Inflammatory Protein Elicitation In Response To Whole-Body Vibration Exposure, Nicholas Miller

This study aimed to assess the effects of whole-body vibration (WBV) on the brain through changes in blood protein concentrations, seat and head accelerations, and symptomatology over time. Subjects were assigned to control, 1-hour, or 8-hour vibration exposure groups. Glial fibrillary acidic protein (GFAP) and S100B concentrations were measured. Root mean square average weighted vibration (Aws), vibration dose value (VDV), and seat-to-head transmissibility (STHT) were calculated. There were no differences in GFAP (p = 0.79) or S100B (p = 0.97) concentrations between 1-hour and 8-hour subjects (p = 0.79). Average weighted head acceleration (p …

The use of ultrasound-guided regional anesthesia (UGRA) in anesthetic practice is an invaluable tool for anesthesia providers to implement into their daily practice. One of these regional techniques is the erector spinae plane block (ESPB) which has anesthetic implications in numerous surgical procedures including cervical spine, thoracolumbar spine, thoracic wall, and chest wall procedures. Because of the novelty of this regional anesthetic technique, it is important to educate first-year anesthesia providers about the implications, skillset, and complications of the ESPB.

Purpose: High reliability principles and patient safety tenets emphasize the importance of preventative measures and mitigation in near-miss safety events to prevent patient harm. Near-Miss Event (NME) reporting at VCU Health was recognized as waning in the face of higher-level harm events. Misnomers in the environment led front line team members to believe that their Good Catches, acts of preventing harm from reaching the patient, did not warrant safety event reporting as they were “just doing their job.”

การวิจัยเชิงพรรณนานี้ ศึกษาสมรรถนะตามบันไดวิชาชีพของพยาบาลหอผู้ป่วยวิกฤตโรคหลอดเลือดสมอง โรงพยาบาลตติยภูมิขั้นสูง ด้วยเทคนิคการวิจัยแบบเดลฟาย ผู้ให้ข้อมูลคือ กลุ่มผู้เชี่ยวชาญจำนวน 18 คน แบ่งเป็น 5 กลุ่ม ได้แก่ 1) พยาบาลผู้เชี่ยวชาญการดูแลผู้ป่วยวิกฤตโรคหลอดเลือดสมองจำนวน 6 คน 2) หัวหน้าหอผู้ป่วยวิกฤตโรคหลอดเลือดสมองจำนวน 4 คน 3) แพทย์ผู้เชี่ยวชาญการรักษาผู้ป่วยวิกฤตโรคหลอดเลือดสมอง จำนวน 3 คน 4) พยาบาลผู้ปฏิบัติการขั้นสูงด้านอายุรศาสตร์และศัลยศาสตร์จำนวน 3 คน และ 5) อาจารย์พยาบาลผู้เชี่ยวชาญการพยาบาลผู้ป่วยวิกฤตโรคหลอดเลือดสมองจำนวน 2 คน ดำเนินการวิจัย 3 ขั้นตอน ได้แก่ 1) สัมภาษณ์เกี่ยวกับสมรรถนะพยาบาลตามบันไดวิชาชีพ ซึ่งประกอบด้วย พยาบาลระดับปฏิบัติการ ชำนาญการ ชำนาญการพิเศษ และเชี่ยวชาญ 2) วิเคราะห์ข้อมูล และสร้างแบบสอบถามเพื่อให้ผู้เชี่ยวชาญประเมินระดับความสำคัญ และ 3) คำนวณค่ามัธยฐานและค่าพิสัยระหว่างควอไทล์เพื่อสรุปผล ผลการศึกษาพบว่า สมรรถนะตามบันไดวิชาชีพของพยาบาลหอผู้ป่วยวิกฤตโรคหลอดเลือดสมอง โรงพยาบาลตติยภูมิขั้นสูง ประกอบด้วย 8 ด้าน ดังนี้ 1) การปฏิบัติการพยาบาลในระยะวิกฤต 2) การบริหารยาความเสี่ยงสูงที่ใช้ในผู้ป่วยวิกฤต 3) การป้องกันภาวะแทรกซ้อน 4) การฟื้นฟูสภาพหลังพ้นวิกฤต 5) การสื่อสารและประสานงานเพื่อการรักษาพยาบาล 6) การพยาบาลระยะท้าย 7) การใช้เทคโนโลยีและเครื่องมือดิจิทัลทางการแพทย์ และ 8) การจัดการและพัฒนาคุณภาพการดูแลผู้ป่วย ซึ่งพยาบาลที่ปฏิบัติงานในหอผู้ป่วยประกอบด้วยพยาบาลระดับปฏิบัติการ ชำนาญการ ชำนาญการพิเศษ และเชี่ยวชาญที่มีสมรรถนะทั้ง 8 ด้าน แต่สมรรถนะย่อยมีความแตกต่างกัน ดังนี้ 1) ระดับปฏิบัติการ เน้นงานพื้นฐานภายใต้การดูแล 2) ระดับชำนาญการ สามารถดูแลผู้ป่วยซับซ้อนและทำงานเป็นทีมได้ 3) ระดับชำนาญการพิเศษ สามารถแก้ปัญหาเฉพาะหน้าและใช้หลักฐานเชิงประจักษ์ และ 4) ระดับเชี่ยวชาญ สามารถพัฒนาองค์ความรู้ใหม่ทำให้คุณภาพการดูแลผู้ป่วยดีขึ้น การศึกษานี้สามารถใช้เป็นแนวทางในการประเมินสมรรถนะและมอบหมายงานตามความสามารถของพยาบาลแต่ละระดับ

Adolescent drug use has consistently been a concern for researchers and policymakers for decades as its early onset and continued use can lead to a variety of negative life outcomes. Concerns related to marijuana use have emerged in recent years as states have begun to change laws regarding recreational cannabis use. The current study uses data from a cohort of students in Massachusetts to understand how these changes may impact marijuana use. Only a fraction of individuals living with serious mental illness (SMI) receive adequatetreatment. This study investigates members’ experiences of a virtual adaptation of the Clubhouse model, a psychosocial rehabilitation program. During the COVID-19 lockdowns, the virtual Clubhouse, referred to as virtual programming, was established to provide an alternative to the traditional brick-and-mortar Clubhouse. This qualitative inquiry utilizes a phenomenological approach to understand participant experiences of the virtual model, including factors related to motivation, recovery, and impact. Leishmaniasis is a neglected tropical disease caused by the protozoan parasite of the genus Leishmania. Presently no vaccine against Leishmaniasis is available, so there is a need for new anti-leishmanial treatments, including an effective preventive vaccine. We focus on developing vaccine candidates based on leishmania’s cell surface carbohydrate antigens. Saccharides heavily envelop the cell surface of Leishmania, lipophosphoglycan (LPG) becoming a significant glycan in most leishmania species. Its intrinsic antigenicity makes this polysaccharide a useful target for developing conjugated vaccine candidates against all leishmaniasis variants. A subarachnoid hemorrhage (SAH) is a type of stoke where a blood vessel bursts and causes the subarachnoid space to become filled with blood. Traditional nursing management of SAH includes close monitoring of blood pressure goals in order to prevent complications such as vasospasm. Vasospasm causes delayed cerebral ischemia and neurological deficits secondary to the initial injury. Recent research has expanded understanding of the underlying mechanisms behind the neuroinflammatory cascade that occurs after a SAH. In October 2020, a new population of invasive brown treesnakes (Boiga irregularis) was discovered on the 33-ha Cocos Island, 2.5 km off the south coast of Guam, United States. Cocos Island is a unique conservation resource, providing refuge for many lizards and birds, including endangered species, which were extirpated from mainland Guam by invasive predators including brown treesnakes. We sought to evaluate the usefulness of toxic baiting with acetaminophen-treated carrion baits and cage trapping, common tools for the control of brown treesnakes on mainland Guam, as potential eradication tools on Cocos Island. North American populations of Black Vultures Coragyps atratus have increased and expanded their distribution in the southern and eastern USA. In conjunction with these patterns has been a rise in human–vulture conflicts. To improve our understanding of space use patterns and better inform management, we evaluated the movements of Black Vultures (n = 23) in the southeastern USA using a long-term GPS tracking database. Our specific objectives were to: (1) quantify home-range sizes in relation to season and geographical study location and (2) examine within-home-range resource selection to identify landscape and anthropogenic factors influencing roost and diurnal space use. …

Antipredator Blood-Squirting Defense In Horned Lizards (Phrynosoma): Chemical Isolation Of Plasma Component(S), Pogonomyrmex Ant Dietary Origin, And Evolution, Wade C. Sherbrooke, Bruce A. Kimball

North American horned lizards of the genus Phrynosoma are largely myrmecophagous and squirt systemic blood from circumocular sinuses during encounters with certain mammalian predators. The predators react with distinct revulsion. Using bioassay-guided fractionation of blood plasma, we endeavored to identify the source and characteristics of the active chemical compound(s) responsible for this negative oral response. Our results of coyote bioassays and a series of novel mouse bioassay experiments were largely concordant. The active compound(s) was likely plasma-borne with a molecular weight (mw) of between 800 and 1,600. To identify the source of the active compound(s), we noted bioassay responses …

Feral swine (Sus scrofa) were targeted for and successfully eradicated from Saint Vincent Island (SVI), a National Wildlife Refuge (NWR) along the coast of Florida’s panhandle to protect its habitats and uncharacteristically high diversity of wildlife species for barrier islands in the region, including federal and state-listed threatened and endangered species. The eradication effort was initiated in early 2015 and concluded in 2019. A total of 438 feral swine were removed from the Island, 417 by federal control experts and 21 by recreational hunters. Temperature is a primary factor influencing organismal development, and the fluctuating daily and seasonal thermal regimes of temperate climates may challenge the ability of viviparous reptiles to optimize body temperatures during gestation. Testing how viviparous reptiles navigate highly variable thermal conditions (e.g., relatively cold nights and/or highly fluctuating temperatures) is a powerful way to understand how they use microhabitats for thermoregulatory benefits. Globalization has led to the frequent movement of species out of their native habitat. Some of these species become highly invasive and capable of profoundly altering invaded ecosystems. Feral swine (Sus scrofa × domesticus) are recognized as being among the most destructive invasive species, with populations established on all continents except Antarctica. Within the United States (US), feral swine are responsible for extensive crop damage, the destruction of native ecosystems, and the spread of disease. Migratory birds exhibit seasonal geographic range (hereafter, range) dynamics during the annual cycle. Few studies have examined how migratory birds select their habitats for range occupancy at the species level and space use at the individual level simultaneously. We hypothesized that environmental variables directly related to fitness components would affect the range occupancy probabilities of migrants, whereas environment variables related to movements and flights would affect the space use intensities of migrants.
