การเปรียบเทียบการรับความรู้สึกของไส้ตรงระหว่างผู้ป่วยที่มีความผิดปกติในการขับถ่ายร่วมกับมีและไม่มีภาวะรีบเร่งเข้าห้องน้ำเมื่อปวดถ่ายอุจจาระโดยใช้เครื่องบาโรแสตท, สลิล สมุทรรังสี

ความสำคัญและที่มา: ภาวะรีบเร่งเข้าห้องน้ำเมื่อปวดถ่ายอุจจาระสามารถพบได้ในประชากรทั่วไปแต่จะมีอุบัติการณ์เพิ่มมากขึ้นในผู้ป่วยที่มีการขับถ่ายอุจจาระที่ผิดปกติไม่ว่าจะเป็นภาวะกลั้นอุจจาระไม่ได้ หรือ ภาวะท้องผูก แต่อย่างไรก็ตามยังไม่มีการศึกษาที่ครอบคลุมกลไกในการเกิดภาวะรีบเร่งเข้าห้องน้ำเมื่อปวดถ่ายอุจจาระ วัตถุประสงค์: เพื่อศึกษาระดับการรับความรู้สึกของไส้ตรงในผู้ป่วยที่มีความผิดปกติในการขับถ่ายร่วมกับมีภาวะรีบเร่งเข้าห้องน้ำเปรียบเทียบกับผู้ป่วยที่มีความผิดปกติในการขับถ่ายแต่ไม่มีภาวะรีบเร่งเข้าห้องน้ำโดยใช้เครื่องบาโรสแตท ระเบียบวิธีการวิจัย: งานวิจัยนี้มีการเปรียบเทียบข้อมูลพื้นฐาน คือ เพศ อายุ ดัชนีมวลกาย ซึ่งไม่แตกต่างระหว่างอาสาสมัครที่ไม่มีความผิดปกติในการขับถ่าย และผู้ป่วยที่มีความผิดปกติในการขับถ่ายร่วมกับมีและไม่มีภาวะรีบเร่งเข้าห้องน้ำ โดยนิยามของภาวะรีบเร่งเข้าห้องน้ำคือ มีภาวะรีบเร่งเข้าห้องน้ำ อย่างน้อย 1 ครั้ง/สัปดาห์ ในช่วง 3 เดือนที่ผ่านมา ระดับความรู้สึกของไส้ตรงแบ่งออกเป็น 8 ระดับ คือ 0 ไม่มีความรู้สึกในไส้ตรง 1 รู้สึกภายในไส้ตรงแต่ไม่ใช่ลมและอุจจาระ 2 รู้สึกว่ามีลมในไส้ตรง 3 รู้สึกอยากผายลม 4 รู้สึกว่ามีอุจจาระในไส้ตรงแต่ไม่ปวดอยากถ่าย 5 รู้สึกปวดอยากถ่ายอุจจาระ 6 รู้สึกต้องรีบเข้าห้องน้ำแต่สามารถกลั้นอุจจาระได้ 7 รู้สึกปวดอยากถ่ายมากไม่สามารถกลั้นอุจจาระได้ โดยใช้เครื่องบาโรสแตทในการตรวจวัดระดับการรับความรู้สึกของไส้ตรงและค่าความยืดหยุ่นของผนังไส้ตรง แบ่งออกเป็น 2 ขั้นตอน คือ การตรวจโดยเพิ่มความดันครั้งละ 4 มม.ปรอท เริ่มต้นที่ 4 มม.ปรอท คงไว้ที่ 1 นาที จากนั้นปล่อยออก มีระยะเวลาห่างกัน 1 นาที และเพิ่มความดันขึ้นเรื่อย ๆ จนกระทั่งถึง 50 มม.ปรอท และขั้นที่ 2 ตรวจโดยการเพิ่มความดันขึ้นเรื่อย ๆ จาก 4 มม.ปรอท ทุก 1 นาที โดยไม่มีการปล่อยลมออกจนกระทั่ง 50 มม.ปรอท ผลการวิจัย: ผู้ป่วยที่มีภาวะรีบเร่งเข้าห้องน้ำมีจำนวนการขับถ่ายต่อสัปดาห์มากกว่าผู้ป่วยที่ไม่มีภาวะรีบเร่งเข้าห้องน้ำเฉลี่ย 7 (4-14) vs 5(2-7) นอกจากนี้ผู้ป่วยที่มีภาวะรีบเร่งเข้าห้องน้ำมีลักษณะของอุจจาระที่เหลวมากกว่า พบว่ามีโรคประจำตัวคือลำไส้แปรปรวน ชนิดท้องเสีย และกลั้นอุจจาระไม่ได้มากกว่าผู้ป่วยที่มีไม่มีภาวะรีบเร่งเข้าห้องน้ำ ผู้ป่วยที่มีภาวะรีบเร่งเข้าห้องน้ำจำนวน 8 ราย (57.14%) ไม่มีความรู้สึกปวดอยากถ่ายอุจจาระ แต่จะมีความรู้สึกต้องรีบเข้าห้องน้ำทันทีหลังจากที่รู้สึกว่ามีอุจจาระในไส้ตรงซึ่งแตกต่างจากผู้ป่วยที่ไม่มีภาวะรีบเร่งเข้าห้องน้ำซึ่งพบเพียง 2 ราย (14.25%) และ อาสาสมัครที่ไม่มีความผิดปกติในการขับถ่ายซึ่งพบเพียง …

ที่มาของงานวิจัย: ภาวะทุพโภชนาการเป็นปัจจัยเสี่ยงที่สำคัญต่อการเสียชีวิตในผู้ป่วยล้างไตทางช่องท้องที่มีภาวะเยื่อบุช่องท้องอักเสบ ข้อมูลของการให้โปรตีนเสริมในผู้ป่วยที่มีภาวะติดเชื้อเยื่อบุช่องท้องยังค่อนข้างจำกัด ระเบียบวิธีวิจัย: เป็นการศึกษาแบบการทดลองหลายแหล่งวิจัยแบบเปิดที่ศึกษาในศูนย์ล้างไตทางช่องท้องทั้งหมด 9 แห่งในประเทศไทย ที่ศึกษาผู้ป่วยล้างไตทางช่องท้องที่อายุมากกว่าหรือเท่ากับ 18 ปีที่ล้างไตทางช่องท้องและได้รับการวินิจฉัยว่ามีภาวะติดเชื้อเยื่อบุช่องท้อง ร่วมกับมีระดับแอลบูมินในกระแสเลือดต่ำกว่า 3.5 กรัม/ดล. และคัดผู้ป่วยที่ติดเชื้อจากวัณโรค เชื้อรา ติดเชื้อแบบทุติยภูมิ หรือมีภาวะติดเชื้อในกระแสโลหิตรุนแรงออกจากการศึกษา โดยทำการสุ่มผู้ป่วยในอัตรา 1:1 เพื่อเข้าสู่กลุ่มศึกษา และกลุ่มควบคุม ในกลุ่มที่ศึกษาผู้ป่วยจะได้รับโปรตีนเวย์ปริมาณ 30 กรัม/วัน เป็นระยะเวลา 30 วัน โดยที่กลุ่มควบคุมจะไม่ได้รับโปรตีนเสริม ผู้ป่วยในทั้งสองกลุ่มจะได้รับคำแนะนำทางโภชนาการจากนักกำหนดอาหารตามแนวทางการดูแลผู้ป่วยติดเชื้อเยื่อบุช่องท้อง โดยศึกษาผลลัพธ์ของการศึกษาหลักเป็นผลลัพธ์รวมของการเสียชีวิตจากภาวะติดเชื้อเยื่อบุช่องท้องและการติดเชื้อเยื่อบุช่องท้องซ้ำแบบ relapsing และ repeat และผลลัพธ์ของการศึกษารองคือระดับแอลบูมินในกระแสเลือดระหว่างทั้งสองกลุ่ม ผลการศึกษา: ผู้ป่วยทั้งหมด 74 คนจากเดือนมิถุนายน พ.ศ. 2564 ถึงเดือนกุมภาพันธ์ 2565 เข้าร่วมการศึกษาและได้รับการสุ่มไปยังกลุ่มศึกษา 37 คน และกลุ่มควบคุม 37 คน ลักษณะผู้ป่วยในทั้งสองกลุ่มไม่ได้มีความแตกต่างกันอย่างมีนัยสำคัญทางสถิติ จากผลการศึกษาพบว่าผู้ป่วยในกลุ่มศึกษามีอัตราการเสียชีวิตหรือการติดเชื้อเยื่อบุช่องท้องซ้ำแบบ relapsing หรือ repeat ที่ 15 คน (ร้อยละ 41) และ 19 คน (ร้อยละ 51) (hazard ratio 0.84; ช่วงเชื่อมั่นร้อยละ 95 0.43-1.67) แต่พบว่าระดับแอลบูมินในกระแสเลือดในกลุ่มศึกษามีระดับสูงกว่ากลุ่มควบคุมอย่างมีนัยสำคัญเมื่อวิเคราะห์แบบตลอดการศึกษา (P<0.001) โดยมีค่าแตกต่างที่วันที่ 120 เฉลี่ยอยู่ที่ 0.35 กรัม/ดล. (ช่วงเชื่อมั่นร้อยละ 95 0.06-0.64) อาการผิดปกติที่พบในผู้ป่วยไม่แตกต่างในทั้งสองกลุ่ม โดยส่วนมากเป็นอาการระบบทางเดินอาหาร ได้แก่ ถ่ายอุจจาระเหลว อืดแน่นท้อง คลื่นไส้อาเจียน และเบื่ออาหาร สรุปผลการศึกษา: การให้โปรตีนเสริมในผู้ป่วยติดเชื้อเยื่อบุช่องท้องจากการล้างไตทางช่องท้องส่งผลให้ระดับแอลบูมินในกระแสเลือดสูงขึ้นได้อย่างมีนัยสำคัญ อย่างไรก็ตามผลต่อการเปลี่ยนแปลงการเสียชีวิตหรือการติดเชื้อเยื่อบุช่องท้องซ้ำยังไม่พบอย่างชัดเจน จำเป็นต้องการมีการศึกษาเพิ่มเติมในผู้ป่วยที่มากกว่านี้และมีการติดตามผู้ป่วยที่ยาวนานกว่านี้เพื่อศึกษาผลที่แท้จริงต่อไป

This study aimed to evaluate the effect of brushing on the percentage change of retentive force in Locator R-Tx attachment. There were 3 types of nylon inserts; blue, pink, and clear. The nylon inserts were brushed in 2 media; water and toothpaste. Each group had 6 specimens (n=6). All specimens were soaked in deionized water at 37 ºC for 7 days, after that they were placed in the simulated brushing machine. Objective: To compare esthetic perception between dentists and laypersons by difference of asymmetrical width and length of maxillary central and lateral incisors under 5 and 15 seconds evaluation time. Materials and Methods: Photos of women were digitally altered by Adobe Photoshop® program to create image with symmetrical and asymmetrical tooth width or length in 0.5 mm increments. Final images were randomly present on computer screen using an equal ratio to actual tooth. Osteoblast differentiation requires interaction of several cell signaling to coordinately modulate cell responses. Understanding the integration between signaling pathways provides an insight into molecular mechanism that governs stem cell differentiation. Notch, Wnt, and Interleukin15 (IL15) signaling are among the crucial pathways that control biological processes including osteogenic differentiation. The authors surveyed hospitals across the country on their policies regarding overlapping surgery, and found large variation between hospitals in how this practice is regulated. Specifically, institutions chose to define “critical portions” in a variety of ways, ultimately affecting not only surgical efficiency but also the autonomy of surgical trainees and patient experiences at these different hospitals.

The converging crises of COVID-19 and racist state violence in 2020 shifted public discourse about marginalization, public health, and racism in unprecedented ways. Nursing responded to the pandemic with heroic commitment and new politicization. But public engagement with systemic racism is forcing a reckoning in nursing. The profession has its own history of racism and of alliance with systems of state control with which to contend. In this article, we argue nursing must adopt an ethics of abolitionism to realize its goals for health and justice. Extracellular vesicles are thought to facilitate pathogen transmission from arthropods to humans and other animals. Here, we reveal that pathogen spreading from arthropods to the mammalian host is multifaceted. Extracellular vesicles from Ixodes scapularis enable tick feeding and promote infection of the mildly virulent rickettsial agent Anaplasma phagocytophilum through the SNARE proteins Vamp33 and Synaptobrevin 2 and dendritic epidermal T cells. However, extracellular vesicles from the tick Dermacentor andersoni mitigate microbial spreading caused by the lethal pathogen Francisella tularensis. The transovarial transmission of tick-borne bacterial pathogens is an important mechanism for their maintenance in natural populations and transmission, causing disease in humans and animals. The mechanism for this transmission and the possible role of tick hormones facilitating this process have never been studied. Injections of physiological levels of the tick hormone, 20-hydroxyecdysone (20E), into part-fed (virgin) adult females of the American dog tick, Dermacentor variabilis, attached to the host caused a reduction in density of Rickettsia montanensis in the carcass and an increase in the ovaries compared to buffer-injected controls. A workshop on MCQ development using cognitive model framework was conducted for health educators from Aga Khan University (AKU) and other academic institutions. The aim was to develop the skill of preparing MCQs for assessing higher cognitive levels. A pre-post study was conducted, participant satisfaction was evaluated and pre-post test scores were used to assess learning capability of the workshop participants. Graduate nurse transition programs are the entry point for most graduates into professional practice, and this review focused on both empirical studies and gray literature to identify at what point practice readiness occurs and what can assist graduate nurses’ transition to become practice ready.

Background: Motor vehicle accidents (MVA) represent a significant burden on health systems globally. Tens of thousands of people are injured in Australia every year and may experience significant disability. Associated economic costs are substantial. There is little literature on the health service utilization patterns of MVA patients. Background and aims: 18F-Sodium Fluoride Positron Emission Tomography (18F–NaF PET) non-invasively detects micro-calcification activity, the earliest stage of atherosclerotic arterial calcification. We studied the association between coronary 18F–NaF uptake and high-risk plaque features on intra-coronary optical coherence tomography (OCT) and CT-angiography (CTCA) and the potential application to patient-level risk stratification.

Aims: We investigated the sex-based risk of mortality across the spectrum of left ventricular ejection fraction (LVEF) in a large cohort of patients in Australia.
Methods and results: Quantified levels of LVEF from 237 046 women (48.1%) and 256 109 men undergoing first-time, routine echocardiography (2000–2019) were linked to 119 232 deaths (median 5.6 years of follow-up). Overall, 17.6% of men vs. 8.3% of women had an LVEF P< 0.001] in women and 1.21 (95% CI 1.05–1.39; P = 0.008) in men. Background: Individuals with obstructive sleep apnoea (OSA) experience a higher burden of atrial fibrillation (AF) than the general population, and many cases of AF remain undetected. We tested the feasibility of an artificial intelligence (AI) approach to opportunistic detection of AF from single-lead electrocardiograms (ECGs) which are routinely recorded during in-laboratory polysomnographic sleep studies.

Atherosclerosis is the commonest cause of death in Australia. Cardiovascular (CV) risk calculators have an important role in preventative cardiology, although they are are strongly age-dependent and designed to identify individuals at high risk of an imminent event. The imprecision around “intermediate” or “low” risk generates therapeutic uncertainty, and a significant proportion of patients presenting with myocardial infarction come from these groups, often with no warning. This highlights a conundrum: “Low” risk does not mean “no” risk.

There are currently no established prognostic models for “low-gradient” severe aortic stenosis (AS), including those with low-flow, low-gradient (LFLG) or normal-flow, low-gradient (NFLG) severe AS. Background: Echocardiographic measures of elevated left ventricular filling pressures are associated with an adverse prognosis. The aim of this study was to determine the relationship between acute (ratio of early transmitral flow to mitral annular velocities; E/e’) and chronic (indexed left atrial volume; LAVI) markers of left ventricular filling pressure (LVFP) and mortality in patients with non-severe aortic stenosis (AS), within the National Echo Database of Australia cohort.
