Novel methods of ensuring survival following cardiac arrest and resuscitation are of supreme importance to the medical community. Targeted temperature management (TTM) has become increasingly utilized pre-hospital, in emergency departments, and within intensive care units to increase the likelihood of survival to hospital discharge. TTM has further been used to attempt to improve neurological functioning. The efficacy and mechanism behind TTM remains poorly understood. In several patient populations it also remains unproven. The purpose of this study is to assess the physiological mechanism, survival, neurological recovery and methodology of TTM use and implementation.

The circadian system is important in all living organisms because it generates a 24-hour rhythm for physiological and behavioral processes enabling anticipation and adaptation to daily changes in the environment. The prevalence of cardiometabolic diseases, which are linked to lifestyle choices, has been rising at an alarming rate. Modernization and globalization are two of many factors to blame for lifestyle changes resulting in circadian disruption. The purpose of this study was to determine whether supplementation with creatine monohydrate is efficacious beyond the realm of its most popular use, which is in athletics. This study investigated the safety of creatine supplementation in the general population. References were collected through a review of PubMed and Scopus databases. Initial keywords searched were creatine supplementation and creatine safety. Where possible, trials with human subjects were utilized. Studies focused on creatine’s effects in athletes or weightlifting parameters were eliminated. The genetic cause of significant disorders like Down syndrome (DS) and Duchenne muscular dystrophy (DMD) is well documented. Pulmonary arterial hypertension (PAH), a co-morbid condition, is tied to mortality among these groups. Individuals with the ability to communicate their symptoms and actively participate in treatment are at an advantage with subsequent benefit. DS and DMD significantly decrease physical ability and communication capacity. Genetic research has allowed for earlier identification of, or predisposition for, the development of PAH. Certain nonlinear systems can switch between dynamical attractors occupying different regions of phase space, under variation of parameters or initial states. In this work we exploit this feature to obtain reliable logic operations. With logic output 0/1 mapped to dynamical attractors bounded in distinct regions of phase space, and logic inputs encoded by a very small bias parameter, we explicitly demonstrate that the system hops consistently in response to an external input stream, operating effectively as a reliable logic gate. This system offers the advantage that very low-amplitude inputs yield highly amplified outputs. Additionally, different dynamical variables in the system …

The epidermal growth factor (EGF)-receptor ligand amphiregulin (AREG) is a potent growth factor implicated in proliferative skin diseases and in primary and metastatic epithelial cancers. AREG, synthesized as a propeptide, requires conversion to an active peptide by metalloproteases by a process known as ectodomain shedding. Although (ADAM17) a disintegrin and metalloprotease 17 is a key sheddase of AREG, ADAM8-, ADAM15-, and batimastat (broad metalloprotease inhibitor)-sensitive metalloproteases have also been implicated in AREG shedding. Background: Ebola virus disease (EVD) is a dangerous, often fatal disease that spreads through infected bodily fluids. In 2014, an Ebola outbreak swept through West Africa, including Liberia.
Purpose: The purpose of this study is to examine how social vulnerability affected different aspects of life during the 2014 Ebola outbreak in Monrovia, Liberia. The association between vulnerability and hardship that people experience during the outbreak was examined. This was measured by indicator variables for hardship such as availability of food, cash, or medical care. Vulnerability was also compared with EVD knowledge and vigilance.

Background: East Liverpool, Ohio, the site of a hazardous waste incinerator and a manganese (Mn) processor, has had air Mn concentrations exceeding United States Environmental Protection Agency reference levels for over a decade. Save Our County, Inc., a community organization, was formed to address community environmental health concerns related to local industry. Researchers from the University of Cincinnati partnered with Save Our County to determine if air Mn had an impact on the neurocognitive function of children in the community.

An alarming occurrence in academia involves the discipline of faculty, under the guise of violating civility or collegiality codes, for engaging in what should be protected academic free speech. This often occurs when unprincipled and/or corporate-minded administrators seek to punish or dissuade faculty from challenging or questioning their decisions or policy initiatives, or for speaking up about policy violations or lack of due process. The ambiguity of terms such as civility and collegiality, when selectively defined by administrators, can be used to stifle, dissuade or punish academic free speech. Ways to identify and address these problems are presented.

Background and Purpose. Subacromial impingement syndrome (SAIS) is a common shoulder pathology. The purpose of this case report is to discuss the outcomes of subsequent physical therapy after operative treatment for SAIS and the clinical decision making involved in seeking surgical intervention.
Case Description. This case report describes a 54 y/o female who participated in eight weeks of physical therapy after undergoing shoulder decompression surgery while failing conservative physical therapy prior to surgical intervention.

Background and Purpose. The purpose of this pilot study was to see if there was a difference in amount of electromyographic (EMG) activity in the quadriceps (vastus medialis and vastus lateralis) compared to amount of EMG activity in the hamstrings (biceps femoris and semitendinosus) in active male and female subjects during vertical jumping and landing. In addition, we looked at the amount of knee flexion that occurred in the male and female subjects shortly after landing from a vertical jump.

Burnout is a term that continues to raise considerable concern among high school coaches and administrators (Raedeke, 1997) and has shown to have negative effects on coaches and athletes alike (Price & Weiss, 2000; Vealey, Armstrong, & Comar, 1998). While burnout has shown to have significant adverse effects, few studies have examined how the coach-athlete relationship may affect a coach s level of perceived burnout. Prolonged exposure to organic barn dusts can lead to chronic inflammation and a broad range of lung problems over time, mediated by innate immune mechanisms. The immune surfactant or collectin surfactant protein D (SP-D) is a crucial multifunctional innate immune receptor. Little work to date has examined the effect of such collectins in response to organic dusts. We provide evidence here that agricultural organic dusts can inhibit mRNA and protein expression of SP-D in a human alveolar epithelial cell line, and an in vivo mouse model. Within the ambulatory care environment, healthcare organizations are challenged to address the rapidly advancing technological demands associated with electronic health records (EHR) when building new facilities or renovating existing infrastructures. The Clemson Healthcare Work System Ergonomic Assessment Toolkit is a tool for evaluating the ergonomics of alternative computer workstation configurations within the broader context of the ambulatory work system which they are placed. The toolkit provides guidance for analyzing the strengths and weaknesses of varying applications of technology integration into ambulatory workspaces to support evidence-based design decisions.

Elucidating the biology of yeast in its full complexity has major implications for science, medicine and industry. One of the most critical processes determining yeast life and physiology is cellular demise. However, the investigation of yeast cell death is a relatively young field, and a widely accepted set of concepts and terms is still missing. Here, we propose unified criteria for the definition of accidental, regulated, and programmed forms of cell death in yeast based on a series of morphological and biochemical criteria. The United States is in the midst of an opioid overdose epidemic resulting to 40,000 accidental drug overdose deaths. In 2016, the number of deaths related to opioid overdose increased to 116 people per day resulting to $504 billion in economic costs. This research study explored the lived experiences of drug addicts having experienced opioid withdrawal symptoms. Phenomenology is useful in discovering the narrative lived experiences that cannot be obtained in quantitative approach. This study evaluated the effect of an educational intervention on improving evidence-based nursing practice (EBP) skills of practicing nurses. In addition, the effects of educational preparation and years of nursing experienced were examined. Using a quasi-experimental pretest-posttest design, the Evidence Based Practice Questionnaire (EBPQ) was administered to a convenience sample of registered nurses from a large regional hospital. Importance: The pathophysiologic mechanisms of Guillain-Barré syndrome (GBS) associated with Zika virus (ZIKV) infection may be indicated by differences in clinical features.
Objective: To identify specific clinical features of GBS associated with ZIKV infection.
Design, Setting, and Participants: During the ZIKV epidemic in Puerto Rico, prospective and retrospective strategies were used to identify patients with GBS who had neurologic illness onset in 2016 and were hospitalized at all 57 nonspecialized hospitals and 2 rehabilitation centers in Puerto Rico. The 2013-16 Ebola virus disease outbreak in west Africa was associated with unprecedented challenges in the provision of care to patients with Ebola virus disease, including absence of pre-existing isolation and treatment facilities, patients' reluctance to present for medical care, and limitations in the provision of supportive medical care. Case fatality rates in west Africa were initially greater than 70%, but decreased with improvements in supportive care. This article examines how cognitive load theory can help instructional designers in medical education design material and content to best suit their audience. Through the examination of schema construction, working and long-term memory, biologically primary/secondary knowledge, and novice and experienced learners, this article proposes instructional design best practices. This article separates these best practices into three categories: activities, pre-lecture resources, and teaching strategies, which can be applied to either novice learners or experienced learners.

Introduction: Cigarette use is associated with substance use and mental health problems among youth, but as- sociations are unknown for non-cigarette tobacco product use, as well as the increasingly common poly-tobacco use.

Anecdotal evidence indicates that naloxone prices have risen in recent years, but limited research has examined the magnitude of these increases and potential causes. We contribute nationally representative evidence to help answer each of these questions, including wholesale pricing data from a proprietary drug sales database span- ning January 2006 to February 2017. We find that all formulations of naloxone increased in price since 2006 except for Narcan Nasal Spray. These cumulative increases totaled 2281% for the 0.4 MG single-dose products, 244% for the 2 MG single-dose products, 3797% for the 4 MG multi-dose products, and 469% for the 0.4 …
