Background: Characterization of fatal and nonfatal reactions to food indicates that the majority of reactions are due to the ingestion of prepared foods rather than the nonprocessed allergen. In an ongoing study that used a double-blind placebo-controlled food challenge to investigate peanut allergy and clinical symptoms, the observed reaction severity in four of the first six subjects was greater than anticipated. We hypothesized that this was due to differences in the composition of the challenge vehicle.

Midazolam And Thiopentone Co-Induction: Looking For Improvement In Quality Of Anaesthesia, M A. Khan, F A. Khan

Objective: To evaluate improvement in quality of anaesthesia induction using thiopentone and midazolam for co-induction of anaesthesia. An additional end point was taken as loss of response to a tetanic stimulation (50 Hz) delivered for 5 seconds after the loss of verbal contact and eyelash reflex.Methods: Ninety ASA I and II patients, within the age range of 20-60 years were studied. Patients were randomly divided into three equal groups; A, B and group C.Results: Onset of induction and loss of response to a tetanic stimulation was achieved earlier in group B as compared to the other study groups (p < 0.05).CONCLUSION: Co-induction with midazolam 0.02 mg x kg(-1) followed by thiopentone 3 mg x kg(-1) was superior to other two groups. Induction of anaesthesia in this group was found to be smoother and faster, provided better hemodynamic stability, better airway maintenance and with lesser incidence of untoward effects.

The health care industry, in its quest to improve outcomes with fewer resources, began to look at CQI/TQM as a possible solution. To date, the most comprehensive and enthusiastic response to efforts to improve quality while containing or lowering costs is reflected in hospitals' commitment to continuous quality improvement/total quality management (CQI/TQM). The purpose of this article is to describe the process of creating a faculty development program for online instruction. Both educational and technological issues are discussed. Factors that will facilitate and barriers that will impede the implementation of online courses are included. Many faculty concerns regarding online courses relate to the issues of quality and student learning. Faculty development activities are directed to ensure that courses are developed using sound educational theory and principles. In a study of health care expenditures attributable to smoking, we seek to compare the distribution of medical costs for persons with lung cancer or chronic obstructive pulmonary disease (cases) to those without (controls) using a national survey which includes hundreds of cases and thousands of controls. The distribution of costs is highly skewed toward larger values, making estimates of the mean from the smaller sample dependent on a small fraction of the biggest values. One approach to deal with the smaller sample is to rely on a simple parametric model such as the log-normal, but this makes the undesirable …

On March 3, 2003, a 22-year-old Hispanic female laborer (decedent) died when she fell from a roof. She had been working on the roof with her brother, who was a sub-contractor for a local roofing company. The work crew consisted of two brothers and the decedent. Two days prior to the incident, the decedent had worked for her brother cleaning and organizing materials on the ground at another location. It was the decedent’s first day on this job site. It is unknown whether or not the decedent had worked on other roofing jobs. Her job was to organize materials and …

Black and White mock jurors’ sensitivity to the cross-race effect was investigated by varying the race of the eyewitness in a simulated murder trial of a Black defendant. Participants heard an audiotape of a trial after which they rendered a verdict and rated the credibility of the witnesses. White participants found the prosecution witnesses (including the eyewitness) more credible, and the defense witness less credible, than did Black participants; they were also more likely to find the defendant guilty. The Black eyewitness was perceived as more credible than was the White eyewitness, but eyewitness race had no effect on verdict. …

Near-infrared spectroscopic tomography was used to measure the properties of 24 mammographically normal breasts to quantify whole-breast absorption and scattering spectra and to evaluate which tissue composition characteristics can be determined from these spectra. The absorption spectrum of breast tissue allows quantification of (i) total hemoglobin concentration, (ii) hemoglobin oxygen saturation, and (iii) water concentration, whereas the scattering spectrum provides information about the size and number density of cellular components and structural matrix elements. These property data were tested for correlation to demographic information, including subject age, body mass index, breast size, and radiographic …

We previously showed that COL1A1 expression is up-regulated at the transcriptional level in systemic sclerosis (SSc) fibroblasts and that the CCAAT-binding factor (CBF) is involved in this increased expression. Ecteinascidin 743 (ET-743) is a chemotherapeutic agent that binds with sequence specificity to the minor groove of DNA and inhibits CBF-mediated transcriptional activation of numerous genes. Therefore, we examined the effects of ET-743 on the increased COL1A1 expression in SSc fibroblasts. The drug caused a potent and dose-dependent inhibition of type I collagen biosynthesis, which reached 70-90% at 700 pM without affecting cell viability. The purpose of this secondary analysis was to examine the difference in quality of life (QOL) in home-care patients with heart failure (HF) receiving nursing intervention. The specific nursing intervention of interest was supportive-education (SE), based on Orem's (1995) self-care deficit theory. The second intervention of general health teaching acted as a placebo effect. The convenience sample of randomly assigned patients with HF was from two home-care agencies. The paired t-test was used to analyze the difference in QOL within the SE group and the placebo group from baseline to six months. The independent t-test was used to compare the …

On June 19, 2003, a 58-year-old bulldozer owner/operator (decedent) died when he was thrown off the track of his bulldozer. The bulldozer owner/operator and a laborer were having difficulty starting the bulldozer. Together, the two men had tried several different methods to start the bulldozer including: spraying ether into the air chamber, having the battery recharged, replacing the battery cables with new ones and tightening wires to the starter. They worked from the left side track of the bulldozer. The laborer sat towards the front of the track tightening the wires to the starter while the owner stood on the …

The Kentucky Fatality Assessment and Control Evaluation program was notified July 31, 2003, via newspaper surveillance, of an occupational fatality involving a 55-year-old bulldozer owner/operator. On July 29, 2003, a bulldozer owner / operator drowned in the cab of his machine as he was trying to find a leak in a pond dam. The operator had been hired by a local farm owner to find and repair a leak in one of the ponds on her farm. As the operator dug a trench through the earthen dam, a section at the top collapsed sending sludge into the trench. The water/sludge …

The Health Enhancement of the Rural Elderly (HERE) project is a federally funded grant project designed to empower the rural elderly in Butler and Edmonson counties to maximize their use of the health care system. This project was developed after identifying the needs of approximately 25% of the population in the U.S. that reside in rural areas. Of this population in rural Kentucky, 14-15% is 65 years or older (U.S. Bureau of Census, 2000). A higher incidence of chronic diseases, disabilities, difficulties with daily living activities, and cutbacks in social services have been identified as factors causing this population to …
