Purpose: Evidence suggests there is limited benefit in using injectable calcitonin for the treatment of hypercalcemia of malignancy. Based on this evidence and in the setting of increased drug costs, many institutions have opted to either restrict or remove calcitonin from their formulary. The purpose of this study was to describe the patient populations receiving treatment for hypercalcemia of malignancy and to evaluate the clinical and financial impact of injectable calcitonin being removed from formulary in an acute care setting. Abstract: Cancer associated venous thromboembolisms (VTE) are associated with higher health care resource utilization (number of hospitalizations, hospital length of stay, inpatient/outpatient medical services) and can complicate treatment of cancer. The Khorana Predictive Model for Chemotherapy-Associated VTE is a validated risk assessment model that determines a patient’s risk of developing VTEs and provides recommendations for starting prophylactic anticoagulation in ambulatory oncology patients. The Khorana Score is composed of various patient characteristics including site of primary cancer, prechemotherapy platelet count, hemoglobin level, prechemotherapy leukocyte count, and BMI. National Comprehensive Cancer Network (NCCN) supportive care guidelines recommend prophylactically starting an anticoagulant if …

Abstract: Cisatracurium is a non-depolarizing neuromuscular blocking agent used for paralysis in paitents with acute respiratory distress syndrome on mechanical ventilation. Paralysis prevents ventilator dyssynchrony and reduces patient’s work of breathing, which decreases the risk of pneumothorax. On November 1, 2023, PSVMC critical care units transitioned from fixed dosing of cisatracurium for ARDS to a weight-based, titratable dosing protocol. A single-center retrospective cohort study demonstrated cisatracurium titrated using train-of-four was associated with similar ventilatory and clinical outcomes with three folds reduction in cisatracrurium dose in comparison to fixed dose strategy. Margalefidinium polykrikoides is a mixotrophic dinoflagellate harmful algal bloom (HAB) species that blooms annually in the lower Chesapeake Bay. M. polykrikoides undertakes a diel vertical migration (DVM) which may give it a competitive advantage over purely phototrophic organisms who cannot access deeper nutrient pools and allow it to form large toxic blooms. Laboratory-based estimates of M. polykrikoides’ DVM rates suggest that it is one of the fastest known dinoflagellate swimmers and understanding this behavior is likely important for modeling and predicting M. polykrikoides blooms. This research was designed as a mixed methods study that aimed to 1) develop and validate a pesticide labeling literacy scale for Thai rice farmers. and 2) evaluate the pesticide labeling literacy level by using the scale development among 400 Thai rice farmers in the central region of Pathum Thani, Chachoengsao, Suphan Buri, Nakhon Sawan, Chi Nat, Sing Buri, Uthai Thani, and Phra Nakhon Si Ayutthaya. การศึกษานี้เป็นการศึกษาเชิงบรรยาย เพื่อศึกษาบทบาทพยาบาลผู้เชี่ยวชาญไตเทียม โดยใช้เทคนิคเดลฟาย ผู้ให้ข้อมูลคือผู้เชี่ยวชาญจำนวน 18 คน วิธีดำเนินการวิจัยประกอบด้วย 3 ขั้นตอน คือ ขั้นตอนที่ 1 สัมภาษณ์ผู้เชี่ยวชาญเกี่ยวกับบทบาทพยาบาลผู้เชี่ยวชาญไตเทียม ขั้นตอนที่ 2 นำข้อมูลที่ได้จากการสัมภาษณ์มาวิเคราะห์เนื้อหาแล้วสร้างเป็นแบบสอบถามให้ผู้เชี่ยวชาญประมาณค่าแนวโน้มความสำคัญของบทบาทพยาบาลผู้เชี่ยวชาญไตเทียม และขั้นที่ 3 นำข้อมูลที่ได้จากรอบที่ 2 มาคำนวณหาค่ามัธยฐาน ค่าพิสัยระหว่างควอไทล์ และส่งแบบสอบถามให้ผู้เชี่ยวชาญกลุ่มเดิมยืนยันคำตอบ จากนั้นจึงนำข้อมูลที่ได้มาคำนวณค่ามัธยฐาน และค่าพิสัยควอไทล์อีกครั้ง เพื่อสรุปเป็นบทบาทพยาบาลผู้เชี่ยวชาญไตเทียม ผลการวิจัยพบว่า บทบาทพยาบาลผู้เชี่ยวชาญไตเทียม ประกอบด้วย 6 บทบาท จำนวน 87 ข้อรายการ ดังนี้ 1) ด้านผู้ปฏิบัติการพยาบาลผู้เชี่ยวชาญไตเทียม จำนวน 40 ข้อ 2) ด้านผู้สอนและให้คำปรึกษา จำนวน 11 ข้อ 3) ด้านผู้ประสานงานและสื่อสาร จำนวน 7 ข้อ 4) ด้านผู้จัดการความปลอดภัยและบริหารความเสี่ยง จำนวน 10 ข้อ 5) ด้านผู้พัฒนาคุณภาพการพยาบาลการดูแลผู้ป่วยไตเทียม จำนวน 12 ข้อ และ6) ด้านผู้สร้างและใช้นวัตกรรมและเทคโนโลยี จำนวน 7 ข้อ

Indonesia experienced new outbreaks of foot and mouth disease (FMD) in 2022 after being officially free from the disease for several decades. The outbreak started in April 2022 in East Java and rapidly spread to many regions in Indonesia. This study aimed to investigate the distribution pattern and risk factors associated with the FMD outbreaks in Java Island, Indonesia, from 2022 to 2023. Descriptive, spatial, spatiotemporal, and risk factor analyses were employed to examine the outbreak dynamics and contributing factors. Lumpy skin disease (LSD) is a vector-borne viral disease of cattle caused by the Lumpy skin disease virus (LSDV). LSD is considered as an emerging infectious disease in cattle in Asia and poses a significant threat to cattle production across Africa, Europe, and Asia. This study aimed to describe the epidemiological characteristics of LSD outbreaks in beef cattle in Thailand and to conduct a molecular survey of LSDV infection in beef cattle from May 2021 to December 2022. New strategies for surface modification have been explored to promote osseointegration and osteogenesis, which are crucial factors for the success of titanium dental implants. Coating the titanium surface with mRNA encoding therapeutic growth factors may achieve these objectives. In our pilot study, we employed green fluorescent protein (GFP) mRNA as a model. Les charges imposées sur la colonne vertébrale lombaire constituent un facteur de risque important pour les maux de dos. Connaître la distribution de ces forces internes lors d’activités de travail est nécessaire autant en prévention qu’en réadaptation des travailleurs. Surface-enhanced Raman spectroscopy (SERS) is an ultrasensitive spectroscopic technique that generates signal-enhanced fingerprint vibrational spectra of small molecules. However, without rigorous control of SERS substrate active sites, geometry, surface area, or surface functionality, SERS is notoriously irreproducible, complicating the consistent quantitative analysis of small molecules. While evaporatively prepared samples yield significant SERS enhancement resulting in lower detection limits, the distribution of these enhancements along the SERS surface is inherently stochastic. Acquiring spatially resolved SERS spectra of these dried surfaces, we have shown that this enhancement is governed by a power law as a function of analyte concentration. Enterovirus genomic replication initiates at a predicted RNA cloverleaf (5′CL) at the 5′ end of the RNA genome. The 5′CL contains one stem (SA) and three stem-loops (SLB, SLC, SLD). Here, we present an analysis of 5′CL conservation and divergence for 209 human health-related serotypes from the enterovirus genus, including enterovirus and rhinovirus species. Phylogenetic analysis indicates six distinct 5′CL serotypes that only partially correlate with the species definition. The bacterial stringent response (SR) is a conserved transcriptional reprogramming pathway mediated by the nucleotide signalling alarmones, (pp)pGpp. The SR has been implicated in antibiotic survival in Clostridioides difficile, a biofilm- and spore-forming pathogen that causes resilient, highly recurrent C. difficile infections. The role of the SR in other processes and the effectors by which it regulates C. difficile physiology are unknown. C. difficile RelQ is a clostridial alarmone synthetase. Deletion of relQ dysregulates C. difficile growth in unstressed conditions, affects susceptibility to antibiotic and oxidative stressors and drastically reduces biofilm formation. Background
Cooking-related emissions contribute to air pollutants in the home and may influence children’s health outcomes.
Objective
In this pilot study, we investigate the effects of a cooking ventilation intervention in homes with gas stoves, including a video-based educational intervention and range hood replacement (when needed) in children’s homes.
Methods
This was a pilot (n = 14), before-after trial (clinicaltrials.gov #NCT04464720) in homes in the San Francisco Bay Area that had a school-aged child, a gas stove, and either a venting range hood or over-the-range microwave/hood. Chagas disease, caused by the protozoan parasite Trypanosoma cruzi, remains a neglected tropical disease and a growing health threat in the United States. Raccoons (Procyon lotor) often coexist with humans in urban and suburban areas and serve as reservoir hosts for T. cruzi. While T. cruzi is rare in humans and domestic animals in the U.S., it is common in wildlife species, particularly raccoons and Virginia opossums (Didelphis virginiana). The mammalian liver, a vital organ with complex functions, relies on a network of transcription factors to regulate gene expression. Fusion of hepatoma cells with fibroblasts often leads to gene extinction, silencing approximately 400 liver-enriched genes, including critical transcription factors such as HNF4. Previous studies revealed that ectopic expression of HNF4 in fibroblasts failed to prevent the extinction of SERPINA1, a liver-specific gene, upon subsequent fusion with hepatoma cells. Here, we sought to investigate the extent to which ectopic expression of HNF4 can reprogram fibroblast cells and prevent gene extinction in hybrid cells.

Complementary Health Approaches (CHAs) encompass a diverse range of practices which are often used both independently and alongside conventional medical treatments. Understanding how graduate students training in different fields perceive CHAs is important because these professional trainees will go on to occupy roles as healthcare practitioners, researchers, policy makers, and consumers. While bacteria play a large role in the initiation of this disease, excessive and prolonged inflammatory immune responses are fundamental for its progression, making host directed therapy (HDT) a possible and potentially valuable treatment. Purpose:
To determine the effect of local anesthesia on postoperative comfort in patients undergoing dental care under general anesthesia.
Methods:
This study was modeled as a double-blinded randomized control trial. Patients included healthy children under the age of 6 years scheduled for restorative dental treatment under general anesthesia (GA). Patient behavior was evaluated preoperatively and postoperatively using the Faces, Legs, Activity, Cry and Consolability (FLACC) or other pain score and postoperatively using the Parent’s Postoperative Pain Measure (PPPM). Purpose: Evaluate current hiring practices and employee retention characteristics in orthodontic private practices.
Methods: Orthodontists (n=1000) were surveyed regarding their hiring methods and outcomes. Responses were summarized with descriptive statistics. Chi-square tests were used to compare hiring practices and perceptions. Nonparametric Kruskal-Wallis tests were utilized in analysis of retention thresholds.
Results: Survey response rate was 7.2% (72 responses). When comparing thresholds previous to, and after the COVID-19 pandemic, there was an increase in open clinical positions compared with non-clinical and managerial positions (p=.0008). 68% of orthodontists reported a decrease in qualified applications for clinical positions compared with non-clinical and managerial …
