Aim: To investigate the prevalence of the physical and sexual abuse of women before and during pregnancy, and to determine whether pregnancy affected the abuse of women. Materials and Methods: This study was performed at a state hospital in the Turkish city of Trabzon and included 762 women that gave birth between July and September 2004. A questionnaire was administered during a face-to-face interview to collect data on sociodemographic factors, exposure to abuse before and during pregnancy, and the women´s post-abuse attitudes. Aims: Families with children afflicted with febrile convulsions must be given adequate information regarding seizures and fever, and they need emotional support and first-aid demonstrations. In this study, we aimed to determine parental knowledge and practices regarding febrile convulsions in Turkish children. Materials and Methods: Questionnaires were given to 122 parents whose children were admitted to the emergency service of Behçet Uz Children´s Hospital between July 2006 and April 2007 with febrile convulsion episodes. Benign tumors of the parotid gland such as pleomorphic adenoma present as a painless mass and pose no difficulty in diagnosis. It is very rare to find a benign mass coexisting with a malignant entity in the parotid gland. We present a case of pleomorphic adenoma coexisting with ductal carcinoma.

Mixed Medullary-Follicular Thyroid Carcinoma : A Report Of Two Cases, Özge Han, Tuba Ünal, Selda Seçki̇n, Sezer Kulaçoğlu

Turkish Journal of Medical Sciences

Mixed medullary-follicular carcinoma is an uncommon tumor of the thyroid. It is characterized by histological and immunohistochemical features of both follicular and parafollicular C cells. We report two female patients, aged 26 and 78 years, with mixed medullary-follicular carcinoma. Histologically, the tumors are composed of follicular carcinoma areas and solid nests of polygonal cells consistent with medullary carcinoma. Immunohistochemically, thyroglobulin and calcitonin positivity was detected. Precise diagnosis of mixed medullary-follicular carcinoma is essential for accurate treatment and follow-up.

Reactive oxygen species may play a role in the mediation of acetaminophen (APAP) hepatotoxicity. N-acetylcysteine (NAC) is the standard therapy for treatment of the APAP overdose patient. The suspected mechanism of hepatic injury appears to be similar to that of APAP and the empiric use of NAC may be justified, especially in significant endosulfan poisoning. An 18-year-old male presented to the emergency department because of unconsciousness and seizure that developed after acute intentional ingestion of endosulfan. The septate uterus is the most common form of structural uterine anomalies. A rare developmental variant is a complete septate uterus with cervical duplication and a complete vaginal septum. We present a case of unilateral traumatic retinal angiopathy following a mandibular fracture. An 11-year-old child involved in a bicycle accident developed Purtscher´s retinopathy in the left eye with retinal hemorrhages, cotton wool spots, and optic disc swelling on funduscopy. The visual acuity in this eye was counting fingers at one meter.

Many hypokinetic and hyperkinetic movement disorders (e.g. parkinsonism, chorea, myoclonus, dystonia) are associated with acquired immune deficiency syndrome (AIDS) and may sometimes represent the initial manifestation of the illness. We described a 37-year-old human immunodeficiency virus (HIV)-infected patient with generalized choreoathetoid movements associated with AIDS. Cranial MRI revealed multiple cerebral and cerebellar abscesses. The diagnosis of toxoplasmosis was established by brain biopsy. Hemichorea and hemiballismus in AIDS patients are pathognomonic findings of cerebral toxoplasmosis. Generalized choreiform movement is a rare condition and related to the bilateral cerebral involvement in toxoplasma disease affecting the subcortical structures.

Aims: Dihydrofolate reductase (DHFR) is necessary for the reduction of the ingested folates before they are used in the body metabolism. Thus, the DHFR enzyme has an important role in folate supplementation and the DHFR gene is a strong candidate for a teratogenic locus for neural tube defects (NTDs). There is a 19 bp deletion within the first intron of the DHFR gene, which may have an effect on folate reduction. Aims: We prospectively evaluated the diagnostic value of serum prostate specific antigen (PSA)-based diagnostic parameters and serum dehydroepiandrosterone sulfate (DHEAS) level for early detection of prostate cancer in patients with serum PSA level between 2.5 and 4 ng/ml. Materials and Methods: This study included 83 consecutive men with serum PSA levels between 2.5 and 4.0 ng/ml. All patients underwent transrectal ultrasound-guided prostate needle biopsy. Total PSA level, free/total PSA ratio, PSA density of the total volume (PSAD-TV), PSA density of the transition zone (PSAD-TZ), and serum DHEAS values were determined. Aims: To investigate radiation-induced oxidative damage in erythrocytes after administration of radioiodine-131 (^{131}I) in patients with hyperthyroidism. Materials and Methods: Twenty patients with hyperthyroidism (11 F, 9 M) treated with ^{131}I were included into the study. Blood samples were taken from patients just before, 1 hour after and 3 hours after applying radioiodine. Aim: In this paper, we present the hematological findings of 787 cases diagnosed with acute brucellosis. Materials and Methods: Records of the cases seen between 1994 and 2006 were retrospectively investigated regarding hematological changes. Results: Three hundred sixty-five (46%) patients were male and 422 (54%) were female. Mean age was 32 (range: 11-78 years). Mean hematological values were as follows: Hb 12 g/dl (4-19), Htc 36% (12-56), leukocyte 6.9 x 10^9/L (0.5-25) and platelets 213 x 10^9/L (9-617). Aim: The aim of this study was to establish a classification that will aid in the surgical treatment and prognosis of sigmoid volvulus (SV). Materials and Methods: The records of 420 patients with SV who were treated surgically during a 41.5-year period were reviewed retrospectively. The preoperative and operative criteria that were significantly correlated with mortality were determined. Results: Of 420 patients, 68 (16.2%) died. Age over 60 years (P < 0.001), associated disease (P < 0.001), presence of shock (P < 0.001), and presence of sigmoid colon gangrene (P < 0.001) were significantly correlated with mortality. A classification was made using these criteria. Conclusions: Recommended treatment options and potential mortality rates were determined for each class in the new classification based on the evaluation of the present series, our clinical experience, and literature findings. This classification may be helpful in determination of the surgical treatment and prognosis of SV.

Aims: To show the efficacy of oral contraceptive (OC) treatment for hospitalized adolescent patients with abnormal uterine bleeding (AUB). Materials and Methods: Thirty-one girls with AUB and anemia hospitalized within the last two years were included. The patients´ medical records were evaluated retrospectively. All patients were given combined OC containing 0.03 mg ethinyl estradiol plus 0.15 mg desogestrel according to a defined protocol. Aim: To document the efficacy of low-dose long-term oral macrolide treatment in the management of sinonasal polyposis. The study was designed prospectively in a group of patients with sinonasal polyposis and negative history of surgery. Materials and Methods: Ten patients were administered roxithromycin (RXM) 150 mg/day single oral dose for eight weeks. All patients underwent Tc-99m rhinoscintigraphic evaluation of mucociliary activity and endoscopic evaluation before and after the treatments. A number of surgical procedures can be performed for partial auricular defects occurring due to various causes. The most appropriate reconstruction method should be employed with regard to the defect dimensions and the status of local adjacent tissues. von Recklinghausen disease is a type I neurofibromatosis originating from the nervous system. In this syndrome, there is abnormal deposition of neural tissue in the nervous system, endocrine system, visceral structures, and skin. We report the anesthetic management of a 31-year-old female patient with multiple thorax deformities who was diagnosed as von Recklinghausen disease. The patient was admitted to the Plastic and Reconstructive Surgery Department because of a huge subcutaneous neurofibroma on her right upper extremity. Temporary breast enlargement may be seen in normal newborn and adolescent boys. Cysts of the breast are uncommon in childhood. Galactocele, defined as an encysted collection of milk products, is an extremely rare cause of breast enlargement in infants and children. We herein report a male infant with bilateral galactocele who was otherwise healthy, and we also review the literature.

Acute bacillary dysentery (shigellosis) occurs due to infections with shigella sp. The spectrum of acute shigellosis ranges from mild watery diarrhea to severe dysentery. Surgical complications of shigellosis described in children have included appendicitis with or without perforation, colonic perforation, intestinal obstruction, and intraabdominal abscess. Acute appendicitis and infective diarrheal illness can be difficult to differentiate, particularly in children. Thus, patients must be followed up closely until the differentiation between bacterial gastroenteritis and acute appendicitis can be made. We report herein a 10-year-old boy who had acute appendicitis due to Shigella flexneri.

Hemophagocytic syndrome (HPS) is a disorder that might be transferred genetically due to an autosomal recessive genetic defect of the long arm of chromosome 9 and chromosome 10 (9q21.3-22 and 10q21-22), termed as primary hemophagocytic syndrome (PHPS) or familial hemophagocytic hemophagocytosis (FHPS) and it may be associated with a variety of infections, malignant neoplasms, drugs, autoimmune diseases and various immuno deficiencies, termed as secondary hemophagocytic syndrome (SHPS). Aim: In this study, we aimed to assess the expression of CD117 in a large series of both Hodgkin´s and non-Hodgkin´s lymphoma, and to investigate the association between the expression patterns of CD117, IRF4/MUM1, and CD38. Materials and Methods: A total of 237 lymphoma cases were selected and examined within six tissue microarray paraffin blocks. Immunohistochemistry was performed using antibodies against CD117, IRF4/MUM1 and CD38. Results: CD117 and IRF4/MUM1 immunoreactivity were seen in a subset of high-grade and low-grade B-cell lymphomas, as well as T-cell lymphomas and classical type Hodgkin´s lymphomas.
