The past decade has been characterized by increasing concern about the medical, social, and economic problems associated with reproductive tract infections (RTIs). The goal of preventing and curing RTIs is now being prioritized by public health agencies in the developing world. Very little research has been conducted on the problem of RTIs in the local context, and it would be helpful for program managers if the knowledge and beliefs now being held about RTIs were more clearly delineated. This study’s objective was to assist the Kenya Ministry of Health (MOH), the National Council for Population and Development (NCPD), NGOs, and donor agencies in planning the expansion and improvement of family planning (FP) and other reproductive health (RH) services provided in Kenya. Data on the functioning of program subsystems and the quality of care provided were collected in May 1995 from 254 maternal and child health/family planning (MCH/FP) facilities throughout the country using a situation analysis approach. Comparisons of College Adjustment Scales (CAS) profiles with interview data from 748 clients generally supported the clinical usefulness of the CAS, although further refinements appear warranted.

The effects on feeding of moving animals from individual- to pair-housing were explored in the present three experiments. Wheel access is known to result in a temporary suppression of feeding. The move to pair-housing was compared with the effect of wheel access. In Experiment l, a group of rats was moved from individual to pair-housing (IP group) and compared to a group of individually-housed rats (IND group) and a group of chronically pair-housed rats (PAIR group). The animals in the IP group showed a temporary (three days) suppression in feeding (initially 23%). When half of the IND and PAIR group …

This study proposes that family systems therapy can prove useful in understanding Biblical families and thus add to our understanding of the Bible. Employing family systems therapy is appropriate because the Biblical narrative contains a comprehensive treatment of the human family. The Bible deals with family overall, many specific families as in the Jacob cycle, and ends with a vision of the family of God that exists already in the kingdom of heaven. This thesis is a feminist deconstruction of anorexia nervosa (AN) in women. It begins with questioning the truth claims about AN established by the psychological and psychiatric fields. After an overview of the medical model literature and the feminist literature on eating problems, a critical analysis is used to deconstruct the sociocultural elements of AN. The purpose of this analysis is to critically examine the assumptions which have shaped how clinicians approach the phenomenon of AN in their practice with clients. What has been accepted as underlying psychopathology is deconstructed to reveal normative culturally constructed factors. A paradigm shift towards …

How problematic is power imbalance in divorce mediation? Research in the last fifteen years has drawn attention to this issue, resulting in criticisms levelled at mediation from women’s groups, lawyers and others. Using six in-depth interviews with Ontario divorce mediators, this study examined power imbalance and its management. Three main themes emerged from the findings related to power: (1) the ‘emotional readiness’ of the couple, (2) the effects of the legal system, in particular, the relationships with lawyers, and, (3) physical intimidation and financial intimidation. Techniques employed by the mediators were similar across the interviews. These findings led to a …

This qualitative research study explored the use of a narrative therapeutic summary letter format as an adjunctive intervention with clients engaged in individual therapy. The study’s sample consisted of six client subjects who received summary letters from six therapist subjects. Client subjects were furnished with written summaries of four consecutive counselling sessions. The summaries were based on pre-established narrative summary guidelines supplied to each respective therapist. After client subjects had received four consecutive summary letters, in-depth individual interviews were conducted with each client. Worldwide in the 1990s over one billion persons are estimated to have a purchasing power of below a dollar per day, the conventional demarcation of “absolute poverty.” Other dimensions of poverty, extending beyond income measures to encompass a person’s broader capabilities and social functioning, are less empirically accessible. Poverty is commonly thought to be associated with high fertility and rapid population growth (regionally, South Asia and Africa have the highest poverty rates), but that view finds little support in the extensive statistical research literature on population and poverty. In an expansion of its efforts to fight a growing HIV epidemic and high teenage pregnancy rates, the Government of Botswana is encouraging nongovernmental organizations (NG0s) to be more active in providing reproductive health (RH) services, and especially HIV/AIDS education and care, to adolescents. NG0s have the potential to assume a larger role as health and education providers as well as implement pilot projects that the public health sector could learn from to increase the availability and quality of RH services for youth. This report documents the proceedings of a training workshop on reproductive tract infection (RTI) case management for physicians, nurses, and midwives in selected health centers of the Philippines. The training started with presentation of focus group discussion results emphasizing existing beliefs and perceptions of the community regarding RTIs. The results of the situation analysis served as an eye opener for many in that clients with RTI-related symptoms were found to be rather rare and not systematically managed in the health clinics. Many providers report that since they are not trained to handle such cases, they refer whoever comes with symptoms …

The overall goal of this study was to develop an integrated program of reproductive tract infection/sexually transmitted disease (RTI/STD) clinical services within two family planning clinics in low-income neighborhoods in North Jakarta. Multiple training programs to introduce a standardized clinical evaluation for the detection, treatment, and management of RTI/STDs were conducted. Prior and subsequent to training, observations of health care provider (HCP) and client interactions were conducted to evaluate behavior changes among HCPs. Each consenting client received a standardized reproductive health history, a pelvic exam, and provided specimens for laboratory testing to detect RTIs. An ANE OR/TA Project qualitative study conducted in 1995 probed into women’s perceptions of abortion in Egypt, and the stress that postabortion patients experience during recovery. That study drew attention to the important role husbands can play in their wives’ recovery and subsequent use of contraception. This study was designed to test the effects of involving husbands in the postabortion medical-care process. Overall, the study indicates that providing counseling to husbands of postabortion patients is feasible, as the majority of husbands either accompanied their wife on admission or at discharge from the hospital. Lactose intolerance is frequently encountered in children with persistent diarrhoea (PD). Selection of an appropriate milk-based formulation is a major management problem in the developing world. In a consecutive series of studies, we evaluated the role of feeding a traditional rice-lentil (khitchri) diet alone (KY) or in combination with either soy formula (KY-Soy) a dilute buffalo milk (KY-B), in children (age 6 months-3 years) with PD. Serial observations of stool output, caloric intake and weight gain of these children over a 14 day period indicated satisfactory tolerance of the KY diet with adequate weight gain. The prevalence of oral mucosal lesions in Malaysia was determined by examining a representative sample of 11707 subjects aged 25 years and above throughout the 14 states over a period of 5 months during 1993/1994. A two-stage stratified random sampling was undertaken. A predetermined number of enumeration blocks, the smallest population unit in the census publication, was selected from each state. With the selected enumeration block, a systematic sample of living quarters was chosen with a random start.
