Discharge Implementation Project: Ascertaining If Discharge Implementation Orders Overall Improves Discharge Metrics At A Texas Hospital, Zachary Paras, Omar Nawab, Komthorn Cunvong, Jackson V. Littlejohn, Sathish Karmegam
North Texas GME Research Forum 2024
There are several factors preventing discharges out of the hospital within 24 hours that ultimately affects metrics for both hospital systems and physicians. These factors include pending labs, imaging, or procedures; pending placement approval at a facility outside of the hospital upon discharge; notification of family and caretakers; secured funding for devices and medicines; and others. In an attempt to mitigate preventable extra days spent in the hospital once a patient is medically ready for discharge, a Texas hospital implemented a discharge order set for providers to utilize that informs all involved parties including case management, PT/OT, social work, nurses, …

Mentoring The Mentor: A Mentor Training Program, Sheri Gillis
Regis University Student Publications (comprehensive collection)
Mentorship has played an important role in nursing and nursing education for decades. However, there have yet to be any standard qualifications or training programs for those who mentor others. Poor mentorship increases faculty turnover, stress, and poor student outcomes. A quality improvement initiative was implemented in the Associate Degree Nursing Program using an evidence-based approach. Using a pre-post survey design with convenience sampling, five (N=5) nursing faculty met the qualifications to participate in the mentor training program. The Mentor Efficacy (MES) survey, which consisted of 30 questions, was administered to measure the participants' feelings of self-efficacy in the mentoring …

Peptide receptor radionucleotide therapy (PRRT) with 177Lu-dotatate is widely used for the treatment of patients with neuroendocrine tumors (NETs). We analyzed data from 104 patients with NETs treated with 177Lu -dotatate at a US academic center between December 2017 and October 2020 to better understand patterns of long-term efficacy, safety, and toxicity in the real-world setting. 177Lu-dotatate (200 mCi) was administered every eight weeks for four doses. The most common sites of primary disease were small intestine NETs (n = 49, 47%), pancreatic NETs (n = 32, 31%), and lung NETs (n = 7, 7%). Twenty-seven percent had Ki-6720%. The …

Understanding The Importance Of Administrative Mental Health Support For Frontline Nurses Post Covid-19, Sarah Mah
Nursing | Senior Theses
During the peak of the COVID-19 pandemic, intensive care units and critical care units were overwhelmed with the number of patients that were on the floors. It was during this time that nurses were experiencing the most mental health decline.
Several factors can be associated with the rapid decline of nurses’ mental health. However, these issues could ultimately stem from the lack of action taken by healthcare administration. This includes increased workload and the lack of PPE, resources institutional support. Because these units played such a vital role during the pandemic, it is imperative that healthcare administrators do what they …
Reimagining Equitable Care: Simulation-Based Education And The Dismantling Of Implicit Bias And Stigma Of Vulnerable Populations In Hospitals, Vanessa Smith
Nursing | Senior Theses
Background: Simulation-based education consists of using high-fidelity mannequins and equipment to provide a safe space for healthcare professionals to practice crucial skills within healthcare. Used within hospitals and schools, simulation most commonly surrounds practicing hands-on skills such as central line insertion, IV insertion, Ultrasound-guided procedures, code blue response, etc. However, a large portion of healthcare that is overlooked is tackling issues within patient-provider relationships. Simulation can help dismantle the negative behaviors and feelings providers push onto patients through the use of standardized patients and role-playing. By using these methods, simulation can reduce providers’ personal implicit biases and the stigma associated …

Factors That Influence The Prevalence Of Food Allergy In Children Born In Industrialized Countries Vs. Developing Countries: A Literature Review & Feasibility Study, Dimitri Miguel Lacson Torres
Nursing | Senior Theses
Background
Food allergy (FA) is known to be more prevalent in industrialized countries compared to developing countries. FA is a chronic disease that can cause a range of symptoms from regional urticaria to lethal anaphylaxis. FA affects 10% of the general population and is more common in children, affecting 8% of children versus 3% of adults (Seth, D. et al., 2020). It is unclear why food allergy is more common in industrialized countries versus developing countries. Research Question What are the influential factors that affect the increasing incidence of food allergy in toddlers born in industrialized countries versus those that …

BACKGROUND: Cannabis is a risk factor in the onset and persistence of psychotic disorders. There is concern that non-medical cannabis legalization in Canada may have population-level impacts on psychotic disorders. We sought to examine changes in health service use and incident cases of psychotic disorder following cannabis legalization, during a period of tight restrictions on retail stores and product types.
METHODS: We conducted a cross-sectional interrupted time-series analysis using linked population-based health administrative data from Ontario (Canada) from January 2014 to March 2020. We identified psychosis-related outpatient visits, emergency department visits, hospitalizations, and inpatient length of stay, as well as …

This article addresses the “why,” “what,” and “how” of teaching for transpersonal learning. Why transpersonal education is relevant for addressing modern social problems and the global ecological crisis, what it is like to teach for transpersonal learning, and how transpersonal learning becomes transformative are discussed. Three promising options in technology and consciousness studies for transpersonal learning—transpersonal media, multistate studies, and entheogenic education—are described and the challenges of introducing transpersonal education into academic settings and assessing educational (student) learning outcomes are reviewed. The relevance of Mezirow’s Transformation Theory to transpersonal educational psychology, with special attention to ecological perspective transformation, is then …

Role Of Insulin-Like Growth Factor-1 Signaling In Diet-Enhanced Bone Elongation, Cassaundra A. White
Theses, Dissertations and Capstones
Childhood obesity has increased precipitously to the point that almost 20% of children between the ages of 2 and 19 have been diagnosed with obesity. While obesity is a complex metabolic disorder, one contributing factor is diet, particularly a diet high in fat. Among many problems associated with diet-induced obesity, the impact on skeletal health is often overlooked. In addition to increased stress on the skeleton due to excess weight, obesity also disrupts the endocrine and immune systems that, in turn, can secondarily alter skeletal growth. Obesity during the early years of childhood is particularly detrimental because the skeleton is …

Determining The Need For 23-Hour Observation Following Transforaminal Lumbar Interbody Fusions: A Patient Perspective On Recovery At Home, Lacey Irias
Doctor of Nursing Practice Projects
Minimally invasive surgery (MIS) for transforaminal lumbar interbody fusion (TLIF) is becoming more frequently performed in ambulatory surgery centers (ASCs). Subsequently, patients are scheduled for an option to undergo a 23-hour observation which requires staff preparation. The preparation increases costs and creates a shortage during a standard daytime shift which can reflect negatively on patient care. Most patients elect to be discharged the same day, on account of experiencing minimal pain and seeking the comfort of their own home. The goal of this DNP project was to assess the necessity of scheduling a patient 23 hours for observation following a …
