The purpose of this study is to determine if spontaneous pushing during the second stage of labor produces better maternal outcomes than the more commonly used directed Valsalva pushing. A literature review of the recent studies has been performed to determine if Valsalva pushing is simply archaic or absolutely necessary for healthier mother/baby outcomes. To recommend that women go against their primal urges to push in the manner that their own bodies are directing them, indicates that there must be research-based evidence to intervene in the natural order of things and use Valsalva pushing instead. When in fact, there are …

The human gut microbiome has been an area of interest for researchers due to its established link with human metabolism, nutrition, physiology, and immune functions. The research presented further investigates the microbiome to better understand it. Stools from two healthy young African males and a French obese individual were analyzed using MALDI-TOF MS and the enrichment culture technique. For the identification of bacterial species metagenomics, genomics, and culturomics were used. The results showed a much broader biodiversity in the human gut than previously predicted, as well as better results when using culturomics for measuring biodiversity in large-scale isolation.

If one was asked “which is better: breastfeeding or formula feeding?” the most common answer would be “breastfeeding.” This would be the answer 9 times out of 10. It is a known fact by a wide range of people, that in general, breastfeeding is the better option. Infants who are breastfed exclusively in the first six months of life tend to have better overall health outcomes than children that are fed only iron-fortified formula or a combination of the two. Therefore, it is likely that breastfeeding will indeed also improve cognitive developmental outcomes. That is the purpose of this study. …

Intimate partner violence (IPV), is a significant public health concern involving physical or psychological threats from a current or former romantic partner. (Centers for Disease Control and Prevention [CDC], 2014). Previous investigations have revealed that nearly half of all women and men have suffered from psychological aggression by an intimate partner at some point (Black et al., 2011). While IPV affects both genders, females suffer more than five million IPV incidents each year and nearly two million injuries (National Center for Injury Prevention and Control, 2003). While many predictors have been linked to IPV, we focused on Attention-Deficit/Hyperactivity Disorder (ADHD) …

The United States is in an opioid crisis. Of all the opioids that are marketed in the United States the most popular is Oxycontin, containing the active ingredient oxycodone, the chemical equiv-alent of heroin which is more powerful than morphine. Along with heroin and fentanyl, Oxy-contin has become one of the great killers of this era. It is estimated that in 2016 there were over 20,000 overdose deaths related to prescription pain relievers, as opposed to 12,990 over-dose deaths related to heroin in 2015. The state of West Virginia has been the one that has suf-fered the most. The rate …

Knee Osteoarthritis: Efficacy Of Platelet-Rich Plasma Injections Are Non-Inferior To Conventional Joint Injection Treatments, John Riley Clark
Physician Assistant Capstones
Platelet-Rich Plasma (PRP) has emerged as a promising treatment of OA, particularly in large joints. For the treatment of knee OA PRP is non-inferior to the traditional methods, in improving function, pain, and ADLs. PRP is promising due to its autologous nature and appeal with the ever-increasing push for holistic or “natural” options in therapeutics. The patient will utilize properties of their own body to bolster an anti-inflammatory response and potentially improve the function of the knee cartilage. Although it requires fluid collection from a patient and processing which may be slightly time intensive, it offers an alternative to synthetic …

Hyperlipidemia, hypertension, diabetes, family history, gender, and obesity are all well-established cardiovascular disease risk factors. Advancements are being made to investigate the role of diet as a modifiable risk factor, and to determine if adults who consume meat compared to those who do not consume meat are at higher risk for hypertension, obesity, and diabetes, thus increasing their cardiovascular disease risk. Because animal protein is ubiquitous in the modern diet, it is imperative that determinations are made in regard to either its benefits or its contribution to the burden of chronic disease. The clinical application of this research is to …
In Patients Presenting To The Emergency Department With Atrial Fibrillation With Rapid Ventricular Response, Are Calcium Channel Blockers More Effective Than Beta-Blockers For Acute Management Of Rate Control?, Jennifer Dang
Physician Assistant Capstones
The evidence on the efficacy of nondihydropyridine calcium channel blockers (CCBs) compared to beta-blockers (BBs) for acute atrial fibrillation (AF) rate control in the emergency department setting was examined. Twelve studies were relevant and revealed that CCBs, specifically diltiazem, were superior to BBs because of their rapid onset of action and the lower number of doses they required to sustain ventricular rate control compared to BBs. However, given the small sample sizes in these studies and the limited number of randomized, double-blinded trials, more research is needed to increase the generalizability and to confirm the significance of these findings.

Most would agree that obesity is among one of the greatest challenges that providers face in healthcare. Most often the focus is on diet and activity as the primary factors responsible for weight management. Is it possible that the type of birth method predisposes a child to a life of uphill battles in management of weight and comorbidities? Overweight and obese children likely develop into obese adults increasing the likelihood of developing diabetes and cardiovascular disease at a younger age.4 Is there global evidence that otherwise healthy babies, who have been born by c-section compared with those born naturally, are …
