Background: Change in the prevalence of raised blood pressure could be due to both shifts in the entire distribution of blood pressure (representing the combined effects of public health interventions and secular trends) and changes in its high-blood-pressure tail (representing successful clinical interventions to control blood pressure in the hypertensive population). Ultrasound-guided central venous cannulation is now considered as the standard of care, and this has largely replaced blind central venous cannulation using anatomical landmarks. We are reporting a case of inadvertent placement of central venous catheter in the right common carotid artery with the use of ultrasound guidance during emergency surgery for the total correction of Tetralogy of Fallot (ToF). This patient luckily had a favourable outcome despite this inadvertent catheter placement which was not recognised even after completion of surgery .The patient also received drug infusions of inotropes and vasopressors through this malplaced central line into the aorta. Background: The objective of study was to determine the pre-anaesthetic status of intracranial pressure (ICP), using ultrasonographic measurement of optic nerve sheath diameter (ONSD) inpatient scheduled for elective tumour craniotomy. The secondary objective was to compare the diagnostic accuracy of ONSD guided rise in ICP with clinical and radiographic parameters. Couple therapy is successful in treating relationship distress. However, couple therapy does not benefit everyone. Consequently, it is important to study factors that predict therapeutic success. One such factor is what predicts the development of the therapeutic alliance in couple therapy. The purpose of this study was to code therapist behaviors, therapist warmth, empathy, presence, validation, collaboration, and technique factors (systemically-based techniques and session structure), in the first session of couple therapy to examine their ability to predict two aspects of the therapeutic alliance, between- and within-alliance, after the session for males and females. The current study examined the effects of bite blocks on articulatory kinematics when producing /r/ within a phrase. Participants consisted of 20 young adults (10 males, 10 females) with no speech, language or hearing disorders. Participants produced the carrier phrase, I say __ with the nonsense words /əri/ (high front vowel), /əræ/ (low front vowel), /əru/ (high back vowel), /ərɑ/ (low back vowel). Children with Developmental Language Disorder (DLD) often face problems in areas of social communication including negotiating with peers, entering ongoing interactions, and engaging in conflict resolution. A potential cause of these social communication difficulties is the decreased ability to make emotional inferences. This thesis investigates the effects of a social communication intervention on the ability of school-aged children with DLD to make inferences about emotions. Diet composition is the major factor influencing the fatty acid composition of products derived from ruminants. Thirty 7-month-old, Barbados Black Belly × Malin crossbred male sheep were used for the trial to determine the effects of dietary supplementation of oil palm (Eleis guineensis) frond (OPF) on the fatty acid profiles of rumen fluid, muscle tissue and blood lipid parameters. Treatment diets were control diet (CON group, n=10), 25% OPF pellet in diet (% w/w) (HAF group, n=10) and 50% OPF pellet in diet (OPF group, n=10). Cranial cruciate ligament rupture is one of the most common problems in dogs. Previous studies have arrived at no absolute conclusion regarding suture attachment sites using the extracapsular technique to repair cranial cruciate ligaments in dogs. Therefore, the objective of this study was to identify and compare suture attachment sites on the femur and tibia between small and large dog breeds at different stifle angles. Diabetic ketoacidosis (DKA) is a life-threatening complication in diabetic dogs. It is caused by an increase in production and accumulation of ketone bodies in the blood, which results in metabolic acidosis. The objective of this study was to compare the diagnostic accuracy between the measurement of blood beta-hydroxybutyrate (β-OHB) concentration using a point-of-care sensor and plasma acetoacetate (AcAc) level using a urine strip test for the diagnosis of canine DKA. Magnocellular neurosecretory cells (MNCs) are large oxytocin (OT)- and vasopressin (VP)-releasing neurons that secrete these hormones into the circulatory system in response to physiological stimuli. These cells exhibit unique phasic and burst firing patterns to release these peptides into the circulatory system where they primarily control milk ejection and parturition (OT) as well as salt-water balance and vasoconstriction (VP). This firing is underlain by intrinsic ionic mechanisms that shape the duration and frequency of these bursts. One of these mechanisms is the Ca2+-dependent afterhyperpolarization (AHP), which activates during bursts and causes spike frequency adaptation. Occupational therapists working with this population have incorporated a variety of methods into their interventions in an attempt to facilitate greater ease and legibility in handwriting. These interventions include finemotor hand exercise programs, practice-based programs, and visual and auditory cueing (Bryant, Rintala, Lai, DeBakey & Protas, 2010). Improvement in handwriting skills can increase quality of life and independence, as well as decrease frustration, for those with PD (Collett et al., 2017). Glucose-6-phosphate dehydrogenase (G6PD) protects erythrocytes from oxidative stress and hemolysis; G6PD deficiency is the most prevalent enzymopathy. The United States military routinely performs tests to prevent exposing G6PD-deficient personnel to antimalarial drugs that might cause life-threatening hemolytic reactions. In addition, G6PD is a key determinant of vascular function, and its deficiency can lead to impaired nitric oxide production and greater vascular oxidant stress—precursors to atherosclerosis and cardiovascular disease. Using military medical records, we performed a retrospective, cross-sectional study to investigate whether deficient G6PD levels are associated with a higher prevalence of cardiovascular disease than are normal levels, and, if so, …

Obstructive sleep apnea is a sleep-related breathing disorder that has a major impact on cardiovascular function. It has been associated with hypertension, coronary artery disease, cardiac arrhythmias, sudden cardiac death, and heart failure. This review focuses on the relationship between obstructive sleep apnea and heart failure with either reduced or preserved ejection fraction. We discuss the pathophysiology of obstructive sleep apnea, as well as its prevalence, treatment outcomes with continuous positive airway pressure, and prognosis in these 2 distinct types of heart failure. Infection is an important complication of cardiac implantable electronic device procedures. To further study the factors associated with infection, we retrospectively reviewed the records of 3,205 consecutive patients who had undergone de novo or revision cardiac electronic device implantation at our institution from March 2011 through March 2015. We recorded all infections and specified whether they were related to the characteristics of the patient, device, or procedure. To identify predictors of infection, we performed multivariate analysis.
Device infections were identified in 85 patients (2.7%), at a mean follow-up time of 27 ± 11 months. The main predictors of device infection …

As aging involves oxidant injury, we examined the role of the recently described Na/K-ATPase oxidant amplification loop (NKAL). First, C57Bl6 old mice were given a western diet to stimulate oxidant injury or pNaKtide to antagonize the NKAL. The western diet accelerated functional and morphological evidence for aging whereas pNaKtide attenuated these changes. Next, human dermal fibroblasts (HDFs) were exposed to different types of oxidant stress in vitro each of which increased expression of senescence markers, cell-injury, and apoptosis as well as stimulated the NKAL. Further stimulation of the NKAL with ouabain augmented cellular senescence whereas treatment with pNaKtide attenuated it. …

Nipple sparing mastectomy (NSM) has quickly become an accepted technique for patients with selected cancers and for risk reducing surgery. Much of its surgical acceptance over the last decade has been based on the low risk of nipple areolar complex (NAC) occurrence in breast cancer patients. Improved patient satisfaction due to improved cosmetic outcomes with reconstruction have also driven its popularity. We reviewed current English journals to determine the NSM techniques which achieve the lowest complications, best outcomes, and best patient satisfaction. Background—Poor prognosis of pancreatic cancer (PanCa) is associated with lack of an effective early diagnostic biomarker. This study elucidates significance of MUC13, as a diagnostic/prognostic marker of PanCa.
Methods—MUC13 was assessed in tissues using our in-house generated anti-MUC13 mouse monoclonal antibody and analyzed for clinical correlation by immunohistochemistry, immunoblotting, RT-PCR, computational and submicron scale mass-density fluctuation analyses, ROC and Kaplan Meir curve analyses.
Results—MUC13 expression was detected in 100% pancreatic intraepithelial neoplasia (PanIN) lesions (Mean composite score: MCS=5.8; AUC >0.8, P<0.0001), 94.6% of pancreatic ductal adenocarcinoma (PDAC) samples (MCS=9.7, P<0.0001) as compared to low expression in tumor adjacent tissues (MCS=4, P<0.001) along with faint or no expression in normal pancreatic tissues (MCS=0.8; AUC >0.8; P<0.0001). Nuclear MUC13 expression positively correlated with nodal metastasis (P<0.05), invasion of cancer to peripheral tissues (P<0.5) and poor patient survival (P<0.05; prognostic AUC=0.9). Submicron scale mass density and artificial intelligence based algorithm analyses also elucidated association of MUC13 with greater morphological disorder (P<0.001) and nuclear MUC13 as strong predictor for cancer aggressiveness and poor patient survival.

Objective: A growing interest in extracorporeal cardiopulmonary resuscitation (ECPR) as a rescue strategy for refractory adult out-of-hospital cardiac arrest (OHCA) currently exists. This study aims to determine current standards of care and practice variation for ECPR patients in the USA and Korea.
Methods: In December 2015, we surveyed centers from the Korean Hypothermia Network (KORHN) Investigators and the US National Post-Arrest Research Consortium (NPARC) on current targeted temperature management and ECPR practices.
