค์ และลดความเสี่ยงทางกฎหมายมากกว่าการบังคับเชิงกฎหมาย ซึ่งองค์ความรู้ที่ได้จากงานวิจัยสามารถนำไปเป็นแนวทางเพิ่มศักยภาพการคุ้มครองผู้บริโภคในยุคดิจิทัลได้

Background: Portal venous circulating tumor cells (CTCs) detection may better reflect vascular metastasis and predict micro-metastasis risk in pancreatic ductal adenocarcinoma (PDAC) than peripheral blood. We hypothesize that portal CTCs could better represent micro-metastasis and predict survival in PDAC patients. Methods: We conducted a single-center, prospective interventional cohort study of patients with stage I-III PDAC between January 2020 and September 2021, with followed up until February 2024. Portal venous blood was obtained via endoscopic ultrasound (EUS)-guided sampling, and peripheral blood was collected on the same day. เมื่อมีการแพร่ระบาดของโรคติดเชื้อไวรัสโคโรนา 2019 ในประเทศไทยจะส่งผลต่อการไปรับการรักษาทางทันตกรรมหรือไม่เป็นเรื่องที่มีความน่าสนใจ การศึกษานี้มีวัตถุุประสงค์เพื่อให้ทราบถึงความชุกและปัจจัยที่เกี่ยวข้องกับการไปรับการรักษาทางทันตกรรมในช่วงการแพร่ระบาดของโรคติดเชื้อไวรัสโคโรนา 2019 เป็นการศึกษาเชิงพรรณนาแบบตัดขวาง ทำการเก็บข้อมูลในผู้มารับบริการที่โรงพยาบาลมหาวิทยาลัยแห่งหนึ่งโดยใช้แบบสอบถาม วิเคราะห์ข้อมูลด้วยสถิติเชิงพรรณนาและวิเคราะห์ปัจจัยที่เกี่ยวข้องด้วยสถิติ multiple logistic regression ผลการศึกษาพบว่าจากข้อมูลจำนวน 378 คน พบว่าความชุกของการไปรับการรักษาทางทันตกรรมในช่วงการแพร่ระบาดของโรคติดเชื้อไวรัสโคโรนา 2019 อยู่ที่ร้อยละ 46.0 (95% CI = 41.0, 51.1) กลุ่มที่ตอบว่าการไปตรวจฟันและขูดหินปูน/ขูดหินน้ำลายมีความสำคัญมีความสัมพันธ์กับการไปรับการรักษาทางทันตกรรมเป็น 2.38 เท่า (95% CI =1.19, 4.73) กลุ่มที่มีความกังวลหรือความกลัวในการไปรับการรักษาทางทันตกรรมมีความสัมพันธ์กับการไปรับการรักษาทางทันตกรรมเป็น 0.52 เท่า (95% CI = 0.29, 0.92) กลุ่มต้องการยกเลิกหรือเลื่อนนัดการรักษาทางทันตกรรมมีความสัมพันธ์กับการไปรับการรักษาทางทันตกรรมเป็น 0.35 เท่า (95% CI = 0.21, 0.60) และกลุ่มมีปัญหาในช่องปากที่คิดว่ามีความจำเป็นต้องไปรับการรักษาทางทันตกรรมมีความสัมพันธ์กับการไปรับการรักษาทางทันตกรรมเป็น 2.70 เท่า (95% CI = 1.66, 4.38) สรุปผลการศึกษาได้ว่า ความชุกของการไปรับการรักษาทางทันตกรรมในช่วงการแพร่ระบาดของโรคติดเชื้อไวรัสโคโรนา 2019 อยู่ที่ร้อยละ 46.0 ปัจจัยที่เกี่ยวข้องได้แก่ ทัศนคติและความรู้ด้านสุขภาพช่องปากและปัญหาในช่องปากที่คิดว่ามีความจำเป็นต้องไปรับการรักษาทางทันตกรรม

Orbital tissue fibrosis and remodeling contribute to the development of exophthalmos, a hallmark symptom of Graves’ ophthalmopathy (GO). The central pathogenesis to this process are orbital fibroblasts (GOF), which express autoantigen, thyroid-stimulating hormone receptor (TSHR). However, there are limitations in preclinical models for studying the pathogenesis and drug discovery of GO, which poses challenges for developing effective treatment options. Background: Neck pain is a common condition that is often difficult to diagnose. Previous literature has investigated diagnostic accuracy of examination measures, but the strength and clinical applicability are limited. This overview of systematic reviews aimed to investigate clinical features for diagnosing neck pain and its associated disorders.

Methods: An overview of systematic reviews was conducted searching four electronic databases for systematic reviews evaluating diagnostic criteria for neck pain. Quality and risk of bias were assessed using the AMSTAR 2 and ROBIS. Clinical features for neck pain were investigated for diagnostic utility.

OBJECTIVE: The purpose of this study was to determine the completeness of reporting of dry needling (DN) as a primary intervention, using the Template for Intervention Description and Replication (TIDieR) and Standards for Reporting Interventions in Clinical Trials of Acupuncture (STRICTA) checklists.

Despite the extensive research successes and continuous developments in modern medicine in terms of diagnosis, prevention, and treatment, the lack of clinically useful disease-modifying drugs or immunotherapeutic agents that can successfully treat or prevent neurodegenerative diseases is an ongoing challenge. To date, only one of the 244 drugs in clinical trials for the treatment of neurodegenerative diseases has been approved in the past decade, indicating a failure rate of 99.6%. Background: Clinicians and researchers often assume that symptom burden is associated with self-care management (SCM) in patients with heart failure (HF). However, that association is often not borne out in simple regression analyses and may be because another variable mediates the association. Fatalism is an appropriate candidate for mediation and is the belief that circumstances are predetermined without opportunity for control by individuals.

Objective: To construct and validate the content of a bundle to quantify vaginal blood loss after childbirth.

Methods: This is a methodological study developed from February to August 2022, divided into bibliographic survey, instrument construction and content validity, by 14 experts. The instrument for validity consisted of 11 items selected from a systematic review. For each item in the bundle, a Likert scale was applied, and to check agreement among experts, the Concordance Index was calculated. Items with agreement above 80% were considered valid. Background: Patients with heart failure (HF) must engage in self-care, yet their self-care is often poor. Cognitive function commonly is impaired in HF and is associated with poor self-care. This study investigated narrative and literacy differences between monolingual and bilingual children with Developmental Language Disorder (DLD) and examined caregivers' perspectives on their language and literacy development. This mixed methods project involved semi-structured ethnographic interviews and case histories for caregiver measures, and it included a narrative production task and a literacy skills assessment for child measures. Using data from the Medical Expenditure Panel Survey (MEPS) we estimate the fraction of the US population that is health poor. A person is health poor if their income prior to paying health insurance premiums and out-of-pocket healthcare costs is above the poverty line, but their income net-of-premiums and out-of-pocket healthcare costs is below the poverty line. Because resources used to finance health insurance premiums and out-of-pocket costs are (A) lexicographically preferred and (B) cannot be used to maintain an individual’s standard of living, they should be excluded from official poverty estimates. Objectives: To assess needle placement accuracy in the rectus abdominis (RA) muscle during dry needling (DN) without entering the peritoneum.

Methods: Two physical therapists performed DN on a cadaver, making 10 attempts each to needle the RA without entering the peritoneum. Techniques followed two common DN approaches. Ultrasound verified needle depth and safety.

Results: Seventy percent of attempts were recorded as safe needle placement, while 30% were unsafe. Accurate RA needle placement without peritoneal entry occurred in 55% of attempts.

Discussion/Conclusion: Inadvertent peritoneal needle placement during RA DN poses risks regardless of experience. Ultrasound guidance may enhance safety and precision …

Background: The COVID-19 pandemic catalyzed a rapid shift in healthcare delivery towards telehealth services, impacting patient care, including opioid use disorder (OUD) treatment. Regulatory changes eliminated the in-person evaluation requirement for buprenorphine treatment, encouraging adoption of telehealth. This study focused on understanding experiences of primary care providers in predominantly rural areas who used telehealth for OUD treatment during the pandemic. Methods: Semi-structured interviews were conducted with 22 primary care providers. Participants practiced in 13 rural and 9 urban counties in Kentucky and Arkansas.
