Below-Knee Amputations: A Review, Erika L. Garbrecht

UNM Orthopaedic Research Journal

At our institution, physicians from the orthopaedic department perform a considerable proportion of below-knee amputations. These amputations are performed to treat non-traumatic and traumatic injuries. However, the outcomes of amputation are not discussed as often in our educational experience as those of other procedures. The current review examines the current and relevant studies for indications and outcomes of patients with non-traumatic and traumatic conditions treated with below-knee amputations.

Background: Radial head arthroplasty (RHA) is typically performed to restore elbow stability or function in patients with fractures or degenerative joint diseases. The procedure requires a specific operating technique to avoid complications such as overstuffing, capitellar erosion, stiffness, instability, micro motion, and loosening. It is difficult to restore native radial head function reliably.

Methods: We reviewed the medical records of 45 patients who underwent radial head arthroplasty using the ALIGN radial head implant (ALIGN Radial Head System, Skeletal Dynamics, Miami, FL) at our institution. Chronic Tenosynovitis Caused By Mycobacterium Avium-Intracellulare Of The Flexor Tendons In A 65-Year-Old Man: A Case Report, Kourosh M. Kolahi, Moheb S. Moneim

We report a rare case of Mycobacterium aviumintracellulare flexor tenosynovitis in the hand of a 65-year-old man. The patient reported no history of penetrating trauma but was an avid gardener and presented nearly 2 years after the onset of symptoms. Our case highlights the difficulty in diagnosing mycobacterial infections owing to their lack of classic clinical and laboratory findings. Our patient underwent successful surgical debridement followed by oral antibiotic therapy, initiated 4 weeks after the culture was positive for a slow-growing organism.

Chronic vascular occlusion in the upper extremity can result from repetitive trauma, atherosclerosis, proximal embolic events, hypercoagulable states, and systemic diseases such as collagen vascular disease and vasculitis. Considerable functional impairment can result from these maladies; however, sometimes the condition develops slowly with minimal effect on the patient. We describe a 16-year-old soccer player with slow-progressing elbow pain and loss of range in motion caused by brachial artery vasculitis and resultant brachial arterial stenosis. การสูญเสียการได้ยินในเด็กเป็นปัญหาสำคัญอย่างหนึ่งของระบบสาธารณสุข ซึ่งถ้าได้รับการตรวจพบและให้การดูแลอย่างทันท่วงทีจะส่งผลดีต่อพัฒนาการด้านภาษาและพัฒนาการอื่น ๆของเด็ก เป้าหมายในการตรวจคัดกรองการได้ยินในเด็กทารกควรจะได้รับการคัดกรองการได้ยินก่อนอายุ 1 เดือน ทารกที่ตรวจคัดกรองเบื้องต้นไม่ผ่านควรได้รับการยืนยันว่าสูญเสียการได้ยินภายในอายุ3 เดือน และควรได้รับการฟื้นฟูการได้ยินภายในอายุ 6 เดือน ปัจจุบันเครื่องมือที่ใช้ในการตรวจคัดกรองการได้ยินในเด็กมี 2 ชนิด คือ automated evoked otoacoustic emission และ automatedauditory brainstem response ส่วนการตรวจเพื่อยืนยันการสูญเสียการได้ยินนั้นประกอบด้วยการตรวจหลายชนิดร่วมกันทั้ง diagnostic evoked otoacoustic emission, diagnostic auditory brainstemresponse และ Tympanometry นอกจากนี้เด็กควรได้รับการตรวจหาสาเหตุการสูญเสียการได้ยินซึ่งเกิดจากหลายสาเหตุด้วยกัน เช่น ความผิดปกติทางพันธุกรรม การติดเชื้อขณะอยู่ในครรภ์และแรกคลอด การได้รับยาบางชนิดที่ส่งผลต่อหู ฯลฯ เด็กที่ผ่านการประเมินการได้ยินยังต้องได้รับการติดตามประเมินพัฒนาการทางด้านภาษาที่อายุ 9, 18, 24 - 30 เดือน หรือเมื่อผู้ปกครองสงสัยว่ามีปัญหาการได้ยิน โดยเฉพาะอย่างยิ่งเด็กที่มีปัจจัยเสี่ยงต่อการสูญเสียการได้ยินควรได้รับการตรวจประเมินการได้ยินอีกครั้งที่อายุประมาณ 24 - 30 เดือน เนื่องการสูญเสียการได้ยินอาจเกิดในภายหลังได้ เด็กที่บกพร่องทางการได้ยินควรได้รับการฟื้นฟูการได้ยินและกระตุ้นพัฒนาการ ซึ่งปัจจุบันมีอุปกรณ์ช่วยฟังหลายชนิดด้วยกัน ได้แก่ เครื่องช่วยฟัง เครื่องช่วยฟังชนิดฝังในกระดูก และประสาทหูเทียมอย่างไรก็ตามเนื่องจากประเทศไทยมีข้อจำกัดทั้งด้านบุคลากรทางการแพทย์ นักตรวจการได้ยินเครื่องมือที่ใช้ตรวจคัดกรองและประเมินการได้ยิน การเข้าถึงอุปกรณ์ช่วยฟังซึ่งส่วนใหญ่มีราคาสูงจึงจำเป็นที่จะต้องปรับเปลี่ยนแนวทางการคัดกรองให้เหมาะสมกับแต่ละสถาบัน.

เหตุผลของการทำวิจัย : การติดเชื้อไวรัสตับอักเสบซี (HCV) เป็นสาเหตุสำคัญของโรคมะเร็งตับในไทย ความชุกของจีโนไทป์ของ HCV แตกต่างกันไปตามที่ตั้งทางภูมิศาสตร์แต่พบมีการศึกษาความชุกของ HCV ในกรุงเทพมหานครน้อยวัตถุประสงค์ : เพื่อศึกษาอัตราการติดเชื้อ การกระจายตัวจีโนไทป์ของไวรัสตับอักเสบซี ในผู้ป่วยที่มารับบริการที่โรงพยาบาลจุฬาลงกรณ์วิธีการทำวิจัย : ศึกษาย้อนหลังจากฐานข้อมูลระบบสารสนเทศจากฝ่ายจุลชีววิทยาโรงพยาบาลจุฬาลงกรณ์ พ.ศ. 2555 - 2558 จากผู้ป่วยทั้งหมด33,911 ราย มีผู้ป่วยที่มีผลบวกต่อแอนติบอดีต่อไวรัสตับอักเสบซี1,262 ราย (ร้อยละ 3.7) ในผู้ป่วยที่มีผลบวกต่อแอนติบอดีต่อไวรัสตับอักเสบซี มีการตรวจพบ HCV อาร์เอ็นเอ และจำแนกจีโนไทป์จำนวน544 ตัวอย่าง โดยเป็นตัวอย่างจากผู้ป่วยชาย 353 ราย ผู้ป่วยหญิง191 ราย มีอายุเฉลี่ย 52.6 ปี โดยการตรวจ HCV อาร์เอ็นเอ ใช้หลักการ real-time reverse transcription-polymerase chain reaction(RT-PCR) โดยวิธี COBAS TaqMan HCV ส่วนการจำแนกจีโนไทป์ใช้วิธี Linear Array HCV Genotypingผลการศึกษา : ตัวอย่างที่มีผลบวกต่อแอนติบอดีต่อไวรัสตับอักเสบซี 1,262 ราย(อายุเฉลี่ย 51.9 ปี เพศชาย 773 ราย และเพศหญิง 489 ราย) ได้ส่งตรวจจีโนไทป์ 672 ราย มีการตรวจพบ HCV อาร์เอ็นเอด้วยวิธี RT-PCRและจำแนกจีโนไทป์จำนวน 544 ราย ซึ่งพบจีโนไทป์ 1 ร้อยละ 48.5จีโนไทป์ 2 ร้อยละ 1.1, จีโนไทป์ 3 ร้อยละ 45.4 จีโนไทป์ 6 ร้อยละ 1.5และจีโนไทป์ผสมร้อยละ 3.5 ตามลำดับสรุป : อัตราการติดเชื้อไวรัสตับอักเสบซีในผู้ป่วยที่มารับบริการที่โรงพยาบาลจุฬาลงกรณ์ เฉลี่ยร้อยละ 3.7 …

เหตุผลของการวิจัย : LINE-1 เป็น non-long terminal retrotransposon ที่อยู่ในจีโนมมนุษย์ประมาณร้อยละ 17 โดยกระบวนการ LINE-1 methylation ที่ผิดปกติเป็นสาเหตุที่ทำให้เกิด mitotic recombination และความไม่เสถียรของจีโนมส่งผลต่อการเกิดโรคเนื้องอกเนื้อเยื่ออ่อน อย่างไรก็ตามการศึกษากระบวนการ LINE-1 methylation ในโรคเนื้องอกเนื้อเยื่ออ่อนยังมีจำนวนน้อยมากวัตถุประสงค์ : เพื่อตรวจสอบระดับ LINE-1 methylation ในเนื้องอกเปรียบเทียบกับในเนื้อเยื่อปกติข้างเคียงของผู้ป่วยโรคเนื้องอกเนื้อเยื่ออ่อน ตลอดจนวิเคราะห์ความสัมพันธ์ของระดับ LINE-1 methylation ระหว่างเนื้องอกและเซลล์เม็ดเลือดขาววิธีการศึกษา : ทำการตรวจวัดระดับ LINE-1 methylation ในเนื้องอก เนื้อเยื่อปกติข้างเคียงและเซลล์เม็ดเลือดขาวของผู้ป่วยโรคเนื้องอกเนื้อเยื่ออ่อนจำนวน 35 ราย รวมถึงกลุ่มควบคุมที่มีสุขภาพดีจำนวน 107 รายด้วยเทคนิค quantitative combined bisulfite restriction analysisผลการศึกษา : ระดับ LINE-1 methylation ในเนื้องอกมีระดับต่ำกว่าในเนื้อเยื่อปกติข้างเคียง อย่างมีนัยสำคัญ (P = 0.036) แต่ระดับ LINE-1 methylationไม่แตกต่างกันทั้งในเนื้องอกชนิดไม่รุนแรงและรุนแรงเมื่อเปรียบเทียบกับเนื้อเยื่อปกติข้างเคียง นอกจากนี้ระดับ LINE-1 methylation ในเนื้องอกเนื้อเยื่ออ่อนแต่ละชนิดไม่แตกต่างกัน อย่างไรก็ตามพบความสัมพันธ์เชิงบวกระหว่าง LINE-1 methylation ในเนื้องอกและในเซลล์เม็ดเลือดขาว(r = 0.812, P < 0.001)สรุป : ผลการศึกษานี้แสดงให้เห็นว่า ระดับ LINE-1 methylation ต่ำลงในเนื้องอกเนื้อเยื่ออ่อน ดังนั้นกระบวนการ LINE-1 methylation ต่ำลงอาจมีบทบาทสำคัญต่อพยาธิกำเนิดของโรคเนื้องอกเนื้อเยื่ออ่อน.

เหตุผลของการทำวิจัย : ผู้สูงอายุที่มีความรู้สึกมีคุณค่าต่อตนเองจะมีพลังเชิงบวกในการพัฒนาตนเอง สามารถดูแลตนเองได้ และลดการเป็นภาระในสังคม ผู้วิจัยจึงมีความสนใจศึกษาเรื่องความรู้สึกมีคุณค่าในตนเองและปัจจัยที่เกี่ยวข้องของผู้สูงอายุในเขตชนบท ซึ่งมีรูปแบบการดำเนินชีวิตอีกลักษณะหนึ่ง เพื่อใช้เป็นแนวทางในการส่งเสริมความรู้สึกมีคุณค่าในตนเองที่สอดคล้องกับบริบทของสังคมผู้สูงอายุในเขตชนบทวัตถุประสงค์ : เพื่อศึกษาระดับความรู้สึกมีคุณค่าในตนเองของผู้สูงอายุ และเพื่อศึกษาปัจจัยที่มีความสัมพันธ์กับความรู้สึกมีคุณค่าในตนเองของผู้สูงอายุวิธีการทำวิจัย : ผู้สูงอายุ อายุเท่ากับหรือมากกว่า 60 ปีขึ้นไปจำนวน 340 ราย ใช้วิธีการสุ่มตัวอย่างแบบง่าย (simple random sampling) เครื่องมือที่ใช้ในการเก็บรวบรวมข้อมูล คือ แบบสอบถามปัจจัยส่วนบุคคล และแบบประเมินความรู้สึกมีคุณค่าในตนเองของ Coopersmith มีค่าความเชื่อมั่นดังนี้ ความรู้สึกมีคุณค่าในตนเอง 0.90 การใช้หลักธรรมะในการดำเนินชีวิต 0.85 สัมพันธภาพกับบุคคลในครอบครัว 0.84สัมพันธภาพกับบุคคลในสังคม 0.76 และสิ่งแวดล้อมในสังคม 0.78ผลการศึกษา : กลุ่มตัวอย่างเป็นเพศหญิงร้อยละ 59.1 กลุ่มอายุ 60 – 69 ปี มากที่สุดร้อยละ 56.2 มีความรู้สึกมีคุณค่าในตนเองระดับปานกลางมากที่สุดร้อยละ 62.9 รองลงมาต่ำและสูงร้อยละ 21.8 และ 15.3 ตามลำดับปัจจัยที่มีความสัมพันธ์กับความรู้สึกมีคุณค่าในตนเองอย่างมีนัยสำคัญ (P < 0.05) ภาวะสุขภาพ ความสามารถในการปฏิบัติกิจวัตรประจำวันโรคประจำตัว การรับรู้ภาวะสุขภาพกายเมื่อเทียบกับคนวัยเดียวกัน การใช้เวลาว่างในการทำกิจกรรม งานอดิเรก กีฬา และการพบปะติดต่อกับบุตร บทบาทในครอบครัว ได้แก่ ช่วยทำงานประกอบอาชีพ เป็นที่ปรึกษาครอบครัว เลี้ยงดูบุตรหลาน ช่วยเหลืองานบ้านเล็ก ๆ น้อย ๆ สัมพันธภาพกับบุคคลในครอบครัว (r = 0.56)สัมพันธภาพกับบุคคลในสังคม (r = 0.59) และสิ่งแวดล้อมในสังคม(r = 0.55) มีความสัมพันธ์ทางบวกปัจจัยที่ไม่มีความสัมพันธ์ทางสถิติกับความรู้สึกมีคุณค่าในตนเอง (P >0.05) ได้แก่ อายุ รายได้เฉลี่ยต่อเดือน การใช้หลักธรรมะในการดำเนินชีวิตสรุป : หน่วยงานที่มีบทบาทหน้าที่ในการดูแลผู้สูงอายุ ควรดูแลผู้สูงอายุในเรื่องภาวะสุขภาพความสามารถในการปฏิบัติกิจวัตรประจำวันโรคประจำตัว การรับรู้ภาวะสุขภาพเมื่อเทียกับคนวัยเดียวกัน การใช้เวลาว่างในการทำกิจกรรม การพบปะติดต่อกับบุตร การมีบทบาทในครอบครัว เรื่อง ช่วยทำงานประกอบอาชีพ เป็นที่ปรึกษา เลี้ยงดูบุตรหลาน ช่วยเหลืองานบ้านเล็ก ๆ น้อย ๆ มีสัมพันธภาพกับบุคคลในครอบครัว มีสัมพันธภาพกับบุคคลในสังคม และสิ่งแวดล้อมในสังคม.

This study evaluated characteristics of food blogs and food bloggers that potentially affect food choices. It was hypothesized that most food bloggers would not have a nutrition-related degree and food blog environments wouldn’t be supportive of health-conscious food choices. A cross-sectional study was conducted using a novel evaluation tool on 187 blogs, out of which 100 blogs met inclusion criteria. Because some blogs had multiple bloggers, two samples were investigated: 111 bloggers (n1 = 111) and 100 food blogs (n2 = 100). Postpartum depression is a prevalent condition (American Psychological Association, 2018). Untreated postpartum depression can have serious consequences for the mother and baby (McCurdy, Boule, Sivak, & Davenport, 2017). Traditional treatment options may not be feasible. Exercise is an effective treatment for adults with depression (McCurdy et al., 2017). Less is known about treating postpartum depression with exercise. This review of the literature sought to understand in postpartum mothers, how a regimen of regular exercise, compared to no regular exercise, impacted the perception of wellbeing and/or postpartum depression scores. Anxiety is common in pregnancies that occur after a miscarriage or perinatal loss and can lead to detrimental effects on the mother and fetus. The objective of this literature review is to examine whether caring-based nursing actions and anxiety-reducing interventions decrease the self-report of anxiety more than self-management alone. A literature search of four journal databases was conducted using related keywords which produced 429 studies for consideration. A total of 57 studies were reviewed for a match with the inclusion criteria. The thesis exhibition I create will be an installation that is centered around a fictional narrative starring a vivacious mental patient, and containing the people, places, and things in which she interacts with in her life at the hospital. As the audience moves through the gallery, they are immersed into the main character's world, seeing and analyzing the rooms and objects through the eyes and mind of the protagonist. The confidence in which the patient speaks of her environment could lead spectators to believe that her experiences in the hospital are an irrefutable truth. It is only after closely looking …

The purpose of this study was to compare ground contact time between an optical measurement system and a force platform. Participants in this study included six collegiate level athletes who performed drop jumps and sprint strike steps for a total of 15 repetitions each. Ground contact data was simultaneously collected from an optical measurement system and a force platform, at a sampling frequency of 1000 Hz. Data was then analyzed with Pearson’s correlation and paired sample t-tests. The measures from the optical measurement system were found to be significantly higher (p < 0.001) than measures from the force platform in both conditions. Although significantly different, the extremely large relationships (0.979, 0.993) found between the two devices suggest the optical sensor is able to detect similar changes in performance to that of a force platform. Practitioners may continue to utilize optical sensors to monitor performance as it may provide a superior user-friendly alternative to more traditional based monitoring procedures, but must comprehend the inherent limitations due to the design of the optical sensors.

The United States is in the throes of an opioid epidemic. Any person who uses an opioid medication may experience an intentional or unintentional overdose. An overdose is more likely to occur when an individual ingests an opioid in combination with certain classes of medications, such as benzodiazepines or nonbenzodiazepines (Z-drugs). These medications are more likely to cause sedation and respiratory depression when taken together. Veterans, due to their unique mental and physical health challenges, are more likely to experience an overdose when they receive a combination of opioid therapy with benzodiazepine or Z-drug therapy as opposed to a single …

Although patterns of African American and white women breast cancer incidence and mortality in St. Louis, Missouri is consistent with those seen elsewhere in the United States, rates vary greatly across zip codes within the city of St. Louis. North St. Louis, whose neighborhoods are primarily African American, exhibits rates of breast cancer mortality that are among the highest in the city and higher than the state as a whole. Purpose: The purpose of this study was to investigate challenges faced by high schools in rural Appalachia in implementing the Healthy, Hunger Free Kids Act of 2010 (HHFKA). Methodology: We used qualitative, secondary analysis to analyze a collection of thirteen focus groups and 22 interviews conducted in 2013-14 among parents, teachers, and high school students in six counties in rural Appalachian Tennessee (n=98). Results: Five basic themes were identified during the thematic analysis: poor food quality prior to implementation of the HHFKA school nutrition reforms; students’ preference for low-nutrient energy-dense foods; low acceptance of healthier options after implementation of the …

Wildnerness therapy is becoming increasingly popular as a treatment for adolescents. It has been established as an efficacious treatment in previous literature, but the mechanisms as to why have thus far remained a mystery. This research is looking at the connection between wilderness therapy and DBT, another evidence-based treatment. This study will examine 156 adolescents, comparing the efficacy of wilderness therapy to traditional inpatient and intensive outpatient. Pre-treatment assessments of suicidal ideation, emotional regulation, and distress tolerance will be taken.
