Ecmo In Pediatric Oncologic And Hsct Patients: A Descriptive And Comparative Analysis, Michelle Brown

Introduction: Pediatric oncologic and hematopoietic stem cell transplant (HSCT) recipients are vulnerable to development of respiratory failure due to chemotherapy, radiation, surgery, and immunosuppression. Yet, providing extracorporeal membrane oxygenation (ECMO) support in these patient populations remains controversial with heterogenous utilization across pediatric institutions. The aim of this study is to characterize and compare outcomes in pediatric oncologic and HSCT patients with respiratory failure in the presence and absence of ECMO support. We present a case of prenatally diagnosed d-transposition of the great arteries (d-TGA) with intact ventricular septum (IVS), postnatally found to have bilateral patent ductus arteriosus (PDA) with potential for isolation of the left subclavian artery (LSA) and an abnormal left coronary artery (LCA) course. Isolation of the LSA is a rare occurrence and defined as an anomalous origin from the pulmonary artery via PDA and apparent discontinuation from the arch following PDA closure. It is exceedingly rare in association with d-TGA. Coronary artery anatomy is particularly important in d-TGA given most patients undergo the arterial switch operation (ASO).

• Background (83 words): Standard delivery protocols for most prenatally diagnosed CCHD is induction of labor at 39 weeks with aims of a vaginal delivery (VD) near a tertiary care center. There is no benefit to planned Cesarean delivery (CD) over VD for most infants with CCHD. However, longer durations of labor (DOL) in CCHD have been described as a risk factor for umbilical artery (UA) acidosis. Cervical cancer screening (CCS) is a crucial test used for preventative measures against cervical cancer, yet transgender men (TM) experience significantly lower screening rates in comparison to cisgender women due to pain, anxiety, and systemic healthcare barriers. A major factor for deterrence is pain during speculum insertion, often because of vaginal atrophy related to testosterone usage. Other factors include fear of discrimination, mistreatment, and lack of provider awareness, particularly among nurses who play a pivotal role in patient preparation and care. Pediatric magnetic resonance imaging is often challenging due to the lack of cooperation and the inability to fully understand the expectations required for the examination. Children are often restless and may have difficulty remaining still for a long period of time. This can lead to motion artifacts and compensate diagnostic value. Background: Vestibular rehabilitation is known to be effective in the treatment of vestibular dysfunction. While patients with Parkinson's disease have both central and peripheral vestibular dysfunction, there is little information regarding the efficacy of vestibular rehabilitation for the treatment of these individuals.

Study Design: Systematic Review

Purpose: The purpose of this systematic review was to examine the effects of vestibular rehabilitation in patients with Parkinson’s disease.

Results: The interventions identified in this study resulted in improved Berg Balance Scale (BBS) and Dizziness Handicap Questionnaire (DHI) scores.

Conclusion: Vestibular rehabilitation improves balance and subjective dizziness in patients with Parkinson’s disease.

This study is based upon the Transtheoretical/Stages of Change model. The model has been studied in adults with a variety of health behaviors such as addiction (Prochaska, 1999; Prochaska &DiClemente, 1984) and has been applied to current stuttering treatment (Zebrowski et al., 2021) to assess readiness for change. The Stages of Change model consists of five stages: precontemplation, contemplation, preparation, action, and maintenance. Whenever considering making a change, as one would do as part of speech therapy, clients may move in and out of these stages.
