Papillary thyroid carcinoma (PTC) has many subtypes. A rare subtype consisting of papillary projections covered by cells oncocytic in character has been defined in recent years. Oncocytes with characteristic nuclear features of PTC are seen in the thyroid aspiration cytology of this subtype. In the cytological diagnosis, this variant could be confused with other oncocytic lesions of the thyroid. We discuss here the cytological and histopathological features of the oncocytic variant of PTC diagnosed in the aspiration biopsy and thyroid resection material in a 42-year-old female patient.

Central core disease is a rare neuromuscular disorder associated with leg weakness. It is a familial disease with autosomal dominant inheritance. Central core disease has been reported to be associated with malignant hyperthermia. A 25-year-old woman with central core disease was scheduled to be operated for lumbar disc hernia at L4-5 and L5-S1 interspaces. Oral dantrolene was administered prophylactically. Spinal anesthesia was performed with a 25 G Whitacre spinal needle at the L3-4 interspace. No complication was observed during the intraoperative and postoperative periods. Isoniazid is one of the most commonly used antituberculous drugs. Acute intoxication is characterized by repetitious convulsions, high anion gap metabolic acidosis and coma. The basis of therapy consists of parenteral pyridoxine administration in a dose equivalent to that of isoniazid ingested. Here we present a case of acute isoniazid intoxication presenting with convulsions and metabolic acidosis with consequent rhabdomyolysis and discuss the clinical signs and pathophysiology of isoniazid intoxication.

Millions of people search online for medical text, but these texts are often too complicated to understand. Readability evaluations are mostly based on surface metrics such as character or words counts and sentence syntax, but content is ignored. We compared four types of documents, easy and difficult WebMD documents, patient blogs, and patient educational material, for surface and content-based metrics. The documents differed significantly in reading grade levels and vocabulary used. WebMD pages with high readability also used terminology that was more consumer-friendly. วัตถุประสงค์ เพื่อประเมินการเปลี่ยนแปลงปริมาณและสัดส่วนของแบคทีเรียในพ็อกเก็ต และลักษณะทางคลินิก หลังการขูดหินน้ําลายใต้เหงือก ด้วยหัวขูดอัลทราโซนิกส์คล้ายคิวเรตต์แบบมินิทิป ในผู้ป่วยโรคปริทันต์อักเสบเรื้อรัง วัสดุและวิธีการ การวิจัยนี้ใช้กลุ่มตัวอย่าง 12 คน ตําแหน่งทดลองแบ่งออกเป็น 2 กลุ่ม ได้แก่ กลุ่มที่มีพ็อกเก็ต 3.1-5.0 ม.ม. จํานวน 97 ตําแหน่ง และกลุ่มที่มีพ็อกเก็ต 5.1-7.0 ม.ม. จํานวน 43 ตําแหน่ง การศึกษาผลทาง จุลชีววิทยาด้วยกล้องจุลทรรศน์ชนิดเฟส-คอนทราสต์ และการวัดผลทางคลินิกด้วยค่าความลึกของพ็อกเก็ต และ ระดับการยืดติดของอวัยวะปริทันต์ ในช่วงเวลา 8 สัปดาห์ จํานวนแบคทีเรียใต้เหงือกที่นับได้ให้คํานวณค่าด้วย ลอการิทึมฐาน 10 ก่อนนําไปวิเคราะห์ผลด้วยสถิตินั้นพาราเมตริก ชนิดการทดสอบมันน์-วิตนีย์ทดสอบความแตกต่าง ระหว่างกลุ่มทดลอง และการทดสอบเชิงเครื่องหมายและลําดับที่แบบวิลคอกชันทดสอบความแตกต่างภายในกลุ่ม ทดลองของแต่ละช่วงเวลา การเปรียบเทียบความแตกต่างของค่าเฉลี่ยความลึกของพ็อกเก็ต และระดับการยืดตัว ของอวัยวะปริทันต์ ระหว่างกลุ่มทดลอง โดยใช้สถิติการทดสอบสําหรับกลุ่มตัวอย่างสองกลุ่มที่เป็นอิสระกัน และ การทดสอบที่สําหรับกลุ่มตัวอย่างสองกลุ่มที่สัมพันธ์กันทดสอบความแตกต่างภายในกลุ่มทดลอง ที่ระดับความเชื่อมั่นร้อยละ 95 ผลการศึกษา ผลการวิเคราะห์ระหว่างกลุ่ม พบว่า สัดส่วนของแบคทีเรียทุกประเภทแตกต่างอย่างมีนัยสําคัญทาง สถิติ (p < .05) ในสัปดาห์ที่ 8 โดยสัดส่วนของแบคทีเรียรูปกลม และรูปแท่งเคลื่อนที่ไม่ได้จะเพิ่มขึ้น แต่สัดส่วนของ แบคทีเรียรูปแท่งเคลื่อนที่ได้ และสไปโรคีตส์จะลดลงในทุกช่วงเวลาเมื่อเทียบกับสัปดาห์ที่ 0 อย่างมีนัยสําคัญทางสถิติ (p < .05) นอกจากนี้ ค่าร้อยละของสไปโรคีตส์ในตําแหน่งที่มีพ็อกเก็ตลึกจะมากกว่าตําแหน่งที่มีพ็อกเก็ตขึ้น การเปลี่ยนแปลงลักษณะทางคลินิกได้แก่ ความลึกของพ็อกเก็ตลดลง และระดับการยึดติดของอวัยวะปริทันต์เพิ่มขึ้น ในแต่ละช่วงเวลาอย่างมีนัยสําคัญทางสถิติ (p < .05) ตลอดช่วงเวลาการศึกษา เมื่อเทียบกับสัปดาห์ที่ 0 นอกจากนี้ การเปลี่ยนแปลงลักษณะทางคลินิกระหว่างกลุ่มทดลอง จะแตกต่างกันอย่างมีนัยสําคัญทางสถิติ (p < .05) เช่นเดียวกัน สรุป การใช้หัวอัลทราโซนิกส์ขูดหินน้ําลายคล้ายคิวเรตต์แบบมินิทิป จะเปลี่ยนแปลงปริมาณและสัดส่วนของ แบคทีเรียใต้เหงือก และลักษณะทางคลินิกของอวัยวะปริทันต์เมื่อเทียบกับก่อนการรักษา โดยความลึกของพ็อกเก็ตมีแนวโน้มต่อการเปลี่ยนแปลงสัดส่วนของแบคทีเรียใต้เหงือกภายหลังการรักษา (ว ทันต จุฬาฯ 2549;29:22-32)

ความแข็งแรงดัดขวางชนิดไบแอกเซียลของวัสดุบูรณะเรซินคอมโพสิต, กมลา กฤโตปการ, รังสิมา สกุลณะมรรคา, สุชิต พูลทอง

วัตถุประสงค์ เพื่อศึกษาเปรียบเทียบความแข็งแรงดัดขวางชนิดไบแอกเชียลของวัสดุเรซินคอมโพสิตในปัจจุบัน จํานวน 5 ผลิตภัณฑ์ที่มีฟิลเลอร์และเรซินเมทริกซ์ต่างกัน วิธีการทดลอง วัสดุเรซินคอมโพสิตที่ใช้ในการทดลองนี้จํานวน 5 ผลิตภัณฑ์คือ Admira (Voco), CeramX (Dentsply), Filtek Supreme translucent (3M ESPE), Filtek Supreme standard (3M ESPE) และ Z250(3M ESPE) แต่ละผลิตภัณฑ์จะถูกเตรียมเป็นชิ้นทดสอบรูปแผ่นกลมเส้นผ่านศูนย์กลาง 13 มม.หนา 2 มม. จํานวน 15 ชิ้น และเก็บไว้ในน้ํากลั่นอุณหภูมิ 37 องศาเซลเซียสก่อนการทดสอบ ค่าความแข็งแรงตัดขวางชนิด ไบแอกเชียลของวัสดุเรซินคอมโพสิตได้จากการทดสอบด้วยเครื่องทดสอบสากล ที่ความเร็วการกด 0.5 มม. ต่อ นาที จากนั้นนําข้อมูลมาวิเคราะห์ด้วยสถิติความแปรปรวนแบบทางเดียว และการทดสอบทุกย์ ผลการศึกษา วัสดุ Filtek Supreme translucent มีค่าความแข็งแรงดัดขวางชนิดไบแอกเชียลเฉลี่ย 167.13 เมกะปาสคาลซึ่งสูงกว่าวัสดุทุกชนิดที่นํามาทดสอบ ค่าความแข็งแรงดัดขวางชนิดไบแอกเชียลเฉลี่ยของ Filtek Supreme standard และ Z250 เท่ากับ 154.78 และ 147.01 เมกะปาสคาลตามลําาดับโดยแตกต่างกันอย่างไม่มี นัยสําคัญ วัสดุ Admira มีค่าความแข็งแรงดัดขวางชนิดไบแอกเชียลเท่ากับ 122.67 เมกะปาสคาลและวัสดุ CeramX มีค่าความแข็งแรงดัดขวางชนิดไบแอกเชียลเท่ากับ 109.89 เมกะปาสคาล โดยวัสดุ Admira มีค่า ความแข็งแรงต่ํากว่าวัสดุ Z250 แต่สูงกว่าวัสดุ CeramX อย่างมีนัยสําคัญ สรุป จากวัสดุเรซินคอมโพสิตที่ใช้ในการศึกษานี้ วัสดุเรซินคอมโพสิตที่มีฟิลเลอร์ขนาดเล็กมีค่าความแข็งแรงดัดขวางชนิดไบแอกเซียลสูงกว่าวัสดุมีขนาดฟิลเลอร์ใหญ่กว่าและวัสดุเรซินคอมโพสิตที่มีโมเลกุลของออโมเซอร์ เป็นส่วนประกอบไม่ได้มีความแข็งแรงดัดขวางชนิดไบแอกเชียลมากกว่าวัสดุเรซินคอมโพสิตชนิดอื่น (ว ทันต จุฬาฯ 2549;29:1-12)

การรักษาด้วยหน้ากากดึงขากรรไกรบน, ชวาศรี พูนวุฒิกุล

การใช้หน้ากากดึงขากรรไกรบน (facemask) เป็นวิธีหนึ่งในการแก้ไขการสบฟันแบบคลาสทรี ที่เกิด จากโครงสร้างขากรรไกรบนมีตําแหน่งถอยหลังในผู้ป่วยที่ยังมีการเจริญเติบโตอยู่ ทําให้เกิดการเปลี่ยนแปลงของ โครงสร้างใบหน้า โดยเกิดการเคลื่อนของขากรรไกรบนและฟันบนมาข้างหน้า นิยมทําการขยายขากรรไกรบนร่วมด้วย เพื่อชดเชยกับการหดตัวในบริเวณส่วนหน้าของขากรรไกรบนขณะทําการดึงมาข้างหน้า แม้ก่อนการรักษาไม่ปรากฏว่าขากรรไกรบนแคบก็ตาม ตําแหน่งที่ใช้ดึงยางควรดึงจากบริเวณส่วนหน้าของขากรรไกรบนในทิศทางลงล่าง เพื่อลด การหมุนส่วนหน้าขึ้นของขากรรไกรบน ช่วงระยะเวลาที่เหมาะสมในการดึงขากรรไกรบน ควรทําก่อนที่จะมีการ เจริญเติบโตสูงสุด เพื่อให้เกิดการเปลี่ยนแปลงทางโครงสร้างมากกว่าการเคลื่อนของฟัน ปริมาณแรงที่ใช้เพื่อให้เกิดการเคลื่อนของโครงสร้างนั้นมีปริมาณมากกว่าการเคลื่อนของฟัน ระดับแรงที่พบว่าทําให้เกิดการเคลื่อนที่ของขา กรรไกรบนมาข้างหน้ามีค่าประมาณ 300-500 กรัมต่อข้าง ผู้ป่วยควรใส่เครื่องมืออย่างน้อย 10-12 ชั่วโมงต่อวัน หากปริมาณแรงน้อย ระยะเวลาการใส่แต่ละวันและช่วงระยะเวลาการรักษาก็จะยาวนานขึ้น ทั้งนี้ขึ้นกับระดับความ รุนแรงและอายุของผู้ป่วยด้วย หลังการขยายและดึงขากรรไกรบนมาข้างหน้าจนสําเร็จแล้ว ควรติดตามผลหลังการ รักษา และ/หรือให้ผู้ป่วยใส่เครื่องมือคงสภาพเฉพาะตอนกลางคืนจนกว่าผู้ป่วยหมดการเจริญเติบโต (ว ทันต จุฬาฯ 2549;29:44-51)

Children of alcoholics are those directly affected by familial alcoholism. Characteristics that are common in children of alcoholics in youth and in adulthood are isolation, approval seeking, fear of angry people, and addiction (Seixas & Youcha, 1985). Treatment for children of alcoholics is available through several forms of therapy including self-help groups and family therapy. วัตถุประสงค์ เพื่อประเมินผลการใช้สารลดการเสียวฟันชนิดสารเรซินที่มีตัวเติมต่อการป้องกันการสึกกร่อนของเนื้อฟันจากการแปรงฟันในห้องทดลอง วัสดุและวิธีการ การวิจัยใช้ฟันกรามน้อย 40 ซี่ นํามาเตรียมชิ้นตัวอย่าง โดยตัดแต่งและกําจัดชั้นเคลือบรากฟัน ออกจะได้ชิ้นเนื้อฟันบริเวณรากฟันทั้งหมด 40 ชิ้น นําไปอาบกัมมันตรังสี และแบ่งเป็น 4 กลุ่มตามชนิดของสารที่ใช้ เคลือบบนผิวเนื้อฟัน ได้แก่ กลุ่มที่หนึ่งใช้สารเรซินที่มีตัวเติม Seal&ProtectTM กลุ่มที่สองใช้สารเรซินชนิดเดียวกับ กลุ่มที่หนึ่งแต่ไม่มีตัวเติม Seal&ProtectTM กลุ่มที่สามใช้สารเรซินที่ไม่มีตัวเติมคือใช้ All-bond 2 และกลุ่มที่สี่ เป็นกลุ่มควบคุมที่ไม่มีการเคลือบด้วยสารใด ๆ บนผิวเนื้อฟัน นําชิ้นตัวอย่างไปทดสอบการสึกกร่อนของเนื้อฟันด้วยวิธี เรดิโอเทรเซอร์ โดยดัดแปลงจากวิธีของเอดีเอ วัดปริมาณสารกัมมันตรังสีในสารละลายยาสีฟันที่ได้จากการแปรงฟัน ด้วยเครื่องแปรงฟันวี8 โดยตรวจนับปริมาณและคํานวณหน่วยเป็นจํานวนนับต่อนาที ด้วยเครื่องตรวจนับ กัมมันตรังสี และวิเคราะห์ทางสถิติด้วยการวิเคราะห์ความแปรปรวนทางเดียว ที่ระดับนัยสําคัญ 05 ผลการศึกษา ปริมาณการสึกกร่อนของเนื้อฟันมีความแตกต่างระหว่างกลุ่มอย่างมีนัยสําคัญทางสถิติ (p < .05) โดยกลุ่มที่เคลือบสารเรซินที่มีตัวเติม Seal&ProtectTM มีค่าเฉลี่ยจํานวนการสึกกร่อนของเนื้อฟันน้อยที่สุด และ แตกต่างจากกลุ่มอื่นอย่างมีนัยสําคัญทางสถิติ สรุป ผลจากการวิจัยนี้แสดงว่าสารลดการเสียวฟันชนิดสารเรซินที่มีตัวเติม Seal&ProtectTM สามารถลดการ สึกกร่อนของเนื้อฟันจากการแปรงฟันได้ และอาจเป็นแนวทางในการนําไปใช้รักษาผู้ป่วยที่เสียวรากฟัน อย่างไรก็ดีควรมีการประเมินผลทางคลินิกด้วย (ว ทันต จุฬาฯ 2549;29:13-21)

Loudness Discomfort Levels: A Retrospective Study Comparing Data From Pascoe (1988) And Washington University School Of Medicine, Jennifer N. Keller

Loudess discomfort levels (LDLs) were gathered from three Washington University School of Medicine sites, for a total of 325 subjects (total ears=454). These levels were compared to mean LDLs reported by Pascoe (1988). The results revealed that the mean LDL measured at WUSM (ie., the IHAFF procedure) is significantly different that the LDL reported by Pascoe (1988).

This study examined oral education components that could be successfully implemented with culturally and linguistically diverse deaf and hard of hearing (DHH) children and their families. A literature review of oral program strategies used with culturally diverse families and their children with special needs, and federal guidelines related to programs serving DHH children were conducted. Recent statistics of children in programs for DHH students who are from racially and linguistically diverse backgrounds were discussed. This paper is a study of the effectiveness between video and toy reinforcer types to motivate infants/toddlers for hearing tests. No significant differences were found between age groups or gender. Toy reinforcers were found to produce on average two more threshold estimates compared to video reinforcers. Variety, color and animation of animals may have contributed to this difference.

Ankle sprains account for 17% to 20% of all injuries in most sports. Other ankle conditions such as posterior tibialis tendon disorder and Achilles tendinosis are quite common in the nonathletic population and require many of the same interventions. Some of these interventions include range of motion, stretching, strengthening and proprioception. Many studies have shown that strength and proprioceptive training are the intervention of choice for those with lateral ankle instability. Examines the contextual effects of social persuasion (represented by self, client, peer and supervisor's feedback) and mastery experiences (represented by formal level of education and work experience) on specific self-efficacy outcomes and perceived advancement potential in a sample population of nurses at a Southern California hospital. Results of the study suggest that self, client, peer, and supervisor's feedback consistently predict significant self-efficacy outcomes.

Different structures within the medial-temporal lobe likely make distinct contributions to declarative memory. In particular, several current psychological and computational models of memory predict that the hippocampus and parahippocampal regions play different roles in the formation and retrieval of declarative memories [e.g., Norman, K. A., & O'Reilly, R. C. Modeling hippocampal and neocortical contributions to recognition memory: A complementary-learning systems approach. Psychological Review, 110, 611-646, 2003]. Here, we examined the neuronal firing patterns in these two regions during recognition memory. Analyzes the motivation of clients beginning a drug and alcohol treatment program to see if their level of motivation affects their success in the program, as measured by drug screens. Thirty-six participants from the Perris Valley Recovery Program (PVRP) in Southern California took a motivation for treatment questionnaire reflecting their levels of motivation for treatment. Results of the study found significant relationships between clients' motivation to be in substance abuse treatment and clean or dirty drug screens.
