Selective Increases In Antibody Isotypes And Immunoglobulin G Subclass Responses To Secreted Antigens In Tuberculosis Patients And Healthy Household Contacts Of The Patients, R Hussain, G Dawood, N Abrar, Z Toossi, A Minai, M Dojki, J J. Ellner

Mycobacterium tuberculosis-specific antibodies (immunoglobulin M [IgM], IgE, IgG, and IgG subclasses) were determined in 164 tuberculosis patients (pulmonary involvement, n = 135; lymph node involvement, n = 29), 59 healthy household contacts (HC), and 51 healthy endemic donors (EC) by a quantitative enzyme-linked immunosorbent assay for reactivity with culture filtrate. Among the isotypes, significant differences between tuberculosis patient groups with either pulmonary or lymph node involvement and healthy control groups (HC and EC) were detected only for IgG (P < 0.001) and IgG1 (P < 0.001) antibodies. Pulmonary patients also showed a significant difference with IgM (P < 0.01) and IgE (P < 0.05) antibodies. HC showed elevation of only IgM antibodies compared with EC, indicating that IgM antibodies may be an indicator of recent infection with M. tuberculosis. These results suggest that the switching of IgM antibody response to IgG1 is a critical event in disease progression. Polyclonal IgG1, IgG3, and IgE antibodies also showed significant elevation (P < 0.05) in patients compared with EC. A strong correlation (rho = 0.254; P < 0.003) was observed between M. tuberculosis-specific IgG1 and polyclonal IgG1 in patients, suggesting that activations of antigen-specific and polyclonal antibodies are related events. No correlation was found between IgG1 antibodies and purified protein derivative skin test results. Since IgG1 antibody responses to culture filtrate are present only after disease establishment, IgG1 responses could provide a useful diagnostic marker of disease.

Perinatal health professionals are in key positions to either promote or dissuade the use of Natural Family Planning (NFP). The purpose of this article is to describe a survey conducted with perinatal physicians and nurses on their knowledge and professional use of NFP. Four hundred and fifty physicians and nurses (150 MDs and 300 RNs) were sent a questionnaire on the use of and knowledge of NFP. One hundred sixty-six (or 37%) returned the completed questionnaires. A 21-year-old farm worker was killed in a tractor rollover incident. The victim was driving a tractor on a secondary public road, pulling an unloaded 18-foot flatbed trailer, when he lost control. After the tractor wheels went off the right side of the road, the victim overcorrected, which caused the vehicle to become unbalanced; he was thrown from the tractor as it overturned into a ditch. It was not equipped with an appropriate rollover protective structure (ROPS) or a seatbelt. A 34-year-old male welder was killed after being runover by an unmanned pipelayer. The victim worked for a pipelaying construction company as a contracted welder. On the day of the incident the victim, a welder's helper, and an equipment operator drove to the work site together to begin the day's activities. Novel classes of inhibitors which selectively inhibit the cellular transport of normally transported substances, specifically polyamines are taught which comprise (i) polymers of the transported substance or (ii) protein or polypeptide conjugates of the transported substance. These inhibitors may be used in vitro to assess the effect of the transported substance on cellular functions and in vivo for treating disease conditions involving transport of the particular substance, e.g., a polyamine.

A 22-year-old logger was killed after being struck on the head by a falling snag (dead standing tree). The victim was working with one co-worker, who was selectively cutting trees which were approximately 22 inches in diameter. Standing approximately three feet from the tree being cut, the victim was not wearing a hard hat. The cut tree broke a snag about 35 feet up, causing the snag to fall and strike the victim on the head. A 41-year-old self-employed logger was killed when the endloader he was driving down a steep mountain road rolled over. Recent rainfall had made the dirt road wet and muddy. Tire marks left in the mud indicated that as the vehicle began to slide downhill, it hit the right embankment, causing it to veer to the left side of the road and roll over several times before coming to rest at the bottom of the dropoff on its left side. Monopsony Power And Relative Wages In The Labor Market For Nurses, Barry T. Hirsch, Edward J. Schumacher

This paper examines the thesis that monopsony power is an important determinant of wages in nursing labor markets. Using data from the 1985-93 Current Population Surveys, measures of relative nursejnon-nurse wage rates for 252 labor markets are constructed. Contrary to predictions from the monopsony model, no positive relationship exists between relative nursing wages and hospital density or market size. Nor is support found for the presence of monopsony power based on evidence on union wage premiums, slopes of experience profiles, or the mix of RN to total hospital employment.

Changing Relationships Between Self-Help Groups And Mental Health Professionals: Shifting Ideology And Power, Vic Constantino, Geoffrey Nelson

Using qualitative methods, we studied current and desired relationships between members of mental health consumer/survivor and family self-help groups and mental health professionals. Two major themes, professional ideology and practices of power and control, captured both the positive and negative aspects of the relationships the participants had experienced. To move toward more desired relationships, participants suggested education and ongoing interaction between self-helpers and professionals are needed to change professional ideology and practices, while consumer/survivor and family participation and involvement in decision making are required to reduce the power imbalance between self-helpers and professionals. This essay consists of five sections. The first section describes the problem of sex-selective abortion, including an analysis of sociological data regarding adult preference for male children and its current effects. Section Two discusses various philosophical paradigms and analyses of sex-selective abortion with the goal of developing a coherent philosophical base from which to argue for a policy regarding sex-selective abortion which furthers the goals of gender equality. Section Three addresses the constitutionality of sex-selective abortion prohibitions in light of the Supreme Court's pronouncement in Planned Parenthood of Southeastern Pennsylvania v. Casey. Approximately 1.6% of all women will develop cancer of the cervix during their lifetime (4). Cervical cancer is the second leading type of cancer in women and accounts for approximately one-sixth of all cancer deaths in females. In many ways, cervical cancer behaves as a sexually transmitted disease. The major risk factors associated with cervical cancer are the early onset of sexual intercourse, multiple sexual partners, and/or sexual contact with promiscuous partners (66, 94). When evaluated for promotion or tenure, faculty members are increasingly judged more on the quality than on the quantity of their scholarly publications. As a result, they want help from librarians in locating all citations to their published works for documentation in their curriculum vitae. Citation analysis using Science Citation Index and Social Science Citation Index provides a logical starting point in measuring quality, but the limitations of these sources leave a void in coverage of citations to an author's work. This article discusses alternative and additional methods of locating citations to published works.

Mycobacterium avium complex strains and Mycobacterium paratuberculosis are closely related intracellular pathogens affecting humans and animals. M. avium complex infections are a leading cause of morbidity and mortality in AIDS patients, and M. paratuberculosis is the agent of Johne's disease in ruminants. Genetic manipulation of these micro-organisms would facilitate the understanding of their pathogenesis, the construction of attenuated vaccine strains and the development of new drugs and treatment methods. This paper describes the replication of mycobacterial shuttle phasmids and plasmids, and the expression of the firefly luciferase reporter gene in M. avium complex and M. paratuberculosis. Combined treatment with allogeneic small lymphocytes or T-depleted small lymphocytes plus a blocking antibody to CD40 ligand (CD40L) permitted indefinite pancreatic islet allograft survival in 37 of 40 recipients that differed from islet donors at major and minor histocompatibility loci. The effect of the allogeneic small lymphocytes was donor antigen-specific. Neither treatment alone was as effective as combined treatment, although anti-CD40L by itself allowed indefinite islet allograft survival in 40% of recipients. Our interpretation is that small lymphocytes expressing donor antigens in the absence of appropriate costimulatory signals are tolerogenic for alloreactive host cells. With adequate cost containment unlikely in the foreseeable future, health care use will have to be curtailed, ideally with open and explicit criteria for equitably allocating resources or rationing. Yet, consensus on any such criteria appears remote because Americans cannot say no to health care. Americans may refuse to accept rationing for two reasons. The absence of any global limitation on health care resources may encourage patients to believe that health care resources are not scarce and do not need to be rationed. A belief in vitalism--that everyone is morally entitled to unlimited longevity and good health--may discourage setting limits …
