Macrodystrophia Lipomatosa: A Casereport, Pai-Yin Chen, Shu-Min Chen, Chii-Jeng Lin

Macrodystrophia lipomatosa is a rare congenital anomaly of localized gigantism, characterized by progressive overgrowth of all mesenchymal components, particularly fibroadipose tissue. The anomaly occurs frequently in the distribution regions of the median nerve in the upper extremity and the plantar nerve in the lower extremity. It is usually noticed at birth with predominately unilateral distal involvement of the digits. The cause of this condition is still unknown. Physicians use the patient's clinical history, physical examination, and imaging techniques to diagnose the condition, with characteristic imaging findings revealing macrodactyly with hypertrophy of the soft tissue and osseous structures. Background: Cartilage functions as a highly successful load-bearing tissue. However, frequent mechanical loading can affect cartilage metabolism and viability significantly. Intrinsic changes in the metabolism of chondrocytes have also been proposed as an important feature of osteoarthritis. Although many aspects of chondrocyte catabolism have been extensively investigated, the signals that initiate such metabolic changes, and their correlation with structural changes, remain unclear. We hypothesized that matrix metaproteinase-3 (MMP-3) may play a significant role. T12 Compression Fracture Complicated With Delayed Spinal Epidural Hematoma: A Casereport, Chia-Wei Wu, Ching-Shiang Tseng, Lin-Fen Hsieh

Delayed spinal epidural hematoma (SEH) after a spinal compression fracture is a very rare but important cause of symptomatic spinal cord compression. We report a patient with spinal cord compression and cauda equina syndrome due to delayed SEH after a traumatic T12 compression fracture. A 74-year-old male patient had severe back pain without neurologic deficits after a traumatic T12 compression fracture. He developed progressive bilateral lower extremity weakness, sensory loss, and sphincter dysfunction 2.5 months after the injury. Magnetic resonance imaging of the spine revealed a huge spinal epidural hematoma at the level of T12 to L5. He then received …

Numerous serotonergic drugs have been increasingly used in recent years to treat depression, pain, insomnia and obesity. However, concomitant use of these drugs may cause serotonin syndrome. The three main clinical features of serotonin syndrome are neuromuscular abnormalities, autonomic hyperactivity, and mental status changes. A wide variety of the clinical presentations range from: very mild symptoms such as tremor, diarrhea, muscle rigidity, or clonus, and hyperreflexia; to fatal consequences such as disseminated intravascular coagulation or multiple-organ failure. Symptoms resolve spontaneously after prompt discontinuation of the precipitating medication. The diagnosis is based on clinical presentation, history of medication, and resolution after …

In investigating the determinants of survival of prostate cancer cohorts, characterization of the cohort in terms of competing underlying causes of death would be appropriate. Atherosclerotic heart disease, bronchitis, acute myocardial infarction, chronic obstructive pulmonary disease, atherosclerotic cardiovascular disease, and stroke were found to be the major causes of death among prostate cancer cohorts aside from prostate cancer. A population-based assessment of the codeterminants of mortality and a demographic and clinical determinant of prostate cancer survival was undertaken. Kesehatan maternal yang tergolong tinggi di Indonesia merupakan indikator keberhasilan pembangunan pada Millenium Development Goals yang terus diupayakan untuk diperbaiki. Penelitian ini bertujuan mengetahui hubungan antara karakteristik sosial, demografi, dan ekonomi dengan tingkat kesehatan ibu. Penelitian yang menggunakan sumber data sekunder Survei Demografi Kesehatan di Jawa Barat yang mencakup 86 blok sensus yang meliputi 2.150 rumah tangga meliputi 1.100 rumah tangga di perkotaan dan 1.050 rumah tangga di pedesaan. Anak penderita Human Immunodeficiency Virus/Acquired Immune Deficiency Syndrome (HIV/AIDS) di Indonesia memperlihatkan tren yang semakin meningkat karena meningkatnya proporsi perempuan terinfeksi HIV/AIDS. Pertimbangan pada dampak besar yang dihadapi anak penderita HIV mendorong kebutuhan pengembangan instrumen khusus untuk mengukur kualitas hidup mereka. Penelitian ini bertujuan mengembangkan instrumen kualitas hidup anak penderita AIDS dengan memodifikasi instrumen yang ada sesuai dengan konteks Indonesia. Demensia Alzheimer menempati urutan kesembilan penyebab kematian di Amerika Serikat. Demensia adalah kondisi yang sering dialami yang berhubungan dengan berbagai faktor dan gaya hidup terutama diet. Penelitian ini bertujuan untuk mengetahui hubungan defisiensi asupan riboflavin (vitamin B12) dengan demensia pada usia lanjut (usila). Penelitian ini menggunakan desain studi cross sectional dan dilaksanakan pada bulan September 2007 sampai dengan Januari 2008. Sampel penelitian adalah 141 lansia berumur lebih dari sama dengan 60 tahun yang diambil secara purposive sampling. Beberapa tahun terakhir angka obesitas di Indonesia cenderung meningkat. Obesitas terjadi akibat ketidakseimbangan antara asupan dan output energi dalam jangka waktu cukup lama. Tujuan penelitian ini adalah untuk mengetahui faktor risiko kejadian obesitas pada pekerja on shore di perusahaan minyak dan gas. Desain penelitian ini adalah studi cross sectional. Sampel yang diteliti adalah seluruh pekerja on shore (laki-laki maupun perempuan) berusia 35 – 55 tahun yang sesuai dan memenuhi kriteria inklusi dan ekslusi yang telah ditetapkan (n = 378). Provinsi Nusa Tenggara Timur (NTT) memiliki Total Fertility Rate (TFR) tertinggi (4,2 anak) dan yang terendah adalah Provinsi Yogyakarta (1,8 anak). It is often said that justice requires us to treat like cases alike. Accordingly, the U.S. refugee resettlement program provides all refugees—no matter where they are from, no matter their pasts—with very similar funding and services. Refugees, however, are far from alike. In this essay, I invoke Borgmann’s distinction between a “thing” and a “device” and draw on stories from my work with a resettlement agency to argue that our current, employment-driven system is in need of reform. The lack of diagnostic gold standards and the multiplicity of vantage points used to conceptualize mental illness renders psychiatric diagnostic and therapeutic approaches susceptible to individual and cultural biases. Developments in therapeutic approaches have often provided vantage points from which to conceptualize psychopathology. The identification of predictors of treatment response may be a useful element for clinicians to select between multiple treatment tools in an expanding therapeutic armamentarium while fostering refinements in current diagnostic systems.

The medical literature devotes little attention to the application of competency to the general medical patient. Commonly, the only cases recognized as necessitating a competency evaluation are overtly psychotic patients, patients requiring written informed consent, and those patients who refuse treatments strongly recommended by their physician. Clarifying terminology and redefining competency provides a practical screening method for assessing competency in all patients. When physicians neglect the evaluation of competency, patient autonomy is compromised. Not only are all physicians capable of performing a competency evaluation, they are ethically and professionally required to do so.

An individual's decision to pursue a psychiatric residency following medical school training is shaped by many factors. Beginning residents are often ill prepared for the relative impact that the shift from medical doctor to psychiatric resident entails. This paper reviews the literature regarding demographic and psychological factors relating to recruitment, dynamic and practical issues confronting the beginning resident, and various coping styles adopted. These factors are considered in the context of an inpatient setting where the majority of residents begin their training. Much of the resident response is seen as adaptive and a number of strategies for coping are suggested.

Upon arriving at an outpatient clinic, many patients request to see a female psychotherapist. Yet little has been written on this topic. The aim of this study was to explore the demographic and clinical characteristics of such patients. We compared patients who voiced a preference for a female therapist with controls on the demographic variables of age, sex, race, marital and financial status and the clinical characteristics of diagnosis, history of sexual abuse, sexual preference, overt presence of sexual or social difficulties, history of prior treatment, gender of the previous therapist, and treatment drop out rate. Non-Neuroleptic Pharmacotherapy Of Schizophrenia, Sajiv John, Md, Atul Mahableshwarkar, Md

Patients with schizophrenia are typically treated with a neuroleptic and some form of psychosocial intervention. In spite of optimum treatment, up to one-third of patients continue to remain severely disabled due to persistent positive or negative symptoms. A variety of drugs have been tried as alternatives or additions to traditional neuroleptics in these patients. This paper is a review of these nonneuroleptic treatment alternatives and an attempt to establish some guidelines for their use.

Obsessive-Compulsive Disorder (OCD) was considered a rare disorder prior to 1984 when the initial results from the Epidemiologic Catchment Area survey demonstrated a substantial prevalence of the disorder (1). Thus there may be many patients today who entered treatment prior to 1984 with undiagnosed OCD. This paper gives the history of one such patient and reviews the current literature on OCD as it pertains to the diagnostic evidence in this case.

Concepts And Models Of Empathy: Past, Present, And Future, Elizabeth Ann Baxter, Md

The subject of empathy has been increasing in popularity and importance; correspondingly, the theoretical aspects of empathy are in need of further development. This paper outlines key concepts and processes related to empathy, particularly the mechanisms of identification and regression as they are discussed in the models of empathy articulated by Fleiss, Greenson, and Jordan. Similarities and differences of the models are examined, in addition to each model's description of the roles of identification and regression. Although each model has made significant contributions towards a theoretical perspective of empathy, none of them have distinguished pathological from non-pathological uses of identification. …

"Sometimes a cigar is just a cigar. " Others had learned, "My son, sometimes a cigar is just a good smoke." Like many of our colleagues, we believed that these were Freud's words during a question and answer session after a lecture he gave at Clark University. The words were comforting in a way, giving us a chance to chalk at least some things up to pure coincidence, thus releasing us from relentless unconscious agendas and hidden sexual interpretations.
