Medulloblastoma, one of the most malignant brain tumors in children, is thought to arise from undifferentiated neural stem/progenitor cells (NSCs) present in the external granule layer of the cerebellum. However, the mechanism of tumorigenesis remains unknown for the majority of medulloblastomas. In this study, we found that many human medulloblastomas express significantly elevated levels of both myc oncogenes, regulators of neural progenitor proliferation, and REST/NRSF, a transcriptional repressor of neuronal differentiation genes. Previous studies have shown that neither c-Myc nor REST/NRSF alone could cause tumor formation. To determine whether c-Myc and REST/NRSF act together to cause medulloblastomas, we used a …

Zonisamide, a new antiepileptic drug, has been approved in the US as adjunctive therapy for the treatment of partial seizures in adults.^1,2 Chemically a sulfonamide analogue, zonisamide is thought to have several mechanisms of action, including a rate-dependent blockade of voltage-gated sodium channels and reduction of ion flow through T-type calcium channels.^3-5 It is also a weak carbonic anhydrase inhibitor. Zonisamide has a favorable pharmacokinetic profile that includes high oral bioavailability and a long half life (63 hours), permitting a once- or twice-daily dosing regimen.⁶

Follicular dendritic cells (FDC) are non-lymphoid, non-phagocytic accessory cells in the immune system that are essential for antigen presentation and germinal center reaction regulation1. These cells are CD21+, CD35+, CD1a- and S100 protein + and they show desmosomes ultrastructurally.

The most commonly involved sites by FDC tumors are lymph nodes but may arise at a variety of extranodal sites including oral cavity, tonsil, gastrointestinal tract and liver. Most studies represent single case reports or case series.

Integrated Delivery Systems (IDSs) now become a primary means of care provision in healthcare domain. However, existing password systems (under either the single-server model or the multi-server model) do not provide adequate security when applied to IDSs. We are thus motivated to present a practical password authentication system built upon a novel two-server model. We generalize the two-server model to an architecture of a single control server supporting multiple service servers, tailored to the organizational structure of IDSs. Infectious bovine keratoconjunctivitis (IBK) is a highly contagious ocular disease that affects cattle of all ages and that occurs worldwide. Piliated hemolytic Moraxella bovis is recognized as the etiologic agent of IBK. According to data from the Nebraska Veterinary Diagnostic Laboratory System, however, Moraxella (Branhamella) ovis has been isolated with increasing frequency from cattle affected with IBK. The objective of this study was, therefore, to examine M. ovis field isolates for the presence of the putative virulence factors of M. bovis. The association between young adolescents' psychological profiles and their subsequent adjustment was examined in a sample of 606 adolescents (ages 12-13) drawn from the mother-child data set of the National Longitudinal Survey of Youth. Cluster analysis was used to identify distinct groups of youth based on self-regulation, proneness to risk, self-worth, and perceived academic competence. Five replicable clusters were identified corresponding to optimal, average, behavioral risk, low self-regulation, and emotional risk groups. These clusters were associated with distinct patterns of adjustment 4 years later. At ages 16-17, youth in the optimal group tended to report better academic performance, less problem …

The National Children's Study (NCS) is a large, long-term study designed to detect environmental influences on the health and development of children. Waukesha County, Wisconsin, was selected as 1 of 7 "Vanguard Centers" currently funded to finalize and lead the implementation of the study protocol. The authors provide an overview of key design and planning processes that will be used at all NCS Vanguard locations, the specific approaches to be used in the NCS Waukesha Country Vanguard Center, and information about Wisconsin physicians and other health care professionals can become involved in working with the NCS.

Understanding Sexual Abstinence In African American Teens, Kristin Haglund

Purpose: To explore the perspectives of teenage girls on how life contexts influenced sexuality and sexual abstinence.

Study Design and Methods: A qualitative descriptive study with a convenience sample of 14 sexually abstinent African American adolescent females who were interviewed to obtain their life histories. Narrative analysis was used to identify unique and common experiences and to develop themes.

Results: For these participants, being abstinent was a way to demonstrate their emerging identities as adult women. They described themselves as faithful, unique persons who defied negative stereotypes, avoided risky situations, wanted to be strong women like their …

Patient Insurance Status And Do-Not-Resuscitate Orders: Survival Of The Richest?, Gigi Nordquist

This study investigated the effect of patient insurance status upon physicians' decisions to write do-not-resuscitate orders (DNRs). Ninety-four physicians completed a questionnaire consisting of demographic data and a case vignette. In addition to the main research question, the study explored the effect of religious affiliation on writing DNRs and performing "slow codes." Results indicate that insurance status has a significant effect upon the likelihood of writing a DNR, with physicians more likely to write DNRs for patients covered by public (i.e., government-funded, as compared to private) insurance. With rising costs and increasing complexities, many hospitals seek to better understand the intricate details of their operations. Increasingly, these organizations have a strong desire to accurately predict the resources required to effectively treat their patient load. This research investigates patient length-of-stay in a hospital neurological unit using an empirical modeling approach. Factors significantly affecting patient length of stay were identified and used to construct a regression model. The predictive model provides hospital decision makers with a compact tool to input what-if scenarios and predict future patient treatment lengths, thus, allowing the hospital to properly allocate resources.

Two decades ago, Aerospace Medical Technicians received robust skills sustainment training through exposure to multifaceted patient treatment environments. Available training environments included inpatient care, outpatient care, and emergency services. This diverse training environment made possible through large operating budgets and an extraordinary infrastructure could not last. Today (after five separate base closure and realignment initiatives), medical funding and infrastructure is but a shadow of what it once was. However, weight loss is generally associated with negative effects on bone mass, but CLA is reported to have beneficial effects on bone. Objective: To investigate the effectiveness of X-ray and echocardiography (ECHO) as a diagnostic tool for cardiac tamponade in adult cardiac surgery patients.Methods: Thirty five coronary artery bypass and graft surgery patients who developed cardiac tamponade at Cleveland clinic foundation, were included in this study. Their diagnosis was confirmed at the time of re-exploration in the operating room. These patients were followed retrospectively for demographics, X-ray and echocardiography findings. Abnormal and enlarged cardiac silhouette which was different from first postoperative X-ray was used as radiological criteria for tamponade while echocardiographic diagnosis was left to the discretion of cardiologist.Results: Twenty four males …

Objective: Anaesthesia-Ready Time (ART) is the time taken by the anaesthetist to provide sufficient anaesthetic depth for start of surgery. Our aim was to set benchmark timings for ART and compare it with our current practice.Methods: Benchmark ART time of 15 minutes was set for American Society of Anesthesiologists (ASA) class I and II patients, 30 minutes for ASA III and IV patients, 20 minutes for spinal and 30 minutes for epidural anaesthesia. The investigation of the relative roles of Soluble Microbial Products (SMP) in membrane fouling was studied in submerged membrane bioreactors (SMBRs). Three lab-scale SMBRs were operated at a constant permeate flux (12.5 l/m2 h) with a flat sheet microfiltration membrane (hydrophilic polyolefin, pore size of 0.25 μm) using different sludge retention times (SRTs of 8, 20 and 80 days, respectively). The concentrations of SMP in terms of dissolved organic carbon (DOC) and protein at the steady-state period were compared. 288 kidneys complete with renal arteries and abdominal aorta, were collected from 144 dogs (75 males and 69 females), dissected and studied according to the number, branch, length and position of the renal arteries bifurcation. The positions of the left and right renal arteries were also observed. Double renal arteries were noted in 14 of 144 dogs (9.72%) but only on the left side. Single renal arteries had one to six branches. Most kidneys had 2 (39.42%) or 3 (33.58%) branches. The bifurcation mostly occurred in the middle of the renal artery (60.61%). Ribosome recycling, the last step in translation, is now accepted as an essential process for prokaryotes. In 2005, three laboratories showed that ribosome-recycling factor (RRF) and elongation factor G (EF-G) cause dissociation of ribosomes into subunits, solving the long-standing problem of how this essential step of translation occurs. However, there remains ongoing controversy regarding the other actions of RRF and EF-G during ribosome recycling. Thermocycling simulates the temperature dynamics in the oral environment. With direct bonding, thermocycling reduces the bond strength of orthodontic adhesives to tooth structure. The purpose of this study was to evaluate the shear bond strengths (SBS) of one direct and two indirect bonding methods/adhesives after thermocycling. Sixty human premolars were divided into three groups. Teeth in group 1 were bonded directly with Transbond XT. Teeth in group 2 were indirect bonded with Transbond XT/Sondhi Rapid Set, which is chemically cured. Teeth in group 3 were indirect bonded with Enlight LV/Orthosolo and light cured. Malignant catarrhal fever (MCF) is a highly fatal disease of ruminants which has not been extensively studied in Thailand. The aim of this study was to describe the clinical signs and pathological findings of MCFaffected buffaloes (n = 13), in the Nakhonpathom province of Thailand. All cases were collected between May 1996 and August 2000. Before the 1998 outbreak, four clinically normal sheep were moved into a pen adjacent to the buffaloes’ pens. The animals developed clinical signs of severe depression, anorexia, high fever, hypersalivation, and corneal opacity-associated excessive lacrimation. Necropsy revealed severe haemorrhages in various organs including the oropharynx, …
