Background: Opioids are the mainstay of therapy for painful vasoocclusive episodes (VOEs) in sickle cell disease (SCD). Based on limited studies, low-dose ketamine could be a useful adjuvant analgesic for refractory SCD pain, but its safety and efficacy has not been evaluated in pediatric SCD.

Procedure: Using retrospective chart review we recorded and compared characteristics of hospitalizations of 33 children with SCD hospitalized with VOE who were treated with low-dose ketamine and opioid PCA vs. a paired hospitalization where the same patients received opioid PCA without ketamine. Both pathogen- and tissue damage-associated molecular patterns induce inflammation through toll-like receptors (TLRs), while sialic acid-binding immunoglobulin superfamily lectin receptors (Siglecs) provide negative regulation. Here we report extensive and direct interactions between these pattern recognition receptors. The promiscuous TLR binders were human SIGLEC-5/9 and mouse Siglec-3/E/F. Mouse Siglec-G did not show appreciable binding to any TLRs tested. Correspondingly, Siglece deletion enhanced dendritic cell responses to all microbial TLR ligands tested, while Siglecg deletion did not affect the responses to these ligands. TLR4 activation triggers Neu1 translocation to cell surface to disrupt TLR4:Siglec-E interaction. Conversely, sialidase inhibitor Neu5Gc2en prevented TLR4 ligand-induced …

In our academically based, psychiatric outpatient practice, we have encountered those who wish to avoid medications for treatment for psychosis. This study is a qualitative/quantitative study in progress of a patient population diagnosed with psychosis and managed primarily without medication. We reflect upon the transpersonal foundations of an approach in which voices and visions are accepted as real, given full ontological status, and addressed within a dialogical framework. Background: Hyperacusis is the difference in dB between a normal threshold of audibility for a stimulus and the dB level at which that stimulus becomes intolerably loud. Some persons with autism exhibit symptoms of hyperacusis possibly attributable to phobias to auditory stimuli. Desensitization procedures have been used as a treatment for phobias and appear a promising treatment for patients with autism and hyperacusis. Methods: Our participant was a 26 year old male previously diagnosed Asperger's syndrome who presented with hyperacusis to low-pitched voices and a concomitant vocal disorder manifesting mainly as high vocal pitch. PURPOSE: Breathlessness is a predominant symptom of chronic obstructive pulmonary disease (COPD), making it a valuable outcome in addition to lung function to assess treatment benefit. The phosphodiesterase-4 inhibitor roflumilast has been shown to provide small but significant improvements in dyspnea, as measured by the transition dyspnea index (TDI), in two 1-year studies in patients with severe to very severe COPD.

Substance use regularly occurs among individuals that distribute drugs and there is a limited amount of research that has examined both use and distribution. A grounded theory approach was used to develop a framework for analyzing relationships between use and distribution behaviors. 15 participants (five female, 10 male) that had experience selling drugs and using drugs were recruited from a residential substance abuse treatment center. Each participant was interviewed for one hour with an interview protocol aligning with the stage of analysis. This quantitative pilot study examined the quality of life of palliative care patients as perceived by the patient and their caregiver. Patients with palliative care consults at The University of Colorado and The Medical Center of Aurora were asked to participate in this study. The McGill Quality of Life Questionnaire survey tool was selected to measure the patient's quality of life due to its validity and reliability in measuring quality of life in the palliative care patient population. Objective: The purpose of this in vitro study was to determine if there is a difference in force decay between three sliding retraction methods under a standardized force delivery system (200 gm at 25 mm stretch) at 2, 4, 6, and 8 weeks. Background: In order to achieve proper esthetics, occlusion and stability, orthodontic treatment may require extractions. Background . Little is known about the long-term clinical bonding effectiveness of the Fiber-reinforced composite (FRC) posts cemented with self-etch adhesive systems. Bond stability and longevity of the cemented post are adversely affected by physical and chemical factors over time, such as expansion and contraction stresses caused by thermal changes and occlusal load. The aim of this study was to investigate a sensory intervention of moderate pressure touch on children with sensory modulation disorder on the outcomes of sleep behaviors and social participation. 50 children, aged 6-11 years, with both sensory modulation disorder and sleep difficulties were randomly divided into an experimental group and a control group. The experimental group participants received three weeks of nightly massage by their parents. The parents filled out questionnaires reporting on sleep behaviors, sensory modulation, and social participation, as well as recording a sleep log, and determining goal attainment scaling goals. Breast cancer is one of the largest causes of cancer related deaths in women. Less than 5% of breast cancer cases are genetically inherited and most often develop after menopause. The BRCA gene mutation is a genetic inheritance which increases ones chances of developing breast cancer at a young age tenfold. Recent research has proposed a method of treatment in genetically inherited breast cancers by taking advantage of the impaired DNA repair pathway caused by the BRCA mutation.
