Dyskalemias are often seen in children with chronic kidney disease (CKD). While hyperkalemia is common, with an increasing prevalence as glomerular filtration rate declines, hypokalemia may also occur, particularly in children with renal tubular disorders and those on intensive dialysis regimens. Dietary assessment and adjustment of potassium intake is critically important in children with CKD as hyperkalemia can be life-threatening. Manipulation of dietary potassium can be challenging as it may affect the intake of other nutrients and reduce palatability. The Pediatric Renal Nutrition Taskforce (PRNT), an international team of pediatric renal dietitians and pediatric nephrologists, has developed clinical practice recommendations …

Parkinson's disease (PD) is a highly prevalent and irreversible neurodegenerative disorder that is typically diagnosed in an advanced stage. Currently, there are no approved biomarkers that reliably identify PD patients before they have undergone extensive neuronal damage, eliminating the opportunity for future disease-modifying therapies to intervene in disease progression. This unmet need for diagnostic and therapeutic biomarkers has fueled PD research for decades, but these efforts have not yet yielded actionable results. Recently, studies exploring mechanisms underlying PD progression have offered insights into multisystemic contributions to pathology, challenging the classic perspective of PD as a disease isolated to the brain. …

Background
Arm care programs consisting of upper extremity strengthening and stretching have been recommended for injury prevention for pitchers of all ages. There has been no investigation into high school baseball coaches’ usage and perceptions of arm care programs to mitigate physical impairments associated with injuries in baseball players.

The purpose of this study was to investigate the current usage of arm care programs by high school baseball coaches. Following cardiovascular injury, neonatal mammalian hearts are capable of restoring damaged tissue while adult hearts cannot. Adults are hindered by their inability to replenish lost cardiomyocytes resulting in increased scar tissue formation and reduced cardiovascular function. The Hippo pathway and its downstream effector Yap1 have recently been identified as key modulators of cardiac development and regeneration in neonates. We investigated the role of Yap1 in a sheep model of cell-based cardiovascular repair in which regenerative neonatal cardiovascular progenitor cells (CPCs) were introduced following LAD ligation. Mice have been increasingly used as preclinical model to elucidate mechanisms and test therapeutics for treating intervertebral disc degeneration (IDD). Several intervertebral disc (IVD) histological scoring systems have been proposed, but none exists that reliably quantitate mouse disc pathologies. Here, we report a new robust quantitative mouse IVD histopathological scoring system developed by building consensus from the spine community analyses of previous scoring systems and features noted on different mouse models of IDD. The new scoring system analyzes 14 key histopathological features from nucleus pulposus (NP), annulus fibrosus (AF), endplate (EP), and AF/NP/EP interface regions. Each feature is categorized and …

Transgender and nonbinary (TNB) people, more than ever, are in need of and actively seeking out services in order to cope with the reality of being transgender in the United States. This includes requiring assistance from local and federal governmental systems that do not provide protections for equal housing, workplace discrimination, and healthcare provisions, much less hate crime investigation and prosecution. In 2011, 75% of transgender people were or had been in therapy in the past, with a further 14% intending to participate in therapy in the future. Schizophrenia is an exceptionally debilitating mental disorder. Its effects permeate all facets of psychosocial functioning and it is associated with premature mortality. As such, suicide is of clinical concern. Positive symptoms such as hallucinations and delusions may influence the risk of suicide among individuals diagnosed with schizophrenia, but additional research is necessary, as the limited literature on this topic is dated and includes heterogeneous results. Early recognition of and intervention for suicide risk factors, such as the presence of positive symptoms of psychosis, may reduce mortality rates for individuals with schizophrenia. The prevalence of hearing loss increases with age, with age-related hearing loss (ARHL) being one of the most prevalent forms of sensory decline in older adults. Hearing loss is often overlooked in medical settings including palliative and hospice care. Screening for hearing loss in these settings is rare as is formal staff training on assessing and managing hearing loss in palliative and hospice care. An evidence-based toolkit for integrating audiologists into end-of-life care protocols is presented. Aporphine alkaloids, belonging to the isoquinoline class of compounds, have been investigated as a potential source of ligands for Central Nervous System (CNS) receptors. Previous research indicates that the aporphine scaffold may be manipulated to synthesize selective ligands for serotonin and dopamine receptors. Novel aporphine alkaloids containing C10 nitrogen substitutions were synthesized, and their affinities were evaluated at serotonin (5-HT1A, 5-HT1B, 5-HT2A, 5-HT7A) receptors and dopamine (D1, D2, D3, D4, and D5) receptors. Two series of racemic aporphine compounds with C10 nitrogenous functionalities were synthesized and analyzed at the aforementioned receptors. Adaptive regulation of fear is dependent on successful fear extinction learning; therefore, investigating factors that both enhance and diminish fear extinction learning is a critical line of research. In the present study, we induce mild anticipatory anxiety during fear extinction learning in an attempt to modulate how participants extinguish fear memory. In the experiment, we apply a classic three-day fear learning protocol to both control participants (N = 20) and an experimental group (N = 20) with fear acquisition, fear extinction, and fear recovery phases; each phase is separated by a period of 24 hours and we use a skin …

Objective: This systematic review aims to evaluate whether deafness has a significant effect on one’s health literacy skills, and, if there is a difference between health literacy of a normal hearing individual compared to that of a Deaf individual. The credibility and usability of lay physical activity promotion material are a persistent problem (Thomas & Cardinal, 2020, TJACM). Anti N-methyl-D-aspartate receptor (NMDA)receptor encephalitis is one of the most common type of auto immune encephalitis. Its diagnosis is often delayed due to initial clinical presentation with psychiatric clinical features. Initial clinical features resemble those of acute psychosis or depression which later evolve into frank neurological dysfunction. This was first described in association with ovarian teratoma, but it was later also found to exist without neoplastic association and in men as well. Autoimmune and paraneoplastic encephalitides are a group of neurologic disorders that present with cognitive decline, neuropsychiatric symptoms, movement disorders, or seizures. These diseases can present before, with, or after the diagnosis of malignancy. We report two cases of GABA-B encephalitis from Pakistan. The first patient was a 67-year-old gentleman presented with cognitive decline and neuropsychiatric symptoms. His CSF analysis showed increased protein levels and GABA-B receptor antibodies. He was treated with pulse steroids, which improved his symptoms. The second patient was a 34-year-old lady with cognitive decline and psychomotor symptoms. Her serum autoimmune antibody panel showed antibodies against GABA-B receptors. …

Determine if hypomagnesemia & hypocalcemia on admission was associated with ICH and outcome.
Magnesium is a vascular smooth muscle relaxant and appears to provide protection against cellular injury in experimental stroke models. The implication of low serum magnesium in primary ICH is not well defined.
All patients with a primary ICH presented to PIMS, Islamabad prospectively analyzed in a pilot study. Demographics, initial lab values, ICH location, Mg+2 level, NIHSS & MRS at presentation were recorded. All patients with INR <= 1.5 were included in this study. We took normal serum magnesium in our study was 1.8 - 2.5 mEq/dL. Statistical significance was determined using linear regression adjusting for admission systolic blood pressure (SBP).

Parenting is considered imperative in the development of juvenile's cognitions, and beliefs. The parent-child relationship might significantly influence juvenile’s thought patterns and social communications. Different parenting styles perhaps indicate criminal involvement of juveniles, resultantly turns them into delinquent. The present study was done to discover whether parental authority styles moderate the path that links criminal thinking styles to criminal social identity amongst juvenile delinquents. Valproate is one of the oldest known anti-epileptic drugs (AED) in practice which was discovered in 1881 and later in 1962 was approved for treating epilepsy. It is indicated in most of the seizure types including generalized onset (motor, non-motor and myoclonic), focal onset, unknown onset and unclassified. Valproate has other indications in migraine prophylaxis, and bipolar disorder.

Neurological deterioration after acute ischemic stroke (AIS)is a common occurrence, leading to increased mortality, morbidity, and poor quality of life among stroke survivors. Recognition of the individuals at a greater risk for neurological deterioration may contribute to making early decisions and monitoring the therapy. Neurological deterioration arises in up to 40% of subjects after AIS during hospitalization and is followed by neurological and functional debility. Numerous predictors have been devised that predict this progression. Migraine is considered as the second leading cause of disability worldwide, most commonly seen in young women. Skipping meals, dehydration and fasting are among many triggering factors which have been described in literature as a cause of increased frequency and severity of migraine attacks. The underlying pathophysiology, prevention and management of fasting induced migraine has been documented, however, there is a lack of randomized controlled trials for the same. This review article discusses these important aspects in the light of published literature and gives directions on future areas of research.

Juvenile dermatomyositis or JDM is a rare but debilitating pediatric disease that affects the proximal muscles and skin, as implied by its name. Although rare, it is the most common childhood idiopathic inflammatory myopathy (1). The estimated incidence is 3.2 cases per million children (2). The diagnosis is based on clinical signs and symptoms supported by investigations including, but not limited to, muscle biopsy, muscle enzymes and EMG studies. Bohan and Peter’s criterion is most widely used for diagnosis. As the world struggles to deal with the COVID-19 pandemic, this article talks about the experience and findings of the first COVID-19 screening facility in a tertiary care hospital in Karachi, Pakistan. Department of Family Medicine at Aga Khan University (AKU) took initiative where COVID-19 screening of stable patients first began in a dedicated area. The primary focus of this manuscript comprises three sections. Initially, we introduce the concept of a simplified intuitionistic neutrosophic soft set. We impose an intuitionistic condition between the membership values of truth and falsity such that their sum does not exceed unity. Similarly, for indeterminacy, the membership value is a real number from the closed interval [0, 1]. Hence, the sum of membership values of truth, indeterminacy, and falsity does not exceed two. Aim
Electronic cigarette (e-cigarette) use among Irish teenagers has risen significantly. In 2019, prevalence of current use (last 30 days) among 15–17-year-olds was 17.3%. We examine social determinants of adolescent e-cigarette current use.
Subject and methods
A stratified random sample of 50 schools in Ireland was surveyed in 2019, part of the European School Survey Project on Alcohol and other Drugs (ESPAD), with 3495 students aged 15, 16, and 17. Bivariate and multivariable logistic regression [providing adjusted odds ratios (AORs)] analyses were performed using Stata version 16.
