Atypical/anaplastic meningiomas are prone to aggressive behaviour which affects treatment planning and prognostication. Our aim was to assess the role of Apparent Diffusion Coefficient (ADC) values of MRI brain in differentiating typical from atypical/anaplastic meningioma. We reviewed 84 typical and 37 atypical/anaplastic meningiomas and compared mean ADC values and ADC ratios of their preoperative MRI brain. At 3 Tesla, mean ADC value for typical meningioma was1.03±0.10x10-3 and 0.63±0.05x10-3 for atypical/anaplastic meningioma. วัตถุประสงค์: เพื่อศึกษารูปแบบการจัดการเรียนการสอนของอาจารย์พยาบาล มหาวิทยาลัยราชภัฏเพชรบุรี ในศตวรรษที่ 21\n\nแบบแผนการวิจัย: การวิจัยแบบผสมผสาน (Mixed method)\n\nวิธีดำเนินการวิจัย: ผู้ให้ข้อมูลหลัก มี 2 กลุ่ม คือ 1) อาจารย์พยาบาล จำนวน 10 ราย และ 2) นักศึกษาพยาบาล จำนวน 160 คน เก็บข้อมูลโดยการสัมภาษณ์เชิงลึกและสนทนากลุ่มในกลุ่มอาจารย์พยาบาล และนำข้อมูลมาวิเคราะห์เนื้อหา (Content analysis) และเก็บข้อมูลโดยใช้แบบสอบถามความคิดเห็นที่มีต่อรูปแบบการจัดการเรียนการสอนในศตวรรษที่ 21 ในกลุ่มนักศึกษาพยาบาล วิเคราะห์ข้อมูลโดยใช้สถิติพื้นฐาน ได้แก่ ค่าเฉลี่ย และค่าเบี่ยงเบนมาตรฐาน\n\nผลการวิจัย: อาจารย์พยาบาลมีมุมมองของการจัดการเรียนการสอนในศตวรรษที่ 21 ประกอบด้วย 2 ประเด็นหลัก คือ 1) การพัฒนาหลักสูตร และ 2) การจัดการเรียนการสอน ประกอบด้วย 4 รูปแบบ คือ 1) การสอนในโลกไร้พรมแดนที่มีการเชื่อมต่อความรู้ผ่านเครือข่าย 2) การสอนแบบที่ช่วยให้ผู้เรียนเกิดการเรียนรู้โดยไม่ต้องสอน 3) การสอนแบบเน้นการลงมือปฏิบัติ และ 4) การสอนที่มีบรรยากาศส่งเสริมการเรียนรู้อย่างสร้างสรรค์ โดยนักศึกษาพยาบาลมีความคิดเห็นด้วยมากที่สุด คือ รูปแบบการสอนแบบเน้นการลงมือปฏิบัติ อยู่ในระดับดี (= 4.19, SD = 0.56) รองลงมา คือ รูปแบบการสอนที่มีบรรยากาศส่งเสริมการเรียนรู้อย่างสร้างสรรค์ อยู่ในระดับดี (= 4.18, SD = 0.58) ต่อมาคือ รูปแบบการสอนในโลกไร้พรมแดนที่มีการเชื่อมต่อความรู้ผ่านเครือข่าย พบว่า อยู่ในระดับดี (= 4.11, SD = 0.49) และรูปแบบการสอนแบบที่ช่วยให้ผู้เรียนเกิดการเรียนรู้โดยไม่ต้องสอน พบว่า อยู่ในระดับดี (= 4.06, SD = 0.48)\n\nสรุป: ข้อมูลที่ได้ทำให้เห็นรูปแบบการจัดการเรียนการสอนของอาจารย์พยาบาล สามารถนำไปเป็นข้อมูลพื้นฐานสำหรับอาจารย์พยาบาลในการพัฒนารูปแบบการจัดการเรียนการสอนเพื่อให้บัณฑิตพยาบาลบรรลุความสำเร็จของการจัดการศึกษา และสอดรับกับคุณลักษณะบัณฑิตที่พึงประสงค์ในศตวรรษที่ 21 และทำให้นักศึกษาพยาบาลมีคุณภาพที่ดีขึ้นต่อไป

วัตถุประสงค์: เพื่อศึกษาผลของโปรแกรมการเผชิญความเครียดแบบกลุ่มต่อการแสดงออกทางอารมณ์\n\nของผู้ดูแลผู้ป่วยจิตเภท\n\nแบบแผนการวิจัย: การวิจัยแบบกึ่งทดลอง (Quasi-experimental design)\n\nวิธีการดำเนินการวิจัย: กลุ่มตัวอย่างเป็นผู้ดูแลผู้ป่วยจิตเภท และผู้ป่วยจิตเภท ที่เข้ารับบริการในคลินิกจิตเวชแผนกผู้ป่วยนอก โรงพยาบาลมหาราชนครศรีธรรมราช จำนวน 40 คน จับคู่ด้วยระยะเวลาการเผชิญหน้ากับผู้ป่วยในแต่ละสัปดาห์และความเพียงพอของรายได้ สุ่มเข้ากลุ่มทดลองและกลุ่มควบคุม กลุ่มละ 20 คน กลุ่มทดลองได้รับโปรแกรมการเผชิญความเครียดแบบกลุ่ม กลุ่มควบคุมได้รับการพยาบาลตามปกติ เก็บรวบรวมข้อมูลโดยใช้แบบประเมิน 3 ชุด ดังนี้ 1) แบบสอบถามข้อมูลส่วนบุคคล 2) แบบวัดการแสดงออกทางอารมณ์ของผู้ดูแลผู้ป่วยจิตเภท 3) แบบวัดการเผชิญความเครียดแบบมุ่งแก้ปัญหา โดยแบบประเมินชุดที่ 2 และ 3 มีค่าความเที่ยงสัมประสิทธิ์อัลฟ่าครอนบาค เท่ากับ .89 และ .82 ตามลำดับ วิเคราะห์ข้อมูลโดยใช้สถิติ ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐานและทดสอบค่าเฉลี่ยโดยใช้สถิติทดสอบที\n\nผลการวิจัย: \n1) การแสดงออกทางอารมณ์ของผู้ดูแลผู้ป่วยจิตเภทหลังได้รับโปรแกรมการการเผชิญความเครียดแบบกลุ่มลดลงกว่าก่อนได้รับโปรแกรมการเผชิญความเครียดแบบกลุ่ม อย่างมีนัยสำคัญทางสถิติที่ระดับ .05\n2) การแสดงออกทางอารมณ์ของผู้ดูแลผู้ป่วยจิตเภทกลุ่มที่ได้รับโปรแกรมการเผชิญความเครียดแบบกลุ่มลดลงกว่ากลุ่มที่ได้รับการดูแลตามปกติ อย่างมีนัยสำคัญทางสถิติที่ระดับ .05 \n\nสรุป: โปรแกรมการเผชิญความเครียดแบบกลุ่มมีผลทำให้การแสดงออกทางอารมณ์ของผู้ดูแลผู้ป่วยจิตเภทลดลง

วัตถุประสงค์: เพื่อศึกษาบทบาทของพยาบาลโรงพยาบาลชุมชนในยุคประเทศไทย 4.0\n\nแบบแผนการวิจัย: การวิจัยเชิงพรรณนาโดยใช้เทคนิคเดลฟาย\n\nวิธีดำเนินการวิจัย: กลุ่มตัวอย่าง คือ ผู้เชี่ยวชาญ จำนวน 19 คน ประกอบด้วย ผู้บริหารทางการพยาบาลโรงพยาบาลชุมชน 5 คน ผู้ปฏิบัติการพยาบาลขั้นสูงสาขาเวชปฏิบัติชุมชน 5 คน อาจารย์พยาบาลสาขาพยาบาลชุมชน 3 คน อาจารย์พยาบาลสาขาบริหารการพยาบาล 2 คน แพทย์เวชศาสตร์ครอบครัว 2 คน และผู้กำหนดนโยบาย 2 คน คัดเลือกแบบเจาะจงตามคุณสมบัติ คือ เป็นผู้เชี่ยวชาญการบริหารงานและการปฏิบัติการพยาบาลโรงพยาบาลชุมชน และใช้วิธีการบอกต่อ วิธีดำเนินการวิจัยประกอบด้วย 3 ขั้นตอนคือ ขั้นตอนที่ 1 สัมภาษณ์ผู้เชี่ยวชาญเกี่ยวกับบทบาทของพยาบาลโรงพยาบาลชุมชนในยุคประเทศไทย4.0 ขั้นตอนที่ 2 นำข้อมูลที่ได้มาวิเคราะห์สาระสำคัญแล้วนำมาสร้างแบบสอบถามเพื่อให้ผู้เชี่ยวชาญแสดงความคิดเห็นเกี่ยวกับระดับความสำคัญขององค์ประกอบย่อยในแต่ละบทบาท และขั้นตอนที่ 3 นำความคิดเห็นของผู้เชี่ยวชาญมาคำนวณหาค่ามัธยฐานและค่าพิสัยระหว่างควอไทล์ และให้ผู้เชี่ยวชาญยืนยันความคิดเห็นอีกครั้ง แล้วนำข้อมูลมาคำนวณหาค่ามัธยฐาน และค่าพิสัยระหว่างควอไทล์ เพื่อสรุปผลการวิจัย\n\nผลการวิจัย: บทบาทของพยาบาลโรงพยาบาลชุมชนในยุคประเทศไทย 4.0 มีองค์ประกอบทั้งหมด 7 ด้าน และมีข้อรายการย่อย 55 ข้อ ได้แก่ 1) ด้านผู้ปฏิบัติการพยาบาล จำนวน 9 ข้อ 2) ด้านผู้จัดการรายกรณีโรคเรื้อรัง จำนวน 7 ข้อ 3) ด้านการส่งเสริมสุขภาพผู้สูงอายุ จำนวน 8 ข้อ 4) ด้านหุ้นส่วนสุขภาพ จำนวน 9 ข้อ 5) ด้านผู้จัดการการดูแล จำนวน 8 ข้อ 6) ด้านการจัดการการดูแลระยะกลาง จำนวน 7 ข้อ และ 7) ด้านการพัฒนาคุณภาพ จำนวน 7 ข้อ โดยองค์ประกอบทั้ง 7 ด้าน พบว่า มีข้อรายการย่อยจำนวน 42 …

วัตถุประสงค์: 1) ศึกษาภาวะซึมเศร้า การสนับสนุนทางสังคม และคุณภาพชีวิตของผู้ป่วยโรคไตเรื้อรังระยะสุดท้ายที่ได้รับการฟอกเลือดด้วยเครื่องไตเทียม 2) ศึกษาความสัมพันธ์ระหว่างภาวะซึมเศร้า การสนับสนุนทางสังคม กับคุณภาพชีวิตของผู้ป่วย\n\nแบบแผนการวิจัย: รูปแบบการวิจัย: วิจัยความสัมพันธ์เชิงพรรณนา\n\nวิธีดําเนินการวิจัย: กลุ่มตัวอย่างคือ ผู้ป่วยโรคไตเรื้อรังระยะสุดท้ายที่ได้รับการฟอกเลือดด้วยเครื่องไตเทียม โรงพยาบาลมิชชั่น และโรงพยาบาลธนบุรี จำนวน 80 คน คัดเลือกด้วยวิธีเจาะจง เก็บข้อมูลโดยการตอบ แบบประเมินภาวะซึมเศร้า แบบสอบถามการสนับสนุนทางสังคม และแบบสอบถามคุณภาพชีวิต วิเคราะห์ข้อมูลด้วยสถิติเชิงพรรณนาและสหสัมพันธ์เพียร์สัน \n\nผลการวิจัย: ผู้ป่วยโรคไตเรื้อรังระยะสุดท้ายที่ได้รับการฟอกเลือดด้วยเครื่องไตเทียมไม่มีภาวะซึมเศร้า\n\nมีการสนับสนุนทางสังคมระดับปานกลาง มีคุณภาพชีวิตระดับสูง ภาวะซึมเศร้ามีความสัมพันธ์ทางลบกับคุณภาพชีวิต และการสนับสนุนทางสังคมมีความสัมพันธ์ทางบวกกับคุณภาพชีวิตอย่างมีนัยสำคัญทางสถิติที่ระดับ .01 และ.05 ตามลำดับ (r= -.655** p < .01) (r= .282* p < .05) \n\nสรุป: คุณภาพชีวิตของผู้ป่วยโรคไตเรื้อรังระยะสุดท้ายที่ได้รับการฟอกเลือดด้วยเครื่องไตเทียมมีความสัมพันธ์กับภาวะซึมเศร้าและการสนับสนุนทางสังคม ดังนั้นพยาบาลหรือผู้ดูแลสามารถใช้เป็นข้อมูลในการวางแผนดูแลและพัฒนาคุณภาพชีวิตของผู้ป่วยให้ดีมากขึ้นได้

Eslicarbazepine acetate is an anticonvulsant drug with a recent U.S. Food and Drug Administration approval for expanded use in children and adolescents. Currently, eslicarbazepine acetate is only available in the U.S. as 200-mg to 800-mg strength tablets (Aptiom), which are not easy to administer for pediatric patients. This study was initiated to develop an oral suspension formulation for extemporaneous compounding by pharmacists and to generate stability data for storage recommendations. Nine suspension formulations of eslicarbazepine acetate were prepared from Aptiom tablets and commercially available liquid vehicles using the standard mortar/pestle method. Clinical trials of imetelstat therapy have indicated that this telomerase inhibitor might have disease-modifying effects in a subset of patients with myelofibrosis (MF). The mechanism by which imetelstat induces such clinical responses has not been clearly elucidated. Using in vitro hematopoietic progenitor cell (HPC) assays and in vivo hematopoietic stem cell (HSC) assays, we examined the effects of imetelstat on primary normal and MF HSCs/HPCs. Treatment of CD34(+) cells with imetelstat reduced the numbers of MF but not cord blood HPCs (colony-forming unit-granulocyte/macrophage, burst-forming unit-erythroid, and colony-forming unit-granulocyte/erythroid/macrophage/megakaryocyte) as well as MF but not normal CD34(+)ALDH(+) cells irrespective of the …

Introduction:
Observational studies indicate that cigarette package inserts with efficacy messages about the benefits of quitting (i.e. response efficacy) and recommendations for successful cessation increase smokers’ self-efficacy to quit and promote sustained cessation. However, the effects of inserts with such efficacy messages have not been studied using experimental designs. This study used ecological momentary assessment (EMA) to assess smokers’ responses to efficacy inserts.

Material and Methods:
In a randomized case-crossover study among smokers from the United States (n=23), participants received a one-week supply of cigarettes with efficacy inserts and a one-week supply without any inserts, and were randomized to use …

Listeria monocytogenes, a Gram-positive facultative anaerobic bacterium, is becoming a popular vector for cancer immunotherapy. Indeed, multiple vaccines have been developed utilizing modified Listeria as a tool for generating immune responses against a variety of cancers. Moreover, over a dozen clinical trials testing Listeria cancer vaccines are currently underway, which will help to understand the utility of Listeria vaccines in cancer immunotherapy. This review aims to summarize current views on how Listeria-based vaccines induce potent antitumor immunity and the current state of Listeria-based cancer vaccines in clinical trials. © 2018 by the authors. HIV-1 can downregulate HLA-C on infected cells, using the viral protein Vpu, and the magnitude of this downregulation varies widely between primary HIV-1 variants. The selection pressures that result in viral downregulation of HLA-C in some individuals, but preservation of surface HLA-C in others are not clear. To better understand viral immune evasion targeting HLA-C, we have characterized HLA-C downregulation by a range of primary HIV-1 viruses. 128 replication competent viral isolates from 19 individuals with effective anti-retroviral therapy, show that a substantial minority of individuals harbor latent reservoir virus which strongly downregulates HLA-C. Since the introduction of supported employment in the Developmental Disabilities Act of 1984 and the Rehabilitation Act Amendments of 1986, there has been continued development and refinement of best practices in employment services and supports. Progress includes creative outcomes for individuals with significant support needs including customized jobs and self-employment, community rehabilitation providers that have shifted emphasis to integrated employment, and states that have made a substantial investment in Employment First policy and strategy. The purpose of this brief is to examine the quality of employment services available to job seekers with disabilities, and to offer recommendations for improvement. Findings are from a longitudinal study that involved 61 employment 37 employment programs in 17 states. Chronic granulomatous disease (CGD) is the most common of the primary immunodeficiency in children. It is caused by single gene defect resulting in dysfunctional nicotinamide adenine dineucleotide phosphate (NADPH) oxidase complex causing recurrent bacterial and fungal infections. Here we present the case of a 9 year old boy who was a known case of CGD since three years of age. He presented with recent history of fever, left sided pain in the scapular region and difficulty in breathing. Background: Malaria remains an endemic disease in Pakistan with an estimated healthcare burden of 1.6 million cases annually, with Plasmodium vivax accounting for 67% of reported cases. P. vivax is the most common species causing malaria outside of Africa, with approximately 13.8 million reported cases worldwide.
Method: We report a series of P. vivax cases with cerebral involvement that presented at Aga Khan University Hospital, Karachi, Pakistan.

High-throughput platforms allow the characterization of thousands of previously known methylation sites. These platforms have great potential for investigating the epigenetic effects that are partially responsible for gene expression control. Methylation sites provide a bridge for the investigation of real-time environmental contributions on genomic events by the alteration of methylation status of those sites. Using the data provided by GAW20’s organization committee, we calculated the heritability estimates of each cytosine-phosphate-guanine (CpG) island before and after the use of fenofibrate, a lipid-control drug.
