Within the past few decades, lactate research has expanded from initial findings deeming lactate as a dead-end metabolic product to recognition of lactate’s role as a potential energy substrate in the CNS. Exercise with blood flow restriction (BFR) allows healthy, clinical, and athletic populations to improve their strength and exercise capacity. The main advantages exercise with BFR has over traditional training are: 1) increases in muscle size, strength, and exercise capacity are elicited at low training loads, 2) these adaptations occur faster with blood flow restriction, 3) increases in muscle size and strength can be stimulated during both resistance and aerobic exercise. Sweat test or pilocarpine iontophoresis test is the existing method for diagnosis of cystic fibrosis that is recommended to perform at 48 hours after birth. However, the current method is painful, with several side effects such as full thickness skin burn, skin rash, erythema etc. Therefore, the purpose of this study is to develop a pilocarpine topical formulation which can stimulate sweat secretion when applied on skin without iontophoresis. Blood flow restriction (BFR) with low load exercise increases muscular size and strength. Little is known of vascular adaptations to this training modality, and nothing was known at very low loads. These studies examined cardiovascular responses to high load (70% of one-repetition maximum (1RM)) and very low load (15%1RM) exercise, alone or at two levels of BFR. Study 1: Participants performed unilateral biceps curls using either 15%1RM with no BFR (15/00), 40% of arterial occlusion pressure (AOP) (15/40), or 80%AOP (15/80), or 70%1RM (70/00). For 60-plus years the explanatory model for Major Depressive Disorder (MDD) has been the monoamine theory, which states that low levels of monoamine neurotransmitters or alterations in their post-synaptic receptors are responsible for depressive symptoms. Drugs designed to elevate synaptic monoamine levels have remained the first line of pharmacotherapy prescribed by clinicians for MDD patients. However, these drugs have shortcomings. Despite evidence that current antidepressants elevate synaptic monoamine levels within hours of administration, these drugs require four to six weeks of daily administration before symptom relief appears. Furthermore, approximately half of patients taking typical antidepressants never achieve full symptom relief. …

การวิจัยครั้งนี้มีวัตถุประสงค์ เพื่อศึกษาบทบาทวิสัญญีพยาบาล โดยใช้เทคนิคเดลฟาย ผู้ให้ข้อมูลคือผู้เชี่ยวชาญจำนวน 20 คน ประกอบด้วย วิสัญญีแพทย์จากราชวิทยาลัยวิสัญญีแพทย์แห่งประเทศไทย จำนวน 4 คน คณะกรรมการชมรมวิสัญญีพยาบาลแห่งประเทศไทย จำนวน 4 คน คณะอนุกรรมการฝึกอบรมและสอบความรู้ความชำนาญเฉพาะทางการพยาบาลและการผดุงครรภ์เพื่อรับวุฒิบัตรในสาขาการพยาบาลด้านการให้ยาระงับความรู้สึก จำนวน 3 คน หัวหน้าวิสัญญีพยาบาล จำนวน 4 คน และวิสัญญีพยาบาล จำนวน 5 คน วิธีดำเนินการวิจัยประกอบด้วย 3 ขั้นตอน คือ ขั้นตอนที่ 1 สัมภาษณ์ผู้เชี่ยวชาญเกี่ยวกับบทบาทวิสัญญีพยาบาล ขั้นตอนที่ 2 นำข้อมูลที่ได้จากการสัมภาษณ์มาวิเคราะห์เนื้อหาแล้วสร้างเป็นแบบสอบถามให้ผู้เชี่ยวชาญประมาณค่าแนวโน้มความสำคัญของบทบาทวิสัญญีพยาบาล และขั้นตอนที่ 3 นำข้อมูลที่ได้จากรอบที่ 2 มาคำนวณหาค่ามัธยฐาน ค่าพิสัยระหว่างควอไทล์ และส่งแบบสอบถามให้ผู้เชี่ยวชาญกลุ่มเดิมยืนยันคำตอบ จากนั้นจึงนำข้อมูลที่ได้มาคำนวณค่ามัธยฐาน และค่าพิสัยระหว่างควอไทล์อีกครั้ง เพื่อสรุปเป็นบทบาทวิสัญญีพยาบาล ผลการวิจัย พบว่า บทบาทวิสัญญีพยาบาล ประกอบด้วย 4 บทบาท ดังนี้ 1) บทบาทด้านผู้ปฏิบัติการพยาบาลเฉพาะทางวิสัญญี จำนวน 19 ข้อ 2) บทบาทด้านผู้จัดการความปลอดภัย และบริหารความเสี่ยงทางวิสัญญี จำนวน 6 ข้อ 3) บทบาทด้านผู้ให้ความรู้และคำปรึกษา จำนวน 5 ข้อ และ 4) บทบาทด้านผู้พัฒนาและควบคุมคุณภาพบริการวิสัญญี จำนวน 7 ข้อ

จุฬาลงกรณ์เวชสาร มกราคม – กุมภาพันธ์ พ.ศ. 2561 ฉบับนี้ก้าวขึ้นสู่ปีที่ 62 ของการจัดพิมพ์เผยแพร่วารสาร โดยในฉบับนี้ได้เพิ่มหัวข้อขึ้นใหม่ภายใต้ชื่อ “จุฬาลงกรณ์เวชประวัติ” (History Medicine) ซึ่งเป็นการรำลึกถึงพระคุณของบุพพาจารย์หรือบูราพาจารย์อาจารย์ท่านก่อนในยุคบุกเบิกก่อตั้งคณะแพทยศาสตร์จุฬาลงกรณ์มหาวิทยาลัย ที่ได้มีคุณูปการต่อเหล่าบรรดาลูกศิษย์รุ่นแล้วรุ่นเล่า โดยในฉบับนี้ได้รับเกียรติจากศาสตราภิชาน นายแพทย์นภดล นพคุณ เขียนรำลึกศาสตราจารย์กิตติคุณ นายแพทย์สุนิตย์ เจิมศิริวัฒน์ซึ่งเป็นปรมาจารย์ผู้บุกเบิกทางการแพทย์ด้านผิวหนัง และร่วมก่อตั้งชมรมแพทย์ผิวหนังเมื่อวันที่ 24 กรกฎาคมพ.ศ. 2499 และต่อมาเป็นสมาคมแพทย์ผิวหนังแห่งประเทศไทย เอกลักษณ์สำคัญของท่านคือ การแต่งกายสะอาด สุภาพ ผูกหูกระต่ายอย่างสง่างาม ใจดี มีอัธยาศัยเป็นกันเองกับลูกศิษย์ และมีอารมณ์ขัน ผู้เขียนได้มีโอกาสเรียนรู้โรคผิวหนังที่ตึกผู้ป่วยนอกกับท่านเมื่อครั้งเป็นนิสิตแพทย์ และเคยได้เดินจูงมือท่านกลับมายังห้องทำงานของท่านที่ตึกกายวิภาคศาสตร์หลังเก่า ท่านเป็นผู้ตั้งชื่อโรคผิวหนังเป็นภาษาไทย เช่น โรคขี้กลากหนุมาน (Tinea imbricata) และโรคสะเก็ดเงิน (psoriasis) เป็นต้น ท่านอาจารย์เคยเขียนบทความร่วมกับรองศาสตราจารย์นายแพทย์วินัย พากเพียร เรื่องการบำรุงและรักษาผิวหนัง รวมทั้งได้เขียนบทความเรื่องศิลปและจรรยาแพทย์ ร่วมกับศาสตราจารย์กิตติคุณ นายแพทย์ภิรมย์ กมลรัตนกุล ซึ่งได้นำมาลงตีพิมพ์เผยแพร่อีกครั้งในวารสารฉบับนี้.

เมื่อ 60 ปีก่อนนิสิตแพทย์และคนไข้ต่างคุ้นเคยกับการเห็นคุณหมอค่อนข้างท้วม ผูกหูกระต่าย อารมณ์ดี สอนหนังสือหรือตรวจคนไข้ อยู่ที่ห้องตรวจเล็ก ๆ ที่ชั้นล่าง ตึกจักรพงษ์ คุณหมอท่านนั้นคือ ศาสตราจารย์กิตติคุณนายแพทย์ สุนิตย์ เจิมศิริวัฒน์ ปรมาจารย์ทางโรคผิวหนังของประเทศไทย และคณะแพทยศาสตร์ จุฬาลงกรณ์มหาวิทยาลัย ท่านอาจารย์จบมัธยม 8 จากโรงเรียนอัสสัมชัญ และสำเร็จการศึกษา จากคณะแพทยศาสตร์และศิริราชพยาบาล (ศิริราชรุ่น 53) ท่านอาจารย์ได้ไปศึกษาต่อสาขาโรคเขตร้อน ที่เมืองกัลกัตตา ประเทศอินเดีย หลังจากนั้นได้รับทุนฟูลไบรท์ ไปเรียนต่อที่มหาวิทยาลัย Pensylvania ประเทศสหรัฐอเมริกา และไปเรียนโรคผิวหนังที่ Case Western Reserve University, เมือง Cleveland, มลรัฐ Ohio ประเทศสหรัฐอเมริกา หลังจากเรียนจบท่านอาจารย์กลับมาทำงานเป็นแพทย์โท ที่โรงพยาบาลบางรัก และเป็นบุญวาสนาของเราที่ท่านอาจารย์ได้รับการแต่งตั้งให้มาทำงานที่ภาควิชาอายุรศาสตร์ โรงพยาบาลจุฬาลงกรณ์ ท่านได้รับปริญญาแพทยศาสตร์ดุษฎีบัณฑิตกิตติมศักดิ์จากจุฬาลงกรณ์มหาวิทยาลัยในปีการศึกษา 2532.

Histoplasmosis is an endemic, systemic mycosis caused by Histoplasma capsulatum. Clinical manifestations are various depending on immunity status. We reported a case of isolated histoplasmosis of the tongue. A 48-year-old Thai woman, steroid-dependence systemic lupus erythematosus, presented with rapid-growing, painful mass on the right lateral aspect of the tongue without systemic symptoms. Histologic examination confirmed the diagnosis of histoplasmosis and tissue culture identified Histoplasma capsulatum. The patient received oral itraconazole for 6 months until the lesion was completely healed. The oral lesions of histoplasmosis could be a part of localized form or disseminated form. This study used a focus group in August 2017 (n = 9) to explore the perceptions of rural physicians to a state request to incorporate diabetes prevention screening into their West Virginia medical practice. Analysis of the data revealed that the participants did not think private physicians were equipped to incorporate diabetes prevention programming into their practice. Three categories emerged from the data analysis to explain the reasoning of the health practitioners on the incorporation of pre-diabetes screening and management into their practice.

Background: Pinch strength is indicative of later functional hand use following injury to hand. However, we believe clinicians (and more specifically CHTs) currently do not use a consistent measurement of pinch grip, despite availability of a “gold standard” of current (2015) ASHT standards for measuring pinch grip.

Methods: This cross-sectional survey (N = 175, 35% response) was designed to determine whether CHTs adhere to the standardized testing position for palmar pinch strength as recommended by the ASHT.

Objectives:

Previous research with participants in weight management has primarily focused on participant weight-loss expectations. The purpose of this study was to explore participant expectations and strategies for measuring progress during a community-based weight management program.

Methods:

Semi-structured interviews were completed with 22 participants with overweight or obesity, who were currently enrolled in a 2-year weight management program in order to understand their expectations and strategies for measuring their progress.

Results:

Achieving surface design and control of biomaterial scaffolds with nanometer- or micrometer-scaled functional films is critical to mimic the unique features of native extracellular matrices, which has significant technological implications for tissue engineering including cell-seeded scaffolds, microbioreactors, cell assembly, tissue regeneration, etc. Compared with other techniques available for surface design, layer-by-layer (LbL) self-assembly technology has attracted extensive attention because of its integrated features of simplicity, versatility, and nanoscale control. People with disabilities (PWDs) constitute about 15-20% of the total population. Health disparities among PWD's are due, in part, to stereotypes about PWDs as incompetent. These stereotypes may lead some healthcare providers (HCP) to patronize PWDs, over-focus on the PWDs' impairments and potentially neglect their presenting problem. Moreover, if the PWD violates stereotypical assumptions, the HCP may actively interfere with the PWD's goals. External stabilization is reported to improve reliability of hand held dynamometry, yet this has not been tested in burns. We aimed to assess the reliability of dynamometry using an external system of stabilization in people with moderate burn injury and explore construct validity of strength assessment using dynamometry.

Participants were assessed on muscle and grip strength three times on each side. Assessment occurred three times per week for up to four weeks. Background: Visualizing one’s own painful body part appears to have an effect on reported pain intensity. Furthermore, it seems that manipulating the size of the viewed image can determine the direction and extent of this phenomenon. When visual distortion has been applied to clinical populations the analgesic effects have been in opposition to those observed in some experimental pain models. Background: Major burns are life threatening. Fluid resuscitation is required for survival to maintain intravascular volumes and prevent hypovolemic shock. Bioimpedance spectroscopy (BIS) has been recognised as a potential method of monitoring fluid shifts after burn and in other disease states. Background: The Fremantle Back Awareness Questionnaire (FreBAQ) claims to assess disrupted self-perception of the back. The aim of this study was to develop a German version of the Fre-BAQ (FreBAQ-G) and assess its test-retest reliability, its known-groups validity and its convergent validity with another purported measure of back perception.

Methods: The FreBaQ-G was translated following international guidelines for the transcultural adaptation of questionnaires.
