Neoadjuvant therapy is integral to the treatment of early-stage breast cancer. Goals of treatment include surgical downstaging of the tumor, rendering inoperable tumors resectable, and de-escalating axillary surgery in those with clinically positive nodes. Additionally, response to treatment provides important prognostic information regarding risk of recurrence and guides future adjuvant treatment. Although chemotherapy serves as the backbone of neoadjuvant treatment, an increased understanding of the tumor's clinical course as well as its molecular and genetic make-up aids in individualizing treatment and developing novel agents. This review summarizes current clinical approaches and the future direction to the management of breast cancer …

The United Network for Organ Sharing (UNOS) recently revised its heart allocation policy to address numerous shortcomings of the previous system. Implemented in 2018, the changes sought to reduce waiting list mortality, clearly define urgency status based on objective physiologic variables, decrease exemption requests, and introduce geographic modifications to ensure organ distribution favors the highest urgency candidates. In large part, UNOS policy revisions were driven by the growing use of continuous flow left ventricular assist devices (CF-LVADs) and the relevant device complications that led to an unacceptably high number of status exemptions. The new 6-tiered system assigns a comparatively lower …

Biological Activity And Solubility Of 5-Methoxy-1,4-Benzoquinone Having Bromoheptyl And Bromodecyl Substituents In The N-Octanol/Water System, Siti Mariyah Ulfa, Fath Dwisari, Laras Pangesti, Mohammad Farid Rahman
Makara Journal of Science
The biological activity and solubility of compounds are influenced by its chemical structure. These properties can be improved by substituting alkyl, alkoxy, and/or haloalkane in the parent skeleton. In this research, the synthesis of 3-(7-bromoheptyl)-2-methyl-5-methoxy-1,4-benzoquinone (3a) and 3-(10-bromodecyl)-2-methyl-5-methoxy-1,4-benzoquinone (3b) was achieved through the decarboxylation reaction. The solubility and biological activity of 3a and 3b were compared with that of thymoquinone (TQ), which acts as an anti-inflammatory agent. Compounds 3a and 3b were successfully synthesized and analyzed using Fourier Transform Infra-Red (FTIR) and Nuclear Magnetic Resonance (NMR). The FTIR spectrum showed the increasing intensity of …

Makara Journal of Science
This study describes the development of N-benzyl-1,10-phenantrolinium salt as an antiplasmodium agent. The salt, that is, 1-(4-ethoxy-3-methoxybenzyl)-1,10-phenanthrolin-1-ium bromide, was prepared using vanillin as the starting material in four simple synthetic steps. First, the alkylation of vanillin using diethyl sulfate produced 4-ethoxy-3-methoxybenzaldehyde in 79% yield. Second, the reduction of the protected vanillin by NaBH4 through the grinding method allowed us to obtain 4-ethoxy-3-methoxybenzyl alcohol in 96% yield. Next, the bromination of the benzyl alcohol under solvent-free condition led to the formation of the corresponding benzyl bromide, which in turn underwent bimolecular nucleophilic substitution with 1,10-phenanthroline to produce the desired …

Antibodies against glutamic acid decarboxylase (GAD), originally linked to stiff person syndrome (SPS), now denote the “GAD antibody-spectrum disorders” (GAD-SD) that also include autoimmune epilepsy, limbic encephalitis, cerebellar ataxia and nystagmus with overlapping symptomatology highlighting autoimmune neuronal excitability disorders. The reasons for the clinical heterogeneity among GAD-antibody associated syndromes remain still unsettled, implicating variable susceptibility of GABAergic neurons to anti-GAD or other still unidentified autoantibodies. Although anti-GAD antibody titers do not correlate with clinical severity, very high serum titers, often associated with intrathecal synthesis of anti-GAD-specific IgG, point to in-situ effects of GAD or related autoantibodies within the central nervous …

Department of Medicine Faculty Papers
Introduction: Persistent misconceptions of frailty and dementia in geriatric patients impact physician-patient communication and leave patients vulnerable to disempowerment. Physicians may inappropriately focus the discussion of treatment options to health care proxies instead of patients. Our study explores the consenting process in a decision-making capable orthogeriatric trauma patient population to determine if there is a relationship between increased patient age and surgical consent by health care proxy.
Methods: Patients aged 65 and older who underwent operative orthopaedic fracture fixation between 1 of 2 Level 1 Trauma Centers were retrospectively reviewed. Decision-making capable status was defined as an absence of patient …

It is no secret that modern medicine has improved people’s lives in virtually every are of healthcare. What is also not a secret is how expensive it is. . Not only does the average American spend roughly $5000 on healthcare, they also spend roughly $1200 on pharmaceuticals. The candidates gunning for the U.S. presidency often bring up the topic of rising pharmaceuticals and what they would do to change it. Medical services and products as a whole are of concern for virtually all Americans, and many of them are considered overpriced. The development of these products isn’t cheap, and it …

CareCloud, a privately held technology company headquartered out of Miami, states that according to recent figures, the average cost of an organ transplant in the U.S. can reach well over $1 million. Furthermore, according to the United Network for Organ Sharing (UNOS), more than 113,000 patients in the U.S. are currently waiting for an organ transplant. Anne Paschke of UNOS stresses this need for organs by expressing that only 1 to 2 percent of the population dies in a way that makes them potential organ donors. Although the need for organs has yet to be met, bioprinting has the potential …

Imagine a condition in which lesions that grow on your appendages are so severe that you cannot use your hands and feet properly. Moreover, these lesions are large and painful, and prevent you from being able to perform daily activities. Surgery, a common answer for this condition, can temporarily control the growths, but the lesions continue to grow back after each surgical operation. Currently, there is no cure, and the treatments only offer a temporary fix. Therefore, you are faced with a lifelong condition that severely limits your ability to live a normal life. Though there are many debilitating diseases, …

I barely felt the cold, dry Montana air pierce my lungs as I looked through my scope for an iconic animal in our culture: a wolf. While the dramatic thermal features on the south side of Yellowstone National Park attract the most visitors, the north is quiet and rich with life. The Lamar Valley is a stunning stage for Yellowstone’s wildlife to interact with one another. As the sun rises and hits the mountain tops, the cold air rushes into the valley and creates a thick fog, a curtain. Backstage, the wooded mountain ranges slowly bleed into the tall grasses …

Biological weapons, as defined by the Federation of American Scientists, are “toxins and microorganisms, such as viruses and bacteria, used to deliberately inflict disease among people, animals and agriculture.” Biological weapons have been used for hundreds of years on varying scales, from the catapulting of plague-infected corpses into enemy cities in the 14th century, to the testing of infectious diseases in China during WWII, to the 2001 anthrax attacks. These weapons act discreetly, as it is hard to trace an outbreak to a particular attacker and it takes several days for an infected individual to show signs of the disease. …

Partnerships between schools, universities, and community organizations have mutual benefits for all involved. These partnerships value the contributions of all participants and capitalize on the expertise and knowledge that each brings. This reflective paper details a collaborative third-space partnership between a university, a primary school, and a community organization. The partnership facilitated the design, development, and implementation of a unique program called Project DARE (Dementia knowledge, Art, Research, and Education). A research-based evaluation of the Project DARE feasibility study can be found elsewhere (Burns et al., 2020). The aim of this paper is to reflect upon the formation of the …
