BACKGROUND: Pyloric stenosis, the most common surgical condition of infants, is treated by longitudinal myotomy of the pylorus. Comparative studies to date between open and laparoscopic pyloromyotomy have been retrospective and report conflicting results. To scientifically compare the 2 techniques, we conducted the first large prospective, randomized trial between the 2 approaches.

METHODS: After obtaining IRB approval, subjects with ultrasound-proven pyloric stenosis were randomized to either open or laparoscopic pyloromyotomy. Postoperative pain management, feeding schedule, and discharge criteria were identical for both groups. The interactions between CD80 and CD86 on antigen-presenting cells and CD28 on T cells serve as an important costimulatory signal in the activation of T cells. Although the simplistic two-signal hypothesis has been challenged in recent years by the identification of different costimulators, this classical pathway has been shown to significantly impact antiviral humoral and cellular immune responses. How the CD80/CD86-CD28 pathway affects the control of chronic or latent infections has been less well characterized. A 53 year old woman presented to the office with complaint of severe bilateral thumb nail pain and the deformity shown (Figure 1). She denied previous trauma to her thumb nails and had no history of previous bacterial or fungal infections of her nails. Based on the curvature of her nails, which increased from proximal to distal, a diagnosis of bilateral thumb pincer nail deformity was made.

Classical swine fever virus (CSFV), serum neutralizing antibody titers of 854 sows were determined using a neutralizing peroxidase linked assay (NPLA). Blood samples were collected one-week post-partum from 1st- 6th parity sows, from 30 swine fever-free farms. The farms were divided according to their vaccination programs, 10 farms for each program. Program A sows had been vaccinated 3 weeks pre-partum (288 samples). Program B sows had been vaccinated 3 weeks post-partum (276 samples). Job insecurity has been increasing since the 1980s. While researchers have found job insecurity to be negatively associated with multiple indicators of well-being for workers and their families in cross sectional studies, less is known about the long term effects of prolonged job insecurity. Specifically, there is a need to collect measures of both insecurity and its consequences at multiple time periods. The current study followed workers for 3 1/2 years to assess the effects of chronic job insecurity on psychological distress. Results indicate that while workers reported increased feelings of security over time, there were longer term negative effects …

We conducted a study to evaluate the use of a pediatric rigid otoendoscope for determining the extent of middle ear disease and for assessing ossicular integrity and mobility during tympanoplasty. Our study population was made up of 132 patients who were undergoing surgery for the treatment of chronic suppurative otitis media; of this group, 41 patients underwent otoendoscopy and 91 underwent scutum lowering for purposes of visualization. In the otoendoscopy group, the ossicles were successfully visualized and their mobility assessed in 34 patients; the remaining 7 patients subsequently underwent scutum lowering. Blood cells and platelets from twenty-seven animals belonging to two genera of the Family Felidae, were studied using both light and transmission electron microscopes (TEM). Generally, blood cells can be categorized into red blood cells, white blood cells and platelets. The red blood cells or erythrocytes from these species were round and had neither a nucleus nor organelles. They showed a central pale area, Heinz bodies and rouleaux formation. The white blood cells could be divided into two groups; granulocytes and agranulocytes. Aspects of the social environment, including social conditions (socio-economic status, household situations, chronic illnesses) and social relations (attitude and behaviors of relations) are major determinants of depression among women. This study evaluates the relative power of social relations and social conditions in predicting depression among pregnant women in Pakistan. In the qualitative phase of the study, social environmental determinants were identified through literature search, and experts' opinions from psychologists, psychiatrists, gynecologists, sociologists and researchers. Radiation-induced injuries from fluoroscopic procedures in pediatric patients have occurred, and young patients are at greatest risk of many radiation-induced neoplasms. Some fluoroscopists have been injured from their use of fluoroscopy, and they are known to be at risk of radiation-induced neoplasm when radiation is not well-controlled. This article reviews the circumstances that lead to radiation injury and delineates some procedural methods to avoid injury and limit radiation exposure to both the patient and the fluoroscopist.

Magnetic resonance imaging (MRI) is an essential diagnostic tool in modern medicine. Conditions such as cerebral vascular accidents, epilepsy and other seizure disorders, autism, cancer, migraine and cluster headaches benefit from its use. It has been discovered that 4-10% of all children and adolescents will experience a seizure at some point (Hauser, 1994) and MRI is one of the most common imaging modalities for seizure and epilepsy diagnosis. Endodontic intracanal medicaments are commonly tested using standardized bovine cylinders infected with bacteria. Twenty cadaver canine forelimbs, 9 right and 11 left, were used to evaluate the most effective radiographic position that can enhance the cranial aspect of the medial coronoid process (MCP) of the ulna. A lead pellet (1 mm) was buried to represent the area representing the cartilegenous fragment in the canine patient in the fragmented medial coronoid process (FMCP). The distance between the pellet and the radial surface and the superimposition of the humeral condyle silhouette on the pellet were used as parameters for analysis. Porcine small intestinal submucosa biomaterial was used to repair corneal staphyloma in 10 dogs. There was no evidence of leakage of aqueous humor and graft rejection following PSIS grafting. Anterior synechia was presented in 25-50% of the corneal area. Granulation tissue vanished within 50 days of the operation. Even though scar formation and anterior synechia were observed at the end of the study, all dogs had vision.

Immunopresence of the progesterone receptors (PR) and proliferative activity in the bitch uterus were studied during different stages of the oestrous cycle. Uterine samples were collected at 4 different stages of the oestrous cycle; prooestrus, oestrus, dioestrus and anoestrus. Immunohistochemistry was used to evaluate the localization and to semiquantitate PR and uterine proliferation by using monoclonal antibodies to the PR and the proliferative marker, Ki-67 respectively. The results showed that PR and Ki-67 levels were highest during prooestrus and oestrus while they were low at other stages. Nuclear transfer from adult somatic cells is an advanced reproductive biotechnology. This technique can be applied in animal production for producing a high genetically identical copy for endangered species conservation and for therapeutic cloning for human medicine. Our three-year preliminary study indicates the possibility of somatic cell cloning by using the rabbit as model of technical development. Later, interspecies cloning in wild cats and domestic cats was performed successfully in terms of cloned embryo production. Meanwhile, goat cloning served as the model for farm animals.

Psoralen plus UVA (PUVA) is used as a very effective treatment modality for various diseases, including psoriasis and cutaneous T-cell lymphoma. PUVA-induced immune suppression and/or apoptosis are thought to be responsible for the therapeutic action. However, the molecular mechanisms by which PUVA acts are not well understood. We have previously identified platelet-activating factor (PAF), a potent phospholipid mediator, as a crucial substance triggering ultraviolet B radiation-induced immune suppression. Enterococcus faecalis, the third most frequent cause of bacterial endocarditis, appears to be equipped with diverse surface-associated proteins showing structural-fold similarity to the immunoglobulin-fold family of staphylococcal adhesins. Among the putative E. faecalis surface proteins, the previously characterized adhesin Ace, which shows specific binding to collagen and laminin, was detectable in surface protein preparations only after growth at 46 degrees C, mirroring the finding that adherence was observed in 46 degrees C, but not 37 degrees C, grown E. faecalis cultures. Background: The Multicenter Study of Hydroxyurea (HU) in Sickle Cell Anemia (MSH) previously showed that daily oral HU reduces painful sickle cell (SS) crises by 50% in patients with moderate to severe disease. The morbidity associated with this disease is known to have serious negative impact on the overall quality of life(QOL) of affected individuals.
