Access to anticancer drugs is limited mainly due to their high cost. To support policymakers in Thailand to develop policies and programs to facilitate better access to high-cost anticancer drugs, global and local evidence are needed. A systematic review of literature was conducted using PubMed, Embase and Web of Science between October 8-11, 2018 to identify peer-reviewed articles published in English from 2000 to 2018. Studies were included if they described health policies or programs facilitating access to high-cost anticancer drugs. This study aimed to compare the effectiveness of three recovery protocols on muscle oxygenation, blood lactate, and subsequent performance during a 200-m repeated swimming. Twelve swimmers (6 females and 6 males) from Chulalongkorn University swimming club completed three sessions of two consecutive 200-m front crawl trials separated by one of three recovery protocols: a 15-min of active recovery (AR), a 15-min of passive recovery (PR), and a recovery combination of 5-min of active and 10-min of passive recovery (CR) by using a counterbalance design. The main overall goal of this study is to investigate the application of modified chitosan as a potential vector for apoptotic gene delivery to gonadotropin-releasing hormone receptor (GnRHR)-expressing cells (i.e. testicular cells and mammary cancer cells). This study reported Gonadotropin Releasing Hormone-modified Chitosan (GnRH-CS) nanoparticle as a promising vector for targeted gene delivery, and a GnRH peptide was used for active targeting of a transgene in GnRHR expressing cells. The PCR technique is always a definitive confirmatory test of Ehrlichia canis infection. However, the drawback of PCRs test is crucially needed of laboratory equipments. This drawback is somehow enabled newly DNA based technology to be developed in order to eliminate laborious thermocycler and gel electrophoresis. The aim of the present study was to assess the effect of the dietary inclusion of Jerusalem artichoke, Lactobacillus rhamnosus, or their combination, on serum biochemical, growth performance, intestinal morphology, immune parameters, intestinal bacterial count and protection against Aeromonas veronii in juvenile red tilapia (Oreochromis spp.). It is widely used in clinical dermatology such as acne, acne scar, photoaging, wrinkles, psoriasis, skin neoplasia and seborrheic dermatitis. Retinal is a pivotal key to regulate keratinocyte proliferation and differentiation in the epidermis resulting in epidermal thickening and inhibit collagen destruction in the dermis. Flavobacterium columnare is Gram negative bacteria which caused columnaris disease in various kinds of freshwater fishes. This study is the first report of F. columnare infection in freshwater culturing Asian sea bass in Thailand. Phenotypic characterization, antimicrobial susceptibility test and quinolone resistant-associated genes of F. columnare were performed. A total 15 of F. columnare were isolated from diseased fishes and phylogenetic analysis revealed that these isolates were belonged to genetic group 2 and 4. Campylobacter has been known as an important cause of gastroenteritis in humans. The primary source is raw or undercooked poultry meat. Previous study showed that Campylobacter isolated from slaughterhouses were associated with Campylobacter isolated from broiler farms. Thus, detection of Campylobacter flock status and sorting flocks prior to slaughtering processes may help prevent Campylobacter contamination between Campylobacter-positive and negative flocks during slaughtering processes. A total of 809 samples of pig rectal swab from slaughterhouses (n=441) and pork from retail market (n=368) were collected in the border provinces among Thailand, Cambodia, Lao PDR and Myanmar between October 2016 and March 2017. The objective of this study was to determine resistance to extended-spectrum ß-lactams (ESBLs) and colistin in Salmonella enterica and Escherichia coli. A total of Salmonella (n=463) and E. coli (n=767) were collected and determined for the ESBL-production and for the Minimum Inhibitory Concentrations (MICs) of colistin and; the presence of ESBL gene and mcr gene. In this study, we aimed to detect antimicrobial resistance determinants and virulence factor genes in V. parahaemolyticus isolates from cultivated oysters and estuarine waters in southern Thailand. A total of 594 V. parahaemolyticus isolates were collected from pooled oysters (n=361) and estuarine waters (n=233). The samples were collected monthly between March 2016 and February 2017 from Thap Put district, Phang Nga province in southern Thailand. Twenty-six osteoarthritic cats concurrent with chronic kidney disease (CKD) enrolled in this study. All cats were divided randomly into 2 groups; control group (n=12) received placebo and treatment group (n=14) received PCSO-524 for 60 days. Cats had verified stable CKD stage II and had treated and followed up for CKD more than 3 months were included in our study. Fifty stifles from Pomeranians and Chihuahuas with over five years old were enrolled in this study, which are 8 normal stifles and 42 medial patellar luxation (MPL) stifles. All MPL stifles were assigned randomly into 2 groups, including control and treatment groups. The control group was received placebo daily for 16 weeks, while treatment group was fed with PCSO-524 for 16 weeks. Brachycephalic airway obstruction syndromes (BAOS) are the most common respiratory problems in brachycephalic dogs. Surgical treatment which are staphylectomy and alaplasty is commonly used for correction primary lesions of BAOS. The purpose of this study is to compare clinical outcome of post staphylectomy using ultrasonic scalpel and bipolar sealing devices. Twenty brachycephalic dogs were included for staphylectomy and randomly separated into two groups. การศึกษาแบบเก็บข้อมูลย้อนหลังนี้มีวัตถุประสงค์เพื่อสร้างแบบจำลองทางเภสัชจลนศาสตร์และหาปัจจัยที่มีผลต่อค่าพารามิเตอร์ทางเภสัชจลนศาสตร์ของยาแวนโคมัยซินในผู้ป่วยเด็กชาวไทย ค่าความเข้มข้นของ ยาแวนโคมัยซินในซีรั่มจานวน 348 ตัวอย่าง จากผู้ป่วยเด็กอายุระหว่าง 1 เดือน ถึง 18 ปี ที่เข้ารับการรักษา ณ โรงพยาบาลจุฬาลงกรณ์ สภากาชาดไทย ระหว่างปี พ.ศ. 2555-2560 จานวน 212 คน วิเคราะห์ข้อมูลเภสัช จลนศาสตร์ประชากรด้วยโปรแกรม NONMEM® โดยวิธี first-order conditional estimation with interaction ลักษณะทางเภสัชศาสตร์ของยาแวนโคมัยซินอธิบายได้ด้วยแบบจำลองชนิดหนึ่งห้องที่มีการกำจัดยา แบบปฏิกิริยาอันดับหนึ่ง ค่าเฉลี่ยอัตราการกำจัดยาแวนโคมัยซิน (CL) มีค่า 0.13 ลิตร/ชั่วโมง/กิโลกรัม และ ค่าเฉลี่ยปริมาตรการกระจายยา (Vd) มีค่า 0.88 ลิตร/กิโลกรัม ปัจจัยที่มีผลต่อค่าพารามิเตอร์ทางเภสัชศาสตร์ ได้แก่ น้ำหนัก (กิโลกรัม) และอัตราการกรองของไต (มิลลิลิตร/นาที/1.73 ตารางเมตร) ดังแสดงในสมการ CL (ลิตร/ชั่วโมง) = 1.66*(น้ำหนัก/14)[superscript 0.75]*(อัตราการกรองของไต/108.9) และ V (ลิตร) = 12.7*(น้ำหนัก/14) ค่า ความผันแปรระหว่างบุคคลของ CL เท่ากับร้อยละ 34.8 ค่าความผันแปรระหว่างบุคคลของ V เท่ากับร้อยละ 39.6 และแบบจำลองสุดท้ายถูกตรวจสอบความสมเหตุสมผลด้วยวิธี bootstrap และ visual predictive check (VPC) ค่าพารามิเตอร์จากแบบจำลองสุดท้ายมีค่าใกล้เคียงกับค่าพารามิเตอร์ที่ได้จากการทำ bootstrap 1,000 ครั้ง และอยู่ในช่วงความเชื่อมั่นที่ร้อยละ 95 ผลการจำลองข้อมูลด้วยวิธี VPC 1,000 ครั้ง พบค่าความเข้มข้นของ ยาแวนโคมัยซินร้อยละ 6 ที่อยู่นอกช่วงความเชื่อมั่นที่ร้อยละ 90 แสดงถึงความเหมาะสมของแบบจำลองดังกล่าว ในการอธิบายเภสัชจลนศาสตร์ของยาแวนโคมัยซิน

สาร Oxyresveratrol เป็นสารสำคัญในแก่นของต้นมะหาด โดยได้มีการศึกษาถึงฤทธิ์ทางเภสัชวิทยาของสาร Oxyresveratrol ในหลายด้าน แต่อย่างไรก็ตามสาร Oxyresveratrol เป็นสารที่มีค่าชีวปริมาณออกฤทธิ์ต่ำจึงเป็นอุปสรรคที่สำคัญในการนำมาพัฒนาเป็นยารับประทาน ดังนั้นจุดประสงค์ในการศึกษานี้ เพื่อศึกษาเภสัชจลนศาสตร์ของสาร Oxyresveratrol ที่เปลี่ยนไปเมื่อให้สาร Piperine เป็นสารเพิ่มชีวปริมาณออกฤทธิ์ในหนูแรท ซึ่งขนาดที่ให้แบ่งเป็นสาร Oxyresveratrol เดี่ยวขนาด 10 mg/kg หรือ สาร Oxyresveratrol ขนาด 10 mg/kg ให้ร่วมกับสาร Piperine 1 mg/kg โดยการบริหารยาทางหลอดเลือดดำ และ สาร Oxyresveratrol เดี่ยวขนาด 100 mg/kg หรือ สาร Oxyresveratrol ขนาด 100 mg/kg ให้ร่วมกับสาร Piperine 10 mg/kg โดยการบริหารยาทางปากจากนั้นเก็บตัวอย่างเลือด, อวัยวะ, ปัสสาวะ และ อุจจาระ ตามเวลาที่กำหนดนำไปวิเคราะห์ด้วยเครื่อง LC-MS/MS เพื่อวิเคราะห์ข้อมูลทางเภสัชจลนศาสตร์ จากผลการศึกษาพบว่าสาร Piperine สามารถเพิ่มค่าชีวปริมาณออกฤทธิ์ของสาร Oxyresveratrol ขึ้นประมาณ 2 เท่า รวมถึงสามารถช่วยเพิ่มค่าความเข้มข้นของสาร Oxyresveratrol สูงสุดในพลาสมาโดยการบริหารทางปากและ เมื่อคำนวณอัตราส่วนปริมาณยาในอวัยวะต่อพลาสมามีค่าประมาณ 10-100 เท่า ซึ่งสอดคล้องกับค่าปริมาณการกระจายยาที่สูง ประมาณ 50 L/kg ในกระบวนการเปลี่ยนแปลงสภาพของสารเมื่อได้รับร่วมกับสาร Piperine มีค่า Conversion ratio ลดลงในทั้งสองรูปแบบการบริหาร โดยสรุปสาร Piperine สามารถปรับปรุงค่าทางเภสัชจลนศาสตร์ของสาร Oxyresveratrol ได้ทั้งในทั้งสองรูปแบบการบริหารยา การศึกษาทางเภสัชจลนศาสตร์นี้จึงสามารถนำมาเป็นข้อมูลในการพัฒนาสาร Oxyresveratrol เพื่อใช้ประโยชน์ต่อไปได้

ผู้ป่วยมะเร็งเต้านมที่มีการแสดงออกของตัวรับเอสโตรเจนในระยะลุกลามร้อยละ 40 สามารถกลับเป็นมะเร็งซ้ำเนื่องจากมีการดื้อยา tamoxifen แพทย์จึงเปลี่ยนไปใช้ยาต้านฮอร์โมนกลุ่มอื่น แต่ยังเกิดการดื้อยาและมีผลข้างเคียงที่รุนแรง ดังนั้นการนำสารจากธรรมชาติมาพัฒนาเป็นยาที่มีฤทธิ์ยับยั้งการเจริญของเซลล์มะเร็งจึงเป็นแนวทางที่อาจลดปัญหาดื้อยาได้ สารสกัดจากต้นจันทน์หอม (Mansonia Gagei Drumm) ยับยั้งการเจริญของเซลล์มะเร็งได้หลายชนิด โดย mansonone G (MG) สารสำคัญที่แยกจากสารสกัดสามารถแย่งจับกับตัวรับเอสโตรเจนได้ ในการศึกษานี้จึงทำการทดสอบฤทธิ์อนุพันธ์ MG คือ Ethoxy mansonone G (EMG) สารกึ่งสังเคราะห์ในการยับยั้งการเจริญ การลุกลามและการยับยั้งการเจริญแบบไร้การยึดเกาะ พบว่าเมื่อเพิ่มความเข้มข้นและระยะเวลาในการทดสอบ EMG สามารถยับยั้งการเจริญในเซลล์มะเร็งเต้านมชนิดที่ไม่ดื้อยา (MCF-7) และเซลล์มะเร็งเต้านมที่ดื้อต่อยาต้านฮอร์โมน (MCF-7/LCC2, MCF-7/LCC9) อย่างมีนัยสำคัญทางสถิติและมีความเป็นพิษน้อยกว่ายาเคมีบำบัดในเซลล์ไฟโปรบลาสต์ และพบว่ากลไกการออกฤทธิ์ของ EMG ในการยับยั้งการเจริญของเซลล์ MCF-7 ส่วนหนึ่งผ่านการลดการแสดงออกของยีนที่เป็นเป้าหมายของตัวรับเอสโตรเจนและสาร EMG ยังสามารถลดการแสดงออกของยีนที่เกี่ยวกับการดื้อต่อยา tamoxifen ใน MCF-7/LCC2 และ MCF-7/LCC9 นอกจากนี้ EMG ยังยับยั้งการลุกลามของ MCF-7/LCC2 และ MCF-7/LCC9 โดยลดการแสดงออกของ MMP-9 และ uPA ทั้งยังสามารถลดการเจริญแบบไร้การยึดเกาะผ่านการลดจำนวน ขนาดของ colony ใน MCF-7/LCC2 และ MCF-7/LCC9 สรุปว่าสาร EMG มีคุณสมบัติในการต้านมะเร็งเต้านมชนิดไม่ดื้อยาและชนิดที่ดื้อต่อยาต้านฮอร์โมน ต้านการลุกลาม รวมไปถึงลดการเจริญแบบไร้การยึดเกาะของ MCF-7/LCC2 และ MCF-7/LCC9 ซึ่งอาจนำไปพัฒนาเป็นยาชนิดใหม่ในการรักษาผู้ป่วยมะเร็งเต้านมที่ดื้อต่อยาต้านฮอร์โมน

ยาฟิแนสเทอไรด์ (finasteride) เป็นยาในกลุ่ม 5-alpha reductase inhibitor ใช้รักษาโรคผมร่วงทางพันธุกรรมในเพศชาย โดยยาฟิแนสเทอไรด์อาจทำให้เกิดผลข้างเคียงหลายประการได้แก่ การเสื่อมสมรรถภาพทางเพศ ปริมาณน้ำอสุจิลดลง ดังนั้นควรมีการถูกใช้ในรูปแบบยาทาหนังศีรษะเพื่อลดผลข้างเคียงดังกล่าว โดยการศึกษานี้มีวัตถุประสงค์เพื่อศึกษาระดับยาฟิแนสเทอไรด์ในเลือดและในชั้นผิวหนัง หลังการให้ยาความแรง 0.5% ด้วยการทาผิวหนังเพียงครั้งเดียว (single dose) และแบบทาผิวหนังหลายครั้ง (repeated dose) บนพื้นที่ 4 ตารางเซนติเมตรของหนูแรทเพศผู้สายพันธุ์ Sprage-Dawley โดยมีการเก็บพลาสมาและผิวหนังภายใน 24 ชั่วโมง หลังการทายาครั้งสุดท้ายและสกัดยาออกมาเพื่อตรวจวัดระดับยาฟิแนสเทอไรด์ด้วยเครื่อง LC-MS/MS ผลการศึกษาแสดงให้เห็นว่า ระดับยาฟิแนสเทอไรด์ในพลาสมาหลังการให้ยาด้วยการทาผิวหนังเพียงครั้งเดียวมีแนวโน้มการดูดซึมเข้ากระแสเลือดเพิ่มขึ้นตามปริมาตรยาที่ทา โดยเริ่มตรวจพบระดับยาที่เวลา 4 ชั่วโมง หลังทายาในปริมาตร 300 ไมโครลิตร เพียงครั้งเดียว แต่ไม่พบระดับยาในพลาสมาของกลุ่มที่ได้รับยาปริมาตร 75 หรือ 150 ไมโครลิตร นอกจากนี้การทายาซ้ำวันละ 1 ครั้งและทายาซ้ำวันละ 2 ครั้ง ติดต่อกัน 7 วัน ไม่มีนัยสำคัญต่อระดับยาในพลาสมา ซึ่งเห็นได้ว่ายาฟิแนสเทอไรด์ถูกดูดซึมเข้ากระแสเลือดได้น้อยหลังการให้ยาด้วยการทาผิวหนัง สำหรับอัตราส่วนระหว่างระดับยาในผิวหนังและระดับยาในพลาสมาที่เวลา 24 ชั่วโมง หลังได้รับยาทางปากเพียงครั้งเดียวมีค่าประมาณ 2.6 เท่า โดยพบอัตราส่วนระหว่างระดับยาในผิวหนังบริเวณที่ไม่สัมผัสยา (control sites) และระดับยาในพลาสมาเกินกว่า 2.6 เท่า ในหนูแรทกลุ่มที่ได้รับยาทางผิวหนังทุกกลุ่ม ทั้งนี้ค่าที่ตรวจพบระดับยาในพลาสมามีความแปรปรวนสูงมาก อย่างไรก็ตามผลการศึกษานี้แสดงให้เห็นว่า ยาฟิแนสเทอไรด์สามารถสะสมอยู่ในผิวหนังชั้นหนังแท้ (dermis) และถูกดูดซึมเข้าสู่กระแสเลือดได้น้อยหลังให้ยาด้วยการทาผิวหนัง ทั้งนี้จำนวนครั้งการทาทั้งการทาซ้ำวันละ 1 ครั้งและการทาซ้ำวันละ 2 ครั้งอาจไม่มีผลอย่างมีนัยสำคัญต่อการดูดซึมยาเข้าสู่กระแสเลือด

งานวิจัยนี้ศึกษาการเปลี่ยนแปลงของค่าคะแนนอาการปวดเข่าลูกสะบ้า (Kujala Score), Pain Scale, ค่า Single Leg Hop Test (SLHT), ค่า Step Down Test (SDT) และค่าอัตราส่วนแบบทำงานของความแข็งแรงกล้ามเนื้อเหยียดเข่าต่อกล้ามเนื้องอเข่า (Functional Qecc/Hcon ratio) หลังได้รับโปรแกรมการออกกำลังกายที่บ้าน 24 สัปดาห์ ในนักวิ่งสมัครเล่นเพศชายและเพศหญิงที่มีอาการปวดเข่าลูกสะบ้า จำนวน 59 คน เป็นเพศชาย 30 คน เพศหญิง 29 คน วัดค่า Kujala Score, Pain Scale, SLHT, SDT และค่า Functional Qecc/Hcon ratio ก่อนเริ่มต้นการออกกำลังกาย (T0) ครบ 8 สัปดาห์ (T8) ครบ 16 สัปดาห์ (T16) และเมื่อสิ้นสุด 24 สัปดาห์ (T24) ผลการศึกษาพบว่า ผู้เข้าร่วมวิจัยทั้งหมดมีค่า Kujala Score เพิ่มขึ้น จากที่ T0 มีค่า 78.95 ± 9.42 และที่ T24 มีค่าเป็น 99.50 ± 0.82, ค่า Pain Scale ที่ T0 มีค่า 5.61±1.43 ที่ T24 มีค่า 0.03±0.18, ค่า SLHT และค่า SDT มีค่าผลลัพธ์เปลี่ยนแปลงดีขึ้น ค่า Functional Qecc/Hcon ratio ที่ T0 มีค่า 1.34 ± …

การวิจัยโดยการทดลองในครั้งนี้ มีวัตถุประสงค์ เพื่อศึกษาผลของโปรแกรมการฝึกเกร็งกล้ามเนื้อหน้าท้องต่ออัตราการตอบสนองของกล้ามเนื้อที่ใช้ในการเหยียดหลังขณะยืนก้มลำตัวในอาสาสมัครที่มีอาการปวดเอวที่มีอายุระหว่าง 20-40 ปี จำนวน 44 คน โดยถูกสุ่มออกเป็น 2 กลุ่ม คือกลุ่มทดลอง (N=22) และกลุ่มควบคุม (N=22) ซึ่งกลุ่มทดลองได้รับโปรแกรมการฝึกออกกำลังกายจำนวน 3 ครั้งต่อสัปดาห์ เป็นระยะเวลา 4 สัปดาห์ สำหรับกลุ่มควบคุมได้รับคู่มือเกี่ยวกับอาการปวดหลัง ผลการศึกษาพบว่าหลังจากการทดลองกลุ่มที่ได้รับโปรแกรมการออกกำลังกาย มีความแตกต่างกันอย่างมีนัยสำคัญทางสถิติระหว่างก่อนและหลังการทดลอง ของเปอร์เซ็นต์ของอัตราการตอบสนองของกล้ามเนื้อที่ใช้ในการเหยียดหลังเท่ากับ 42.52±25.32 และ 29.08±14.21 ตามลำดับ (P=0.015) มุมการเคลื่อนไหวของกระดูกสันหลังระดับเอวเท่ากับ 44.95±9.78 และ 37.28±8.66 องศา (P=0.017) มุมการเคลื่อนของข้อสะโพกเท่ากับ 42.17±17.62 และ 54.81±19.26 องศา (P=0.001) ค่าดัชนีชี้วัดระดับความสามารถในการทำกิจวัตรประจำวันเท่ากับ 16.89±2.16 และ 3.91±1.06 (P<0.05) และระดับความปวดระหว่างทำกิจกรรม 5.32±1.67 และ 2.26±1.76 (P<0.05) สำหรับกลุ่มควบคุมดังนั้นผลการศึกษานี้แสดงให้เห็นว่าโปรแกรมการฝึกเกร็งกล้ามเนื้อหน้าท้องส่งผลต่อการลดลงของอัตราการตอบสนองของกล้ามเนื้อที่ใช้ในการเหยียดหลังขณะก้มลำตัวในระยะสุดท้าย มีองศาการเคลื่อนไหวของกระดูกสันหลังระดับเอวลดลงและข้อสะโพกเพิ่มขึ้น การจำกัดการใช้ชีวิตประจำวันเนื่องจากอาการปวดน้อยลงและระดับปวดบริเวณเอวลดลง โดยโปรแกรมการออกกำลังกายนี้สามารถใช้เป็นทางเลือกในการรักษาและให้คำแนะนำสำหรับผู้ป่วยที่มีอาการปวดเอว
