Measures Of Sleep Are Not Routinely Captured In Trials Assessing Treatment Outcomes In Knee Osteoarthritis - A Scoping Systematic Review And Call To Action, Jessica Feda, Tyler Miller, Jodi L. Young, Brett Neilson, Daniel I. Rhon

Objective

To identify and map the extent to which trials for pain interventions in individuals with knee osteoarthritis (OA) track measures of sleep, characterize the type of sleep measure assessed, and assess their influence on pain-related effect sizes.

Design

A scoping review was conducted, searching seven bibliometric databases from 2000 to 2022. We included all randomized controlled trials with a primary purpose of assessing non-surgical pain management interventions for adults with knee OA. All non-surgical interventions and any comparator or control were included. Demographic data were pooled from all trials.

Results

926 trials conducted in 61 countries met eligibility. Nineteen …

The incidence of empyema is increasing and associated with a mortality rate of 20% in patients older than 65 years. Since 30% of patients with advanced empyema have contraindications to surgical treatment, novel, low-dose, pharmacological treatments are needed. A Streptococcus pneumoniae-induced rabbit model of chronic empyema recapitulates the progression, loculation, fibrotic repair, and pleural thickening of human disease. Treatment with single chain (sc) urokinase (scuPA) or tissue type (sctPA) plasminogen activators in doses 1.0–4.0 mg/kg were only partially effective in this model. This article describes efforts within the All of Us Research Program to integrate research participants into governance and operational decision-making roles. Through intentional engagement strategies, participants were recruited to serve on steering and executive committees and contribute to program oversight. Evaluation of the initiative included a survey measuring researcher readiness for participant engagement across the consortium. Over a three-year period, survey responses demonstrated increased support for participant involvement and recognition of the value of participant perspectives in shaping research priorities. This study investigates the self-efficacy of undergraduate and graduate TAs associated with changes in instruction after the COVID-19 disruption to the spring 2020 semester.

Guiding research questions include: (i) What are the relationships among TA teacher efficacy and stress resulting from the COVID-19 disruption? (ii) What experiences do TAs describe that contribute to their stress and teacher efficacy?

Implications of the results for supporting TAs during similarly stressful events and circumstances are discussed.

Rural communities in the state of Ohio face barriers to care that affect the health of the population. This paper investigates the extent to which the general state of health, life expectancy, uninsured rates, and access to primary care providers can be affected by living in a rural community and how these variables have changed over time. Data from urban and rural counties in Ohio, taken from 2016 to 2022 via the 2022 County Health Rankings, will be used to explore health-related concerns. Objective: The object of this paper is to investigate the relationship between rurality and the national top five causes of death in Kentucky counties.

Methods: We used Spearman correlations to quantify the relationship between percent rurality and the crude death rate for heart disease, malignant neoplasia, unintentional injury, chronic lower respiratory disease, and cerebrovascular disease for each Kentucky county. We also used a Spearman correlation to compare and correlate percent rurality and mortality due to heart disease as these two factors changed between data from 2000 and 2010.

Loop diuretics have been investigated for managing postpartum hypertensive disorders, but there is currently insufficient clinical evidence pointing to any single agent. This study aims to compare blood pressure (BP) outcomes for patients with gestational hypertensive disorders receiving labetalol alone versus labetalol + furosemide during postpartum hospital stay. The primary aim was to learn if loop diuretic incorporation in conjunction with labetalol could lower the need for additional anti-hypertensive agents.

Aim: The aim of this study is to examine the prevalence of multimorbidity among middle-aged adults in the Atherosclerosis Risk in Communities Study at baseline by examining the demographic and socioeconomic characteristics of the participants.

Methods

I included 15,698 participants from the Atherosclerosis Risk in Communities (ARIC) baseline visit. Participants were randomly selected from 4 different locations in the United States. Multimorbidity was defined as the prevalence of 2 or more chronic conditions in an individual. The purpose of this study was to implement and examine a new surveillance system for Kentucky State Police’s forensics unit to track trends of illicit drugs and drug trafficking, and to see if the implementation of this new system was associated with a decrease in overall drug counts over a period of five months. An extensive review of the literature showed that the drug epidemic in the United States continues to worsen, with public health and law enforcement institutions leading the fight in attempting to end the epidemic, but little evidence showing these two joining forces to work together.

Objectives: To identify differences in demographic and contextual characteristics of suicide mortality among healthcare support workers (HCSWs) and to estimate annual suicide rates among HCSW in the United States from 2003-2019 using the National Violent Death Reporting System (NVDRS) and the United States Bureau of Labor Statistics (BLS).

Methods: Demographic variables of healthcare support worker suicide decedents were collected using the NVDRS. Odds ratios for circumstantial risk factors by sex were calculated. Crude rates for healthcare support worker suicide decedents were calculated and compared to the U.S. general population suicide crude rates.

Nationally representative data have indicated that less than one in ten individuals with a substance use disorder (SUD) have received recent treatment, suggesting a need for research to examine potential barriers to SUD services. Although research has been conducted to identify some barriers to treatment, there is little research focused on gender differences. To address this gap in research, the present study aimed to examine gender differences in barriers to entering SUD treatment, as well as differences in factors that may impact decisions to enter treatment.

Three-dimensional convolutional neural networks (3D CNNs) have been widely applied to analyze Alzheimer’s disease (AD) brain images for a better understanding of the disease progress or predicting the conversion from cognitively impaired (CU) or mild cognitive impairment status. It is well-known that training 3D-CNN is computationally expensive and with the potential of overfitting due to the small sample size available in the medical imaging field. This article describes the creation and outcomes of simulation as Level I Fieldwork for entry level graduate occupational therapy students. The simulations were created by a team of interprofessional educators following the International Nursing Association for Clinical Simulation and Learning best practices. Additionally, the simulations were designed to meet the student learning outcomes of Level I Fieldwork. Students participated in eight high-fidelity simulations and were evaluated with self-ratings, peer ratings, and faculty ratings. Implicit (unconscious) gender bias and stereotypes can affect clinical decisions and interactions between healthcare professionals, as well as impact careers. However, there is no research exploring the implicit gender bias or stereotypes of occupational therapy students or practitioners. Therefore, the aim of this study was to examine the gender-career bias of occupational therapy students. This Article argues that, even if courts are unpersuaded by the broadest arguments in favor of a public health approach to regulation of addictive design, they should nonetheless reject the platforms’ efforts to make addictive design a public-health-law-free zone. The public health and internet paradigms can be reconciled as a policy matter because addictive design threatens both public health and innovation online. This Article explores the interaction between addiction and liberty and identifies a firm legal basis for recognition of a fundamental constitutional right to freedom from addiction. Government interferes with freedom from addiction when it causes addiction or restricts addiction treatment, and government may protect freedom from addiction through legislation empowering individuals against private actors’ efforts to addict them without their consent. In Dobbs v. Jackson Women’s Health Organization, the Supreme Court overruled Roe v. Wade and Planned Parenthood of Southeastern Pennsylvania v. Casey. These two cases held that the Due Process Clause of the Fourteenth Amendment encompassed a right of women to terminate a pregnancy. Roe reflected over 60 years of substantive due process precedent finding and reaffirming a constitutional right of privacy with several animating themes, including bodily integrity, equality, and dignity. Red tilapia (Oreochromis sp.) is a prominent freshwater fish in Thai aquaculture. It has long been afflicted by severe bacterial diseases, ulcerative syndrome, and hemorrhagic septicemia, primarily caused by Aeromonas veronii. Vaccination is a key strategy for control and prevention. In this study a chitosan nano-immersion vaccine has been developed using a nanoemulsion technique and surface modification method with chitosan polymers. The physicochemical properties, mucoadhesive properties, immune response assays, and vaccine efficacy were investigated. The results showed that the vaccine is spherical-shaped, nanometer-sized, and having monodispersed positive charge. Streptococcus suis serotype 2 (S. suis 2) is an important porcine pathogen that causes diseases in both swine and humans. For rapid S. suis 2 identification, a novel latex agglutination test (LAT) based on heavy-chain variable domain antibody (VH) was developed. Firstly, the soluble 47B3 VH antibody fragment from a phage display library, in which cysteine residues were engineered at the C-terminus, was expressed in Escherichia coli. The purified protein was then gently reduced to form monomeric soluble 47B3 VH subsequently used to coat with latex beads by means of site-specific conjugation. ประชากรโลกมีอายุสูงขึ้นอย่างรวดเร็ว การพัฒนาการจัดบริการสุขภาพปฐมภูมิด้านโรคผิวหนังในผู้สูงอายุจึงมีความสำคัญและมีความจำเป็น เพื่อให้ผู้สูงอายุเข้าถึงการวินิจฉัยและรักษาได้อย่างมีประสิทธิภาพอย่างต่อเนื่องและเท่าเทียมกัน การศึกษานี้ใช้กระบวนการวิจัยเชิงปฏิบัติการแบบมีส่วนร่วม (Participatory action research; PAR) โดยศึกษาองค์ประกอบ, ระบบงาน, เส้นทางการมารับบริการ, ความจำเป็นทางสุขภาพและความคาดหวังของผู้สูงอายุในการมารับบริการสุขภาพปฐมภูมิด้านโรคผิวหนัง โดยทำกิจกรรมสนทนากลุ่ม, การสัมภาษณ์เชิงลึก, การสำรวจด้วยแบบสอบถาม แล้วนำผลไปออกแบบแนวทางการพัฒนาบริการสุขภาพปฐมภูมิด้านโรคผิวหนัง ผลการศึกษาพบว่า การพัฒนาบริการสุขภาพปฐมภูมิด้านโรคผิวหนังควรประกอบด้วย 8 สิ่งสำคัญ คือ 1. การคัดกรองผู้ป่วย 2. การอธิบายการวินิจฉัยรักษาและมียาเพียงพอ 3. การพัฒนาศักยภาพของบุคลากรสาธารณสุข 4. การส่งต่อผู้ป่วย 5. การปรึกษาและเครือข่ายแพทย์ผิวหนัง 6. การนัดหมาย 7. การดูแลตนเองและการดูแลจากอาสาสมัครสาธารณสุขประจำหมู่บ้าน 8. สถานที่และอุปกรณ์เพียงพอ จากการสำรวจความเห็นของหน่วยบริการปฐมภูมิ 12 เขตสุขภาพและได้รับการตอบกลับจากโรงพยาบาลอำเภอและโรงพยาบาลส่งเสริมสุขภาพตำบล 397 แห่งจาก 576 แห่ง (อัตราการตอบกลับ 68.9%) มีความเห็นว่า มีความเป็นไปได้ในการทำวิธีการพัฒนาด้วย PAR 71%, มีประโยชน์ 96% และน่าจะทำได้ในระยะเวลา 6-12 เดือน ส่วนรูปแบบการพัฒนาการจัดบริการสุขภาพปฐมภูมิด้านโรคผิวหนัง มีความเป็นไปได้ 82%, มีประโยชน์ 98% และน่าจะทำได้ในระยะเวลามากกว่า 12 เดือน สรุปผลการศึกษาได้ว่า การพัฒนาการจัดบริการสุขภาพปฐมภูมิด้านโรคผิวหนังมีความจำเป็นและหน่วยบริการปฐมภูมิส่วนใหญ่ลงความเห็นว่าการพัฒนาการจัดบริการสุขภาพปฐมภูมิด้านโรคผิวหนังมีความเป็นไปได้และมีประโยชน์

ประเทศไทยและทั่วโลกมีประชากรประสบภาวะซึมเศร้าเพิ่มขึ้นทุกปี มีจำนวนมากที่ไม่สามารถเข้าถึงบริการสาธารณสุขได้จึงเริ่มมีการนำเทคโนโลยีปัญญาประดิษฐ์มาช่วยค้นหาภาวะซึมเศร้าพร้อมแยกระดับความรุนแรง การศึกษานี้มีวัตถุประสงค์เพื่อพัฒนาเทคโนโลยีปัญญาประดิษฐ์ Application DMIND ประเมินความเชื่อมั่น ความเที่ยงตรง และความถูกต้องในการคัดกรองภาวะซึมเศร้าในประชากรไทย เป็นการศึกษาแบบ Cross-sectional เป็น Research and development โดย มี 2 ระยะ คือ ระยะที่ 1 เป็น การพัฒนา DMIND Questionnaire ที่เตรียมนำเข้าไปใช้กับแอพลิเคชั่น มีการศึกษาความเชื่อมั่น ความเที่ยงตรงและความถูกต้องของแบบสอบถาม ระยะที่ 2 เป็นการศึกษาความเที่ยงตรงและความถูกต้องของ Application DMIND เทียบกับ HDRS-17 Thai ระยะที่ 1: ผู้เข้าการศึกษาทั้ง 81คน ถูกประเมินด้วยแบบสอบถาม DMIND ตามด้วยแบบประเมินภาวะซึมเศร้า Hamilton Depression Rating Scale (HDRS-17 Thai) ผลการศึกษาพบว่า แบบสอบถาม DMIND แสดงให้เห็นถึงคะแนนความถูกต้องของเนื้อหาสูง โดยมี I-CVI และ S-CVI ที่ 1.0 เครื่องมือยังแสดงให้เห็นถึงความสอดคล้องภายในที่สูง Cronbach's alpha coefficient = 0.96 การวิเคราะห์ ROC แสดง AUC = 0.88 ซึ่งบ่งบอกถึงความแม่นยำสูงในการคัดกรองภาวะซึมเศร้า คะแนนตัดที่เหมาะสมที่ 11.5 ระยะที่ 2: เป็นการศึกษาความถูกต้องและเที่ยงตรงของ Application DMIND เทียบกับ HDRS-17 Thai โดยมีผู้เข้าร่วมงานวิจัย ที่มีคุณสมบัติตามเกณฑ์ จำนวน 388 ราย ใช้แอปพลิเคชัน DMIND แล้วถูกประเมินต่อด้วย HDRS-17 Thai ผลการศึกษาพบว่า แอปพลิเคชัน DMIND มีระดับความสอดคล้องกับ HDRS-17 Thai สูง 87.53 …

The deleterious health consequences associated with alcohol consumption, ranging from addiction and liver disease to accidents and mental health disorders, have been extensively documented. This study aimed to assess the efficacy of an educational program utilizing Facebook and Line platforms in mitigating alcohol consumption intentions among such workers in Bangkok. Utilizing a randomized controlled trial methodology, the study focused in Bangkok. A total of forty-four participants were allocated into either the intervention (n=22) or control (n=22) groups.
