Background: Injuries sustained from motor vehicle accidents are one of the leading causes of death in children every year. In one year, more than 618,000 children 0-12 years of age rode in vehicles without the use of a proper restraint device at least some of the time. Of the children ages 12 years and younger who died in a motor vehicle accident in 2016 (for which safety restraint use was known), 35% were not buckled up (Centers for Disease Control and Prevention, 2017).

Abstract
BACKGROUND: Nationally, approximately one third of antibiotics are currently being prescribed unnecessarily. Antibiotic misuse can result in disability, longer hospital stays, increased costs to organizations, antibiotic resistance and death.
PURPOSE: The purpose of this project was two-fold: (1) explore current management of acute adult rhinosinusitis in an outpatient setting (2) assessing providers perceptions on the barriers to preventing antibiotic misuse.
METHODS: A retrospective chart review of 150 charts was performed to examine prescribing practices by providers treating rhinosinusitis at two primary care practices. An interview was conducted with the practice providers on barriers for compliance in antibiotic stewardship.

Behavioral health problems impact approximately 19% of the United States population and nurses are not exempt from these experiences. In fact, nurses may be at higher risk for developing a behavioral health issue than the general public. The purpose of this study was to identify specific demographic, work-related, and behavioral variables associated with substance use and personal experience with any mental and or behavioral health problems among nurses in an academic healthcare system. Background: Neonatal abstinence syndrome (NAS) is the set of signs and symptoms present in a newborn whose mother was addicted to drugs while pregnant. NAS can occur due to sudden withdrawal from several drugs, including opioids, benzodiazepines, methamphetamines, and caffeine. It can cause many different issues in the newborn, including diarrhea, tremors, fever, and seizures. While these specific signs and symptoms are self-limited, research is showing that these children grow in to childhood and adolescence with increased risk of Attention-Deficit/Hyperactivity Disorder (ADHD), autism, motor dysregulation, and other cognitive and behavioral sequelae.

BACKGROUND: Depression in adults with type 2 diabetes can lead to non-adherence to self-management behaviors and treatment recommendations, and ultimately worse health outcomes. Currently, only 50% of adult patients with type 2 diabetes treated in the primary care setting are screened for depression. Without screening for depression, a diagnosis of depression and subsequent treatment is likely to be missed and the patient is more likely to experience worse health outcomes and worse quality of life.

PURPOSE: This DNP project is relevant to nurses of the psychiatric and Intensive Care Unit (ICU) setting, as high stress environments may produce higher rates of compassion fatigue and burnout. BACKGROUND: In 2018, there were approximately 234,030 new lung cancer diagnoses and 154,500 lung cancer deaths in the United States. Kentucky leads the nation in new lung cancer cases each year. The USPSTF estimates that 10% to 12% of lung cancers detected by routine screening would not have been detected until the cancer advanced; therefore, the USPSTF recommends yearly lung cancer screenings using low dose CT (LDCT) scans of the chest for patients 55 to 80 years old who have a 30 pack-year smoking history and are currently smoking or have quit within the last 15 years.

Introduction: Prevalence of heart failure (HF) in the US is projected to rise 46% by 2030, resulting in more than 8 million people with HF. Negative psychologic states including depression and pessimism (negative future expectations) have been linked with poor cardiovascular outcomes, including HF. Despite the fact that optimism (positive future expectations) and other positive affective states have been associated with superior outcomes, there has been little focus on interventions designed to increase positive psychological states in patients with HF.

PURPOSE: The purpose of this project is to determine if an educational intervention with families decreased overall screen time use in school-aged children with attention-deficit/hyperactivity disorder (ADHD).
METHODS: This study was a single group pre-test, post-test interventional study design. The sample population consisted of the parents/guardians of 30 school-aged children diagnosed with ADHD (5-12 years) recruited from the University of Kentucky Developmental and Behavioral Pediatrics Clinic between November 2019 and February 2020. Background: Heuristics, commonly known for introducing bias, are now being standardized to reduce biases in clinical decision-making (CDM). In a healthcare environment that recognizes the importance of clinical creativity, it remains unclear how standardized heuristics such as the fast-and-frugal model impact creative features of CDM. One framework of CDM, proposed by Engebresten et al., emphasizes the importance of uncertainty and creativity through four aspects: imagination, reflective questioning, understanding and critical judgment.

This project combines reflective and creative processes in a theater collaboration between patients, caregivers, and healthcare students. The goal was to explore the ways in which theater can heal, and to better understand the patient experience of dementia.

Background: Vitamin E δ-tocotrienol (VEDT), a vitamin E compound isolated from sources such as palm fruit and annatto beans, has been reported to have cancer chemopreventive and therapeutic effects.
Methods: We report a novel function of VEDT in augmenting tumor necrosis factor-related apoptosis-inducing ligand- (TRAIL-) induced apoptosis in pancreatic cancer cells. The effects of VEDT were shown by its ability to trigger caspase-8-dependent apoptosis in pancreatic cancer cells.
Results: When combined with TRAIL, VEDT significantly augmented TRAIL-induced apoptosis of pancreatic cancer cells. VEDT decreased cellular FLICE inhibitory protein (c-FLIP) levels without consistently modulating the expression of decoy death receptors 1, …

Even though pancreatic ductal adenocarcinoma (PDAC) is associated with fibrotic stroma, the molecular pathways regulating the formation of cancer associated fibroblasts (CAFs) are not well elucidated. An epigenomic analysis of patient-derived and de-novo generated CAFs demonstrated widespread loss of cytosine methylation that was associated with overexpression of various inflammatory transcripts including CXCR4. Co-culture of neoplastic cells with CAFs led to increased invasiveness that was abrogated by inhibition of CXCR4. Metabolite tracing revealed that lactate produced by neoplastic cells leads to increased production of alpha-ketoglutarate (aKG) within mesenchymal stem cells (MSCs). Splice variants have been shown to play an important role in tumor initiation and progression and can serve as novel cancer biomarkers. However, the clinical importance of individual splice variants and the mechanisms by which they can perturb cellular functions are still poorly understood. To address these issues, we developed an efficient and robust computational method to: (1) identify splice variants that are associated with patient survival in a statistically significant manner; and (2) predict rewired protein-protein interactions that may result from altered patterns of expression of such variants. การวิจัยกึ่งทดลองนี้ มีวัตถุประสงค์เพื่อ 1) เปรียบเทียบภาวะซึมเศร้าของผู้ป่วยจิตเภท ก่อนและหลังได้รับโปรแกรมการสร้างความเข้มแข็งทางใจแบบกลุ่ม และ 2) เปรียบเทียบภาวะซึมเศร้าของผู้ป่วยจิตเภทระหว่างกลุ่มที่ได้รับโปรแกรมการสร้างความเข้มแข็งทางใจแบบกลุ่มกับกลุ่มที่ได้รับการพยาบาลปกติ กลุ่มตัวอย่าง คือ ผู้ป่วยจิตเภทที่เข้ารับการรักษาที่แผนกผู้ป่วยใน โรงพยาบาลจิตเวชแห่งหนึ่ง ที่มีคุณสมบัติตามเกณฑ์ที่กำหนดจำนวน 40 คน สุ่มเข้ากลุ่มทดลองและกลุ่มควบคุมกลุ่มละ 20 คน โดยได้รับการจับคู่ด้วย เพศ อายุ และอาการทางจิต กลุ่มทดลองได้รับโปรแกรมการสร้างความเข้มแข็งทางใจแบบกลุ่ม ส่วนกลุ่มควบคุมได้รับการพยาบาลตามปกติ เครื่องมือที่ใช้ในการวิจัย ได้แก่ 1) โปรแกรมการสร้างความเข้มแข็งทางใจแบบกลุ่ม 2) แบบประเมินอาการซึมเศร้าในผู้ป่วยจิตเภท เครื่องมือทุกชุดผ่านการตรวจสอบความตรงเชิงเนื้อหาจากผู้ทรงคุณวุฒิ จำนวน 5 ท่าน เครื่องมือชุดที่ 2 มีค่าสัมประสิทธิ์อัลฟาครอนบราคเท่ากับ 0.85 วิเคราะห์ข้อมูลโดยใช้ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐานและสถิติทดสอบที ผลการวิจัยสรุปได้ดังนี้ 1) ภาวะซึมเศร้าของผู้ป่วยจิตเภทหลังได้รับโปรแกรมการสร้างความเข้มแข็งทางใจแบบกลุ่มต่ำกว่าก่อนได้รับโปรแกรมฯอย่างมีนัยสำคัญทางสถิติที่ระดับ .05 2) ภาวะซึมเศร้าของผู้ป่วยจิตเภทกลุ่มที่ได้รับโปรแกรมการสร้างความเข้มแข็งทางใจแบบกลุ่มต่ำกว่ากลุ่มที่ได้รับการพยาบาลตามปกติอย่างมีนัยสำคัญทางสถิติที่ระดับ .05

การวิจัยครั้งนี้เป็นการศึกษาแบบบรรยายเชิงความสัมพันธ์ มีวัตถุประสงค์เพื่อศึกษา 1) คุณภาพชีวิตด้านสุขภาพของผู้ป่วยเบาหวานที่มีแผลที่เท้า โรงพยาบาลระดับตติยภูมิ กรุงเทพมหานคร 2) ศึกษาความสัมพันธ์ระหว่าง ประสบการณ์การมีอาการ ระดับน้ำตาลเฉลี่ยสะสมในเลือด ภาวะแทรกซ้อนจากโรคเบาหวาน พฤติกรรมการดูแลตนเอง และการสนับสนุนทางสังคมกับคุณภาพชีวิตด้านสุขภาพของผู้ป่วยเบาหวานที่มีแผลที่เท้า โรงพยาบาลระดับตติยภูมิ กรุงเทพมหานคร กลุ่มตัวอย่างคือ ผู้ป่วยเบาหวานที่มีแผลที่เท้า อายุ18-59 ปี จำนวน 135 คน ที่มารับบริการทำความสะอาดแผล ณ คลินิกศัลกรรม หอผู้ป่วยนอก โรงพยาบาลจุฬาลงกรณ์ สภากาชาดไทย โรงพยาบาลราชวิถี กรมการแพทย์ และโรงพยาบาลวชิรพยาบาล มหาวิทยาลัยนวมินทราธิราช คัดเลือกกลุ่มตัวอย่างตามเกณฑ์การคัดเข้า เครื่องมือที่ใช้ในการวิจัย คือ แบบสอบถามข้อมูลส่วนบุคคล แบบประเมินผลลัพธ์ทางคลินิก ประกอบด้วย ค่าระดับน้ำตาลสะสม ภาวะแทรกซ้อน และแบบสอบถามคุณภาพชีวิตด้านสุขภาพของผู้ป่วยเบาหวานที่มีแผลที่เท้า แบบสอบถามประสบการณ์การมีอาการ แบบสอบถามพฤติกรรมการดูแลตนเอง และแบบสอบถามการสนับสนุนทางสังคม แบบสอบถามทั้งหมดผ่านการตรวจสอบความตรงตามเนื้อหาโดยผู้ทรงคุณวุฒิ 5 คน และมีค่าความเชื่อมั่นสัมประสิทธิ์แอลฟาครอนบาค เท่ากับ 0.77,0.72,0.77 และ 0.92 ตามลำดับ วิเคราะห์ข้อมูลโดยหาค่าเฉลี่ย ร้อยละ และส่วนเบี่ยงเบนมาตรฐาน สถิติสัมประสิทธิ์สหสัมพันธ์ของเพียร์สัน และสเปียร์แมน ผลการวิจัยสามารถสรุปได้ดังนี้ 1. คุณภาพชีวิตด้านสุขภาพของผู้ป่วยเบาหวานที่มีแผลที่เท้า โรงพยาบาลระดับตติยภูมิ กรุงเทพมหานคร อยู่ในระดับปานกลาง (Mean = 3.36, SD = 0.38) 2. พฤติกรรมการดูแลตนเองมีความสัมพันธ์ทางบวกกับคุณภาพชีวิตด้านสุขภาพของผู้ป่วยเบาหวานที่มีแผลที่เท้าอย่างมีนัยสำคัญทางสถิติที่ระดับ .05 3. ประสบการณ์การมีอาการ ภาวะแทรกซ้อนจากโรคเบาหวาน ระดับน้ำตาลเฉลี่ยสะสมในเลือด และการสนับสนุนทางสังคมไม่มีความสัมพันธ์กับคุณภาพชีวิตด้านสุขภาพของผู้ป่วยเบาหวานที่มีแผลที่เท้า

A novel multiplex PCR (mPCR) was developed to identify and differentiate malaria parasite (Plasmodium caprae), and two piroplasms (Babesia bigemina and Theileria luwenshuni) in Thai goats. Three pairs of primer were designed to be species-specific, targeting cytochrome c oxidase subunit I (COI) gene in mitochondria. Plasmids containing each parasite’s DNA was serially diluted to evaluate the sensitivity of singleplex and mPCR assays. The specificity of the primer sets was confirmed by testing for amplification from DNA of each pathogen and other related hemoparasites. การวิจัยครั้งนี้เป็นการวิจัยเชิงพรรณนา ณ จุดเวลาใดเวลาหนึ่ง เพื่อศึกษาความชุกของความตั้งใจไม่มีบุตร และความเกี่ยวข้องกับบุคลิกภาพและการเผชิญปัญหา ในผู้ที่มาจดทะเบียนสมรสใหม่ ณ สำนักงานเขตบางรัก กรุงเทพมหานคร เก็บรวบรวมข้อมูลจากผู้มาจดทะเบียนสมรสเพศชายและหญิงจำนวน 380 คน โดยใช้แบบสอบถามแบบตอบด้วยตนเอง ซึ่งประกอบด้วยแบบสอบถามข้อมูลพื้นฐานทั่วไป แบบทดสอบบุคลิกภาพ MPI (The Maudsley Personality Inventory) และแบบวัดการเผชิญปัญหา ใช้ Univariate analysis (ได้แก่ T-Test และ Chi-Square) เพื่อทดสอบปัจจัยที่มีความสัมพันธ์กับความตั้งใจไม่มีบุตร และใช้ Multivariate Analysis เพื่อหาปัจจัยทำนายของความตั้งใจไม่มีบุตรในผู้มาจดทะเบียนสมรสกลุ่มนี้ ผลการศึกษาพบว่าความชุกของความตั้งใจไม่มีบุตรในผู้มาจดทะเบียนสมรสมีอัตราร้อยละ 13.7 เมื่อทดสอบความสัมพันธ์พบว่าบุคลิกภาพ มิติที่ 1 ด้านพฤติกรรม (Scale E – extraversion) มีความสัมพันธ์กับความตั้งใจไม่มีบุตรอย่างมีนัยสำคัญ โดยกลุ่มตัวอย่างที่มีบุคลิกภาพแบบเก็บตัว (Introvert) มีความตั้งใจไม่มีบุตรสูงกว่ากลุ่มตัวอย่างที่มีลักษณะบุคลิกภาพแบบแสดงตัว (Extravert) นอกจากนี้ยังพบว่าการใช้กลวิธีการเผชิญปัญหาแบบมุ่งจัดการปัญหา (1.8 - การแสวงหาการเป็นส่วนหนึ่งของกลุ่ม) มีความสัมพันธ์เชิงลบกับความตั้งใจไม่มีบุตรอย่างมีนัยสำคัญ และการใช้กลวิธีการเผชิญปัญหาแบบหลีกหนี การใช้กลวิธีการเผชิญปัญหาแบบหลีกหนี (3.4 - การเก็บความรู้สึกไว้คนเดียว) และการใช้กลวิธีการเผชิญปัญหาแบบหลีกหนี (3.2 - การไม่แสดงออกทางพฤติกรรม) มีความสัมพันธ์เชิงบวกกับความตั้งใจไม่มีบุตรในผู้มาจดทะเบียนสมรสอย่างมีนัยสำคัญ

วัตถุประสงค์ในการวิจัยครั้งนี้มีวัตถุประสงค์เพื่อศึกษาปัจจัยที่ส่งผลต่อการตัดสินใจเลือกเข้าร่วมการแข่งขันไตรกีฬาในประเทศไทยของนักไตรกีฬา โดยใช้ปัจจัยส่วนประสมทางการตลาด 7Ps รวมถึงลักษณะทางประชากรศาสตร์ทางด้านเพศ อายุ ระดับการศึกษา รายได้เฉลี่ยต่อเดือนของนักไตรกีฬาการวิจัยนี้ใช้กลุ่มตัวอย่าง จำนวน 400 คน เครื่องมือที่ใช้ในการวิจัย คือ แบบสอบถาม วิเคราะห์ข้อมูลโดยใช้สถิติสมการถดถอยเชิงเส้นพหุคูณ การทดสอบสมมติฐานโดยใช้สถิติ Independent Sample t-test และการวิเคราะห์ความแปรปรวนแบบจำแนกทางเดียว One-Way ANOVA โดยตั้งระดับความสำคัญทางนัยสถิติที่ 0.05 ผลการวิจัย ในภาพรวมกลุ่มตัวอย่างมีความเห็นระดับมากที่สุดทั้งในปัจจัยส่วนประสมทางการ ตลาด 7Ps และการตัดสินใจเลือกเข้าร่วมการแข่งขันไตรกีฬาในประเทศไทยของนักไตรกีฬา ผลการทดสอบ สมมติฐาน พบว่าปัจจัยส่วนประสมทางการตลาดด้านส่งเสริมการตลาด ด้านบุคคล ด้านลักษณะทางกายภาพ และด้านกระบวนการเท่านั้น ที่ส่งผลต่อการตัดสินใจเลือกเข้าร่วมการแข่งขันไตรกีฬาในประเทศไทยของนักไตรกีฬาอย่างมีนัยสำคัญทางสถิติที่ระดับ 0.05 ขณะที่ลักษณะประชากรศาสตร์ทาง ด้านเพศ อายุ ระดับการศึกษา รายได้เฉลี่ยต่อเดือน ที่แตกต่างกันมีระดับการตัดสินใจเลือกเข้าร่วมการแข่งขันไตรกีฬาในประ เทศไทยไม่แตกต่างกัน สรุปผลการวิจัย มีเพียงปัจจัยส่วนประสมทางการตลาดด้านส่งเสริมการตลาด ด้านบุคคล ด้าน ลักษณะทางกายภาพ และด้านกระบวนการเท่านั้น ที่ส่งผลต่อการตัดสินใจเลือกเข้าร่วมการแข่งขันไตรกีฬา ในประเทศไทยของนักไตรกีฬา อย่างไรก็ตาม ลักษณะประชากรศาสตร์ทางด้านเพศ อายุ ระดับการศึกษา รายได้เฉลี่ยต่อเดือน ที่แตกต่างกันไม่ส่งผลต่อการตัดสินใจเลือกเข้าร่วมการแข่งขันไตรกีฬาในประเทศไทย

ที่มาและความสำคัญ: ปัจจุบันยังไม่มีการศึกษาเกี่ยวกับการวางแผนดูแลรักษาตนเองล่วงหน้าในผู้ป่วยสูงอายุโรคมะเร็งเม็ดเลือดขาวชนิดไมอีลอยด์ในประเทศไทย วัตถุประสงค์: เพื่อศึกษาว่าผู้ป่วยสูงอายุโรคมะเร็งเม็ดเลือดขาวชนิดไมอีลอยด์ที่ได้รับการดูแลแบบประคับประคอง ที่มีการวางแผนดูแลรักษาตนเองล่วงหน้ามีการตายดี กว่าผู้ที่ไม่มีการวางแผนดูแลรักษาตนเองล่วงหน้าหรือไม่ วิธีการวิจัย: การวิจัยเชิงพรรณนาโดยการสังเกตแบบไปข้างหน้า ในผู้ป่วยสูงอายุโรคมะเร็งเม็ดเลือดขาวชนิดไมอีลอยด์ที่อายุมากกว่าหรือเท่ากับ 60 ปี ได้รับการดูแลแบบประคับประคองที่เข้ามารับการรักษาที่โรงพยาบาลจุฬาลงกรณ์ ศึกษาประเมินโดยการสัมภาษณ์เกี่ยวกับคุณภาพชีวิต และการวางแผนดูแลรักษาตนเองล่วงหน้าทุก 3 เดือน ติดตามจนกระทั่งผู้ป่วยเสียชีวิต และเก็บข้อมูลวาระสุดท้ายของชีวิตเพื่อประเมินว่าตายดีหรือไม่ โดยนิยามของตายดี ประกอบด้วย (1.) ผู้ตายยอมรับได้พร้อมที่จะจากไป ได้มีการวางแผนถึงสภาวะแวดล้อมขณะเสียชีวิต (2.) การตายที่ไม่เจ็บปวด ไม่ทุกข์ทรมาน และ (3.) ได้รับการดูแลทางอารมณ์และจิตวิญญาณตามความต้องการ การศึกษาจะประเมินญาติผู้ดูแลเรื่องความทุกข์ และการวางแผนดูแลรักษาตนเองล่วงหน้าต่อผู้ป่วยด้วย ผลการศึกษา: ตั้งแต่ เดือนมกราคม พ.ศ. 2562 ถึงเดือนมกราคม พ.ศ. 2563 มีผู้ป่วย 20 รายเข้าร่วมการศึกษา ได้รับการรักษาด้วย hypomethylating agents 14 ราย, cytoreductive agents 4 รายและ novel agents 2 ราย ไม่มีผู้ป่วยถูกส่งปรึกษาทีมเชี่ยวชาญดูแลระยะสุดท้าย ผู้ป่วย 2 รายไม่ทราบการวินิจฉัย ค่ามัธยฐานระยะเวลาติดตามอาการ 4 เดือน มีผู้ป่วยเพียง 7 ราย (ร้อยละ 35) ที่มีการวางแผนดูแลรักษาตนเองล่วงหน้า มีผู้ป่วย 13 รายเสียชีวิตระหว่างติดตามค่ามัธยฐานระยะเวลารอดชีวิต 10.3 เดือน โดยผู้ป่วยส่วนใหญ่ร้อยละ 92 เสียชีวิตในโรงพยาบาล ค่ามัธยฐานจำนวนวันที่นอนรักษาในโรงพยาบาล 7.5 วัน สาเหตุการเสียชีวิตส่วนใหญ่คือโรคมะเร็งเม็ดเลือดขาวชนิดไมอีลอยด์เอง 11 ราย (ร้อยละ 84.7) ผู้ป่วยที่ได้รับการทำหัตถการกู้ชีพ เช่นใส่ท่อช่วยหายใจ, ส่องกล้องหลอดลม, ได้รับยากระตุ้นหัวใจและหลอดเหลือด และ leukapharesis จำนวน 3 รายซึ่งอยู่ในกลุ่มที่ไม่มีการวางแผนดูแลรักษาตนเองล่วงหน้าทั้งหมด ในบรรดาผู้เสียชีวิต กลุ่มที่มีการวางแผนดูแลรักษาตนเองล่วงหน้า 5 รายได้รับการประเมินว่าตายดีทั้งหมด เปรียบเทียบกับ กลุ่มที่ไม่มีการวางแผนดูแลรักษาตนเองล่วงหน้า 8 …
