Advance Care Planning Interventions For Racially And Ethnically Diverse Populations: A Literature Review, Haley Kirkpatrick

DNP Qualifying Manuscripts

Background and purpose: Diverse populations are less likely to engage in end-of-life-care-planning. This literature review examined research on end-of-life planning interventions for diverse patient populations.

Methods: This systematic search included Academic Search Complete, Cochrane, CINAHL, and PubMed databases. Results were limited to peer-reviewed articles published in English between 2015 to 2019.

Conclusions: Each intervention was effective at increasing advance care planning (ACP) engagement. Community-based programs are costlier. PREPARE used less resources. If diverse populations are specifically targeted, ACP engagement increases.

Implications for practice: Healthcare institutions and practitioners should employ targeted interventions for ACP engagement of diverse groups.

Abstract

Introduction: In the United States, lung cancer is the leading cause of cancer death in both men and women. Smoking increases the risk for lung cancer, and smoking cessation after diagnosis improves cancer survival and lowers rates of recurrence. Using the Theory of Planned Behavior (TPB), we examined individual attitudes, perceived control, and subjective norms as predictors of lung cancer survivors’ intention to quit smoking.

Method: We conducted a secondary data analysis to examine attitudes, perceived control, and subjective norms of smoking in a sample of lung cancer survivors (N= 171). Analyses were limited to …

In recent years, communities and institutions have sought new interventions intended to reduce sugar sweetened beverage (SSB) consumption among children. Among these interventions are “SSB-free zones,” where such beverages are not permitted to be consumed on the premises. Insufficient knowledge still exists, however, about the readiness for such restrictive SSB policies within health care institutions. Understanding attitudes toward SSB consumption among adults is necessary to guide an institution-wide policy, where staff and patients serve as role models for parents and their children. Importance: In a randomized clinical trial, heart failure (HF) hospitalizations were lower in patients managed with guidance from an implantable pulmonary artery pressure sensor compared with usual care. It remains unclear if ambulatory monitoring could also improve long-term clinical outcomes in real-world practice.

Objective: To determine the association between ambulatory hemodynamic monitoring and rates of HF hospitalization at 12 months in clinical practice.

With the increased use of cancer immunotherapy, a number of immune-related adverse events (irAEs) are being identified. These irAEs can be compared with known autoimmune disorders in similar tissues, with important similarities and differences. Understanding the etiology of irAEs may bring to light concepts applicable to immune responses in cancer, autoimmunity, and infectious disease. This immunobiology is especially relevant to cancer patients with preexisting allogeneic transplants or autoimmune disease who are undergoing cancer immunotherapy. The goal of the current study was to determine if students who are deaf or hard of hearing (d/hh) would learn more new vocabulary words through the use of retrieval practice than repeated exposure (repeated study). No studies to date have used this cognitive strategy—retrieval practice—with children who are d/hh. Previous studies have shown that children with hearing loss struggle with learning vocabulary words. This deficit can negatively affect language development, reading outcomes, and overall academic success. Falls are a growing public health problem among the aging population. Structured exercise programs are effective in preventing falls, and many have been implemented for community-dwelling older adults in the U.S. However, low adherence rates for structured exercise are reported consistently for this population. The need to translate alternative evidence-based exercise programs the diverse older adult population is important. Permanent junctional reciprocating tachycardia (PJRT) is a rare form of supraventricular tachycardia (SVT). It generally presents in infants but can be difficult to diagnose. The characteristic EKG findings, response to Adenosine and persistence or frequent recurrences are helpful in making the diagnosis. It is usually difficult to manage with the initial and single medications used in SVT. Many patients are misdiagnosed and not treated effectively and end up having end stage cardiomyopathy and are diagnosed in patients referred for transplant. Research over the last thirty years has focused on investigating the mechanisms through which specific elements of the built environment, such as indoor air quality, influence the physical health of occupants. A Unique Role For Clathrin Light Chain A In Cell Spreading And Migration., Oxana M. Tsygankova, James H. Keen

Clathrin heavy chain is the structural component of the clathrin triskelion, but unique functions for the two distinct and highly conserved clathrin light chains (CLCa and CLCb, also known as CLTA and CLTB, respectively) have been elusive. Here, we show that following detachment and replating, CLCa is uniquely responsible for promoting efficient cell spreading and migration. Selective depletion of CLCa, but not of CLCb, reduced the initial phase of isotropic spreading of HeLa, H1299 and HEK293 cells by 60-80% compared to siRNA controls, and wound closure and motility by ∼50%. Surface levels of β1-integrins were unaffected by CLCa depletion. However, …

Introduction: Animal studies and rare human studies have suggested a negative effect of barbiturates on cardiac function. Although intravenous (IV) phenobarbital is used routinely in children in the clinical setting, studies in children are lacking. We performed a study to evaluate effect of IV phenobarbital loading on myocardial systolic function of children.

Methods: In a prospective pilot study in children without congenital heart defects, the effect of IV phenobarbital was evaluated on the left ventricular systolic function measured by ejection fraction (EF) by Simpson's method via an echocardiogram. Opioid dependence during pregnancy may lead to negative outcomes for both the parturient and the fetus. Opioid dependent parturients are often prescribed buprenorphine, a form of medication- assisted therapy. The use of buprenorphine within the parturient population is a relatively new practice and anesthesia providers are encountering difficulties in managing this population during the peripartum period. Despite countless surgical advances over the last several decades refining surgical approaches, repair techniques, and tools to treat tendon and tendon-to-bone injuries, we are still left with repair solutions that rely on fairly crude underlying mechanical principles. Musculoskeletal soft tissues have evolved to transfer high loads by optimizing stress distribution profiles across the tissue at each length scale. However, instead of mimicking these natural load transfer mechanisms, conventional suture approaches are limited by high load transfer across only a small number of anchor points within tissue.
