Introduction:
Most congenital heart disease [CHD] is diagnosed and treated in early childhood in designated children’s hospitals with parents responsible for decision-making and receipt of information. The adolescent assumes this role in preparation for transition to an adult congenital heart program. We studied the knowledge base of our adolescent CHD patients and their parents.
Methods:
Established patients with CHD, >11 y.o. and their parents, were independently surveyed in the outpatient clinic prior to being seen over a one-year period. Participation was voluntary. Cardiomyopathy, transplant and electrophysiology patients were excluded. Scores were assessed as full, partial or incomplete.

Deconvoluting key biological mechanisms forms the framework for therapeutic discovery. Strategies that enable effective translation of those insights along the development and regulatory path ultimately drive validated clinical application in patients and populations. Accordingly, parity in What vs. How we transform novel mechanistic insights into therapeutic paradigms is essential in achieving success. Aligning molecular discovery with innovations in structures and processes along the discovery-development-regulation-utilization continuum maximizes the return on public and private investments for next-generation solutions in managing health and disease.

Infants exposed in utero to opioids will demonstrate a withdrawal syndrome known as neonatal abstinence syndrome (NAS). Buprenorphine is a long-acting opioid with therapeutic use in medication-assisted treatment of opioid dependency in adults and adolescents. Emerging data from clinical trials and treatment cohorts demonstrate the efficacy and safety of sublingual buprenorphine for those infants with NAS who require pharmacologic treatment. Pharmacometric modeling will assist in defining the exposure-response relationships and facilitate dose optimization.

BACKGROUND: Relatively few neonatal drug development studies have been conducted, but an increase is expected with the enactment of the Food and Drug Administration Safety and Innovation Act (FDASIA). Understanding the safety of drugs studied in neonates is complicated by the unique nature of the population and the level of illness. The objective of this study was to examine neonatal safety data submitted to the FDA in studies pursuant to the Best Pharmaceuticals for Children Act (BPCA) and the Pediatric Research Equity Act (PREA) between 1998 and 2015.

Degenerative cervical myelopathy (DCM) is the most common cause of nontraumatic spinal cord injury worldwide. Even relatively mild impairment in functional scores can significantly impact daily activities. Surgery is an effective treatment for DCM, but outcomes are dependent on more than technique and preoperative neurologic deficits.

The problem of racial microaggressions in the United States has important implications for counseling due to the mental and physical health concerns individuals who experience them may develop. Although the current literature addresses racial microaggressions among Black individuals and implications for counseling, no qualitative studies that included the voices of Black individuals using relational cultural theory were found. Therefore, the purpose of this study was to explore the lived experiences of Black individuals who have encountered racial microaggressions and learn their perspectives on what counselors can do to help them in counseling. A great deal of biomedical research focuses on new biotechnologies such as gene editing, stem cell biology and reproductive medicine, which have created a scientific revolution. While the potential medical benefits of this research may be far-reaching, ethical issues related to non-medical applications of these technologies are demanding. We analyze, from a Jewish legal perspective, some of the ethical conundrums that society faces in pushing the outer limits in researching these new biotechnologies.

Background: This case study examines the feasibility of application of an acceptance-based behavioral therapy, acceptance and commitment therapy (ACT), to a patient with end-stage metastatic pancreatic cancer, depression, and anxiety, as a form of integrative palliative care.
Case Presentation: ACT allowed the patient to identify her values of resuming her religious connection, improving relationships with family members and trusted friends, and organizing her affairs before death. As a result, the patient was able to remain engaged in cancer treatments despite side effects that she had previously deemed intolerable. การศึกษาวิจัยนี้มีวัตถุประสงค์เพื่อศึกษาผลของการฝึกเสริมด้วยการฝึกกล้ามเนื้อแกนกลางลำตัวต่อความสามารถทางกีฬาจักรยานของนักกีฬาจักรยานไทม์ไทรอัลระดับเยาวชนชาย อาสาสมัครเป็นนักกีฬาจักรยานประเภทไทม์ไทรอัลระดับเยาวชนชาย อายุเฉลี่ย 16±2 ปี คัดเลือกตัวอย่างแบบเฉพาะเจาะจง จากนั้นเรียงลำดับตามความแข็งแรงของกล้ามเนื้อแกนกลางลำตัว แบ่งกลุ่มตัวอย่างเป็น 2 กลุ่ม ได้แก่ กลุ่มฝึกจักรยาน จำนวน 13 คน และกลุ่มฝึกจักรยานเสริมด้วยการฝึกกล้ามเนื้อแกนกลางลำตัว จำนวน 12 คน ทั้งสองกลุ่มทำการฝึกโปรแกรมการฝึกจักรยาน คือ ปั่นจักรยานที่ความหนัก 65 – 80 เปอร์เซ็นต์ของอัตราการเต้นของหัวใจสูงสุด ระยะเวลา 120 นาทีต่อวัน จำนวน 2 วันต่อสัปดาห์ และปั่นจักรยานที่ความหนัก 80 – 90 เปอร์เซ็นต์ของอัตราการเต้นของหัวใจสูงสุด ระยะเวลา 75 – 90 นาทีต่อวัน จำนวน 4 วันต่อสัปดาห์ กลุ่มฝึกจักรยานเสริมด้วยการฝึกกล้ามเนื้อแกนกลางลำตัว จะทำการฝึกเพิ่มเติมด้วยเอกเซอร์ไซด์บอล และเครื่องกำหนดแรงต้านที่ความหนัก 75% 1RM จำนวน 2 วันต่อสัปดาห์ ทำการทดสอบตัวแปรด้านต่างๆ ก่อนและหลังการฝึก 8 สัปดาห์ ได้แก่ ด้านสรีรวิทยา ด้านความสามารถทางกีฬาจักรยาน ประกอบด้วยการปั่นจักรยานไทม์ไทรอัล 20 กิโลเมตร ความสามารถในการทรงตัว ระยะเวลาที่ทนต่อความเมื่อยล้า และด้านความแข็งแรงของกล้ามเนื้อ ทำการวิเคราะห์ข้อมูลทางสถิติด้วยการวิเคราะห์ความแปรปรวนสองทางแบบวัดซ้ำและเปรียบเทียบความแตกต่างแบบรายคู่โดยใช้วิธีการทดสอบแอลเอสดี ผลการวิจัย ภายหลังการฝึก 8 สัปดาห์ ทั้ง 2 กลุ่มมีความสามารถในการใช้ออกซิเจนสูงสุดเพิ่มขึ้นเมื่อเทียบกับก่อนการฝึกอย่างมีนัยสำคัญทางสถิติที่ระดับ .05 กลุ่มฝึกจักรยานเสริมด้วยการฝึกกล้ามเนื้อแกนกลางลำตัวมีการใช้ระยะเวลาในการปั่นจักรยานไทม์ไทรอัล 20 กิโลเมตรลดลง รวมถึงเวลาที่ทนต่อความเมื่อยล้า ความสามารถในการทรงตัว และความแข็งแรงของกล้ามเนื้อแกนกลางลำตัวเพิ่มขึ้นเมื่อเทียบกับก่อนการฝึกอย่างมีนัยสำคัญทางสถิติที่ระดับ .05 สรุปผลการวิจัย การฝึกจักรยานเสริมด้วยการฝึกกล้ามเนื้อแกนกลางลำตัวสามารถพัฒนาความสามารถทางกีฬาจักรยานในด้านการเพิ่มความสามารถในการทรงตัว การทนต่อความเมื่อยล้า และลดระยะเวลาในการปั่นจักรยานไทม์ไทรอัล 20 กิโลเมตรได้ ในขณะที่การฝึกด้วยโปรแกรมปั่นจักรยานเพียงอย่างเดียวไม่ส่งผลให้เกิดการพัฒนาดังกล่าว

การวิจัยเชิงสหสัมพันธ์ (Correlation research) มีวัตถุประสงค์เพื่อศึกษาคุณภาพชีวิตของผู้สูงอายุโรคมะเร็งตับที่ได้รับการรักษาด้วยเคมีบำบัดทางหลอดเลือดแดง และศึกษาความสัมพันธ์ระหว่างความเหนื่อยล้า ความปวด อาการนอนไม่หลับ ภาวะเครียด และคุณภาพชีวิตของผู้สูงอายุโรคมะเร็งตับที่ได้รับการรักษาด้วยเคมีบำบัดทางหลอดเลือดแดง กลุ่มตัวอย่างคือผู้ที่มีอายุ 60 ปีขึ้นไป ได้รับการวินิจฉัยว่าเป็นมะเร็งตับทุกระยะและรักษาด้วยเคมีบำบัดทางหลอดเลือดแดง เข้ารับการรักษาแผนกผู้ป่วยใน ตึกศัลยกรรมและอายุรกรรม โรงพยาบาลจุฬาลงกรณ์ และโรงพยาบาลรามาธิบดี จำนวน 120 คน กลุ่มตัวอย่างได้จากการสุ่มแบบอย่างง่าย ด้วยวิธีการจับฉลากแบบไม่แทนที่ เครื่องมือที่ใช้ในการทำวิจัยคือ แบบบันทึกข้อมูลทั่วไป แบบประเมินความเหนื่อยล้า แบบประเมินอาการปวด แบบประเมินการรับรู้ความเครียด แบบประเมินอาการนอนไม่หลับ แบบประเมินคุณภาพชีวิตผู้ป่วยมะเร็งตับ ผ่านการตรวจสอบความตรงตามเนื้อหาโดยผู้ทรงคุณวุฒิจำนวน 5 คน หาความเที่ยงจากการคำนวณค่าสัมประสิทธิ์แอลฟาของครอนบาค ได้ค่าความเที่ยงเท่ากับ .96, .78, .95 และ .87 ตามลำดับ สถิติที่ใช้ในการวิเคราะห์ ข้อมูลได้แก่ ร้อยละ ค่าเฉลี่ย ส่วนเบี่ยงเบนมาตรฐาน และค่าสัมประสิทธิ์สหสัมพันธ์ของเพียร์สัน ผลการวิจัยสรุปได้ดังนี้ 1. ผู้สูงอายุโรคมะเร็งตับที่ได้รับการรักษาด้วยเคมีบำบัดทางหลอดเลือดแดงมีคุณภาพชีวิตโดยรวมอยู่ในระดับค่อนข้างสูง (Mean = 122.47, S.D. = 18.76) 2. ความเหนื่อยล้า ความปวด อาการนอนไม่หลับ ภาวะเครียด มีความสัมพันธ์ทางลบในระดับปานกลางกับคุณภาพชีวิตของผู้สูงอายุโรคมะเร็งตับที่ได้รับการรักษาด้วยเคมีบำบัดทางหลอดเลือดแดง อย่างมีนัยสำคัญทางสถิติที่ระดับ .05 (r = - 0.444, - 0.540, - 0.579, - 0.466 ตามลำดับ)

เป้าหมายงานวิจัย: เพื่อศึกษา เปรียบเทียบการเปลี่ยนแปลงของขนาดของก้อนที่ตับอ่อน ของมะเร็งตับอ่อนระยะลุกลาม ระหว่างผู้ป่วยที่รับการรักษาด้วยการเผาทำลายโดยใช้เข็มคลื่นวิทยุผ่านทางกล้องคลื่นเสียงร่วมกับการให้ยาเคมีบำบัด วิธีการศึกษา : เทียบกับผู้ป่วยที่ได้รับการรักษาด้วยยาเคมีบำบัดเพียงอย่างเดียว โดยกลุ่มทดลองจะได้รับการรักษาด้วยการเผาทำลายโดยใช้เข็มคลื่นวิทยุผ่านทางกล้องคลื่นเสียงร่วมกับการให้ยาเคมีบำบัดและกลุ่มควบคุม คือผู้ป่วยที่มี อายุ เพศ ระยะของมะเร็งตับอ่อน และ ECOG ใกล้เคียงกับผู้ป่วยกลุ่มทดลอง แต่ได้รับการรักษาด้วยยาเคมีบำบัดเพียงอย่างเดียว มาเปรียบเทียบขนาดก่อนหลัง,ภาวะแทรกซ้อน และ ปริมานยาแก้ปวดที่ใช้ ก่อนและหลังทำการรักษาในช่วง 3เดือน ผลการศึกษา :เก็บข้อมูลตั้งแต่ กรกฎาคม พ.ศ. 2560 ถึง กันยายน พ.ศ. 2561ที่โรงพยาบาลจุฬาลงกรณ์มีผู้ป่วยเข้าร่วมการศึกษาทั้งสิ้น28 ราย เป็นกลุ่มทดลอง และ กลุ่มควบคุม อย่างละ 14 ราย (อายุเฉลี่ย66.14±10ปี; อัตราส่วน ชาย: หญิง= 1: 3)พบว่า ไม่มีความแตกต่างของลักษณะก่อนทำการรักษาระหว่างประชากรระหว่างทั้งสองกลุ่ม มีการส่องกล้องคลื่นเสียง และเผาทำลายด้วยเข็มคลื่นวิทยุ ทั้งสิ้น 34 ครั้ง(พิสัย 1-4ครั้ง/คน)มีรายงานภาวะแทรกซ้อน ทั้งสิ้น3 ใน 34 ครั้ง ในผู้ป่วย2 ราย คิดเป็นร้อยละ 8.8 ได้แก่ การติดเชื้อในก้อนมะเร็ง จำเป็นต้องได้ยาฆ่าเชื้อ (นอนโรงพยาบาล 7 วัน ), เลือดออกจากรอยเจาะเข็ม ใช้ฮีโมคลิปหยุดเลือด (นอนโรงพยาบาล 7 วัน)และ ตับอ่อนอักเสบไม่รุนแรง (นอนโรงพยาบาล 2วัน)โดยภาวะแทรกซ้อนที่เกิดขึ้น ผู้ป่วยไม่มียังสามารถเข้ารับยาเคมีบำบัดได้ตามกำหนดการเดิม ผลการทดลองพบว่า ในกลุ่มทดลอง ความยาวของก้อนมะเร็งไม่โตขึ้น (ก่อน เทียบกับ หลัง ; 61.37±20.1 ม.ม. เทียบกับ 64.25±22.0 ม.ม. ความเป็นมา อ๊อกซาลิพลาติน เป็นยาเคมีบำบัดหลักที่ใช้ในผู้ป่วยมะเร็งลำไส้ใหญ่และทวารหนัก ผลข้างเคียงที่มักพบบ่อย คือการเกิดปฏิกิริยาการแพ้แบบภูมิไวเกินขณะได้รับอ๊อกซาลิพลาตินทางเส้นเลือด โดยมีแน้วโน้มสูงขึ้นในกลุ่มผู้ป่วยที่ได้รับการรักษาด้วยยาเคมีบำบัดที่มีอ๊อกซาลิพลาตินซ้ำ วัตถุประสงค์ เพื่อศึกษาอุบัติการณ์การเกิดปฏิกิริยาการแพ้แบบภูมิไวเกินขณะได้รับอ๊อกซาลิพลาตินทางเส้นเลือด ในกลุ่มผู้ป่วยที่ได้รับการรักษาด้วยยาเคมีบำบัดที่มีอ๊อกซาลิพลาตินซ้ำ ซึ่งจะได้รับยาป้องกันคือ ยากลุ่ม แอนติฮิสตามีน ก่อนการให้ยาอ๊อกซาลิพลาติน วิธีการศึกษา ผู้เข้าร่วมวิจัยจะได้รับยาคลอร์เฟนิรามีน 10 มิลลิกรัม ร่วมกับ รานิทิดีน 50 มิลลิกรัม ฉีดเข้าทางเส้นเลือดดำ ก่อนเริ่มยาเคมีบำบัด 30นาที ในทุกรอบการรักษา จนกว่าการรักษาด้วยยาเคมีบำบัดสูตรนี้จะยุติลง โดยจะรายงานอุบัติการณ์การเกิดปฏิกิริยาการแพ้แบบภูมิไวเกินขณะได้รับอ๊อกซาลิพลาตินทางเส้นเลือด ผลการวิจัย ตั้งแต่ 1 ก.พ.2561 ถึง 20 เม.ย. 2562 มีผู้เข้าร่วมในวิจัย 35 คน พบการเกิดปฏิกิริยาการแพ้แบบภูมิไวเกินขณะได้รับอ๊อกซาลิพลาตินทางเส้นเลือด ทั้งหมด 5 คน โดยไม่มีผู้เข้าร่วมวิจัยที่ต้องยุติการรักษาเนื่องจากการเกิดปฏิกิริยาการแพ้ดังกล่าว สรุปผลการวิจัย ในผู้ป่วยที่ได้รับการรักษาซ้ำด้วยอ๊อกซาลิพลาติน พบอุบัติการณ์การเกิดปฏิกิริยาการแพ้แบบภูมิไวเกินขณะได้รับอ๊อกซาลิพลาตินทางเส้นเลือดหลังให้การป้องกันด้วยยากลุ่มแอนตี้ฮิสตามีน ร้อยละ 14.28

ที่มาและความสำคัญ ภาวะหัวใจห้องบนเต้นระริกสามารถพบได้บ่อยในแผนกผู้ป่วยวิกฤติ ซึ่งการรักษาภาวะหัวใจห้องบนเต้นระริกนี้มักจะส่งผลทำให้ช่วงเวลา QT ยาวขึ้น ซึ่งเป็นสาเหตุนำมาซึ่งการเกิดหัวใจห้องล่างเต้นผิดจังหวะแบบ ventricular tachyarrhythmias ปัจจุบันยังไม่มีแนวทางมาตรฐานที่ชัดเจนในการวัดช่วงเวลา QT ในคนไข้กลุ่มภาวะหัวใจห้องบนเต้นระริก เพื่อที่จะป้องการการเกดหัวใจห้องล่างเต้นผิดใจหวะแบบ ventricular tachyarrhythmias (VA) ได้อย่างเหมาะสม วัตถุประสงค์ เพื่อหาวิธีการวัดช่วงเวลา QT ที่แม่นยำที่สุดในการคาดคะเนการเกิดหัวใจห้องล่างเต้นผิดจังหวะแบบ VA ในคนไข้กลุ่มภาวะหัวใจห้องบนเต้นระริก ระเบียบวิธีวิจัย เก็บข้อมูลผุ้ป่วยภาวะหัวใจห้องบนเต้นระริกในแผนกผู้ป่วยวิกฤติ โรงพยาบาลจุฬาลงกรณ์ ตั้งแต่ 1 มกราคม 2557 ถึง 30 กันยายน 2561 โดยใช้ ICD-10 ในการสืบหาข้อมุลผู้ป่วย โดยนำ 12-lead EKGs วัดช่วงเวลา QT มาทำการวัดที่ lead II หรือ lead V3 และใช้ สมการ Fridericia's QT interval correction (QTc) รวมถึงกำหนดจุดสิ้นสุดในการวัดช่วงเวลา QT โดยใช้ วิธี Tangent โดยผู้ป่วยทุกคนได้รับการวัด 12-lead EKGs ในทั้งหมด 4 ตำแหน่ง คือ (1) ค่าเฉลี่ยของช่วงเวลา QTc ระหว่าง QTc หลังช่วงเวลา RR ที่ยาวที่สุดและสั้นที่สุด (2) ค่าเฉลี่ยของช่วงเวลา QTc 3 ช่วงโดยมี QTc หลังช่วงเวลา RR ที่ยาวที่สุดอยู่ตรงกลาง (3) ค่าเฉลี่ยของช่วงเวลา QTc 10 ช่วงโดยหนึ่งในนั้นต้องประกอบด้วย QTc หลังช่วงเวลา RR ที่ยาวที่สุดร่วมด้วย (4) ค่าช่วงเวลา QTc โดยเครื่องอ่านผลระบบ Philips DXL 12- lead …

The utilization of canine mesenchymal stem cells (cMSCs) with multipotent capabilities has been regarded for possible therapy of incorrigible bone disease. Although various sources of cMSCs show similar characteristics, they are different in osteogenic potential due to their original cellular sources. This study was designed to globally explore and analyze the in vitro differentiation potential and behavior of canine bone-marrow derived mesenchymal stem cells (cBM-MSCs) and canine dental pulp stem cells (cDPSCs) toward osteogenic lineage. However, cervical branch of facial nerve (CN) and retromandibular vein (RMV) injury can occur with scissors or even with a liposuction cannula during dissection. Background: Piperacillin/Tazobactam is frequently used for empirical treatment in patients with sepsis. Pathophysiological changes during the early phase of sepsis have significant effects on pharmacokinetic/pharmacodynamic (PK/PD) behaviors. This study aimed to characterize the population PKs of piperacillin and investigate probability of target attainment (PTA) and cumulative fraction of response (CFR) of various dosage regimens in critically ill patients during the early phase of sepsis. Methods: Forty-eight patients treated with piperacillin/tazobactam were recruited. Five blood samples were drawn before and during 0-0.5, 0.5-2, 2-4 and 4-6 or 8 hours after administration. BACKGROUND: Tenofovir alafenamide (TAF) is associated with less renal and bone toxicity compared with tenofovir disoproxil (TDF). TAF's recent FDA approval has spurred HIV providers to consider switching antiretroviral therapy (ART) regimens containing TDF to TAF to minimize long term risks. Patient views on the process of such medication switches have not been explored.
METHODS: Patients taking elvitegravir/cobicistat/emtricitabine/tenofovir disoproxil fumarate (E/C/F/TDF) following the Food and Drug Administration's (FDA) approval of elvitegravir/cobicistat/emtricitabine/tenofovir alafenamide (E/C/F/TAF) received medication education from an HIV pharmacist prior to switching to the tenofovir alafenamide (TAF) formulation. Introduction: Increasing incidence of vision impairment in the United States reinforces the urgent need for research and public health awareness. Eye diseases are of common concern in Asian and other ethnic groups globally. Glaucoma, cataracts, macular degeneration, myopia, and retinitis are types of eye diseases and common causes of vision impairment in Asians. This study addressed the current gap in knowledge regarding vision impairment prevalence among Asian-Americans by geographical location and level of education. Socio-ecological theory was used as the theoretical foundation.

The study investigated the association between body mass index (BMI) and depression based upon glycated hemoglobin levels (HBA1C) among United States veterans. Based on Bandura's social cognitive theory, a cross-sectional analysis examined the association between BMI and depression on HBA1C regulation, and if the interaction between BMI and depression affects HBAIC regulation among veterans. Multiple regression analysis was used to test the multivariate associations between depression and BMI on the outcome variable of HBA1C. Linearity, normality, and homoscedasticity were assessed using normal probability plots and residual analysis. The use of an indwelling catheter is common in hospital settings. Objective: A multisite field trial testing whether improved outcomes associated with intensive referral to mutual help groups (MHGs) could be maintained after the intervention was adapted for the circumstances and needs of rural veterans in treatment for substance use disorder (SUD). Methods: In three Veterans Affairs treatment programs in the Midwest, patients (N = 195) received standard referral (SR) or rural-adapted intensive referral (RAIR) and were measured at baseline and 6-month follow-up. Researchers have found that African American men are one of the most marginalized, targeted, and disenfranchised groups of people. In the past, African American men were less likely to attend psychotherapy compared to other ethnicities.
