The Diploma in Family Medicine (DFM) Examination Is a new certification offered by the College of Physicians and Surgeons of Pakistan, and its Department of Medical Education designed a scientific examination. First, the Expert Advisory Committee for Family Medicine was formed, relevant training objectives Were determined, a training programme to achieve the objectives was designed and a valid syllabus was chosen. Then the examination was designed, where the candidates must pass the objective theory papers before taking the clinical examination. The clinical examination consisted of an Objective Structured Clinical Examination (OSCE) and traditional case presentations. The candidates had to pass …

Street youth are at risk of HIV infection worldwide. To develop effective prevention strategies, information about the meanings and functions of sexual activity for street youth is needed. In this paper, data from structured questionnaires, focus group discussions, in-depth interviews and field observations are used to build up a picture of the sexual culture of 9-to-18-year-olds living and/or working on the streets of a large Brazilian city. The findings reveal that these children and adolescents engage in sexual behavior that puts them at risk of sexually transmitted diseases, including HIV/AIDS, and reinforce that sex is a multi-determined and entrenched behavior …

Although there is widespread interest in preventing teenage pregnancy, few interventions have been directed toward young adolescents. In part this neglect is understandable because adolescents under age 15 account for only a small percentage of teenage pregnancies (Pittman & Adams, 1988). At the same time, the lack of attention to younger adolescents is unfortunate on several counts. First, an increasing number of young adolescents are at risk. Rates of sexual activity have been rising in this group (Hofferth, Kahn, & Baldwin, 1987) and, because young adolescents are typically not consistent or effective contraceptive users, an increasing number of girls are …

Sections 307 and 309 of the 1988 Indoor Radon Abatement Act (IRAA) direct EPA to identify areas of the United States that have the potential to produce elevated levels of radon. EPA, the U.S. Geological Survey (USGS), and the Association of, American State Geologists (AASG) have worked closely over the past several years to produce a series of maps and documents which address these directives. The EPA Map of Radon Zones is a compilation of that work and fulfills the requirements of sections 307 and 309 of IRAA. The Map of Radon Zones identifies, on a county-by-county basis, areas of …

Two cases of polyarthritis in young people with extra-articular tuberculosis are described. The clinical features were consistent with the concept of Poncet's disease. After synovial biopsy one patient proved to have tuberculous peripheral arthritis. This observation raised doubts about the diagnosis in the other patient. Multiple joints may be simultaneously infected with Mycobacterium tuberculosis and Poncet's disease, if it exists, should not be entertained in the absence of synovial biopsy.

The impact of fathers on the development and well-being of their children has been the focus of much recent research and debate (e.g., Furstenberg, 1988; Hawkins & Eggebeen, 1991; Lamb, 1987). Traditionally, fathers have been assumed to play an important role in the socialization of their children and, consequently, father-absence has been hypothesized to result in a variety of cognitive and psychosocial vulnerabilities. Evidence For A Second Type Of Fibril Branch Point In Fibrin Polymer Networks, The Trimolecular Junction, Michael W. Mosesson, James P. Diorio, Kevin R. Siebenlist, Joseph S. Wall, James F. Hainfeld

Fibrin molecules polymerize to double-stranded fibrils by intermolecular end-to-middle domain pairing of complementary polymerization sites, accompanied by fibril branching to form a clot network. Mass/length measurements on scanning transmission electron microscopic images of fibrils comprising branch points showed two types of junctions. Tetramolecular junctions occur when two fibrils converge, creating a third branch with twice the mass/length of its constituents. Newly recognized trimolecular junctions have three fibril branches of equal mass/length, and occur when an extraneous fibrin molecule initiates branching in a propagating fibril by bridging across two unpaired complementary polymerization sites. Mental health crises are often presented and thought about as individual problems, activities going on only within the minds of individuals. This presentation considers mental health crisis as a regional problem. It discusses the four county region of North Central West Virginia which is the catchment area for Valley Community Mental Health Center. It is based on a study conducted by the author of the need for a revised and integrated crisis management system for the region.

In today's competitive home health industry, productivity has become a major concern, and establishing productivity expectations is necessary for survival. Productivity can be described as a relationship between the outputs produced by an organization and the inputs provided to create the outputs. This means that the outputs consist of the number of home visits performed and the inputs consist of the time, supplies, mileage, and administrative support necessary for producing home visits. The purpose of this study was to assess the relationship between meaning in life and utilization of health care services. An epidemiological, cross-sectional study was performed on a sample of 282 students at Western Kentucky University and 49 employees at the Barren River District Health Department. Study participants completed the Life Attitude Profile--Revised and the Participant Data Sheet. The data gathered and analyzed via correlation analysis showed no overall significant relationship between the two variables for this sample. The present study sought to determine whether or not state anxiety levels were significantly reduced when individuals participated in aerobic dance workouts of 15 minutes and 30 minutes. Undergraduate university students and faculty enrolled in an aerobic dance class were volunteers in this study. As hypothesized, subjects in the aerobic dance classes did reduce anxiety acutely following both their 15 minute and 30 minute workout as assessed by the STAI (Y-Form State Anxiety). Thus, results indicate that state anxiety may be reduced through aerobic dance with similar situations and individuals. In addition, the study also looked at the posttest state …

The AKXL-5 recombinant inbred mouse strain is positive for the endogenous ecotropic murine leukemia virus emv-14, the only emv present in its germ line. emv-14 is of particular interest because spleen cells expressing emv-14 virus escape recognition by anti-AKR/Gross virus-specific cytotoxic T lymphocytes. We report here the isolation and characterization of a replication-competent emv clone, pAK7, derived from an AKXL-5 mouse. This clone is novel in that it encodes a variant ecotropic murine leukemia virus that, when expressed in SC.Kb target cells, fails to be recognized efficiently by anti-AKR/Gross virus cytotoxic T lymphocytes. Abstract
The red cell distribution width (RDW) is an index of the variation in red cells size (anisocytosis). A study was conducted to examine the validity of using RDW in improving classification of microcytic anaemias. A total of 300 blood samples collected from a patient population aged 3 months to 55 years who were referred for haemoglobin electrophoresis were examined at The Aga Khan University Hospital (AKUH). On complete blood count, initially 200 patients (66.6%) were found to have hypochromic microcytic anaemia. Following haemoglobin electrophoresis 41% (821200) patients were diagnosed to have thalassemia minor and 59% (1181200) had hypochromic microcytic …

Children undergoing chemotherapy for cancer are at increased risk of supervening infection, including bacteremia. This may be due to immunosuppression as well as the presence of intravascular devices such as central venous catheters. Bacteria responsible for these infections include those found in the child's endogenous flora as well as in environmental sources. We report the case of a child with acute lymphoblastic leukemia (ALL) who developed bacteremia secondary to Ochrobactrum anthropi. Details of this extremely rare infection are the focus of this report.

We previously described the pattern of sequence variation in gp120 following persistent infection of rhesus monkeys with the pathogenic simian immunodeficiency virus SIVmac239 molecular clone (D.P.W. Burns and R.C. Desrosiers, J. Virol. 65:1843, 1991). Sequence changes were confined largely to five variable regions (V1 to V5), four of which correspond to human immunodeficiency virus type 1 (HIV-1) gp120 variable regions. Remarkably, 182 of 186 nucleotide substitutions that were documented in these variable regions resulted in amino acid changes. This is an extremely nonrandom pattern, which suggests selective pressure driving amino acid changes in discrete variable domains. In the present study, …

RATIONALE: Advanced glycosylation end products (AGEs) may play an important role in the development of diabetic vascular sequelae. An AGE cross-link, pentosidine, is a sensitive and specific marker for tissue levels of AGEs. OBJECTIVES: To evaluate the role of AGEs in the development of diabetic nephropathy and retinopathy, we studied pentosidine levels and the clinical characteristics of 48 subjects with insulin-dependent diabetes mellitus. Diabetic nephropathy was classified as normal, microalbuminuria, or gross proteinuria, and retinopathy was graded as none, background, or proliferative.
