Nurses spend many hours with dying cancer patients, developing close relationships with both the patients and their families. However, nurses are frequently not involved in the process of requesting an autopsy, either by choice or tradition. During this time of great stress, the nurse can be more involved. The nurse who understands the reasons for autopsy and its ramification for the grieving process can be actively involved in supporting the physician who makes the request and the family members who must make the this difficult decision. The Legionnaire's disease bacterium, Legionella pneumophila, is a facultative intracellular pathogen which invades and replicates within two evolutionarily distant hosts, free-living protozoa and mammalian cells. Invasion and intracellular replication within protozoa are thought to be major factors in the transmission of Legionnaire's disease. Although attachment and invasion of human macrophages by L. pneumophila is mediated in part by the complement receptors CR1 and CR3, the protozoan receptor involved in bacterial attachment and invasion has not been identified. Intrinsic And Extrinsic Control Of Hemopoietic Stem Cell Numbers: Mapping Of A Stem Cell Gene, Gerald De Haan, Gary Van Zant

We evaluated in vivo interactions between extrinsic (growth factor induced) and intrinsic (genetically determined) effectors of mouse primitive hemopoietic stem cell proliferation and numbers. Accordingly, stem cell frequency and cell cycle kinetics were assessed in eight strains of inbred mice using the cobblestone area–forming cell (CAFC) assay. A strong inverse correlation was observed between mouse lifespan and the number of autonomously cycling progenitors (CAFC day 7) in the femur. The population size of primitive stem cells (CAFC day 35) varied widely (up to sevenfold) among strains, unlike total CAFC day 7 numbers (cycling and quiescent), which were similar. The purpose of this research was to assess the lifestyle of international students at Western Kentucky University (WKU) and to compare the lifestyles of international and American students and subgroups of international students. The Lifestyle Questionnaire (LSQ), was a 26-item personal health lifestyle test, which had six sections (nutrition, exercise, smoking, alcohol and drugs, safety, and stress). The test-retest reliability of LSQ was evaluated by Pearson Correlation Coefficient (r) and was .89. During October and November 1997, 55 international students and 64 American students at WKU finished the LSQ. Significant differences were found between the American student group and the …

The rural community has lacked adequate healthcare providers for the past thirty years. Collaboration between physician and family nurse practitioner is an option for meeting the need for delivery of primary healthcare in the rural community. A descriptive design was used to describe the acceptance of the nurse practitioner by patients living in rural southcentral Kentucky who utilize the services of the community hospital. Every fifth inpatient and outpatient discharge was mailed the Kviz Assessment Questionnaire and a demographic survey. The questionnaires (N = 842) were mailed over a four week survey period with reminder postcards mailed to each survey …

Humans have been using plant-derived drugs for as long as we have recorded history. Significant negative consequences related to drug abuse have occurred in the areas of health, social issues, family relations, legal systems, and economic productivity. The purpose of this study was to explore the relationship between meaning in life and drug abuse. An epidemiological, retrospective study was performed to compare personal meaning in life between individuals receiving inpatient treatment for drug abuse and a group of non-drug abusing controls. The present study was conducted to determine if vitamin E and selenium could have a positive synergistic effect during experimental Chagas' disease. In the first phase of the study, 25 female C3HeB/FeJ mice were infected with a Brazil strain of Trypanosoma cruzi while receiving vitamin E supplementation of 0 IU/kg, 200 IU/kg, 400 IU/kg, 600 IU/kg, 800 IU/kg in their diets. In the second phase, 20 female C3HeB/FeJ mice were infected while receiving 400 IU/kg vitamin E plus selenium supplementation of 2 ppm, 4 ppm, 8 ppm, 16 ppm in their drinking water. Prevalence And Severity Of Viral Hepatitis In Pakistani Pregnant Women: A Five Year Hospital Based Study, A B. Aziz, S Hamid, S Iqbal, W Islam, S A. Karim

A hospital based observational study was carried out on pregnant women presenting with either acute hepatitis or fulminant hepatic failure (FHF), during the past years. Of 53 patients, 20 (38%) developed FHF.Non-A, Non-B was the commonest cause (62%) followed by hepatitis B in 17% and hepatitis A in 4% cases. Eight women expired (case fatality rate 15%) with a high maternal mortality (62%) caused by NANB hepatitis. Perinatal mortality was 30%. Poor prognostic factors identified were lack of antenatal care, severity of jaundice, history of somnolence, gastrointestinal bleeding and a high grade of encephalopathy.

D-Cycloserine is an effective second-line drug against Mycobacterium avium and Mycobacterium tuberculosis. To analyze the genetic determinants of D-cycloserine resistance in mycobacteria, a library of a resistant Mycobacterium smegmatis mutant was constructed. A resistant clone harboring a recombinant plasmid with a 3.1-kb insert that contained the glutamate decarboxylase (gadA) and D-alanine racemase (alrA) genes was identified. Subcloning experiments demonstrated that alrA was necessary and sufficient to confer a D-cycloserine resistance phenotype. The D-alanine racemase activities of wild-type and recombinant M. smegmatis strains were inhibited by D-cycloserine in a concentration-dependent manner. Objective: To apply the economic theory of economies of scope to the home healthcare industry. Data Sources: Data on 488 observations obtained from the Cost Report (HCFA Form 1728-86) of all Connecticut state-licensed, Medicare-certified home health agencies. Study Design: The Cost Report was the primary source of data for this study. Information on total cost, scope, and other related factors was collected. Logarithmic and nonlinear regression analyses were used to identify factors related to scope and also to test for economies of scope. Mesenchymal chondrosarcomas (MSCs) are a rare form of chondrosarcoma which usually arise in bone. Extraskeletal chondrosarcomas constitute a minority (14-25%) of MSCs. We describe the imaging features of an extraskeletal mesenchymal chondrosarcoma that arose from the rectus abdominus muscle.

T cell cytokines play an important role in mediating airway inflammation in asthma. The predominance of a Th2 cytokine profile, particularly interleukin (IL)-4 and IL-5, is associated with the pathogenesis and course of asthma. The aim of this study was to test the hypothesis that a stressful life event alters the pattern of cytokine release in asthmatic individuals. Thirteen healthy controls and 21 asthmatic adolescents gave blood samples three times over a semester: midsemester, during the week of final examinations, and 2-3 weeks after examinations. A 67-year-old male farmer (the victim) died when the tractor he was operating flipped over backward, pinning him between it and another tractor. He and his 13-year-old grandson had been working with a tractor and rotary mower, but got stuck in the mud of a creek bed; approximately one-third of the rear tractor tires were buried in mud. CD40 is a member of the tumor necrosis factor (TNF) receptor superfamily. Studies with human B cells show that the binding of CD154 (gp39, CD40L) to CD40 recruits TNF receptor– associated factor 2 (TRAF2) and TRAF3 to the receptor complex, induces the downregulation of the nonreceptor-associated TRAFs in the cell and induces an increased expression of Fas on the cell surface. Combined signaling through the interluekin 4 receptor and CD40 induces an increased expression of Fas with a commensurate increase in the level of TRAF2, but not TRAF3, that is recruited to the receptor complex. An 80-year-old full-time farmer died after being caught under a baler. The victim worked closely with his son on the 300-acre farm. He was working alone, driving a tractor with a baler attached, in an 8-acre field during a midafternoon in May. He had no previous logging experience and had been working with his friend who was a full-time logger for six days when the fatal incident occurred. The First Annual Dr. John Patten Memorial Lecture was held November 14 at the Hunter College of Social Work. CLAGS agreed to cosponsor the lecture, developed to honor the life and work of Dr. John Patten, faculty member of the Ackerman Institute for Family Therapy, Medical Director of the Institute's AIDS Project, and co-founder and co-director f the Institute's Gay and Lesbian Family Study Project. Dr. Patten was also co-founder and senior editor of In The Family Magazine, a family therapy-oriented magazine for gays, lesbians, bisexuals and their relations. He died of AIDS on October 4, 1995.

A 56-year-old part-time farmer was killed when the tractor he was driving overturned while mowing. Using a rotary mower attached to the John Deere 1520 tractor, the farmer mowed an area near the roadway that had a gradual downward slope. The tractor was not equipped with a Rollover Protective Structure (ROPS) or seatbelt. After mowing two rows, he made a left turn to begin the third pass when the back tire began to slip in the moist soil underneath the grass. Since January, at least six Kentucky farmers have been killed in tractor rollovers in which terrain changes caused by recent heavy rainfalls were a factor. In addition to mud and wet, slick grass, the rain created marshy areas and holes where none had been before; often these places are invisible due to weeds and tall grass.
